1.Effect of orthokeratology on the myopia progression of contralateral eyes in adolescents with monocular myopia

AIM:To investigate the effect of orthokeratology on myopia progression in contralateral eyes of adolescents with monocular myopic anisometropia.METHODS:Prospective clinical control study. From January 2021 to August 2021 at Nanning Aier Eye Hospital, 35 patients(70 eyes)diagnosed with monocular myopia and opting for single vision frame glasses correction were recruited as the SV group, and 35 patients(70 eyes)opting for orthokeratology correction were recruited as the OK group, with 1 year of follow-up. Both eyes in the SV group wore single vision lenses, while the myopic eye in the OK group wore an orthokeratology lens, with no intervention in the contralateral eye. After wearing glasses for 6 and 12 mo, the axial lengths of the myopic and contralateral eyes in both groups were measured again, and the data were statistically analyzed.RESULTS:After wearing lenses for 6 and 12 mo, the axial length of the myopic eye of patients in the SV group increased by 0.18±0.07 and 0.35±0.12 mm, respectively, and that of the contralateral eye increased by 0.10±0.05 and 0.23±0.10 mm, respectively. In the OK group, the axial length of the myopic eye of patients, increased by 0.09±0.05 and 0.16±0.09 mm, respectively, and that of the contralateral eye increased by 0.19±0.08 and 0.38±0.13 mm, respectively. The axial length growth of the myopic eye in the SV group was higher than that in the OK group at different time points(t=0.367, P<0.001; t=0.688, P<0.001), whereas the axial length growth of the contralateral eye was lower than that in the OK group(t=0.074, P<0.001; t=0.170, P<0.001). After wearing lenses for 6 and 12 mo, the interocular axial length difference values increased by 0.08±0.05 and 0.12±0.08 mm in the SV group, but decreased by 0.10±0.07 and 0.21±0.12 mm in the OK group, respectively. A statistically significant inter-group difference was observed in the change of interocular axial length discrepancy after wearing lenses for 6 and 12 mo(t=0.111, P<0.001; t=0.084, P<0.001).CONCLUSION:Compared with single vision frame glasses, wearing an orthokeratology lens in the myopic eye can effectively help inhibit the growth of axial length and reduce the degree of anisometropia in adolescents with monocular myopic anisometropia, but there is a potential risk of promoting the growth of axial length in the contralateral eye.
2.Analysis of the management effect of community pharmacy outpatient service on patients with type 2 diabetes mellitus

Objective To explore the effect of community pharmacy outpatient service on patients with type 2 diabetes mellitus. Methods A non-randomized controlled study was conducted, and type 2 diabetes patients managed in the community were divided into an intervention group of 112 cases and a control group of 110 cases. The control group received routine medication guidance during general practice outpatient visits, while the intervention group received comprehensive pharmacy outpatient service intervention based on routine medication guidance in general practice. Follow-up visits were conducted every 3 months. Repeated measurement analysis of variance and multivariate linear regression analysis were used to evaluate the intervention effect of the pharmacy outpatient service. Results Fasting blood glucose and glycosylated hemoglobin levels in the intervention group showed a decreasing trend with the increase of intervention time compared to pre-intervention time (P<0.01), with increased duration of weekly exercise, decreased staple food intake, increased vegetable intake, and increased medication adherence score (P<0.01). After adjusting for confounding factors through multivariate linear regression model, pharmacy outpatient intervention was found to be an independent protective factor for fasting blood glucose level (β=−0.891, P<0.01) and glycosylated hemoglobin level (β=−0.760, P<0.01) in the study subjects. Conclusion The community pharmacy outpatient service could enhance the self-management ability of patients with type 2 diabetes mellitus, and effectively improve patients’ fasting blood glucose and glycosylated hemoglobin.
3.Sedentary behavior among children and adolescents with intellectual disabilities in Jinan City

Objective:
To understand the sedentary behavior level of children and adolescents with intellectual disabilities in Jinan City, and to provide a reference basis for developing health behavior intervention strategies.
Methods:
By used the method of cluster random sampling,the Children s Leisure Activities Study Survey was used to investigate the sedentary behavior level of 285 children and adolescents with intellectual disabilities aged 6-18 years from 7 special education schools in Jinan City.
Results:
The sedentary behavior time during the whole week among children and adolescents with intellectual disabilities in Jinan City was 394.46 min/d, including 378.00 min/d on weekdays(Monday to Friday) and 388.80 min/d on weekends (Saturday and Sunday), the difference was statistically significant ( Z =-2.19, P <0.05). 80.4%(229) of children and adolescents with intellectual disabilities had sedentary behavior time of more than 2 h/d. The sedentary behavior time per day during the whole week among children and adolescents with intellectual disabilities was negatively correlated with the amount of time spent in moderate vigorous physical activity among them ( r =-0.16, P <0.05).
Conclusion
Excessive sedentary behavior has become a growing public health concern among children and adolescents with intellectual disabilities,which warrants targeted healthy behavior intervention.
4.Advances of Angiopoietin-Tie axis in vascular and lymphatic system-related diseases.

