1.The battle within: Command hallucinations driving recurrent urethral foreign body insertion in schizophrenia – A case report.

OBJECTIVES
To present a rare case of recurrent urethral and intravesical foreign body insertion driven by command hallucinations in schizophrenia, to describe the surgical and psychiatric management strategies employed, and to emphasize the importance of interdisciplinary care and treatment adherence in preventing recurrence.
METHODSReported here is the case of a 37-year-old male with schizophrenia who presented with multiple episodes of urethral and intravesical foreign body insertion over a six-year period (2018–2025). Inserted objects included metallic wires, electrical cords, and a LED Christmas light rope, each requiring surgical removal via cystoscopy or open cystotomy. Psychiatric evaluation revealed poor adherence to antipsychotic medication, with recurrent episodes associated with command hallucinations. Psychiatric management was reinitiated with olanzapine and structured follow-up to improve treatment compliance.
RESULTSSix documented episodes of self-inflicted urethral and intravesical trauma required repeated urologic interventions. Despite recurrent instrumentation and foreign body insertion, serial cystoscopic evaluations demonstrated preserved urethral and bladder integrity without evidence of stricture formation. The most recent episode required open cystotomy for removal of a coiled LED light rope, which was successfully extracted without complications. Following coordinated psychiatric management and improved adherence to antipsychotic therapy, the patient remained asymptomatic and free of recurrence at three months follow-up.
CONCLUSIONThis case highlights the unusual preservation of urethral integrity despite recurrent traumatic self-insertion. Effective management requires sustained psychiatric stabilization, multidisciplinary collaboration, and strict treatment adherence. Integration of psychiatric and urologic care is essential to prevent recurrence and improve long-term outcomes in patients with schizophrenia-related self-inflicted genitourinary injury.

2.Bilateral single-curvilinear inguinal incisions in advanced penile cancer: A novel approach to inguinal and pelvic lymphadenectomy.

BACKGROUND
Penile squamous cell carcinoma (SCC) is rare in high-income regions but remains a significant oncologic burden worldwide. Nodal involvement is the most important prognostic factor, and early identification is critical.
THE CASEA 56-year-old circumcised male presented with a six-month history of a progressively enlarging, foul-smelling fungating penile mass and bilateral inguinal lymphadenopathy. Biopsy confirmed moderately differentiated SCC. Imaging showed large bilateral inguinal nodal disease without distant metastasis. Laboratory findings revealed leukocytosis, thrombocytosis, and mildly deranged coagulation parameters. The patient underwent total penectomy with perineali urethrostomy, followed by bilateral inguinal and pelvic lymph node dissection through single-curvilinear incisions. Frozen-section analysis confirmed extensive bilateral nodal metastasis. Postoperatively, a localized left inguinal surgical site infection was treated conservatively. He was discharged on postoperative day seven with drains in situ and was recommended adjuvant chemoradiotherapy in accordance with NCCN Guidelines.
CONCLUSIONThis case highlights the challenges of managing advanced penile SCC with bulky bilateral inguinal metastasis and demonstrates the utility of single-curvilinear inguinal incisions for comprehensive lymphadenectomy. Early recognition, accurate staging, and guideline-based multimodal treatment remain essential for optimizing outcomes.

3.Omental torsion presenting as acute appendicitis: A case report.

Omental torsion is a rare cause of acute surgical abdomen from omental twisting leading to distal vascular compromise. Preoperative diagnosis has been a dilemma as clinical features, laboratory and radiographic findings are nonspecific and variable, closely mimicking acute appendicitis. Surgical excision has been suggested to offer rapid symptom relief and postoperative recovery. Presented here is a case of a 59-year-old male with omental torsion who presented with generalized abdominal tenderness and was preoperatively diagnosed as ruptured appendicitis who underwent laparotomy with uneventful post-operative course.

4.Diffuse intraretinal and optic disc edema in a case of reversed vaulted sulcus-implantation of a multi-piece intraocular lens in a 7-year-old child.

OBJECTIVE
This paper reports a rare presentation of diffuse intraocular inflammation following reversed vaulted multipiece intraocular lens (IOL) implanted in the sulcus of a 7-year-old child.
METHODSThis is a case report.
CASE PRESENTATIONA 7-year-old child presented with persistent posterior pole inflammation and optic disc swelling after an improperly placed multipiece intraocular lens in the sulcus. Comprehensive eye and wholebody evaluations were performed with the aid of optical coherence tomography (OCT), magnetic resonance imaging (MRI) and systemic work-up to rule out other causes of uveitis to definitively diagnose the case. The reverse-vaulted lens was eventually explanted. After 1 week post-explantation, the visual acuity improved markedly to 20/100 with the appropriate spectacle prescription. The patient recovered from the ocular inflammation 8 months postoperatively.
CONCLUSIONThis paper reports significant posterior pole inflammation as an uncommon yet potentially visually-disabling consequence of a reverse-vaulted sulcus placement of a multipiece intraocular lens in a child. Careful attention is warranted to ensure proper IOL orientation during implantation, especially in pediatric cataract surgery. Visual rehabilitation is important postoperatively

5.Ocular cicatricial pemphigoid in a young Filipino male: The first biopsy-proven case report from the Philippines.

