1.Research progress in perioperative immunotherapy for non-small cell lung cancer

It is very limited that the benefit of perioperative chemotherapy in early non-small cell lung cancer (NSCLC), and the 5-year survival rate is only 5% higher than surgery. Antibodies that block programmed cell death protein 1/programmed death-ligand 1 significantly improve the survival of advanced NSCLC. The value of immunotherapy in early NSCLC is also being explored. This paper firstly summarized and analyzed the progress of immunotherapy in the perioperative period of NSCLC. Secondly, the safety and feasibility of surgical resection after neoadjuvant immunotherapy were discussed. Finally, the clinical value of different therapeutic efficacy prediction indicators was summarized, in order to clarify the current status of immunotherapy in the perioperative period, so as to improve the clinical benefits of early NSCLC patients.
2.Analysis of left semiprone position in 3D laparoscopic liver resection for tumor in segment Ⅶ

Objective:To study the feasibility and safety of left semiprone position in 3D laparoscopic liver resection for tumor in segment Ⅶ.Methods:Clinical data of 60 patients with liver tumors treated at Jinhua Central Hospita from January 2018 to September 2022 were retrospectively collected, including 40 males and 20 females, aged (51.5±4.5) years. Perioperative data on operative time, intraoperative blood loss, postoperative time of ambulation, postoperative time to flatulate, duration of hospital stay, pathological type, complications (bile leakage, pleural effusion, etc.) were analyzed. Postoperative follow-ups were conducted through telephone reviews, outpatient visits.Results:Of 57 patients (95.0%) successfully underwent the surgery, while three cases (5.0%) were converted to open surgery due to excessive intraoperative bleeding. The operative time was (210.5±20.5) min. The intraoperative blood loss was (220.5±50.5) ml. The postoperative time of ambulation was (1.5±0.5) d, and the postoperative time to flatulate was (2.0±0.8) d. The postoperative hospital stay was (7.2±1.5) d. Bile leakage occurred in four cases (6.7%) after surgery, which was managed by conservative treatment, and pleural effusion occurred in eight cases (13.3%), which recovered after puncture and drainage. Postoperative pathology included 40 cases (66.7%) of hepatocellular carcinoma, three cases (5.0%) of intrahepatic cholangiocarcinoma, five cases (8.3%) of liver metastases, seven cases (11.7%) of hepatic hemangioma, two case (3.3%) of angiomyolipoma, and three case (5.0%) of focal hepatic nodular hyperplasia. No tumor recurrence was observed during follow-up (median 26 months; range 18-40 months).Conclusion:The application of left semiprone position in 3D laparoscopic liver resection for tumor in segment Ⅶ is safe and feasible.
3.A qualitative study on elderly inpatients' experiences with intelligent nursing services

Objective To analyze and understand the experiences and feelings of elderly inpatients during the use of intelligent nursing services,providing a scientific basis for the development of intelligent nursing models in intelligent ward systems.Methods Adopting the phenomenological research method,purposive sampling was used to select 14 elderly inpatients from a tertiary hospital in Beijing between October and December 2024 for semi-structured interviews.Thematic analysis was applied for data analysis.Results The experiences of elderly inpatients with intelligent nursing services were mainly summarized into 3 themes,including the dual track of acceptance challenges and convenience perception of intelligent nursing services,the integration of aging-appropriate innovation and humanistic care in intelligent nursing services,and the health orientation and privacy security concerns of intelligent nursing services.Conclusion Elderly inpatients can generally recognize the utility of intelligent nursing services in enhancing convenience and their health,but they still face challenges,and have clear aging-appropriate needs,while expressing concerns about privacy security.Managers and healthcare professionals need to design multi-faceted aging-appropriate innovations or intervention strategies to expand the accessibility of intelligent nursing,ensuring that elderly patients can truly benefit from these.
4.Intervention effect of nicotinamide mononucleotide on subacute benzene exposure-induced hematotoxicity in mice

