1.Analysis of patent application status of key technology in capsule endoscopy industry

The patent data in the key technology field of capsule endoscopy industry was retrieved in IncoPat Global Patent Database from January 1,2003 to December 31,2022,and the development trend of key technologies and patent competition in the global capsule endoscopy industry were analyzed in terms of the applicant,concentration of China's patent applications and regional layout.It's pointed out Japan's Olympus company gained advantages in the key technology field,the enterprise played a main role in the innovation of the key technology field,the main exporters and target markets included the United States,South Korea,Japan and China and China became more and more important for the innovation of the key technology field.References were provided for the technology development of capsule endoscopy industry in China.[Chinese Medical Equipment Journal,2025,46(5):60-65]
2.Experimental study on treatment of relapsed and refractory acute myeloid leukemia with DNA methyltransferase 1 inhibitor combined with extracellular signal-regulated kinase 1,homeodomain-interacting protein kinase 2,and glycogen synthase kinase 3β inhibitors

Objective To investigate the mechanism of DNA methyltransferase 1(DNMT1)inhibitor combined with extracellular signal-regulated kinase 1(ERK1),homeodomain-interacting protein kinase 2(HIPK2),and glycogen synthase kinase 3 β(GSK3 β)inhibitors in synergistically inducing apoptosis and protein arrest in relapsed and refractory acute myeloid leukemia(AML)cells.Methods Three therapeutic targets,including ERK1,HIPK2,and GSK3β,were screened based on the Gene Expression Omnibus(GEO),The Cancer Genome Atlas(TCGA)database,and Ex-pression 2 Kinases database.Human AML cell line U937 cells were treated with DNMT1 inhibitor a-lone or combined with ERK1,HIPK2,or GSK3β inhibitors.Cell viability was detected using the CCK-8 method.Apoptosis rate was analyzed by flow cytometry,and cell cycle distribution was de-termined by propidium iodide(PI)staining.The mRNA expression levels of DNMT1,ERK1,HIPK2,and GSK3β were detected by real-time fluorescent quantitative reverse transcriptionpoly-merase chain reaction(RT-qPCR).Protein expressionlevels of DNMT1,ERK1,HIPK2,and GSK3β were detected by immunoblotting.Results DNMT1 inhibitor significantly inhibited the cell viability of U937 cells,and significantly induced apoptosis and cell cycle arrest in U937 cells(P<0.05).When DNMT1 inhibitor was combined with ERK1,HIPK2,or GSK3β inhibitors,cell via-bility and apoptosis rate were significantly reduced(P<0.05).DNMT1 inhibitor alone or its com-bination with ERK1,HIPK2,and GSK3β inhibitors induced U937 cell arrest in the G0/G,phase,with a significant increase in the proportion of cells in the G0/G1 phase in the combination group(P<0.05).The combination of DNMT1 inhibitor with ERK1,HIPK2,and GSK3β inhibitors sig-nificantly reduced the mRNA expression levels in DNMT1,ERK1,HIPK2,and GSK3β in U937 cells(P<0.05).Similarly,the combination therapy significantly reduced the protein expression levels of DNMT1,ERK1,HIPK2,and GSK3β in U937 cells(P<0.05).Conclusion DNMT1 inhibitor combined with ERK1,HIPK2,and GSK3 β inhibitors can synergistically induce apoptosis and protein arrest in relapsed and refractory AML cells,providing a novel strategy for combined tar-geted therapy of AML.
3.Modified Chevron osteotomy for moderate and severe hallux valgus deformity with enlarged distal metatarsal articular angle

