1.Effectiveness of innervated medial plantar flap for reconstruction of soft tissue defects following foot tumor resection.

OBJECTIVE:
To evaluate the effectiveness of the innervated medial plantar flap for reconstructing soft tissue defects, particularly in the weight-bearing zone, after resection of foot tumors.
METHODS:
A retrospective analysis was conducted on 12 patients with malignant skin and soft tissue tumors of the foot treated between October 2023 and December 2024. The cohort included 8 males and 4 females, aged 42-67 years (mean, 57.5 years). Tumor types comprised malignant melanoma (5 cases), squamous cell carcinoma (4 cases), arsenical keratosis (2 cases), and tumor-induced osteomalacia (1 case). Soft tissue defects located in the heel weight-bearing region in 10 cases and non-weight-bearing ankle region in 2 cases, with defect sizes ranging from 4.0 cm×3.0 cm to 6.0 cm×4.0 cm. Preoperative photon-counting CT angiography (PC-CTA) was performed to assess the medial plantar artery and its perforators. All patients underwent radical tumor resection with confirmed negative margins. The resulting defects were reconstructed using a innervated medial plantar flap incorporating sensory branches of the medial plantar nerve. The flap donor site was covered with a split-thickness skin graft harvested from the ipsilateral inguinal region.
RESULTS:
The operation was successfully completed in all 12 patients. All flaps survived completely without vascular compromise, partial necrosis, or total loss. Incisions healed primarily without dehiscence or infection. Minor skin graft necrosis occurred at the donor site in 3 patients, which healed within 2-3 weeks with routine dressing changes. No donor site complication (e.g., tendon or nerve injury) occurred. Patients were followed up 2-16 months (mean, 10.3 months). At last follow-up, there was no tumor recurrence. Flaps exhibited good color and texture match with surrounding tissue, restored sensation, and all feet achieved normal weight-bearing activity.
CONCLUSION
The innervated medial plantar flap, precisely designed based on PC-CTA localization, provides reliable blood supply and effective sensory restoration. It is an ideal method for reconstructing soft tissue defects after foot tumor resection, especially in the heel weight-bearing region.

2.Construction and Validation of Prognostic Nomogram Model for Elderly Pa-tients with Epithelial Ovarian Cancer

Objective:To investigate the prognostic factors of elderly patients with epithelial ovarian cancer(EOC),construct and validate a nomogram prediction model,and provide a basis for clinical diagnosis and treat-ment.Methods:A total of 13128 elderly patients pathologically diagnosed with EOC from 2010 to 2019 in the U.S.SEER database(version 8.4.1)were selected as internal validation data.They were randomly divided in a 7∶3 ratio,with 9138 cases in the training set and 3990 cases in the validation set.All factors were subjected to univari-ate Cox regression analysis;multivariate Cox regression analysis was performed for factors with P<0.05 to ob-tain their independent prognostic risk factors,and a nomogram model for evaluating 1-,3-,and 5-year overall sur-vival rates was constructed,followed by internal validation.At the same time,the clinical data of 73 elderly EOC patients treated in the First Department of Gynecology at the Affiliated Hospital of Chengde Medical University from January 1,2016 to December 31,2022 were selected for external validation of the nomogram.After construc-ting the nomogram model,by obtaining the risk scores of each independent prognostic factor,the training set,vali-dation set,and external validation set were divided into low-and high-risk groups according to comparison of indi-vidual risk scores and the overall median,the K-M curves for different risk groups were plotted based on the medi-an survival time.Results:①Univariate and multivariate Cox regression analyses showed that,age,marital status,histological type,International Federation of Gynecology and Obstetrics(FIGO)stage,differentiation degree,ser-um tumor carbohydrate antigen 125(CA125)level,unilateral of bilateral tumor,tumor size,positive lymph nodes,residual disease size after cytoreductive surgery,postoperative chemotherapy and surgical treatment were inde-pendent influencing factors for elderly women with EOC(P<0.05).②According to the drawn column line dia-gram model,the C index values of the training set,the verification set and the external verification set were:0.750,0.732 and 0.798 respectively;the 5-year Area under the curve(AUC)in the Receiver Operating Characteristic(ROC)curve of the external verification set in the three groups was 0.669,while the AUC values for 1-,3-,and 5-year(except the 5-year of external validation)were all greater than 0.700.③Based on the plotted K-M curves,the median survival time of the high-risk groups in the training set,validation set,and external validation set was 31,32 and 39 months respectively,and more than half of the patients in the low-risk group were still alive.The a-bove results all suggested that the column chart model had a high clinical application value for the prediction the 1-,3-,and 5-year overall survival rates of elderly patients with EOC.Conclusions:The nomogram model in this study can accurately evaluate the overall survival rate of elderly with EOC,and provide a basis for individualized treatment.

