1.Study on the capture of Helicobacter pylori released from Candida using immunomagnetic bead

ObjectiveTo investigate the ability of clinically isolated, Helicobacter pylori (H. pylori)-specific gene polymerase chain reaction (PCR)-positive gastric, vaginal, and fecal Candida to release H. pylori. MethodsResuscitate 4 strains of H. pylori -specific 16S rDNA and ureA gene PCR-positive Candida strains isolated in laboratory from clinical sources, including 1 strain of gastric Candida, 1 strain of fecal Candida, 2 strains of vaginal Candida and the standard Candida albicans strain ATCC10231 (Ca10231). The presence of H. pylori-specific ureA in the 5 strains of Candida isolates was confirmed by PCR. The aforementioned strains of Candida and H.pylori were inoculated into urea medium and cultured in a constant temperature incubator at 37 ℃. The color change of the medium was observed daily. A change in the medium's color from yellow to red indicated the presence of urease activity. Then, the five strains of Candida and H. pylori were co-incubated with the magnetic beads coated with H. pylori antibodies respectively. Scanning electron microscopy (SEM) was employed to observe the presence of bacilli adsorbed on the surface of the magnetic beads. PCR was used to detect the presence of H.pylori-specific 16S rDNA and ureA genes on magnetic beads. ResultsThe PCR analysis of the ureA gene in the four Candida isolates was positive, whereas the Ca10231 strain tested negative. Upon culturing the four Candida isolates on urea medium, the medium color changed from yellow to red which was determined to be urease positive, while the medium containing Ca10231 remained unchanged, which was urease negative. SEM revealed that bacilli could be observed on the surface of magnetic beads co-incubated with the 4 strains of Candida of clinical origin and H.pylori isolate. Specifically, PCR testing of the magnetic beads co-incubated with one vaginal Candida, one gastric Candida and H.pylori isolate showed positive results for the 16S rDNA and ureA genes of H. pylori; however, the PCR tests for the two genes were negative for the magnetic beads co-incubated with the other two Candida isolate. ConclusionThis study demonstrates that H. pylori-specific genes Candida can release H. pylori.
2.Short-term effect of 3D-printed external fixation guide combined with video-assisted thoracic surgery for flail chest: A retrospective cohort study

Objective To investigate the early clinical efficacy of a 3D-printed external fixation guide combined with video-assisted thoracic surgery (VATS) in the treatment of flail chest, and to provide evidence for its clinical application. Methods Patients with flail chest admitted to Xiamen University Affiliated Chenggong Hospital between January 2010 and January 2023 were retrospectively selected as the study subjects. Based on the surgical methods, the patients were divided into two groups: patients treated with the 3D-printed external fixation guide combined with VATS were assigned to a 3D group, and those who underwent open reduction and internal fixation were assigned to an internal fixation group. The operative time, intraoperative blood loss, duration of chest tube drainage, recovery of thoracic volume, visual analogue scale (VAS) scores at 1 month postoperatively, and complications were compared between the two groups. Results A total of 40 patients were included, with 20 patients in each group. The 3D group consisted of 13 males and 7 females, with a mean age of (45.7±3.8) years; the internal fixation group consisted of 14 males and 6 females, with a mean age of (47.3±4.1) years. There were no statistical differences between the two groups in terms of gender, age, number of rib fractures, or preoperative VAS scores (P>0.05). The surgeries were performed successfully in both groups, primary wound healing was achieved in all patients, and pain symptoms were significantly alleviated compared to preoperation. No postoperative complications occurred in the 3D group, whereas 1 patient each of chronic postoperative pain, fracture malunion, and incision infection occurred in the internal fixation group, resulting in a complication rate of 15.0%. The operative time, intraoperative blood loss, and duration of chest tube drainage in the 3D group were significantly shorter or less than those in the internal fixation group (P<0.05). There were no statistical differences in the recovery of thoracic volume or the VAS scores at 1 month postoperatively between the two groups (P>0.05). Conclusion The early clinical efficacy of a 3D-printed external fixation guide combined with VATS in the treatment of flail chest is satisfactory. This technique offers the advantages of being minimally invasive, highly efficient, promoting rapid recovery, and resulting in fewer postoperative complications, and can effectively reconstruct the thoracic contour and restore thoracic volume.
3.Pathogen detection and genetic characteristic analyses of hand-foot-and-mouth disease in Dehong Prefecture of Yunnan Province in 2023