Objective:To evaluate the radiological and functional outcomes of moderate to severe hallux valgus patients with enlarged distal metatarsal articular angle (DMAA) underwent modified Chevron osteotomy.Methods:The clinical data of patients with moderate and severe hallux valgus with increased distal metatarsal joint angle who accepted surgery operation in the Department of Foot and Ankle Surgery of Yantaishan Hospital from October 2020 to December 2022 were retrospectively analyzed. All patients underwent modified Chevron osteotomy. Taking the proximal end of the metatarsal head centre as the osteotomy apex, the vertical arm osteotomy line in the sagittal plane made an angle of ≤80° with the metatarsal stem, the horizontal plane was inclined to the lateral distal end of the metatarsal head by about 10°, and the sagittal plane metatarsal arm osteotomy line made an angle of ≥90° with the vertical arm osteotomy line; at the proximal osteotomy surface, another cuneiform bone with its base on the medial and its apex on the lateral was resected. The deformity correction was insufficient and Akin osteotomy was performed in combination. Weil osteotomy was performed in combination with metatarsalgia. Radiological assessment including the hallux valgus angle (HVA), intermetatarsal angle (IMA), DMAA, the joint congruity angle (JCA), forefoot bone width and soft tissue width was performed preoperatively and at last follow-up postoperatively. American Orthopedic Foot and Ankle Society/hallux metatarsophalangeal-interphalangeal scale (AOFAS/HMIS) was used for clinical and functional evaluation, total score from 0 to 100, with higher scores indicating better function.Visual analogue scale (VAS) was used for pain valuation, total score from 0 to 10, with higher scores indicating more pain. A questionnaire survey on patient satisfaction was conducted at the last follow-up. Shapiro-Wilk test was used for normal distribution test, and measurement data following normal distribution were represented as Mean±SD. Paired t-test was used for comparison before and after operation. Other indicators conformed to non-normal distribution were denoted by M( Q1, Q3) and were tested by Wilcoxon signed-rank test. P<0.05 was considered statistically significant. Results:Fifty-two feet of 48 patients (5 males, 43 females; mean age (52.4±14.9) years; range, 24 to 78 years) were enrolled. Before the operation, 8 feet combined with metatarsalgia, among them, 7 feet underwent modified Chevron+ Akin+ Weil osteotomy, and 1 foot underwent modified Chevron+ Weil osteotomy. Among the 44 feet without metatarsalgia, 11 feet underwent modified Chevron osteotomy and 33 feet underwent modified Chevron+ Akin osteotomy. The mean follow-up time was 17.8 months (12-24 months). The HVA angle decreased from 38.30°±7.59° before surgery to 10.00°±5.73° at the last follow-up; the IMA angle decreased from 16.08°(12.89°, 18.24°) to 4.81°(3.62°, 7.57°); the DMAA angle decreased from 18.35°(13.03°, 27.47°) to 4.52°(2.68°, 7.09°); JCA decreased significantly from 15.93°(10.25°, 23.06°) to 3.56°(1.71°, 6.98°); forefoot bone width decreased from (90.05±6.12) mm to (82.75±5.01) mm; forefoot soft tissue width decrease from 102.25(96.77, 107.15) mm to 98.08(91.01, 100.60) mm; the VAS decreased from 6(5.5, 7) points to 0(0, 0) points; the score according to the AOFAS/HMIS forefoot was increased from 49(42, 52.5) points to 90(83.5, 95) points; which were statistically significant compared with that before the operation (all P<0.01). There was no statistically significant difference in the first metatarsal length before the operation and at the last follow-up [54.60(52.86, 56.42) mm vs. 54.29(51.85, 56.35) mm, P>0.05]. In the post-operative period, there were 8 feet had limited metatarsophalangeal joint movement, 3 feet had limited interphalangeal joint movement, 5 feet had limited movement in both joints, which did not affect walking and function; 3 feet of partial recurrence of hallux valgus, 2 feet of screw irritation pain, 1 foot of cystic degeneration of the first metatarsal head, and no complications such as metastatic metatarsalgia. The satisfaction survey showed that the satisfaction rate of patients with the orthopedic effect was 90.4% (47/52). Conclusion:The modified Chevron osteotomy is effective in the treatment of moderate to severe hallux valgus with enlarged DMAA. Careful intraoperative operation and standardized postoperative rehabilitation training can reduce complications.

The circadian clock plays a critical role in regulating various physiological processes, including gene expression, metabolic regulation, immune response, and the sleep-wake cycle in living organisms. Post-translational modifications (PTMs) are crucial regulatory mechanisms to maintain the precise oscillation of the circadian clock. By modulating the stability, activity, cell localization and protein-protein interactions of core clock proteins, PTMs enable these proteins to respond dynamically to environmental and intracellular changes, thereby sustaining the periodic oscillations of the circadian clock. Different types of PTMs exert their effects through distincting molecular mechanisms, collectively ensuring the proper function of the circadian system. This review systematically summarized several major types of PTMs, including phosphorylation, acetylation, ubiquitination, SUMOylation and oxidative modification, and overviewed their roles in regulating the core clock proteins and the associated pathways, with the goals of providing a theoretical foundation for the deeper understanding of clock mechanisms and the treatment of diseases associated with circadian disruption.

