1.Construction and application of the standardized management index system for traditional Chinese medicine reserved in the department of medical institutions

OBJECTIVE To establish a whole-process quality management index system for traditional Chinese medicine (TCM) reserved in the department of medical institutions, providing a reference for standardized management. METHODS An initial indicator framework was determined by collecting and analyzing relevant laws, regulations, policy documents, group standards, and literature on TCM management. Two rounds of Delphi expert consultation involving 20 experts were conducted to refine and optimize the indicator system. The analytic hierarchy process was used to construct judgment matrices and convert the indicator weights into a percentage-based system; an assessment was conducted on 14 departments with reserved TCM among the affiliated units of the Quality Management and Control Center for Traditional Chinese Medicine in Hebei Province. RESULTS The response rate for both rounds of consultation was 100%, with an expert authority coefficient of 0.89. The final quality management system of TCM reserved in the department included four first-level indicators: management (composite weight: 0.366 3), processing (composite weight: 0.119 7), storage (composite weight: 0.291 7) and usage (composite weight: 0.222 3), and twenty-four second-level indicators, such as establishing an organizational structure for hospital drug quality management and having dedicated regulations for backup drugs in clinical departments. Kendall’s coefficient of concordance confirmed consistency across all levels of indicators. Based on the application of the indicator system for evaluation, the average score for the standardized management of reserved TCM in the department of medical institutions increased from 67.01 points to 85.15 points over three months. CONCLUSIONS The constructed indicator system meets the standardized management requirements for reserved TCM, enabling closed-loop management across the entire process of management, processing, storage and usage. It provides a reference for medical institutions to enhance the precision and standardization of reserved TCM management.
2.Construction and application of clinical inapplicability evaluation system for instructions of oral Chinese patent medicines containing toxic decoction pieces

OBJECTIVE To construct a clinical inapplicability evaluation system for the instructions of oral Chinese patent medicines containing toxic decoction pieces, so as to provide references for the revision and improvement of such drug instructions and the formulation of instructions for new drugs. METHODS The initial indicator framework was determined based on policy documents and literature related to instruction registration and revision. Two rounds of Delphi consultation were conducted among 25 experts to refine and optimize the indicator system. The analytic hierarchy process was employed to construct judgment matrices and obtain indicator weights. The comprehensive weights were converted into a 100-point scale to evaluate 11 instructions of oral Chinese patent medicines containing toxic decoction pieces from the medical institution of the research team. RESULTS The average questionnaire recovery rate of the two rounds of consultation was 96%. The expert authority coefficients were 0.87 and 0.88, respectively, and the Kendall’s W was statistically significant ( P <0.001). The final evaluation system comprised 4 first-level indicators (defect of toxicity identification, defect of dosage information, defect of risk warning, and defect of information guidance) and 24 second-level indicators (e.g., failure to label toxic decoction pieces in 【warnings】, failure to indicate all decoction piece compositions, absence of medication course specifications, etc.). The total scores of the 11 oral Chinese patent medicine instructions ranged from 15.50 to 50.87 points. The main clinical inapplicability issues included the absence of medication course specifications and the failure to provide warnings in items such as 【precautions】 for decoction pieces involving the “eighteen incompatibilities and nineteen mutual antagonisms”. CONCLUSIONS The constructed indicator system can meet the requirements for evaluating the clinical inapplicability of instructions for oral Chinese patent medicines containing toxic decoction pieces. All evaluated instructions exhibited certain clinical applicability defects. Pharmaceutical manufacturers should revise the instructions in accordance with policy requirements and clinical needs.
3.Clinical value of evaluation of anatomical position of inferior mesenteric artery and vein as well as left colic artery in laparoscopic radical resection of colorectal