OBJECTIVE
To describe the first biopsy-proven case of ocular cicatricial pemphigoid (OCP) in a young, Filipino male – an atypical presentation for this disease.
METHODSThis is a case report.
CASE PRESENTATIONA 34-year-old healthy Filipino male presented with a 2-year history of intermittent eye redness, foreign body sensation, and tearing. Slit lamp examination revealed symblepharon formation and fornix shortening of both eyes. Excision of symblepharon with ocular surface reconstruction using amniotic membrane graft was performed, and the conjunctival specimen was sent for immunohistochemical analysis. Direct immunofluorescence was performed which confirmed strong linear deposits of IgG and fibrinogen along the basement membrane zone, confirming diagnosis of OCP. Early diagnosis and prompt treatment is even more important in the younger population as OCP tends to progress more rapidly, and is associated with more severe manifestations. If left untreated, OCP may progress to symblepharon or, in severe cases, ankyloblepharon formation.
CONCLUSIONOCP is a rare, sight-threatening, chronic, autoimmune disease that involves inflammation and cicatrization of the conjunctiva, which typically affects older females and is rarely seen in younger populations. This case report highlights the need for increased awareness among clinicians in the Philippines to consider OCP as a differential diagnosis for chronic conjunctivitis, even in patients outside the typical demographic. Early recognition and confirmatory biopsy are essential to prevent progressive ocular surface scarring and vision-threatening complications.

6.Spontaneous spinal epidural hematoma treated nonsurgically in a Philippine tertiary hospital: A case report.

Spontaneous spinal epidural hematomas (SSEH) is a rare entity with an estimated incidence of 0.1 in 100,000, defined as a an epidural hematoma without any known inciting events such as trauma or an iatrogenic procedure. Currently, there are no local data pertaining to this disease entity. This paper presents a case of a 52 year old male, without any known comorbid coagulopathies or vasculopathies, and no preceding history of trauma or surgical procedures with sudden onset right-sided radicular pain and hemiparesis with left-sided hemianesthesia with spinal MRI findings of a spinal epidural hematoma but no vascular malformations seen on spinal angiogram. He was managed non-surgically, given short-course steroids, and sent home after 7 days in the hospital. There was no noted progression of his baseline deficits nor any recurrence of another acute spinal event. Upon follow up after 6 months, there was noted resolution of weakness and with only residual mild and patchy sensory deficit on the left. Traditionally treated surgically, this case reports highlights the role of conservative management in this rare condition with good functional outcome.

7.The acceptance of stroke telerehabilitation among rehabilitation providers and consumers in two tertiary hospitals in the Philippines.

BACKGROUND AND OBJECTIVES
Telerehabilitation is the remote delivery of rehabilitation services using telecommunication technologies. Its local adoption was catalyzed by the COVID-19 pandemic, prompting the need to assess user acceptance. This study aimed to determine the acceptance of stroke telerehabilitation among patients, carers, and rehabilitation providers in the Department of Physical Medicine and Rehabilitation at St. Luke’s Medical Center – Global City and Quezon City.
METHODSThis descriptive cross-sectional study used purposive sampling to recruit 73 rehabilitation providers and 10 consumers. Data were collected using a self-administered survey based on the Technology Acceptance Model, covering perceived ease of use, usefulness, and behavioral intent. Descriptive and inferential statistics were used for analysis.
RESULTSMost providers (94.4%) were familiar with telerehabilitation, while only half of the consumers were aware of it. Acceptance was moderate among providers (mean score: 35.75 ± 8.67) and high among consumers (mean score: 31.6 ± 7.52). Female providers were less likely to accept telerehabilitation (p=0.049). Consumers identified financial constraints and lack of a companion as key barriers, while providers cited internet issues and technology use. Both groups viewed telerehabilitation positively for teleconsultation, teletherapy, and telemonitoring. Smartphones were the preferred device; Viber and Facebook Messenger were the most commonly chosen platforms.
CONCLUSIONStroke telerehabilitation was moderately to highly accepted by rehabilitation stakeholders in two tertiary private hospitals in Manila. Findings may guide institutional planning for telerehabilitation services. Training, infrastructure support, and awareness campaigns can help address implementation barriers.

8.Mental health status and academic performance of graduating nursing students during COVID-19 pandemic in a government school in Leyte, mental health program model: A correlational study.