6.Synthetic MRI for evaluating structural changes of brain nuclei and white matter in patients with Parkinson disease

Objective To investigate the value of synthetic MRI(syMRI)for evaluating structural changes of brain nuclei and white matter in patients with Parkinson disease(PD).Methods Forty-five PD patients(PD group)and 35 healthy controls(HC group)were prospectively enrolled.Head scanning with strategically acquired gradient echo sequence was performed,then syMRI was obtained with post-processing.The values of T1,proton density(PrD),R2* and magnetic sensitivity values(MSV)of nuclei and white matter were measured based on syMRI and compared between groups.The correlations of syMRI quantitative parameters of nuclei and white matter being significantly different between groups with neurological function scores of subject were analyzed,and a combined model was established using multivariate logistic regression analysis.Receiver operating characteristic(ROC)curve was drawn,the efficacy of single syMRI quantitative parameter and combined model for diagnosing PD was evaluated.Results Compared with HC group,T1 and PrD values of bilateral caudate nuclei decreased,while MSV of bilateral substatia nigra and right red nucleus increased(all FDR correction P<0.05),while T1 and PrD values of bilateral splenium of corpus callosum,PrD values of left posterior limbs of the internal capsule(PLIC),R2* values of bilateral PLIC and right genu of the corpus callosum decreased in PD group(all FDR correction P<0.05).syMRI quantitative parameters of the above brain regions were correlated with scores of Montreal cognitive assessment and Hamilton anxiety scale(r=-0.390-0.416,all P<0.01).The area under the curve of single syMRI quantitative parameter for diagnosing PD was 0.664-0.788,lower than that of combined model(0.905,Z=2.653-4.096,all P<0.05).Conclusion SyMRI could be used to quantitatively evaluate structural changes of brain nuclei and white matter in PD patients.

9.Efficacy of"ditching and ridge removal"with 450 nm semiconductor blue laser in the treatment of large volume benign prostatic hyperplasia

Objective To explore the efficacy and safety of"ditching and ridge removal"with 450 nm semiconductor blue laser in the treatment of large volume benign prostatic hyperplasia(BPH),in order to promote the clinical application of this method.Methods A retrospective study was conducted on 30 patients with large volume BPH treated with"ditching and ridge removal"with 450 nm semiconductor blue laser in Yingsheng Branch of Tai'an Central Hospital during Sep.and Dec.2023.The laser operation time,level of hemoglobin before and after operation,bladder irrigation time after operation,urinary catheter indwelling time,postoperative hospital stay,and intraoperative and postoperative complications were recorded.The changes of international prostate symptom score(IPSS),quality of life scale(QoL)score,maximum urinary flow rate(Qmax)and post-void residual volume(PVR)were compared before and 1 month after operation.Results The volume of prostate was(104.5±14.52)mL,the laser operation time was(20.13±2.98)min,and the bladder irrigation time was(20.27±2.56)h.The catheter was removed in all patients 2 days after operation,and all patients were discharged 3 days after operation.One month after operation,the IPSS,QoL,Qmax and PVR were significantly improved as compared with those before operation(P<0.05).No complications occurred during the follow-up.Conclusion"Ditching and ridge removal"with 450 nm semiconductor blue laser is a new,safe and effective method in the treatment of large volume BPH.
10.Laboratory proficiency testing for creepage distance and electrical clearance test of medical electrical equipment based on GB 9706.1-2020

Objective To clarify the understanding of types of laboratories and manufacturers for GB 9706.1-2020 Medical electrical equipment-Part 1:General requirements for basic safety and essential performance by laboratory proficiency testing for creepage distance and electrical clearance test.Methods An operation guide was formed according to the testing program in GB 9706.1-2020,and the homogeneity and stability of the samples were evaluated according to CNAS-GL003:2018 Guidance on Evaluating the Homogenneity and Stability of Samples Used for Proficiency Testing.Robust statistic methods were used to assess the quantitative parameters of the test results of the participating laboratories according to the requirements in GB/T 28043-2019 Statistical methods for use in proficiency testing by interlaboratory comparison;the results reported by the expert laboratories were used as the specified values of the qualitative parameters.SPSS 25.0 statistical software was used for data analysis.Results All the results of the crreepage distance and electrical clearance tests met the requirements for homogeneity and stability.Of the 46 laboratories involved in,37 ones did have comprehensive satisfactory determinations while the remained 9 ones not.Conclusion Some laboratories don't behave well in understanding the standard,which have to be reformed accordingly to enhance their proficiencies.[Chinese Medical Equipment Journal,2024,45(10):54-59]
