1.Application value of ultrasound-guided multimodal examinations in the diagnosis of lymph node mycobacterial infection

Objective:To investigate the application value of ultrasound-guided multimodal examinations in the diagnosis of lymph node mycobacterial infection.Methods:The clinical data of 42 patients with suspected lymph node mycobacterial infection who were initially diagnosed at the Affiliated Hospital of Shaoxing University from January 2019 to December 2020 were retrospectively analyzed. All patients underwent an ultrasound-guided lymph node-negative pressure puncture. Acid-fast staining, bacterial culture, pathological examination or their combination were used to screen lymph nodes for mycobacterial infection. The results were compared with those of acid-fast staining and bacterial culture of sputum and bronchoalveolar lavage fluid smears.Results:The combined application of acid fast staining, bacterial culture, and pathological examination for the puncture fluid smear showed a positive rate of 71.4% (30/42), which was significantly higher than the positive rate [26.2% (11/42)] for acid fast staining of the puncture fluid smear, the positive rate [42.9% (18/42)] for bacterial culture of the puncture fluid, and the positive rate [50.0% (21/42)] of pathological examination ( χ2 = 17.20, 7.00, 4.04, P < 0.001, P < 0.01, P = 0.040). The positive rate for sputum smear and bacterial culture was 21.4% (9/33). The positive rate for acid fast staining and bacterial culture of the bronchoalveolar lavage fluid was 28.6% (12/30). The differences were statistically significant ( χ2 = 21.11, 15.43, both P < 0.001). Conclusion:Ultrasound-guided negative pressure aspiration and puncture biopsy of lymph nodes combined with acid fast staining, bacterial culture, and pathological examinations can markedly increase the detection rate and diagnostic rate of mycobacterial infection.

The National Medical Center for Major Public Health Events invited experts in fields of infectious diseases, respiratory medicine, intensive care, pediatrics and fever clinic to develop this quick guideline based on the current best evidence and extensive clinical practices. When does the population infection rate rise to its peak? What will eventually be the number of infected patients? How to make early diagnosis? What effective antiviral drugs are available? How to effectively treat with existing drugs? Can it successfully improve the survival rate of critically patients? In response to the above questions, we put forward corresponding suggestions and reflections from the perspective of the infectious clinician.

6.Effects of Assertive Community and Family Treatment on Family Environment and Compliance of Patients with Schizophrenia

Objective To explore the effects of assertive community and family treatment on family environment and compliance of patients with schizophrenia. Methods A total of 122 patients with schizophrenia discharged from January, 2014 to September, 2015 along with their families were enrolled and divided into intervention group (n=61) and control group (n=61). The intervention group received comprehensive intervention including family treatment by a professional team in the assertive community treatment model, while the control group received routine follow-up, for twelve months. They were evaluated with Positive and Negative Symptoms Scale (PANSS), Insight and Treatment Attitude Questionnaire (ITAQ), Family Environment Scale-Chinese Version (FES-CV), duration of pharmaceutical treatment and relapse rate before, and six and twelve months after intervention. Results There were statistically significant differences between the intervention group and the control group in total score of PANSS, score of ITAQ and FES-CV six and twelve months after intervention (FGroup>1.760, P<0.05); 16 patients in the control group and seven patients in the intervention group relapsed during the observation period (χ2=4.340, P=0.037). Kaplan-Meier survival analysis showed statistically significant difference between the two groups in duration of pharmaceutical treatment (χ2=6.338, P=0.012). Conclusion Assertive community and family treatment can improve intimacy between patients with schizophrenia and their family members, reduce high emotional expression, improve medication compliance, and reduce relapse.
7.Cell-in-Cell Death Is Not Restricted by Caspase-3 Deficiency in MCF-7 Cells.