ObjectiveTo genotype the specimens from hand-foot-and-mouth disease (HFMD) cases in Dehong Prefecture, Yunnan Province in 2023 by gene sequencing, and to analyze the genetic characteristics of Coxsackievirus A4 (CVA4), CVA6, CVA10, CVA16 and enterovirus A71 (EV-A71), so as to clarify the serotypes of enteroviruses causing endemic HFMD and the genotype composition of CVA4, CVA6, CVA10, CVA16 and EV-A71 prevalent in HFMD, and to provide baseline genotype data for monitoring the dynamic changes of the five virus genotypes. MethodsStool specimens from HFMD cases in Dehong Prefecture in 2023 that tested positive for EV-A71, CVA16 and other enteroviruses by real-time quantitative reverse transcription polymerase chain reaction (RT-qPCR) were first subjected to gene sequencing and typing of the VP4/VP2 junction region to determine the viral types. Subsequently, the complete VP1 gene of each virus was amplified in two segments using group-specific VP1 gene primers corresponding to enterovirus groups (i.e., enterovirus group A, B, and C) and sequenced (group A virus: primers 486/488 and 487/489, group B virus: primers 490/492 and 491/493, group C virus: primers 494/496 and 495/497). The representative reference sequences of VP1 genes for CVA4, CVA6, CVA10, CVA16 and EV-A71 were downloaded from the literature, and the VP1 gene phylogenetic tree was constructed using MEGA 6.0 software for genetic characterization. ResultsA total of 256 stool specimens were collected from HFMD cases in Dehong Prefecture in 2023, of which 94 tested positive for nucleic acid by RT-qPCR, yielding a positive rate of 36.72%. Of the 94 positive specimens, 83 were identified as group A viruses, and were classified into 5 serotypes (17 strains of CVA4, 3 of CVA6, 6 of CVA10, 49 of CVA16, and 8 of EV-A71). Nine strains were group B viruses, comprising 2 serotypes: 2 strains of echovirus type 3 (E3) and 7 strains of Coxsackievirus B type 5 (CVB5). The remaining 2 strains were group C viruses, both identified as poliovirus type 3 (PV3). Genetic analyses of the VP1 region showed that all 17 CVA4 strains were identified as subgenotype C2, all 3 CVA6 strains as subgenotype D3a, all 6 CVA10 strains as genotype C. Among the 49 CVA16 strains, 46 were identified as subgenotype B1a and 3 as subgenotype B1b, and all 8 EV-A71 strains were identified as subgenotype C4a. ConclusionIn 2023, group A viruses dominated the pathogenic spectrum of HFMD in Dehong Prefecture. Among them, CVA16 had the highest positivity rate. Genetic characteristic analyses showed that CVA4, CVA6, CVA10 and EV-A71 were each composed of a single genotype (subgenotype), while CVA16 was composed of two subgenotypes (B1a and B1b), with B1a being the predominant one. It is recommended that molecular epidemiological surveillance of HFMD pathogens, particularly group A viruses, be continuously strengthened in Dehong Prefecture.
4.Prevalence rates of healthcare-associated infection in a tertiary first-class hospital in the northwest of Hunan Province in 2015-2024