BACKGROUND
The COVID-19 pandemic disrupted education worldwide, prompting a rapid shift to emergency remote teaching that challenged students’ learning and mental health. Nursing students, in particular, faced heightened pressures due to the suspension or online adaptation of essential clinical experiences, alongside the need to master theoretical and practical competencies. Emerging evidence indicates that such stressors adversely affect students’ emotional and psychological well-being, potentially influencing academic outcomes. Understanding the relationship between mental health and academic performance among nursing students is crucial for developing targeted interventions that support their well-being and professional readiness.
OBJECTIVEThis study analyzed the mental health status and academic performance of graduating nursing students during the challenging period of remote learning amid the pandemic in a government school in Leyte.
METHODSThe study utilized a descriptive correlational design to explore the relationships between mental health status and academic performance among nursing students. A modified self-administered questionnaire was utilized to gather data. Ethical approval from Eastern Visayas Health Research and Development ConsortiumEthics Review with ERC number 2023-024 was secured, and data collection occurred through various methods. Data analysis used SPSS version 24, emphasizing the importance of understanding these relationships in educational settings.
RESULTSThe study assessed the demographic profile, online learning attributes, mental health status, and academic performance of 20 nursing students during the pandemic. All students passed their courses, despite reporting moderate emotional loneliness and irritability, but minimal fear of COVID-19. Significant correlations were found between demographic factors and mental health indicators. The null hypothesis, suggesting no relationship between demographic factors and mental health, is void, as significant associations were identified. Recommendations include enhancing mental health support in nursing education to address these challenges.
CONCLUSIONThis study highlights the experiences of 20 nursing students from a government college in Leyte during the COVID-19 pandemic. Predominantly young women from rural, low-income backgrounds, these students faced challenges like poor internet access but successfully completed their academic requirements, showcasing resilience. While they reported low fear of COVID-19, moderate emotional loneliness and irritability indicated underlying mental health issues. The findings stress the need for educational institutions to provide mental health support and address the digital divide to enhance student well-being and success.

9.Association of Food Intake with Metabolic Syndrome among Filipino Adults in the 8th Philippine National Nutrition and Health Survey (NNHeS).

OBJECTIVES
The prevalence of metabolic syndrome among Filipino adults was found to be 12-19%. Diet has been one risk factor targeted to prevent complications. The association of each macronutrient component with metabolic syndrome remains unclear. There is no Philippine data on macronutrient intake and metabolic syndrome, thus, the primary objective of this study is to determine the association of food intake with metabolic syndrome among Filipino adults.
METHODSThis study utilized a cross-sectional design. Data were taken from the results of the 8th National Nutrition and Health Survey (NNHeS). Filipino adults from different regions who consented to participate and with complete anthropometric, biochemical and food recall data were included in this study.
RESULTSThere were 8,056 adults included in the study. The prevalence of metabolic syndrome was 32%. Multivariate analysis showed that increased total protein intake (OR 1.391) and increased daily consumption of meat and poultry (OR 1.397) were associated with increased risk for metabolic syndrome. On the other hand, decreased vegetable intake was associated with increased risk for metabolic syndrome, as well as higher socioeconomic status, female sex, and old age.
CONCLUSIONncreased total protein intake and daily consumptions of meat, poultry, and decreased vegetable intake are associated with an increased risk for metabolic syndrome.

10.Prevalence, Clinical Characteristics, and Outcomes of Intensive Care Unit Patients Requiring Prolonged Mechanical Ventilation in a Tertiary Hospital in the Philippines: A Single-Center Retrospective Cross-sectional Study.

OBJECTIVES
Epidemiology data on prolonged mechanical ventilation (PMV) and PMV patient features in the Philippines is lacking. This retrospective cross-sectional study aimed to determine the prevalence of PMV among intubated patients, describe patient characteristics and outcomes, and identify risk factors associated with PMV.
METHODSA retrospective review of records was done on adult intubated patients admitted under the Medical Intensive Care Unit Service from July 2022 to June 2023. Various clinical characteristics and outcomes of PMV and non-PMV patients were collected, compared, and analyzed. PMV was defined as having MV for ≥6 hours per day
for >21 days.
Among 261 intubated ICU patients admitted, 75 (28.7%) required PMV. PMV patients were older (62 vs.
53.5), had higher Charlson Comorbidity Index scores (4 vs. 3), and required vasopressors (81.33% vs. 54.84%)
and blood products (93.33% vs. 51.08%) more often. Nosocomial infections (86.67% vs. 45.70%), ventila- tor-related (30.67% vs. 12.37%) and in-hospital (66.22% vs. 32.97%) complications developed more frequently.
Outcomes such as ICU length of stay (29.5 vs. 7 days) and hospital mortality (61.33% vs. 41.94%) were longer. Vasopressor use (OR 2.25, 95% CI 1.06-4.76), develop- ment of nosocomial infections (OR 6.20, 95% CI 2.64-
14.56), and development of in-hospital-related compli- cations (OR 2.20, 95% CI 1.13-4.30) were independent predictors of PMV.
In this single-center investigation, 28.7% of ICU patients required PMV. Knowledge of patient characteristics and risk factors aid in the development of interventions that improve outcomes and reduce PMV prevalence. Larger studies are recommended to assess nationwide PMV epidemiology and provide data on the need for step-down units for weaning.
