Objective: To characterize perioperative dynamic changes in immune-cell phenotypes and inflammatory cytokines in patients undergoing CPB (cardiopulmonary bypass) cardiac surgery, and to explore their associations with postoperative outcomes. Methods: In this prospective cohort study, 120 adult patients who underwent elective cardiac surgery under CPB at West China Hospital from May 2022 to March 2023 were enrolled. Perioperative immune-cell phenotypes and concentrations of 40 inflammation-related cytokines were measured. The primary outcomes were the sequential organ failure assessment (SOFA) score at 24 h after surgery and ΔSOFA (the peak SOFA score within 48 h after surgery minus the preoperative SOFA score). Secondary outcomes included major adverse cardiovascular events (MACE), acute kidney injury (AKI), respiratory failure, severe liver injury, and infection. Results: The mean age of enrolled patients was 57±10 years. Of these, 52% (62/120) were male and 90% (108/120) underwent valve surgery. During the rewarming to the end of CPB, neutrophil counts rapidly increased (7.39×10 /L vs preoperative 3.07×10 /L, P<0.001), with significant upregulation of CD11b (7.30×10 /L vs preoperative 3.05×10 /L, P<0.001) and CD54 (7.15×10 /L vs preoperative 2.99×10 /L, P<0.001). Lymphocyte counts increased at the end of CPB (1.75×10 /L vs preoperative 1.12×10 /L, P<0.001) but decreased significantly at 24 h after surgery (0.59×10 /L vs preoperative 1.12×10 /L, P<0.001). Plasma analysis showed that multiple pro-inflammatory cytokines increased during CPB and remained elevated up to 24 h after surgery; five chemokines and the anti-inflammatory cytokine IL-10 peaked at the end of CPB. The SOFA score increased from 1 (1, 2) preoperatively to 7 (5, 10) at 24 h after surgery, with a ΔSOFA of 6 (4, 8). Within 30 days after surgery, 48 patients (40.0%) developed AKI, 17 (14.2%) developed infection, 4 (3.3%) developed severe liver injury, 3 (2.5%) developed respiratory failure, and 3 (2.5%) experienced MACE. During the 2-year follow-up, 8 patients (6.7%) experienced MACE and 5 (4.2%) died. Conclusion: Multi-organ dysfunction is common after cardiac surgery under CPB (median ΔSOFA, 6), accompanied by perioperative activation of multiple immune-cell subsets and upregulation of pro-inflammatory, anti-inflammatory, and chemotactic mediators. This study provides data-driven evidence and research clues for further investigation of the associations between CPB-related immune perturbations and postoperative organ dysfunction and clinical outcomes.

Objective To investigate the impact of a high-salt diet on blood pressure level and the prevalence of hypertension in individuals with varying salt-sensitive phenotypes.Methods In 2004,339 participants from northern China were subjected to a chronic salt loading diet intervention study,and their salt-sensitive phenotypes were determined based on the study results.The participants were divided into salt-sensitive and salt-insensitive groups.In 2018,we randomly selected 152 participants for follow-up from the original cohort,and assessed their salt intake through 24-hour urine sodium level.The participants were divided into different groups based on various criteria to comprehensively investigate the effects of high-salt diet on blood pressure level and hypertension prevalence in individuals with different salt-sensitive phenotypes.Results Among the 339 subjects,154 were identified as salt-sensitive,accounting for 45.4％,while 185 were classified as salt-insensitive,accounting for 54.4％.The average arterial pressure response to high-salt intake after switching from low-salt intake in salt-sensitive individuals was 8.31(7.60,9.02),while the average arterial pressure response in salt-insensitive individuals was-1.09(-1.49,-0.68).A follow-up visit was conducted in 152 participants,It was found that the systolic blood pressure and diastolic blood pressure[divided according to the average daily salt intake(＞11 g).SBP:140.8 mmHg vs.132.9 mmHg,DBP:85.9 mmHg vs.81.6 mmHg,P＜0.05;divided according to the quartile of daily salt intake,SBP:140.8 mmHg vs.132.9 mmHg,DBP:85.9 mmHg vs.81.6 mmHg,P＜0.05]were significantly higher in the high-salt diet group than in the normal salt diet group in salt-sensitive individuals,but there was no significant difference in salt-insensitive individuals(P＞0.05).Additionally,the high-salt diet could significantly increase the incidence of hypertension among salt-sensitive individuals[divided according to the average daily salt intake(＞11 g):58.8％vs.32.8％,P＜0.05;divided according to the quartile of daily salt intake:58.8％vs.32.8％,P＜0.05],but had no effect on salt-insensitive individuals(P＞0.05).Conclusion A high-salt diet can increase systolic blood pressure and diastolic blood pressure in salt-sensitive individuals,and significantly increase the long-term prevalence of hypertension in salt-sensitive individuals,but not in salt-insensitive individuals.Therefore,the salt-sensitive phenotype plays a crucial role in the relationship between salt intake and salt-sensitive hypertension.