Objective To assess the intervention effect of nicotinamide mononucleotide (NMN) on the mouse model of hematotoxicity induced by subacute benzene exposure. Methods Benzene exposure and NMN intervention were adopted in a 2×2 factorial design, as benzene exposure and non-exposure, and NMN intervention and non-intervention. Male specific pathogen-free C57BL/6J mice were randomly assigned to negative control group, NMN control group, simple benzene exposure group and NMN intervention group, with 12 mice in each group. Benzene exposure of mice in simple benzene exposure group and NMN intervention group was conducted by dynamic inhalation of benzene at a concentration of 325 mg/m³ for six hours per day, five days per week for four weeks (28 days). Mice in the negative control and NMN control group inhaled clean air. During benzene exposure, mice in the NMN control group and NMN intervention group received NMN in drinking water at a dose of 300 mg/kg body weight. Peripheral blood samples of mice were collected for complete blood count analysis and calculation of composite inflammatory indices after 28 days. Results Interaction analysis showed that the counts of peripheral white blood cell, neutrophil, lymphocyte, and platelet of mice in the simple benzene exposure group were lower than those in the negative control group (all P<0.05). Neutrophil and platelet counts in the NMN intervention group were higher than those in the simple benzene exposure group (all P<0.05). The results of main effect analysis showed that the monocyte count of peripheral blood, systemic inflammatory index, systemic inflammatory response index, neutrophil/lymphocyte ratio and platelet/lymphocyte ratio of mice in the benzene exposure group increased (all P<0.05), and the basophil count and lymphocyte/monocyte ratio decreased (all P<0.05), compared with the control group. Conclusion Oral NMN alleviates subacute benzene-induced decreases in peripheral neutrophil and platelet counts in mice. This protective effect may be related to the targeted intervention of NMN on mitochondrial energy metabolism disorder and oxidative damage induced by benzene exposure in male mice.
5.Repurposing drugs for the human dopamine transporter through WHALES descriptors-based virtual screening and bioactivity evaluation

Computational approaches,encompassing both physics-based and machine learning(ML)methodolo-gies,have gained substantial traction in drug repurposing efforts targeting specific therapeutic entities.The human dopamine(DA)transporter(hDAT)is the primary therapeutic target of numerous psychi-atric medications.However,traditional hDAT-targeting drugs,which interact with the primary binding site,encounter significant limitations,including addictive potential and stimulant effects.In this study,we propose an integrated workflow combining virtual screening based on weighted holistic atom localization and entity shape(WHALES)descriptors with in vitro experimental validation to repurpose novel hDAT-targeting drugs.Initially,WHALES descriptors facilitated a similarity search,employing four benztropine-like atypical inhibitors known to bind hDAT's allosteric site as templates.Consequently,from a compound library of 4,921 marketed and clinically tested drugs,we identified 27 candidate atypical inhibitors.Subsequently,ADMETlab was employed to predict the pharmacokinetic and toxi-cological properties of these candidates,while induced-fit docking(IFD)was performed to estimate their binding affinities.Six compounds were selected for in vitro assessments of neurotransmitter re-uptake inhibitory activities.Among these,three exhibited significant inhibitory potency,with half maximal inhibitory concentration(IC50)values of 0.753 μM,0.542 μM,and 1.210 μM,respectively.Finally,molecular dynamics(MD)simulations and end-point binding free energy analyses were con-ducted to elucidate and confirm the inhibitory mechanisms of the repurposed drugs against hDAT in its inward-open conformation.In conclusion,our study not only identifies promising active compounds as potential atypical inhibitors for novel therapeutic drug development targeting hDAT but also validates the effectiveness of our integrated computational and experimental workflow for drug repurposing.

