1.Scleromitrion diffusum reverses epithelial-mesenchymal transi-tion of gastric mucosa in rats with gastric precancerous lesions.

OBJECTIVES:
To investigate the effect of Scleromitrion diffusum on gastric mucosal epithelial-mesenchymal transition (EMT) in rats with gastric precancerous lesion.
METHODS:
Fifty SD rats were randomly divided into blank control group (n=11), model control group (n=13), Scleromitrion diffusum (SD) group (n=13) and vitase group (n=13). Gastric precancerous lesion animal model was prepared by 1-methyl-3-nitro-1-nitrosoguanidine complex polyfactor method, and the drugs were administrated by gavage once a day for 6 weeks. The pathological changes of gastric mucosa were observed with hematoxylin and eosin staining, the expression of EMT marker proteins were detected with immunohistochemical staining and Western blotting.
RESULTS:
Compared with the model control group, the gastric mucosal injury was significantly attenuated in the Scleromitrion diffusum group, the mucosal tissue structure gradually recovered, the saccular expansion area was reduced, and the inflammatory infiltration was ameliorated. The expression of epithelial cadherin was higher, and the expression of neural cadherin and vimentin in the Scleromitrion diffusum group were lower than those of model control group (all P<0.05).
CONCLUSIONS
Scleromitrion diffusum can ameliorate gastric mucosal injury in rats with gastric precancerous lesion by reversing the EMT.

Objective To investigate the mechanism by which Erdong Xiaoke Formula(EDXKF)regulates lacrimal gland autophagy in type 2 diabetes-induced dry eye rats through the peroxisome proliferator-activated receptor γ(PPARγ)/mammalian target of rapamycin(mTOR)signal pathway.Methods Healthy male SD rats were fed with high-sugar and high-fat chow and then injected intraperitoneally 30 mg·kg-1 streptozotocin to construct type 2 diabetes-induced dry eye rat models.Healthy male SD rats were selected as a blank group.Type 2 diabetes rats were randomly divided into a model group,a Traditional Chinese Medicine(TCM)group(given 11 g·kg-1 of EDXKF through gavage),an antagonist group(given 2 mg·kg-1 PPARγ antagonist through intraperitoneal injection),and a TCM plus antagonist group(given 11 g·kg-1 EDXKF through gavage and 2 mg·kg-1 PPARγ antagonist through intraperitoneal injection).Fasting blood glucose(FBG),corneal fluorescein(FL)staining,tear film break-up time(BUT)and phenol red cotton thread test(Prtt)were examined before modeling,after modeling,and after intervention.The weight of the lacrimal gland was compared among different groups after sampling.Hematoxylin and eosin(HE)staining was performed to analyze lacrimal gland hismorphol-ogy.The expression of microtubule-associated protein 1 light chain 3(LC3)and Sequestosome-1(p62)in the lacrimal gland was examined by immunofluorescence.Western blot was used to detect the expression of PPARγ,p62,LC3 Ⅱ,LC3 I,mTOR and p-mTOR in the lacrimal gland.Results FBG levels in the antagonist group were significantly increased after intervention,compared with those in the TCM group(P<0.01).Ocular surface examination showed that compared with the model group,the TCM group had increased BUT and Prtt scores and decreased FL scores,the antagonist group had de-creased BUT and Prtt scores and increased FL scores(all P<0.05).Compared with the antagonist group,the TCM plus antagonist group showed increased BUT and Prtt scores and decreased FL scores(all P<0.05).The weight analysis of lac-rimal glands revealed that the lacrimal gland weight increased in the TCM group and decreased in the antagonist group,compared with that in the model group(all P<0.01).The lacrimal gland weight in the TCM plus antagonist group was higher than that in the antagonist group(P<0.01).HE staining of the lacrimal gland showed atrophy of glandular lobules,increased fusion and expansion of follicles,and dense distribution of nuclei in model and antagonist groups.These symp-toms were more obvious in the antagonist group.Compared with antagonist and TCM plus antagonist groups,the TCM group showed improved symptoms,with tightly arranged follicles,partial atrophy and fusion,and a small amount of expan-sion.Immunofluorescence staining for detecting the average fluorescence intensity of LC3 and p62 showed that compared with the model group,the TCM group had increased LC3 levels and decreased p62 levels,and the antagonist group had de-creased LC3 levels and increased p62 levels(all P<0.01).Compared with those in the antagonist group,LC3 levels were increased and p62 levels were decreased in the TCM plus antagonist group(both P<0.01).Western blot results showed that compared with the model group,the TCM group had increased PPARγ and LC3 levels and decreased p62 and p-mTOR/mTOR levels.PPARγ and LC3 levels were decreased and p62 and p-mTOR/mTOR levels were increased in the antagonist group,compared with those in the model group.The TCM plus antagonist group had higher PPARγ and LC3 levels and low-er p62 and p-mTOR/mTOR levels than the antagonist group(all P<0.05).Conclusion EDXKF can regulate the PPARy/mTOR signal pathway to promote lacrimal gland autophagy and thus alleviate dry eyes in type 2 diabetes rats.
3.Efficacy and safety of repetitive transcranial magnetic stimulation in the treatment of postpartum depression: a Meta-analysis