4.Differentially expressed genes of ulcerative colitis and associated microRNAs based on bioinformatics analysis

Objective To analyze differentially expressed genes and potential microRNA (miRNAs) with diagnostic and therapeutic potential in ulcerative colitis (UC) based on bioinformatics. Methods The chip raw data in GEO database was screened by weighted gene coexpression network analysis. UC related differentially expressed genes were obtained for enrichment analysis. Potential miRNAs associated with differentially expressed genes were predicted based on key genes, and gene-miRNA regulatory networks were constructed. Results A total of 277 differentially expressed genes were screened, of which 200 genes were up-regulated and 77 genes were down-regulated. Gene set enrichment analysis (GSEA) showed that the main enrichment pathways were neuroactive ligand-receptor interaction, leishmania infection, prion disease and electrocardiogram receptor interaction. The results of gene ontology (GO) analysis showed that it was mainly involved in chemokine activity, heparin binding as well as chemokine receptor binding and other items. The Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis showed that the main enrichment pathways were cytokine receptor interaction pathway, phosphatidylinositol-3 kinase/protein kinase B(PI3K-AKT) signaling pathway, chemokine signaling pathway as well as nuclear transcription factor kappa B(NF-κB) signaling pathway and other pathway. 5.Exploration of compatibility rules of traditional Chinese medicine and prediction of combination medication for acute rhinopharyngitis based on weighted projection of bipartite networks

Objective To propose a new method for mining compatibility rules of traditional Chinese medicine (TCM) formulation from the perspective of weighted projection of bipartite networks, and to predict the combination of new drugs to provide a basis for guiding the clinical treatment of acute nasopharyngitis. Methods Using the acute nasopharyngitis prescription data in the Traditional Chinese Medicine Integrated Database (TCMID) as the data source, a bipartite network is constructed by extracting the prescription and drug nodes.A drug network projection map was then obtained using weighted projection.Social network analysis was performed combined with weighted projection of bipartite networks, and compatibility rules of "Jun-Chen-Zuo-Shi" in TCM via hierachical cluster analysis based on Pearson correlation.Besides, link prediction was used for core drug prediction. Results The combination of bipartite network weighted projection and Pearson correlation for systematic clustering analysis played a significant role in the study of the compatibility rules of TCM prescriptions.In link prediction, 11 link prediction indicators were selected, and weighted and unweighted algorithms were distinguished.The final calculation showed that the area under the curve (AUC) of the weighted indicators were generally higher than that of the unweighted network.Among the weighted indicators, the indicator with the highest AUC index was the network resource allocation Index.A total of 7 groups of drug combinations were predicted, including Baitouweng-Maokezi, Anxixiang-Shijiaocao, Baihuacha-Fuzi, etc. Conclusion The bipartite network weighted projection method is practical and effective in revealing the compatibility rules of TCM and predicting drug combination.
6.Exploration of the Characteristics, Existing Problems and Research Ideas of Traditional Chinese Medicine Disease Names