Objective To understand the current situation and dynamic changing trends of healthcare-associated infection(HAI)in a tertiary first-class hospital in the northwest of Hunan Province from 2015 to 2024,and provide scientific basis for optimizing infection control strategies.Methods A single-day cross-sectional survey method was employed to investigate the HAI prevalence rates of hospitalized patients on the given survey day each year from 2015 to 2024.The standardized survey protocol on prevalence rate issued by the National Medical Institution Infec-tion Surveillance Network was strictly adhered,lanqingting real-time HAI monitoring management platform was adopted to retrieve cases from the hospital information system,and R4.2.2 was applied for statistical analysis.Results From 2015 to 2024,the prevalence rate of HAI decreased from 3.03%in 2015 to 1.76%in 2024(Z=-3.37,P<0.001),and the HAI case prevalence rate decreased from 3.55%in 2015 to 2.20%in 2024(Z=-2.81,P=0.005).Department of critical care medicine continuously had the highest HAI case prevalence rate,which presented a downward trend over time(Z=-2.84,P=0.004).The main site of HAI was lower respiratory tract,accounting for 39.36%to 48.15%,bloodstream infection increased from 3.57%in 2015-2016 to 10.60%in 2023-2024(Z=2.41,P=0.016).A total of 302 strains of HAI pathogens were detected,including 212 strains(70.20%)of Gram-negative bacteria,mainly Pseudomonas aeruginosa(n=55,18.21%),Escherichia coli(n=45,14.90%),Acinetobacter baumannii(n=33,10.93%),and Klebsiella pneumoniae(n=31i,10.26%).65 strains(21.52%)of Gram-positive bacteria were identified,with Enterococcus faecium(n=19,6.29%)and Staphylococcus aureus(n=18,5.96%)accounting for the highest proportions.25 fungal strains(8.28%)were detected,mainly Candi-da albicans(n=11,3.64%).The use rate of antimicrobial agents showed a downward trend over the past decade(Z=-4.01,P<0.001).Therapeutic antimicrobial use accounting for 82.42%,and its proportion increased over time(Z=6.02,P<0.001).Prophylactic antimicrobial use accounted for 16.42%,showing a decreasing trend(Z=-2.75,P<0.001).The pathogen detection rate presented an upward trend over the past decade(Z=13.01,P<0.001).Conclusion The prevalence rate and case prevalence rate of HAI present a downward trend in this hospi-tal.In the future,it is necessary to establish a monitoring data-based dynamic analysis mechanism,achieve timely feedback and intervention in data monitoring,pay attention to high-risk links in department of critical care medicine,implement precise prevention and control mearsures,perform targeted prevention and control for lower respiratory tract,urinary tract,and bloodstream infection,optimize diagnosis and treatment processes,use antimicrobial agents rationally,and pay attention to the prevalence trend of Gram-negative bacteria.
5.Effect of the number of times to urinate on the modeling rate of neurogenic bladder model in rats after complete spinal cord transection

BACKGROUND:Spinal cord injury often leads to neurogenic bladder with hyperreflexia of the forced urethral muscle,but there is a lack of clear understanding of its pathogenesis and treatment,and establishing a stable and reliable animal model has an important impact on revealing its pathomechanisms and exploring therapeutic approaches.OBJECTIVE:To investigate the effect of the number of times to urinate on neurogenic model rats after complete spinal cord transection in order to improve the postoperative survival and modeling rate of neurogenic model rats.METHODS:Out of 46 female Sprague-Dawley rats,6 were selected as the sham-operated group using the random number table method,and the remaining 40 rats were randomly divided into 0,1,3,and 5 times daily urination groups after complete spinal cord transection modeling,with 10 rats in each group.The residual urine volume was measured every 3 days within 19 postoperative days,and the survival and modeling were observed on the 19th day after the operation,and urodynamics tests and contraction experiments of isolated forced urethra muscle strips were performed.RESULTS AND CONCLUSION:(1)Survival and modeling rate:there was 10%survival rate and 10%modeling rate in the group of 0 times daily urination;20%survival rate and 10%modeling rate in the group of 1 time daily urination;70%survival rate and 70%modeling rate in the groups of 3 and 5 times daily urination.(2)Residual urine volume:compared with the sham-operated group,the residual urine volume of the groups of 3 and 5 times daily urination was significant increased on postoperative days 3,6,9,12,and 15(P<0.01);the residual urine volume of the groups of 3 and 5 times daily urination was increased on the 18th day after surgery(P<0.05).Compared with the 3 times daily urination group,the residual urine volume was decreased in the 5 times daily urination group on the 6th day after surgery(P<0.05),while there was no significant difference in the residual urine volume between the 3 and 5 times daily urination groups on the 3rd,9th,12th,15th,and 18th days after surgery.(3)Urodynamics:Compared with the sham-operated group,the differential pressure at the point of leakage was significantly reduced(P<0.01)and the maximal volume was significantly increased(P<0.01)in the 3 and 5 times daily urination groups.There was no statistical difference in the differential pressure at the point of leakage and the maximal volume between the 3 and 5 times daily urination groups.(4)Muscle-strip contraction test of isolated detrusor muscles:Compared with the sham-operated group,the contraction amplitude and frequency of detrusor muscle strips were significantly reduced in the 3 and 5 times daily urination groups(P<0.01).There was no statistically significant difference in the contraction amplitude and frequency of detrusor muscle strips between the 3 and 5 times daily-urination groups.In conclusion,assisted urination is one of the keys to establish a successful neurogenic model of urethral reflexes,and there is no significant difference in the effects of urinating 3 or 5 times a day on the neurogenic model.It is recommended to urinate at least 3 times a day based on the actual workload and the modeling rate.
6.Brusatol induces apoptosis in small cell lung cancer by inhibiting STAT3 phosphorylation