Objective:To investigate the optimal timing for stage II vitreous surgery for open ocular trauma and its effects on complications, intraocular pressure, and visual acuity.Methods:A total of 98 patients with open ocular trauma were selected as subjects from the Third Department of Ophthalmology at Baoji People's Hospital who received treatment between May 2021 and March 2024. This study used a retrospective design. Based on the different intervals between stage I emergency debridement and suturing and stage II vitreous surgery, the patients were divided into two groups: the control group (with an interval of > 14 days) and the observation group (with an interval of ≤ 14 days), with 49 cases in each group. The postoperative complications, intraocular pressure recovery, and visual acuity improvement were compared between the two groups.Results:At 1 month after surgery, the total cure rate in the observation group was significantly higher than that in the control group [95.92% (47/49) vs. 81.63% (40/49), χ2 = 5.02, P = 0.025]. In the observation group, the visual acuity levels were as follows: level I [2.05% (1/49)], level II [4.08% (2/49)], level III [6.12% (3/49)], level IV [77.55% (38/49)], and level V [10.20% (5/49)]. These results were significantly better than those in the control group, which had the following levels: 18.37% (9/49), 20.41% (10/49), 24.49% (12/49), 34.69% (17/49), and 2.05% (1/49) for levels I-V respectively ( Z = 5.12, P < 0.001). At 1 week after surgery, there were no statistically significant differences in the rates of normal intraocular pressure, high intraocular pressure, or low intraocular pressure between the observation and control groups (all P > 0.05). At 1 week after surgery, the levels of interleukin-4 (IL-4), IL-6, IL-12, IL-17, and interferon-γ in the observation group were (14.85 ± 2.82) ng/L, (7.52 ± 0.54) ng/L, (10.05 ± 2.63) ng/L, (9.17 ± 1.83) ng/L, and (8.95 ± 2.30) ng/L, respectively. These levels in the observation group were significantly lower than those in the control group [(16.48 ± 2.46) ng/L, (7.83 ± 0.64) ng/L, (11.30 ± 2.60) ng/L, (10.22 ± 1.46) ng/L, (10.03 ± 2.24) ng/L, t = -2.79, -2.37, -2.16, -2.87, -2.15, all P < 0.05]. The incidences of silicone oil-dependent eyes, secondary glaucoma, re-vitreal hemorrhage, and ocular atrophy in the observation group were not statistically different from those in the control group (all P > 0.05). The incidence of traumatic proliferative vitreo-retinopathy in the observation group was significantly lower than that in the control group [0 vs. 10.20% (5/49), χ2 = 5.27, P = 0.022]. Conclusions:In patients with open ocular trauma, performing stage II vitreous surgery within 2 weeks after stage I emergency debridement and suturing yields better therapeutic outcomes. This approach can significantly increase the retinal reattachment rate, improve postoperative visual acuity, and effectively prevent and reduce the incidence of traumatic proliferative vitreoretinopathy.

Objective:To explore the current status of hospital-acquired rhinosinusitis (HAR) in Neurocritical Care Unit (NCU) patients and to analyze its risk factors.Methods:From January 2021 to December 2022, 819 NCU patients of Nanfang Hospital, Southern Medical University were selected by whole population sampling method, of which 125 patients met the inclusion and exclusion criteria for analysis. Patients were categorized into HAR group ( n=69) and non-HAR group ( n=56) based on the occurrence of HAR. Differences in demographic and clinical data of the two groups of patients were compared. Logistic regression analysis was used to explore the risk factors for the HAR in patients. Results:HAR occurred in 69 NCU patients, with an incidence of 55.2% (69/125) . There were statistically significant differences in gender, smoking history, Glasgow Coma Scale score on admission, whether or not there was pneumonia, whether or not a nasogastric tube was indwelt before the occurrence of HAR, whether or not tracheal intubation was performed before the occurrence of HAR, whether or not mechanical ventilation was conducted before the occurrence of HAR, whether or not tracheotomy was performed, and the length of NCU stay between patients in HAR group and non-HAR group ( P<0.05) . Logistic regression analysis showed that indwelling nasogastric tube before the occurrence of HAR, failure to quit smoking before admission, and prolonged NCU stay were risk factors for HAR ( OR=6.291, 3.976, 1.089; P<0.05) . Conclusions:NCU patients have a high incidence of HAR, and indwelling nasogastric tube, failure to quit smoking before admission, and prolonged NCU stay are risk factors for HAR in NCU patients.

Blood from a case of group B epidemic cerebrospinal meningitis identified in February 2024 in Ganzhou City,Jiangxi Province,and throat swabs from close contacts were collected for isolation and culture.The isolates were subjected to serogrouping,drug sensitivity testing,and whole genome sequencing and analysis,to provide a basis for epidemiological inves-tigation and clinical drug use.One strain of Neisseria meningitidis was isolated from the blood of the case and denoted group B.The MLST type was ST-7962,with no clonal group attribution.The phylogenetic tree showed that it was genetically close to the 1977 Shanghai carrier isolate(id-52231).Drug sensitivity results indicated that the strain was sensitive to 8 drugs:azithro-mycin,cefotaxime,minocycline,ceftriaxone,chloramphenicol,meropenem,rifampicin,and benzylpenicillin;resistant to cot-rimoxazole,levofloxacin,and ciprofloxacin;and showed an intermediate response to penicillin.This report describes the first case of ST-7962 group B meningoencephalitis found in Jiangxi Province.Monitoring of Neisseria meningitidis carriage,drug re-sistance,and molecular characteristics of strains in the healthy population in this region should be strengthened,to provide la-boratory support for the clinical use of medications,traceability,and control of the pathogen underlying meningoencephalitis infection.