The standardization study on disease names in traditional Chinese medicine (TCM) is a key and challenging aspect of the modernization of TCM. TCM disease names refer to a symbolic system established by traditional medicine for the rational definition and denotation of physical and mental distress, characterized by temporal and spatial features, clinical features, and cultural traits. These features are both the characteristics of TCM disease names and the root causes of issues such as weakened application, standardization difficulties, and lack of methodology. In response to these problems, we propose a research idea for TCM disease names. Firstly, appropriate samples should be selected based on application needs, and by constructing models, the alignment of the TCM disease names' refe-rence system in content and form should be facilitated to address the issue of weakened application. Secondly, studies on semantics, pragmatics, and clinical positioning should be conducted simultaneously to promote the empi-rical nature of linguistic symbols and strengthen the clinical features of TCM disease names. Furthermore, research on methodology should be intensified. By fully referencing traditional naming methods and the nomenclature of western scientific fields and theoretically examining complex systems, the intrinsic relationship between classification systems and naming methods should be clarified, thereby completing the standardization study of the construction process of the TCM disease names reference system.
7.Reviews and Recommendations in Comparative Studies on Quality of Wild and Cultivated Chinese Crude Drugs

By reviewing the research history on quality comparison between wild and cultivated Chinese crude drugs, this paper systematically combed the relevant research reports since the 1950s, and summarized and analyzed the results of existing comparative studies, and found that the existing comparative research on the quality of wild and cultivated Chinese crude drugs were mainly focused on several aspects, including characteristics, microstructures, chemical compositions, pharmacodynamic effects, and genetic diversity. Among these, comparative studies of chemical compositions have been the dominant approach, with a particular emphasis on comparing the contents of index components. However, research on pharmacodynamic effects remained relatively limited. Due to various factors such as sample quantity, sample origin, growth period and cultivation methods, the differences in quality between wild and cultivated Chinese crude drugs vary significantly. In general, most wild Chinese crude drugs exhibited higher quality than cultivated products, with significant differences in their characteristics. The contents and proportions of some chemical components underwent noticeable changes, particularly with a marked increase in the proportion of primary metabolites after cultivation. The quality of cultivated Chinese crude drugs is closely related to the cultivation practices employed. Chinese crude drugs produced through wild nurturing, simulated wild planting, ecological cultivation, and other similar methods demonstrate quality levels comparable to those of wild Chinese crude drugs. Based on the analysis results, it is recommended to explicitly specify the cultivation practices and cultivation period of cultivated Chinese crude drugs in comparative studies of the quality between wild and cultivated Chinese crude drugs. Multiple technical approaches, including characteristics, microscopy, non-targeted metabolomics combined with quantitative analysis of differential components, and bioefficacy evaluation, should be employed to comprehensively assess the quality disparities between wild and cultivated Chinese crude drugs. Moreover, research efforts should be intensified to investigate the changes in pharmacodynamic effects resulting from differences in plant cell wall composition, primary metabolites, and secondary metabolites, in order to guide the production of high-quality Chinese crude drugs.
8.Analysis on Quality of Sojae Semen Praeparatum Based on Traditional Quality Evaluation