Objective:To investigate the clinical value of laparoscopic evaluation of anato-mical position of inferior mesenteric artery (IMA), inferior mesenteric vein (IMV) and left colic artery (LCA).Methods:The prospective one-arm study was conducted. The clinical data of 229 pati-ents who underwent laparoscopic left hemicolectomy for left colon or laparoscopic radical resection of rectal cancer in The Second Affiliated Hospital of Air Force Medical University from December 2022 to December 2023 were selected. The distance between the origin point of IMA and the origin point of the first branch (L1) as well as the distance from the origin point of LCA root to the junction of LCA and IMV (L2) were measured during the operation. IMA classification, the location relation-ship of LCA and IMV junction were recorded. Observation indicators: (1) situations of enrolled patients; (2) difference analysis between L1, L2 and clinical features; (3) distribution characteristics of the location relationship between LCA and IMV in different types of IMA. Mann-Whitney U test was used for comparison of measurement data with skewed distribution between groups, Kruskal-Wallis H test was used for comparison between multiple groups, and Dunn-Bonferroni test was used for pairwise comparison. Comparison of count data between groups was performed by chi-square test. Pearson or Spearman correlation analysis was conducted for correlation of continuous variables. Results:(1) Situations of enrolled patients. A total of 229 eligible patients were screened out, including 146 males and 83 females, aged 64(range, 55-71)years. The height of 229 patients was 168(range, 160-172)cm, the weight was 65.0(55.5,71.5)kg, the body surface area was (1.68±0.17)m 2, the tumor maximum diameter was 3.0(2.5,4.0)cm. The total number of lymph nodes dissected was 19(17,21), and the number of No.253 lymph node dissected was 4(3,5). The L1 was 3.50(1.20,8.00)cm, and the L2 was 2.20(0.50,7.30)cm. There were 58, 31, 32, 71, 22, 90, 26 and 212 patients with smoking, alcohol drinking, diabetes, hypertension, coronary heart disease, neoadjuvant chemo-therapy, neoadjuvant radiotherapy and preservation of the LCA, respectively. Among 229 patients, cases with BMI <18.5 kg/m 2, 18.5-23.9 kg/m 2 and >23.9 kg/m 2 were 11, 133 and 85, respectively. There were 153 cases in pathological stage Ⅰ-Ⅱ and 76 cases in stage Ⅲ. There were 168 cases of Dixon operation, 6 cases of Miles operation and 55 cases of sigmoid colon resection. There were 135 cases of IMA type 1, 44 cases of IMA type 2, 23 cases of IMA type 3, 2 cases of IMA type 4, and 25 cases of IMA type unable to judge. (2) Difference analysis between L1, L2 and clinical features.Correlation analysis showed negative correlation between the height, body surface area and L1 ( r=-0.17, -0.15, P<0.05). The L1 was 3.20(2.68,4.00)cm for male patients and 3.60(3.00,4.20)cm for female patients, respectively, showing a significant difference between the two groups ( Z=-2.37, P<0.05). The L1 of patients with IMA type 1, 2, and 3 was 3.20(2.80,4.00)cm, 3.85(3.00,4.48)cm, and 3.20(2.50,4.30)cm, respectively, showing a significant difference among them ( H=7.54, P<0.05). Further pairwise com-parison showed that there was a significant difference in L1 between patients with IMA type 2 and those with IMA type 1 ( P<0.05). The L2 of smokers and non-smokers were 2.50(1.95,3.20)cm and 2.20(1.60,2.80)cm, respectively, showing a significant difference between the two groups ( Z=-2.24, P<0.05). (3)Distribution characteristics of the location relationship between LCA and IMV in different types of IMA. There was no significant difference in LCA distribution between the anterior and posterior positions of IMV among the three IMA types (type 1, 2, 3) ( χ2=1.63, P>0.05). Conclusions:Patients with greater height have larger body surface area and shorter L1. L1 is significantly longer in female patients than in male patients. L1 is significantly longer in patients with IMA type 2 than in those with type 1. L2 is significantly longer in smokers than in non-smokers. There was no significant difference in the distribution location between LCA and IMV among patients of IMA type 1, 2 and 3.

5.Primary large cell neuroendocrine carcinoma of bladder with urothelial carcinoma: a case report

Large cell neuroendocrine carcinoma of bladder is a rare malignant tumor with high degree of malignancy, strong invasiveness and poor prognosis. We reported a case of a 56-year-old man who underwent transurethral resection of bladder tumor because of bladder mass. Postoperative pathology revealed large cell neuroendocrine carcinoma of the bladder with urothelial carcinoma. Radical cystectomy was performed after postoperative chemotherapy, and there was no recurrence after 3 months of follow-up.
6.Construction and immunogenicity evaluation of candidate recombinant subunit vaccine for dengue virus

Dengue virus(DENV)diseases are a serious threat to human life and health,and there is no effective vaccine for DENV in China.In this study,a recombinant subunit candidate vaccine for DENV-2 was prepared.Using the ED Ⅲ gene of DENV-2 as the target gene,the recombinant eu-karyotic expression plasmid was constructed and transfected into suspension cells to express the target protein.The appropriate immune dose was screened to immunize mice and the immunogenic-ity was analyzed.The recombinant eukaryotic expression vector of DENV was successfully con-structed and transfected into HEK-293F to express the target protein,which was about 15 kDa in size.The mice were immunized with different doses of recombinant protein mixed with aluminum salt as adjuvant and the immunogenicity was analyzed.On the 35th day after the first immuniza-tion,the serum specific antibody levels of mice in the three protein immunization groups were sig-nificantly higher than those in the adjuvant group and PBS group,and the specific antibody levels of 10 μg D2EDⅢ protein group and 20 μg D2EDⅢ protein group were 1.43 and 1.56 times higher than those of 5 μg D2EDⅢ protein group,respectively.The antibody typing was biased to IgG1,in-dicating that the recombinant subunit candidate vaccine of DENV-2 prepared in this study mainly improved humoral immune response,but there was no significant difference in IgG1 antibody levels among the three protein immunization groups.Considering the immune effect and vaccine cost,the optimal target protein immunization dose per mouse was determined to be 10 μg.In this study,a recombinant subunit candidate vaccine of DENV-2 was prepared,which provides a refer-ence for the prevention of dengue virus.
7.Hemorrhagic shock caused by double aortic arch and aortoesophageal fistula in a newborn: a case report and literature review

