1.Research status of non-drug treatment of vestibular migraine

Due to the limitations of drug treatment of vestibular migraine VM,the exploration of non-drug treatment is particularly important.In recent years,the research on non-drug treatment of VM mainly focuses on four aspects:lifestyle management,vestibular rehabilitation training,neuroregulatory therapy and acupuncture therapy.A number of studies suggest that all VM patients should consider vestibular rehabilitation training,but the choice of neuroregulatory therapy stimulation and adverse reactions after treatment still need to be systematically studied.A comprehensive description of non-drug therapy for VM is helpful to develop a more comprehensive and scientific treatment plan for VM patients.

Dendritic cell-based cancer vaccines (DC vaccines) have been proved efficient and safe in immunotherapy of various cancers, including melanoma, ovarian and prostate cancer. However, the clinical responses were not always satisfied. Here we proposed a novel strategy to prepare DC vaccines. In the present study, a fusion protein SNU containing a secretin-penetratin (SecPen) peptide, NY-ESO-1 and ubiquitin was designed and expressed. To establish the DC vaccine (DC-SNU), the mouse bone marrow-derived DCs (BMDCs) were isolated, pulsed with SNU and maturated with cytokine cocktail. Then peripheral blood mononuclear cells (PBMCs) from C57BL/6 mice inoculated intraperitoneally with DC-SNU were separated and cocultured with MC38/MC38
3.Influence of WeChat education on self-management of diabetic nephropathy patients based on IMB model

Objective To explore the effects of information-motivation-behavioral (IMB) skills model based WeChat education on the treatment compliance, the self-efficacy and the quality of life of diabetic nephropathy patients.Methods A total of 92 patients with diabetic nephropathy treated in Danyang People's Hospital from January 2015 to June 2016 were selected as the research object by convenient sampling method and randomly divided into observation group and control group according to the random number table, with 46 cases in each. The control group received conventional WeChat health education, and the observation group implemented WeChat health education based on IMB model. The treatment compliance, the self-efficacy and the quality of life were compared between the two groups 6 months after the nursing intervention. Results There was no statistically significant difference between the two groups of all dimensions and the total scores of treatment compliance before intervention (P>0.05). All the dimensions of treatment compliance of the observation group were higher than those of the control group after intervention, and the differences were statistically significant (t=3.365, 3.258, 3.989, 2.655, 3.513, 4.674, 3.747, 4.187,3.578;P<0.01). The change degree of the 10 dimensions of self-efficacy and total scores were all greater than those of control group, the differences were statistically significant (t=4.854, 5.632, 4.854, 4.241,3.896, 3.785, 3.789, 3.325, 3.314, 3.817,4.123;P<0.01). The improvement of all the dimensions of patients' life quality and its total score were all higher than the control group after intervention, with significant statistical difference (t=5.965, 4.763, 4.746, 4.547, 3.869;P<0.01).Conclusions Application of IMB model based WeChat health education on diabetic nephropathy patients can significantly improve the patient's treatment compliance, self-efficacy and quality of life.
4.Investigation on characteristic of infectious occupational exposure among medical staff in a newly-built hospital

Objective To investigate the characteristics of infectious occupational exposure among medical staff in newly-built hospitals, find out the high risk points of occupational exposure, and formulate occupational safety protection measures accordingly. Methods A retrospective survey in Beijing Tsinghua Changgung Hospital during January 2015 to October 2017 was carried out on occupation exposure, including gender, age, working years, occupation,exposure time, exposure sites, exposed sites and links, exposure to the source of infection and other aspects of the data, using SPSS 16.0 statistical analysis. Results There were 59 infectious occupational exposures. Among 59 infectious occupational exposures, 52 cases (88.1%) were sharp instrument injuries, 7 cases (11.9%) were blood and body fluid exposure. Occupational exposure occurred mainly in nurses (45 cases, 76.3%), followed by doctors (11 cases, 18.6%). The median age was 26 years, and the median length of work was 2 years. The location of exposure was mainly ward (26 cases, 44.1%), followed by operation room (17 cases, 28.8%). The location of sharp instrument injuries was mainly finger (44 cases, 84.6%). Sharps were mainly needles (38 cases, 73.1%), and the proportion of sharp instruments was 80.4%. The main causes of exposure were themselves (41 cases, 77.5%). Hepatitis B was the main pathogen of exposure (27 cases, 45.8%). No nosocomial infection was not found at follow-up. Conclusions New hospitals should establish a perfect occupational exposure management system, control the high-risk points, strengthen the occupational safety protection training, standardize the operation, and reduce the incidence of infection.
5.Survey on awareness and willingness of breast cancer screening between Han and Mongolian women in Ordos , Inner Mongolia Autonomous Region