PURPOSE: Cell-in-cell structures are created by one living cell entering another homotypic or heterotypic living cell, which usually leads to the death of the internalized cell, specifically through caspase-dependent cell death (emperitosis) or lysosome-dependent cell death (entosis). Although entosis has attracted great attention, its occurrence is controversial, because one cell line used in its study (MCF-7) is deficient in caspase-3. METHODS: We investigated this issue using MCF-7 and A431 cell lines, which often display cell-in-cell invasion, and have different levels of caspase-3 expression. Cell-in-cell death morphology, microstructures, and signaling pathways were compared in the two cell lines. RESULTS: Our results confirmed that MCF-7 cells are caspase-3 deficient with a partial deletion in the CASP-3 gene. These cells underwent cell death that lacked typical apoptotic properties after staurosporine treatment, whereas caspase-3-sufficient A431 cells displayed typical apoptosis. The presence of caspase-3 was related neither to the lysosome-dependent nor to the caspase-dependent cell-in-cell death pathway. However, the existence of caspase-3 was associated with a switch from lysosome-dependent cell-in-cell death to the apoptotic cell-in-cell death pathway during entosis. Moreover, cellular hypoxia, mitochondrial swelling, release of cytochrome C, and autophagy were observed in internalized cells during entosis. CONCLUSION: The occurrence of caspase-independent entosis is not a cell-specific process. In addition, entosis actually represents a cellular self-repair system, functioning through autophagy, to degrade damaged mitochondria resulting from cellular hypoxia in cell-in-cell structures. However, sustained autophagy-associated signal activation, without reduction in cellular hypoxia, eventually leads to lysosome-dependent intracellular cell death.

8.Expression of IMP3 and its clinicopathological significance in breast cancer

ObjectiveTo study the expression of insulin-like growth factor Ⅱ mRNA-binding protein 3 ( IMP3 ) and its clinicopathological significance in breast cancer.MethodsThe Maxvision immunohistochemistry was used to detect the expression of IMP3 protein in 103 cases of breast cancer and the adjacent normal breast tissues.Stem-loop real-time RT-PCR was used to detect the expression level of IMP3 mRNA in 30 cases of breast cancer and the matched non-tumor adjacent tissues.The relationship between the expression of IMP3 and the clinicopathological features of breast cancer was analyzed.Results62 cases were infiltrative ductal carcinoma,3cases were invasive lobular carcinoma,9 cases were microinfiltrative intraductal carcinoma,and 29 cases were special type of carcinoma.The difference of IMP3 expression between breast cancer group and normal breast tissue group had statistical significance( t =19.630,P =0.000)by t test.For infiltrative ductal carcinoma patients,IMP3 expression had no significant relation with age,tumor size,lymph node metastasis,histological grade,pTNM stage,ER,PR,or HER-2 ( P > 0.05 ).IMP3 mRNA expression level of was significantly higher in tumor tissues than in the adjacent normal tissues (P < 0.05 ).No significant association was found between the expression of IMP3 and the histological grade,tumor size,lymph node metastasis ( P > 0.05 ).ConclusionIt is possible that IMP3 plays an important role in the generation,progression,invasion and metastasis of breast cancer.
9.The effects of physical exercises on recurrent seizures-induced long-term cognitive deficit and zinc transporter-3 expression in the hippocampus of developmental period rats

Objective To explore the effects of physical exercises on recurrent seizures-induced long-term learning and memory deficits and on the expression of zinc transporter-3(ZnT-3)in hippocampuses of developmental period rats.Methods Sprague-Dawley rats aged 21 d were randomly divided into blank control group,exercises control group,seizure group and seizure plus exercises group.Abdominal cavity injections of penicillin or saline were used to induce recurrent seizure or as a control in the corresponding groups.During the postnatal(P)39-44 d and P61-65 d periods,Morris water-maze tests were administered to evaluate spatial learning and memory capacity.During the P48-53 d period,the rats in exercises control and seizure plus exercises groups were subjected to a 30-min daily aerobic exercises program for 6 d.The real-time reverse transcription-polymerase chain reaction(RT-PCR)method was used to detect the expression of ZuT-3 mRNA in hippocampuses of all rats at P66 d. Results Searching strategy:There was a decreasing incidence of marginal strategy and an increasing trend in the use of taxis and straight line strategy in all four groups.The scores on d 1 and 4 were significantly higher in two control groups than in two seizure groups in water-maze test(all P<0.05).By d 2 the exercises control and seizure plus exercises groups were scoring significantly higher than the bland control and seizure groups(P<0.05).Memory test:The frequency of passing through the platform quadrant decreased significantly in the two seizure groups compared to the two control groups in both probe tests(all P<0.05).RT-PCR test:ZnT-3 mRNA expressions in hippocampuses were significantly higher in seizure plus exercises group than in any other groups. Conclusions Penicillin-induced recurrent seizures can induce long-term damage on learning and memory capacity in developmerital period rats.Physical exercises can improve learning capacity.It's mechanism may be related to the up-regulation of ZnT-3 expression in hippocampus of rats.