8.Clinical characteristics of premature rupture of membranes complicated with histological chorioamnionitis

Objective To analyze the clinical features of premature rupture of membrane(PROM)complicated with histologic chorioamnionitis(HCA),and to provide insights for clinical management of such pregnant women and their newborns.Methods A total of 202 cases of PROM pregnant women who gave birth and performed placental pathological examination in the Affiliated Hospital of Southwest Medical University from January to December 2021 were retrospectively analyzed.According to the histopathological results,the pregnant women were divided into HCA group(n=151)and control group(n=51),and the prenatal factors and short-term maternal-fetal pregnancy outcomes of two groups were compared and analyzed.Results The amniotic fluid contamination rate,white blood cell count,neutrophil count and serum amyloid A levels in HCA group were significantly higher than those in control group(P<0.05).There was a higher incidence of early neonatal septicemia in HCA group than in control group(P<0.05).Conclusion The risk of early-onset sepsis in neonates with PROM complicated with HCA significantly increases.It is necessary to strengthen the screening and early intervention in such cases.Amniotic fluid contamination and increased maternal infection indicators are associated with the occurrence of HCA,which may has potential value in predicting HCA occurrence.
9.Therapeutic effect of Xuanfei Zhisou Mixture combined with ceftriaxone sodium in treatment of cough after infection

OBJECTIVE To observe the therapeutic effect of Xuanfei Zhisou Mixture combined with ceftriaxone so-dium in treating post-infectious cough with wind-evil invading lung syndrome and its effect on Toll-like receptor 4(TLR4)/nuclear factor-KB(NF-κB)signaling pathway.METHODS The clinical data of 100 patients with post-in-fectious cough admitted to Wuhan Hospital of Integrated Traditional Chinese and Western Medicine from Dec.2021 to Dec.2022 were retrospectively analyzed.According to different therapeutic schemes,the patients were di-vided into two groups:48 cases in the single-drug group and 52 cases in the combined group.The single-drug group was treated with ceftriaxone sodium,while the combined group was treated with Xuanfei Zhisou Mixture com-bined with ceftriaxone sodium.The total effective rate,traditional chinese medicine(TCM)symptom score,leicester cough quality of life questionnaire(LCQ),serum levels of inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and inducible nitric oxide synthase(iNOS)]and TLR4/NF-κB signaling pathway expression were analysed to evaluate the therapeutic effect.RESULTS After treatment,the total effective rate of the combined group was higher than that of the single-drug group(94.23%vs.77.08%,x2=6.096,P=0.014);meanwhile,compared to the single-drug group,the TCM symptom score,TNF-α,IL-6,iNOS,TLR4,NF-κB levels,TLR4 protein and NF-κB protein expression in the combined group were significantly decreased(P<0.05),and the LCQ score was significantly increased(P<0.05).CONCLUSIONS Xuanfei Zhisou Mixture com-bined with ceftriaxone sodium has an obvious effect in treating post-infectious cough with wind-evil invading lung syndrome.They can effectively improve the cough symptoms and quality of life of patients and reduce the produc-tion of inflammatory factors by inhibiting TLR4/NF-κB signaling pathway.

To formulate an expert consensus on the principles of preoperative hair removal and provide scientific guidance for standardized removal of hair before surgical procedures so as to reduce the incidence of surgical site infections.METHODS Led by the Hospital Management Institute of National Health Commission of the People's Republic of China,this consensus was reached with the joint efforts from the expects of relevant fields such as surgeries,interventional therapies,nursing,and infection prevention and control.The consensus facilitates the classification and evaluation of literatures by following the evidence grade formulated by Oxford Evidence-based Medicine Center and focuses on the association of preoperative hair removal with surgical site infection,it reaches the evidence grade of expert consensus and recommendation intensity by integrating with discussions on meetings and clinical experience of the expects from relevant fields.RESULTS A total of 6 items of consensus were reached by summarizing the latest evidence on the aspects including the indications for preoperative hair removal,tools,range,timing and places.CONCLUSION The consensus,to some extent,make supplements to and complete the exiting regulations and standards.It provides guidance for the medical institutions to carry out the preoperative hair removal.