Aim To investigate the effect of Brusatol a-gainst small cell lung cancer(SCLC)and its potential mechanism.Methods CCK-8 assay and flow cytome-try were used to detect the cytotoxic effect of Brusatol on SCLC cells.Western blot was employed to measure the expression levels of apoptosis-related proteins,in-cluding cleaved poly(ADP-ribose)polymerase(cleaved-PARP),B-cell lymphoma 2(Bcl-2)and Bcl-2-associated X protein(Bax).Network pharma-cology databases were utilized to identify common tar-gets of Brusatol,SCLC,and apoptosis.Kyoto Encyclo-pedia of Genes and Genomes(KEGG)and Gene On-tology(GO)enrichment analyses were performed on the intersecting genes.Molecular docking simulations between Brusatol and core targets were conducted using the CB-DOCK2 online platform to calculate binding en-ergies and sites.Western blot was further applied to detect the expression levels of signal transducer and ac-tivator of transcription 3(STAT3)and phosphorylated-STAT3(p-STAT3).Results Brusatol inhibited SCLC cell growth and induced apoptosis,significantly downregulating Bcl-2 and cleaved-PARP while upregu-lating Bax expression(P<0.05).Network pharma-cology analysis revealed 108 common targets of Brusa-tol and SCLC,with the top three core targets being ep-idermal growth factor receptor(EGFR),STAT3,and tumor necrosis factor(TNF).Molecular docking re-sults indicated strong binding affinity between bruceine D and these core targets.Western blot validation con-firmed that bruceine D suppressed the expression of STAT3 and p-STAT3.Conclusion Brusatol exerts anti-SCLC effects by inhibiting STAT3 to induce apop-tosis in SCLC cells.

8.Comprehensive evaluation of performance in clinical specialties based on entropy weight-TOPSIS-RSR

Objective To evaluate the medical service performance of a hospital in 2023,providing references for the refined management and discipline advancement.Methods The DRG-related data of patients discharged from the sample hospi-tal in 2023 were obtained through the DRGs in Sichuan Province.A comprehensive evaluation was performed by using the inte-grated approach of TOPSIS and RSR with entropy weight.Results The top three specialties offering a wide range of medical services and handling more complex cases on average were neurosurgery,orthopedics,and cardiovascular medicine.The difficulty level of cases in these departments exceeded the average for both the city and the province.TOPSIS-RSR method was used to e-valuate six weights of the index,and the top three ones were CMI,total weight,and number of diagnostic-related groups.In the DRG performance evaluation,30 clinical departments were divided into 3 grades,with 5,21,and 4 departments in each grade,respectively,revealing differences in case structure across grades.Conclusion The combination of DRG-related indicators and comprehensive evaluation method can be effectively applied to the performance evaluation of medical service for inpatients,thus providing suggestions and strategies for promoting the improvement of medical service capacity,improving medical service effi-ciency,optimizing safety and quality assessment indicators,and achieving overall balanced development in wards.
9.Exploring the neural circuitry mechanisms of acupuncture for depression:Insights centered on the nucleus accumbens