Objective: To improve the efficiency and standardization of preoperative anemia diagnosis and treatment by establishing a systematic intelligent management platform for preoperative anemia. Methods: A multidisciplinary collaborative model was adopted to develop a preoperative anemia management system that integrates intelligent early warning, standardized treatment pathways, and quality control. The system utilizes natural language processing technology to automatically capture laboratory data and establish evidence-based medical decision support functions. A pre-post study design was employed to compare changes in preoperative anemia screening rates, preoperative anemia intervention rates, reasonable use of iron supplements, and perioperative red blood cell transfusion rates before and after system implementation. Results: After system implementation, the standardization of anemia diagnosis and treatment significantly improved: 1) Screening effectiveness: The anemia screening rate increased to 50.00% (an increase of 27.24%); 2) Intervention effectiveness: The anemia treatment rate rose to 56.30% (an increase of 14.02%); 3) Treatment standardization: The reasonable use rate of iron supplements increased to 55.33% (an increase of 21.02%); the red blood cell transfusion rate decreased to 18.29% (a decrease of 4.07%), and the amount of red blood cell transfusions was reduced by 291 units. Conclusion: This system achieves full-process management of preoperative anemia through information technology, significantly enhancing the standardization of diagnosis and treatment as well as intervention effectiveness, providing an effective solution for perioperative anemia management.

OBJECTIVE: Xuebijing injection has been recommended as a therapeutic approach for individuals with severe and critical COVID-19. This study aims to explore the correlation of neutrophil to lymphocyte platelet ratio (NLPR) with the severity and prognosis of COVID-19, and the effect of XBJ on the prognosis of patients with COVID-19 in different inflammatory states. METHODS: This was a retrospective study conducted at Wuhan Union Hospital in China. COVID-19 patients admitted between November 1, 2022 and February 1, 2023 were included. In predicting prognosis for individuals with COVID-19, new inflammatory indicators were used, and their prognostic value was assessed by using Cox regression models and receiver operating characteristic curves. Furthermore, a calculation was made to determine the cutoff value for NLPR. Relative risk and Cox regression models were used to examine the effects of Xuebijing injection on prognosis in patient cohorts that had been stratified by the NLPR cutoff. RESULTS: This research included 455 participants with COVID-19, with a mean age of 72 years. Several inflammatory indicators were found to be strongly correlated with prognosis, and NLPR shows the greatest predictive power. Patients with NLPR > 3.29 exhibited a mortality rate of 17.3%, which was 6.2 times higher than in patients with NLPR ≤ 3.29. Importantly, providing Xuebijing injection to patients with NLPR > 3.29 was associated with a lower risk of 60-day all-cause mortality. However, there was no discernible improvement in survival among patients with NLPR ≤ 3.29 who received Xuebijing injection. CONCLUSION NLPR is the most reliable inflammatory marker for predicting prognosis among individuals with COVID-19, and can accurately identify individuals who may benefit from Xuebijing injection. Geriatric oral health care encounters significant challenges with the increase in the proportion of older individuals. Age-related changes in the dentition, muscles, and joints result in a decline in objective masticatory function, subjective restoration requirements, and acceptability among the elderly population, with individual variations influenced by systemic health. Considering functional requirements, the adaptability of stomatognathic and systemic health conditions, health economics and other factors, the authors believe that it should not be limited to the conventional "one-to-one" strategy for replacing missing teeth in geriatric prosthodontics. There is an urgent need for a precise and adaptable restoration strategy that is more suitable for older individuals. The proposal of a new concept of functional tooth loss updates the minimal restoration standards for elderly patients and establishes the theory of age-friendly functional restoration. Based on the restoration strategy of functional tooth loss, this paper proposes a new concept termed "age-friendly functional restoration of the stomatognathic system", which integrates treatment considerations including endodontics, periodontology, mucosa, muscles, temporomandibular joint, and systemic health. Efforts should be made in four areas as follows. Firstly, the "assessment of accessible function" should be enhanced by considering the interrelationship between stomatognathic and systemic health. Secondly, the "evaluation of appropriate function" is supposed to be optimised in view of subjective needs and objective evaluation of the stomatognathic system. Moreover, the "formulation of treatment plans" needs to be accomplished with the aid of assistive technologies, such as artificial intelligence, to accurately exert appropriate functional restoration. Lastly, the "management and maintenance of health" is likely to be strengthened through follow-ups, propaganda and education, and preventive healthcare, so as to improve quality of life and ultimately achieve healthy ageing among older individuals.