Objective To investigate and explore the awareness and willingness of breast cancer screening between Han and Mongolian women in Ordos, Inner Mongolia Autonomous Region, and to analyze the influence factors. Methods The patients answered investigation questionnaires face to face, and the content of questionnaire included demographic information, breast cancer awareness and relevant affecting factors on the awareness and willingness. Results 11162 (88.2 %) Han women and 1495 (11.8 %) Mongolian women were enrolled in the survey, with the average age of (45.4 ±7.1) years old and (45.4 ±6.8) years old. The education level (high school and above) of Han women was lower than that of Mongolian women [4456 women (40.0%) vs. 750 women (50.2%), χ2= 57.041, P< 0.001]. The Main way to delivery information on breast cancer screening in Han and Mongolian women was from their family members or friends [5002 women (44.9%) vs. 688 women (46.1%),χ2=40.699, P<0.001]. The proportions of never attending screening in the Han and Mongolian women were 41.7 % (4640 women) and 45.9 % (683 women), respectively (χ2=14.075, P=0.001). The breast self-examination rate of Han women were higher than that of Mongolian [71.1 % (7926 women) vs. 58.5 % (872 women), χ2= 98.466, P< 0.001]. According to logistics regression analysis, the cognitive level of women with high education level and breast disease history was higher (all P<0.001). Conclusions The cognitive level of breast cancer screening in Han women is better than that in the Mongolian, however, the overall level are low. It is necessary to strengthen the propaganda and education, and to encourage women to actively participate in breast cancer screening.
6.Comparison of amlodipine besylate and amlodipine besylate in the treatment of hypertension

Objective To study and analyze the effect of amlodipine besylate and amlodipine besylate in the treatment of hypertension. Methods 100 patients with hypertension treated in our hospital from January 2015 to December 2016 were selected and randomly divided into two groups, the control group and the experimental group, with 50 patients in each group. The control group was treated with amlodipine besylate, and the patients in the experimental group were treated with amlodipine besylate and amlodipine besylate. The therapeutic effects of the experimental group and the control group were compared and analyzed. Results After the corresponding treatment, the experimental group of 50 cases, the total effective number of 44 cases, 6 patients invalid, the total effective rate was 88%. In the control group of 50 cases, the total effective number was 37 cases, 13 cases were invalid, the total effective rate was 74%. The treatment effect of the experimental group was significantly higher than that of the control group, with statistical difference (P<0.05). There were 5 cases of adverse reactions in the experimental group, 3 patients had facial flushing, and 2 patients suffered from pain. The incidence was 10%. The number of adverse reactions in the control group was 12 cases, ankle edema occurred in 5 patients, and headache occurred in 7 patients, with a rate of 24%. The rate of adverse reactions in the control group was significantly higher than that in the experimental group, with statistical difference (P<0.05). Conclusion Amlodipine and Levamlodipine besylate in treatment of hypertension can improve the treatment efficiency to a great extent, high safety, low probability of adverse reactions, with further clinical promotion and application significance.
7.Effect of artificial cycle on expression of MMP-9 and TGF-β1 of endometria in patients with uterine adhesion

Objective To investigate the effect of the artificial cycle on the prognosis after transcervical resection of adhesion (TCRA) and its mechanism .Methods 80 patients with intrauterine adhesion were randomly divided into the observation group and the control group .The observation group were performed TRCA and postoperative artificial cycle for successive 3 months :oral es-tradiol valerate(9 mg once daily) for 21 d ,adding oral medroxyprofgesterone acetate(10 mg once dialy) on 15 d ,then the medication discontinuation for 7 d ,which was taken as 1 cycle;the control group received only TCRA .The re-examination was performed after 3 months .The levels of follicle stimulating hormone(FSH) ,luteinizing hormone(LH) ,estradiol(E2 ) ,prolactin(PRL) ,progesterone (P) and testosterone(T) were examined by ELISA after TCRA three months .The expression of MMP-9 and TGF-β1 in adhesive endometrium were detected by real-time PCR and Western blot .Results Compared with the control group ,the artificial cycle thera-py combined with the operation could significantly increase the effective rate of TCRA for treating mild and moderate intrauterine adhesion(P<0 .05);the seum levels of FSH ,LH ,E2 ,PRL ,P and T had no statistical differences between the observation group and the control group(P>0 .05);the artificial cycle significantly improved the adhesive degree of intrauterine adhesion ,increased the MMP-9 expression and decreased the TGF-β1 expression .Conclusion Artificial cycle could improve the degree of uterine cavity ad-hesion after TCRA ,which the mechanism might be related to the increase of MMP-9 expression and decrease of TGF-β1 expres-sion .
8.Promotive effect of neovascularization on rats with cerebral infarction by intranasal administration of granulocyte colony-stimulating factor