Endothelial cells that form the inner layers of both blood and lymphatic vessels are important components of the vascular system and are involved in the pathogenesis of vascular and lymphatic diseases. Angiopoietin (Ang)-Tie axis in endothelial cells is the second endothelium-specific ligand-receptor signaling system necessary for embryonic cardiovascular and lymphatic development in addition to the vascular endothelial growth factor receptor pathway. The Ang-Tie axis also maintains vascular homeostasis by regulating postnatal angiogenesis, vessel remodeling, vascular permeability, and inflammation. Therefore, the dysfunction of this system leads to many vascular and lymphatic diseases. In light of the recent advances on the role of the Ang-Tie axis in vascular and lymphatic system-related diseases, this review summarizes the functions of the Ang-Tie axis in inflammation-induced vascular permeability, vascular remodeling, ocular angiogenesis, shear stress response, atherosclerosis, tumor angiogenesis, and metastasis. Moreover, this review summarizes the relevant therapeutic antibodies, recombinant proteins, and small molecular drugs associated with the Ang-Tie axis.

5. Teaching clinic for training of general practitioners in suburban of a Shanghai community

In order to improve the quality of clinical teaching, Xidu Community Health Service Center of Shanghai Fengxian District established a teaching clinic for training of general practitioners in 2015. The clinic served as a suburban teaching station of Department of General Practice of Zhongshan Hospital, a tertiary university hospital. This article introduces the implementation methods, achievements, practical experiences and work prospects of the clinic. The issues of system evaluation and the generalization of this teaching model for training of general practitioners are also discussed in the article.
6. One family with osteosclerosis caused by D111Y mutation in the low-density lipoprotein receptor-related protein 5 gene

Objective:
To investigate the clinical features and pathogenic genes of a family with osteosclerosis.
Methods:
Six patients and six family members from a family in Jiangsu were tested for biochemical parameters, bone metabolic markers, bone mineral density, thoracolumbar anterior lateral slices, skull positive lateral radiographs, and pelvic plain films. Meanwhile, Sanger sequencing was performed to detect gene mutations of the proband and five other family members with high bone mass. The conformation of the mutational low-density lipoprotein receptor-related protein 5 (LRP5) protein was predicted by SWISS-MODEL.
Results:
Four adult patients (one male and three females) were tall, with mandibular enlargement and kyphosis in the center of the lower jaw, and none of the four had fractures. Their X ray examination revealed that the skull and long bone cortex was thickened, while the sella and mandible was enlarged. In addition, the absolute values of bone mineral density at each site of all patients were significantly higher as compared with the standard age- and sex-matched adults or adolescent mean reference values, with Z scores of L2-4, femoral neck and total hip being (6.31±4.03) SD, (6.56±2.36) SD, and (7.19±2.03) SD, respectively. The results of genetic sequencing revealed that all six patients carried a heterozygous mutation (c.331G>T; D111Y) in exon 2 of LRP5 gene, while other family members showed wild type (c.331G>G; D111D). Functional prediction indicated that this mutation was located at the amino acid terminal of exon 2 of LRP5 gene, which encodes the first β-helix-generating region of LRP5 protein.
Conclusion
The D111Y mutation in LRP5 gene leads to a clinical phenotype characterized by benign increased bone mineral density without increasing the risk of fracture. This mutation may further affect the downstream Wnt signaling pathway by altering the spatial structure of LRP5 protein, thereby promoting maturation and differentiation of osteoblasts and resulting in osteosclerosis.
7.Application of mixed assessment of competence of general practitioners in Shanghai suburban communities

A pilot mixed assessment method was used to evaluate the clinical and teaching ability of general practitioners (GPs) in a suburban district of Shanghai. In December 2018, 29 GPs from 20 community health centers in a suburban district of Shanghai were assessed by a mixed method consisting of 3 domains and 8 stations for one day. The average age of GPs was (37.8±4.7) years and 22 were females (75.9%). In 3 domains, the average score (66.8±9.7) and qualified rate (75.9%) of clinical thinking abilities were the lowest. In 8 stations, the average score [(53.8+13.4) points] and qualified rate (34.5%) of adult SOAP medical record writing station were the lowest. The average score of pediatric outpatients consult station of GPs with<15 years of community work experience was higher than that of GPs with >15 years of work experience [(68.6+10.2) vs. (58.5+9.4), t=2.787, P<0.05]. There was no significant difference in the scores among GPs with different teaching years and titles. The average scores of clinical thinking ability domain ( t=2.115) and adult outpatient consult station ( t=3.410); in the clinical operation ability domain the ophthalmoscopy and readings ( t=3.816) and otoscopy and reporting station ( t=2.286); clinical teaching abilities ( t=2.618) and simulated educational situation station ( t=2.452) and mini lectures station ( t=2.802) of GPs in the community teaching base were higher than those in non-community teaching base (all P<0.05). Mixed assessment method can be used as one of the important means for GPs′ ability assessment. The community teaching bases have effectively improved the clinical and teaching abilities of GPs in suburban areas, however, the clinical thinking abilities of them are still weak.
8.Application of case combined with Micro-lecture teaching in perioperative nursing teaching among gastrointestinal cancer patients