ObjectiveBased on the quality evaluation experience of "it is better to have a fragrant and strong aroma" summarized by materia medica of past dynasties, the chemical components of Sojae Semen Nigrum(SSN) and Sojae Semen Praeparatum(SSP) were systematically compared and analyzed, and the main fermentation products in different fermentation time were quantitatively analyzed, so as to clarify the transformation law of internal components in the processing process and provide scientific basis for the modern quality control of SSP. MethodUltra performance liquid chromatography-quadrupole tandem time-of-flight mass spectrometry(UPLC-Q-TOF-MS) was used for the structural identification of the chemical constituents of SSN and SSP, and with the aid of Progenesis QI v2.3 software, the negative ion mode was employed for principal component analysis(PCA) pattern recognition, and the data were analyzed with the aid of orthogonal partial least squares-discriminant analysis(OPLS-DA) for two-dimensional data to obtain S-plot, and components with |P|>0.1 were selected as the differential constituents. The contents of isoflavonoids in SSP during fermentation was determined by UPLC, and the samples were taken every 8 h in the pre-fermentation period and every 2 d in the post-fermentation period, and the dynamic changes of isoflavonoid contents in different fermentation stages were analyzed. The contents of amino acids and nucleosides in SSP and SSN from different fermentation stages were quantitatively analyzed by phenyl isothiocyanate(PITC) pre-column derivatization and high performance liquid chromatography(HPLC) gradient elution, and the contribution of flavor substances to the "delicious" taste of SSP was discussed by taste intensity value(TAV). ResultA total of 19 kinds of differential components were screened out, mainly soybean saponins and isoflavones, and their contents decreased significantly or even disappeared after fermentation. In the pre-fermentation process of SSP, glycoside bond hydrolysis mainly occurred, and isoflavone glycosides in SSN were degraded and converted into the corresponding aglycones, the content of flavor substances such as amino acids increased gradually. In the post-fermentation process, protein degradation mainly occurred, after 8 d of post-fermentation, the content of isoflavones was basically stable, while the total content of amino acids increased by 8-40 times on average. Different amino acids form the special flavor of SSP, such as the TAV of glutamate is always ahead of other flavor substances, and sweet substances such as alanine and valine have made relatively great contributions to SSP. ConclusionBased on the law of constituent transformation, combined with the traditional evaluation index of "fragrant and strong", it is difficult to control the fermentation degree of SSP by the existing standards in the 2020 edition of Chinese Pharmacopoeia. It is suggested that description of the characteristics of SSP be refined and changed to "fragrant, delicious and slightly sweet", and at the same time, the post-fermentation index compounds such as glutamic acid, alanine and valine should be added as the quality control indicators of SSP, so as to standardize the production process and improve the quality of SSP.
9.Discussion on the TCM syndrome and treatment of depression after myocardial infarction based on the theory of ascending and descending of Qi and blood

Depression after myocardial infarction is closely related to the theory of ascending and descending of qi and blood. The core pathogenesis is analyzed as disorder of qi and blood, mental damage, uncontrolled upward and downward movement, and pivot movement failure. The treatment method is to regulate qi and blood, invigorate qi and activate blood circulation and restore the rise and fall of visceral qi. Nourishing qi and promoting blood circulation should invigorate qi in the first. Appropriate blood activating drugs should be selected according to the degree of blood stasis, so that the blood circulation is smooth and the spirit has dependence. The key to restoring the normal balance of qi movement of visceral organs is to regulate not only liver ascending and lung descending but also spleen. Clinicians need pay attention to the nature of drug lifting and falling, conforming the physiological functions of the organs, to restore the rising and falling of the qi movement of internal organs.
10.Current situation and influencing factors of intrinsic capacity of elderly people in the community

Objective:To explore the current situation of the intrinsic capacity of elderly people in the community and analyze its influencing factors.Methods:From September 2022 to March 2023, convenience sampling was used to select 360 elderly community residents from Xicheng District, Beijing, and Xingtai City, Hebei Province, as the research subjects. The subjects were surveyed using the Intrinsic Capacity Assessment Questionnaire, Intrinsic Capacity Influencing Factors Questionnaire, Barthel Index, and Lawton Instrumental Activities of Daily Living Scale.Results:A total of 360 questionnaires were distributed, and 360 valid questionnaires were collected, with an effective response rate of 100.0% (360/360). The overall impairment rate of intrinsic capacity among 360 elderly people in the community was 90.3% (325/360), and the impairment rates in various fields from high to low were sensation, psychology, vitality, cognition, and exercise. Multiple linear regression analysis showed that age, gender, residential pattern, education level, work status, grip strength, number of geriatric syndrome, self-rated health status, adverse life events, number of social activities, transportation conditions, and social security were the influencing factors of the intrinsic capacity of elderly people in the community, and the difference was statistically significant ( P<0.05) . Conclusions:The impairment rate of intrinsic capacity among elderly people in the community is relatively high. Grassroots medical and nursing staff should assess the intrinsic capacity of elderly people in the community in health promotion work, identify elderly people with decreased intrinsic capacity early and carry out effective interventions to prevent elderly people from becoming disabled and dependent on care.