The pathogenesis and progression of depression are closely associated with functional dysregula-tion in the brain's reward and emotion-modulating circuits.As the central hub of the reward system,the nucleus accumbens(NAc)forms aberrant neural circuits with multiple brain regions-including the ventral tegmental area(VTA),medial prefrontal cortex(mPFC),amygdala(AMY),hippocampus(HIP),and paraventricular nucleus of the thalamus(PVT)-which have been empirically linked to depressive symptomatology.Acupuncture,with its multi-target therapeutic profile,aligns well with the multifactorial and multisystem dysregulation characteristic of depressive disorders.This review systematically examines the functional alterations of the NAc-centered neural circuitry in depression and explores the potential mechanisms of acupuncture intervention.Evidence suggests that acupuncture holistically ameliorates depression by:modulating dopaminergic transmission in the VTA-NAc pathway,enhancing neurotransmitter equilibrium in the mPFC-NAc circuit,bidirectionally regulating emotional stability via the AMY-NAc loop,attenuating HIP-NAc glutamatergic hyperactivity,and optimizing excitatory signaling in the thalamus-NAc circuit.Furthermore,this synthesis emphasizes a systems-level integration of circuit mechanisms,highlighting both the clinical applicability of acupuncture in depression treatment and recent advances in mechanistic research.The findings provide robust theoretical foundations and practical guidance for clinical practice,bridging translational neuroscience with therapeutic innovation.

Icaritin(ICT)is an 8-isopentenylflavonoid,which is the main effective component of the tra-ditional Chinese medicine Epimedium.Previously,we found that Icaritin inhibits the growth of glioblasto-ma(GBM)cells.Herein we aim to study the in vivo anti-GBM effectiveness of Icaritin and explore its mechanism.The results of MTT assay,flow cytometry,comet assay and cellular immunofluorescence as-say in vitro showed that ICT inhibited the proliferation of four kinds of GBM cells,U87,U251,U118 and A172,induced early apoptosis(P<0.001)and late apoptosis(P<0.05)in U87 cells,induced DNA damage in U87 cells,and blocked the growth of U87 cells at the G0/G1 phase(P<0.0001)in a concen-tration-time-dependent manner.In vivo subcutaneous tumor transplantation tumor experiments showed that feeding 200 mg/kg(P<0.01)and 400 mg/kg(P<0.001)ICT had a significant inhibitory effect on the growth of GBM subcutaneous tumors,and had no significant toxic effects on heart,liver,spleen,lung and kidney tissues.The results of network pharmacological analysis,molecular docking and cellular thermodynamic experiments showed that there were 26 possible target proteins between ICT and GBM,a-mong which the expression of p53 in GBM tissues was significantly(P<0.001)higher than in normal tis-sues,and the binding energy of ICT and p53 was lower;cellular thermodynamic experiments verified that ICT significantly enriched the level of p53 in the living cells of GBM,which indicated that ICT could tar-get p53.The expression of key proteins in the DNA damage repair and apoptosis-associated FOXO signa-ling pathway was detected by ICT.The results showed that the expression of ATR(P<0.01),P53(P<0.001),P21(P<0.05)and γ-H2AX(P<0.05)was up-regulated,whereas the expression of Cyc-lin E1(P<0.01),E2F1(P<0.05),CDK2(P<0.01),Rb(P<0.001),p-Rb(P<0.0001)and WRN(P<0.0001)expression were down-regulated.There was no significant change in the expres-sion of FOXO 1 in the FOXO pathway or a significant down-regulation of its phosphorylation level.This study demonstrated that ICT could effectively inhibit the growth of GBM cells in vivo.It targets p53 to regulate the DNA damage repair pathway and FOXO signaling pathway to induce GBM cell cycle arrest and apoptosis.