Objective? To explore the effects of case combined with Micro-lecture teaching in perioperative nursing teaching among gastrointestinal cancer patients. Methods? From January 2015 to December 2018, we selected 128 nursing students with practice in the Hangzhou First People's Hospital as subjects by convenience sampling. Nursing students with practice from January 2015 to December 2016 were in control group (n=64). A total of 64 practice nursing students from January 2017 to December 2018 were in observation group. Control group adopted case teaching, while observation group carried out case combined with Micro-lecture teaching. The teaching effects were assessed with the Self-Rating Scale for Self-Directedness in Learning (SRSSDL) and the Chinese version of the Competency Inventory for Nursing Students (CINS). Results? After teaching,the total scores of SRSSDL and CINS as well as dimension scores of observation group were higher than those in control group with statistical differences (P< 0.05). Conclusions? Case combined with Micro-lecture teaching can improve self-directedness learning ability and core competency of practice nursing students to guarantee the nursing teaching quality.
9.Effect of negative leadership behavior in nursing management on the nurses' sense of professional mission and patient satisfaction

[Abstract]? Objective? To investigate the current situation of negative leadership behavior in hospital nursing management, analyze the relationship and influence between negative leadership behavior and nurses' sense of professional mission and patients' satisfaction, and provide theoretical basis for reducing negative leadership behavior in nursing management and improving nurses' sense of professional mission and nursing quality. Methods? From January 2017 to July 2018, 260 clinical nurses from three hospitals in Hangzhou were selected by convenience sampling method. General Information Questionnaire, Negative Leadership Scale, Professional Mission Questionnaire and Nursing Job Satisfaction Scale were used in the questionnaire survey. The negative leadership behavior in hospital nursing management and its influence on nurses' sense of professional mission and patients' satisfaction were analyzed. A total of 271 questionnaires were distributed to the nursing staff and 260 valid questionnaires were retrieved. Results? Among the 260 nurses surveyed, 22.69% encountered autocratic and selfish leadership, 30.77% encountered unfaithful leadership, 55.00% experienced constructive leadership, and the total average score of items in Negative Leadership Scale was (1.58±0.76). Autocratic and selfish leadership behavior was negatively correlated with nurses' sense of professional mission and nursing quality (P< 0.01), unfaithful leadership behavior was negatively correlated with nurses' sense of professional mission and nursing quality (P< 0.01), and constructive leadership behavior was positively correlated with nurses' sense of professional mission and nursing quality (P<0.01). Conclusions? There are some arbitrary, selfish and unfaithful negative leadership behaviors in hospital management, which have a negative impact on nurses' sense of professional mission and patients' satisfaction. Effective measures should be taken to intervene.
10.Application of OSTE in teaching competence assessment for general practice preceptors in community training bases

Objective To apply objective structured teaching evaluation (OSTE) in teaching competence assessment for general practice preceptors in community training bases.Methods The teaching competency was assessed with OSTE for 28 general practice preceptors in Fengxian District of Shanghai in December 2017.The application of OSTE was evaluated with questionnaire among preceptors,examiners and standardized students(SS).Results Among 28 preceptors there were 23 females (82.1%),with an average age of (37.3±4.2) years.The average teaching duration was (2.8±2.1) years.In the five OSTE station,the highest total score was (88.1 ±2.7) points in the first station self-presentation,while the lowest was (70.8± 14.5) points of the second part physical examination feedback of the third station ambulatory care teaching.The total score in the fifth station (doctor-patient communication teaching) of participants with< 15 years of community practice was higher than those ≥15 years of community practice [(80.1±8.5) vs.(71.6± 7.1),t=2.092,P=0.007].Except"being able to adapt to this form of evaluation",the proportions of choosing "very good"were all more than 50% in other 4 aspects.The overall willingness of "recommending OSTE as the main method to evaluate the teaching competence of GP trainers" was higher in examiners and SS than that inpreceptors (Z=2.836,P=0.005).Conclusion The innovative approach of OSTE has been widely recognized,but the professional capability,the cognition of general practice,the teaching competence and the willingness to use OSTE need to be strengthened for general practice receptors in community bases.