BackgroundPostpartum depression (PPD) is a prevalent postpartum complications that significantly compromises women's psychological and physical well-being. Repetitive transcranial magnetic stimulation (rTMS), a conventional neuromodulation technique, has been increasingly used in the treatment of PPD. However, high-quality evidence regarding its efficacy and safety remains limited. ObjectiveTo evaluate the efficacy and safety of rTMS in the treatment of PPD, thereby providing references for clinical treatment. MethodsDatabases including Cochrane Library, PubMed, Embase, CNKI, Wanfang, VIP and China Biology Medicine disc (CBM) were electronically searched for randomized controlled trials (RCTs) on rTMS for PPD, with the search spanning from database inception to February 8, 2025. Study quality was assessed using the Cochrane Handbook for Systematic Reviews of Interventions 5.0.1, and the certainty of evidence was graded according to the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Meta-analysis was conducted using RevMan 5.3 and Stata 12.0. The outcomes of the Meta-analysis included the total effective rate, Edinburgh Postnatal Depression Scale (EPDS) score, Hamilton Depression Rating Scale (HAMD) score, and adverse reactions (dizziness, headache, nausea, diarrhea, and the overall incidence of adverse reactions). ResultsA total of 11 studies involving 729 patients with PPD were included. Meta-analysis results showed that the total effective rate in the study group was significantly higher than that in the control group (OR=5.54, 95% CI: 3.07–10.01, P<0.01). Both EPDS score (SMD=-2.38, 95% CI: -3.39–-1.37, P<0.01) and HAMD score (SMD=2.53, 95% CI: 1.21–3.85, P<0.01) in the study group were significantly lower than those in the control group, with statistically significant differences. Comparisons between the study group and control group reveal no significant differences in the incidence of dizziness and headache (RR=1.47, 95% CI: 0.63–3.43, P>0.05), nausea (RR=1.46, 95% CI: 0.55–3.86, P>0.05), diarrhea (RR=0.71, 95% CI: 0.23–2.20, P>0.05), and overall adverse reactions (RR=1.30, 95% CI: 0.79–2.15, P>0.05). GRADE assessment rated the four indicators of dizziness and headache, diarrhea, overall incidence of adverse reactions, and EPDS score as "moderate-certainty evidence", and rated the total effective rate, nausea, and the HAMD score as "low-certainty evidence". ConclusionrTMS demonstrates certain therapeutic efficacy for PPD, with a safety profile comparable to conventional treatment. 4.Correlation between free triiodothyronine/free thyroxine ratio and early-phase insulin secretion in patients with type 2 diabetes mellitus

Objective To explore the correlation between the ratio of free triiodothyronine/free thyroxine(FT3/FT4)and the early-phase insulin secretion index(ΔI30/ΔG30)in type 2 diabetes mellitus(T2DM)patients with normal thyroid function.Methods 200 patients with T2DM with normal thyroid function in Hebei General Hospital from September 2019 to June 2021 were selected and divided into the Q1 group with FT3/FT4≤0.26(n=67),the Q2 group with 0.27≤FT3/FT4≤0.29(n=67),and the Q3 group with FT3/FT4≥0.30(n=66)according to the tertiles of the FT3/FT4.The general data,biochemical indicators and islet functions of the three groups were compared,and the relationship between the FT3/FT4 ratio and the ΔI30/ΔG30 as well as the islet β cell function was analyzed.Results The fasting insulin(FIns),2 h postprandial insulin(2 hIns),homeostatic model assessment of islet β cell(HOMA-β)and area under the curve of insulin(AUCI)in Q3 group were higher than those in Q1 and Q2 groups(P<0.05),2 h postprandial blood glucose(2 hPG)in Q3 group was lower than those in Q1 and Q2 groups(P<0.05).Systolic blood pressure in Q2 group was higher than those in Q1 group(P<0.05).Compared with Q1 group,diastolic blood pressure,body mass index(BMI),alanine aminotransferase(ALT),fasting C-peptide(FC-P),area under curve of C-P(AUCC),2 h postprandial C-peptide(2 hC-P)and ΔI30/ΔG30 in Q3 group were significantly higher(P<0.05),and HbA1c was significantly lower(P<0.05).Spearman correlation analysis showed that Δ I30/Δ G30,HOMA-β,AUCI,AUCC and HOMA-IR were positively correlated with BMI,ALT,FC-P,2 hC-P,FIns,2 hIns and FT3/FT4(P<0.05),it was negatively correlated with HbA1c and 2 hPG(P<0.05).Linear regression analysis showed that ΔI30/ΔG30,HOMA-β,AUCI and AUCC were the influencing factors of FT3/FT4 after adjusting for confounding factors.Conclusions ΔI30/ΔG30,HOMA-β,AUCI and AUCC are the influencing factors of FT3/FT4 in T2DM patients with normal thyroid function,suggesting that FT3/FT4 is higher in patients with better islet β cell secretion function.