Objective To study the promotive effect of neovascularization on rats with cerebral infarction by nasal administration of granulocyte colony-stimulating factor.Methods A blinded,vehicle-controlled study of ING-CSF and IHG-CSF administration was performed by intraluminal middle cerebral artery occlusion (MCAO) model.All Sprague-Dawley rats were randomly divided into sham-operation group,model group,INNS group,IHGCSF group and ING-CSF group.The neurologic behavioral tests were assessed after reperfusion 72 h.Mter 72 h of MCAO,the brains of rats were stainned with TTC and the infarcted volume was calculated by computer image analysis.The expression of vascular endothelial growth factor (VEGF) in the brain was determined by immune-histochemistry.The density of angiogenesis in the brain was counted under fluorescence microscope.Results The score of neurological function of ING-CSF group(3.90± 1.65)was improved significantly compared with the IHG-CSF group (10.55±2.19) at the point of 72 h after cerebral infarction (P<0.01).The cerebral infarct volume of ING-CSF group((20.01±3.29) %) was reduced evidently compared with the IHG-CSF group((33.48±4.49) %) at 72 h (P< 0.01);while the cerebral infarct volume of INNS group ((60.20±7.72) %)was not markedly different compared with the model group((61.49±6.41)%) at 72 h (P>0.05).The expression of VEGF in the brains of ING-CSF group was significantly higher than other groups at 72 h.Conclusion Intranasal administration G-CSF can improve neurological function and vascular angiogenesis in rats following MCAO.
9.Effect of gradually induced disordered occlusion on the expression of osteoprotegerin and receptor activator of nuclear factor-kappaB ligand in mandibular condylar cartilage of rats.

OBJECTIVETo investigate the effect of gradually induced disordered occlusion (GIDO) on the expression of osteoprotegerin (OPG) and receptor activator of nuclear factor-kappaB ligand (RANKL) in mandibular condylar cartilage of rats.
METHODSTotally 48 rats, aged 8 weeks were included, and were divided into experimental and control groups randomly at 4 time points, with same gender distribution (n=3). By inserting elastic rubber band the right side mandibular first molar and the left side maxillary first molar were moved mesially. Four weeks later, the right side mandibular third molar and the left side maxillary third molar were moved distally with same method. In this way, the GIDO was established in rats. The rats were sacrificed at the end of 2th, 4th, 6th and 8th week respectively after the application of the GIDO. The expression of OPG and RANKL in condylar cartilage was examined with immunohistochemical method and calculated by the area of positive cell percentage.
RESULTSOPG and RANKL expressed predominantly in condylar cartilage hypertrophic layer. The rats in experimental group expressed a higher OPG level in all of the 4 time points than their age-matched controls (P<0.05), while RANKL were higher in 2, 6, 8 weeks subgroups (P<0.05), but not in 4 weeks subgroup. No differences were found between male and female subgroups.
CONCLUSIONThe present results suggest that both OPG and RANKL take part in the condylar cartilage remodeling procedure in the present rat model.

10.Analysis of full endoscopic transnasal-transphenoidal approach for pituitary adenomas : the summary of 72 patients

Objective To investigate and evaluate the clinical value of full endoscopic transnasal transphenoidal approach for the surgery of pituitary adenomas.Methods Seventy-two patients,who underwent full endoscopic transnasal transphenoidal approach for the surgery of pituitary adenomas,were selected from the Medical Center of Pituitary Adenomas of our hospital from January 2009 to March 2012.To retrospectively investigate pre- and post-operation symptoms,hormone levels,images information,operation information,complications,following-up information and so on.Results Among the 72 consecutive patients,there were 22 nonfunctioning adenomas,twenty-four prolactin secreting adenomas,seven somatotropin secreting adenomas,five adrenocorticotropic hormone secreting adenomas,one thyrotropin secreting adenomas,and 13 multi-secreting adenomas.The tumor removal was total in 56(77.8%),subtotal 13(18.0%),and partial 3(4.2%).Five cases had CSF leaks,and 6 diabetes insipidus.After 3-24 months of follow-up,the levels of increasing-hormone declined to normal levels in most patients.Conclusion Full endoscopic transnasal transphenoidal approach for the surgery of pituitary adenomas is a kind of technique which is safe,minimally invasive,having less complications and fast recovery.However,it is necessary for surgeons to accept systematic and specialized training,and own advanced equipments.