Renal fibrosis is a critical factor in progression of chronic kidney disease to end-stage renal disease.Underlying mechanisms driving renal fibrosis remain unclear.Lymphocytes play a pivotal and dual role in initiation and progression of renal fibro-sis.Through secretion of cytokines and chemokines,as well as direct cell-cell interactions,lymphocytes can exert profibrotic effects while also demonstrating antifibrotic potential under certain conditions.This review aims to summarize current research advances regarding interplay between lymphocytes and renal fibrosis and to explore their potential applications in antifibrotic therapies.
6.Comparison of efficacy between laparoscopic ureteroureterostomy and ureteral reimplantation for the treatment of complete renal and ureteral duplication in children

Objective:To compare the therapeutic value between laparoscopic ureteroureterostomy (LUU) and ureteral reimplantation (UR) for the treatment of pediatric complete renal and ureteral duplication.Methods:This retrospective case-series study included 65 children with complete data, who were diagnosed with complete renal and ureteral duplication at the First Affiliated Hospital of Zhengzhou University from January 2016 to December 2022.Diagnoses were confirmed through preoperative imaging tests, including renal ultrasound, diuretic renography, computed tomography urography, magnetic resonance urography, and voiding cystourethrography.The patients were categorized into 2 groups based on surgical approach.The LUU group had 35 patients, and the UR group had 30 patients.The surgical duration, intraoperative blood loss, the incidence of stent implantation, postoperative length of hospitalization, the upper renal anterioposterior diameter (APD), the upper ureter diameter (UD), and differential renal function (DRF) of the affected kidney before and after surgery were compared between the 2 groups.For continuous variables such as surgery time and intraoperative blood loss that follow a normal distribution, values are expressed as Mean± SD, with between-group differences analyzed using independent t-tests and within-group differences using paired t-tests.For non-normally distributed continuous variables, data are presented as M( Q1, Q3), with group differences assessed by rank-sum tests.Categorical variables like gender and laterality are compared using Chi-square tests. Results:Intraoperative blood loss was significantly different between the LUU and UR groups [(8.5±4.6) mL vs.(12.6±6.4) mL] ( t=2.465, P=0.020).Postoperative length of hospitalization also differed significantly between the 2 groups [(6.4±2.3) days vs.(10.5±2.8) days] ( t=2.308, P=0.027).However, surgical durations of the groups were similar [(143.0±13.2) min vs.(132.0±12.4) min] ( t=-1.965, P=0.057).No significant difference was observed between the two groups [(35/35) cases vs.(26/30) cases] ( χ2=2.932, P=0.087).Four cases in the UR group suffered long-term complications (including anastomotic stricture in 2 cases, vesicoureteral reflux in 1 case, and recurrent urinary tract infection in 1 case).There was 1 case having the long-term complication (which was anastomotic stricture) in the LUU group.The difference in the incidence of long-term complications was not significant between the 2 groups ( χ2=1.239, P=0.266).Both groups showed significant improvements in upper renal APD, UD, and DRF of the affected kidney after surgery.Changes in upper renal APD ( t=-0.032, P=0.962), DRF ( Z=1.895, P=0.073), and UD ( t=1.832, P=0.079) were not statistically significant. Conclusions:Both LUU and UR are safe and effective in the treatment of complete renal and ureteral duplication in children.Compared with UR, LUU has less intraoperative blood loss and shorter postoperative length of hospitalization.LUU does not involve the bladder and causes less damage to the bladder of children.

Results:Intraoperative blood loss was significantly different between the LUU and UR groups [(8.5±4.6) mL vs.(12.6±6.4) mL] ( t=2.465, P=0.020).Postoperative length of hospitalization also differed significantly between the 2 groups [(6.4±2.3) days vs.(10.5±2.8) days] ( t=2.308, P=0.027).However, surgical durations of the groups were similar [(143.0±13.2) min vs.(132.0±12.4) min] ( t=-1.965, P=0.057).No significant difference was observed between the two groups [(35/35) cases vs.(26/30) cases] ( χ2=2.932, P=0.087).Four cases in the UR group suffered long-term complications (including anastomotic stricture in 2 cases, vesicoureteral reflux in 1 case, and recurrent urinary tract infection in 1 case).There was 1 case having the long-term complication (which was anastomotic stricture) in the LUU group.The difference in the incidence of long-term complications was not significant between the 2 groups ( χ2=1.239, P=0.266).Both groups showed significant improvements in upper renal APD, UD, and DRF of the affected kidney after surgery.Changes in upper renal APD ( t=-0.032, P=0.962), DRF ( Z=1.895, P=0.073), and UD ( t=1.832, P=0.079) were not statistically significant. Conclusions:Both LUU and UR are safe and effective in the treatment of complete renal and ureteral duplication in children.Compared with UR, LUU has less intraoperative blood loss and shorter postoperative length of hospitalization.LUU does not involve the bladder and causes less damage to the bladder of children.