Objective:To study the clinical characteristics of double aortic arch (DAA) combined with aortoesophageal fistula (AEF), and summarize the diagnosis and treatment experience.Methods:Retrospective analysis was performed on the diagnosis and treatment of a newborn with hemorrhagic shock caused by DAA combined with AEF in the Guangzhou Women and Children's Medical Center. The key searching words included "double aortic arch", "aortoesophageal fistula", "vascular ring", "newborn or neonate", and "infant, newborn". The relevant reports were retrieved from databases of CNKI, Wanfang, VIP, PubMed, Springer Link, Google Scholar, Web of Science, Embase, Cochrane Library and OVID, to summarize the clinical features, diagnosis and treatment experience of neonates with DAA and AEF. The retrieval deadline was December 31, 2020.Results:A full-term female newborn was hospitalized for dyspnea immediately after birth, and failed to evacuate from the ventilator for several times. The patient was fed with nasogastric tube and transferred to our hospital because of hemorrhagic shock occurring in 32 days after birth, and gastrointestinal bleeding occurred repeatedly with the maximum bleeding volume reaching 200 ml/time. DAA was diagnosed by cardiac ultrasound and CT, AEF hemorrhage was finally confirmed by gastroscopy, aortography and operation. DAA correction and esophagus repair were successfully performed, and the infant recovered well after the operation. At 9-month old, the infant grew and developed well. At present, no reports of DAA combined with AEF neonates have yet to be published in medical literatures in China. Seven English language literatures included 7 cases of AEF complicated with DAA in neonatal period, 5 cases survived and 2 cases died have so far been reported. All patients have a long history of gastric tube indwelling.Conclusions:The incidence of DAA combined with AEF is rare in the newborn with respiratory and swallowing difficulties as the first manifestation. The disease symptoms progressed rapidly, and life-threatening digestive tract hemorrhage may occur, which often requires surgical treatment. Prolonged gastric tube retention should be avoided in DAA children to prevent the occurrence of AEF.
8.Application of Surface Electromyography in Evaluation of Hand Function for Stroke Patients with Hemiplegia

Objective To apply the surface electromyography (sEMG) in the evaluation of hand motor function for patients with hemiplegia after stroke. Methods From January to August, 2018, 43 hemiplegic patients with hand dysfunction after stroke were selected. All the patients received comprehensive rehabilitation for four weeks. Their root mean square (RMS) and integrated electromyography (iEMG) of wrist flexor (WF), wrist extensor (WE), extensor digitorum (ED) and abductor pollicis brevis (APB) were measured before and after treatment, while they were assessed with Fugl-Meyer Assessment (FMA) and Brunnstrom's stages for the affected hand. Results The FMA score and Brunnstrom's stages, as well as the RMS and iEMG of WF, WE, ED and APB increased after treatment (Z > 2.496, t > 5.361, P < 0.05). The FMA score positively correlated with the RMS and iEMG of WF, WE, ED and APB (r = 0.423-0.732, P < 0.05). The Brunnstrom's stages positively correlated with the RMS of WE, ED and ABP (r = 0.343-0.467, P < 0.05) and the iEMG of WF, WE and ABP (r = 0.223-0.328, P < 0.05). Multiple linear stepwise regression drew the RMS of ED and iEMG of WE as the related factors for FMA (t > 4.243, P < 0.001), and RMS of ED as the related factors for Brunnstrom's stages (t = 4.707, P < 0.001). Conclusions EMG is an effective tool to evaluate the motor function of hand for stroke patient as an objective and quantitative assessment, especially those of WE and ED.
9. Clinical trial of simplified drilling method for dental implant site preparation

Objective:
To investigate the clinical effect of simplified drilling method and conventional drilling method in implants.
Methods:
A total of 46 patients (62 implants) were enrolled in this study that with dentition defect from May 2015 to May 2016 in the Implant department of Xi'an Jiao Tong University. The experimental group and the control group were randomly assigned according to the random number method, 23 cases in each group. The experimental group used the simplified drilling method (guided drill+ final drill), the control group using the conventional drilling method (step by step drill). The operation time, implant stability, marginal bone resorption rate and implant retention rate were compared between the two drilling methods.
Results:
The retention of the experiment group was 97% (31/32), the the control group was 100% (30/30). The operative time in the experiment group [(4.9±0.5) min] was significantly lower from the control group [(8.9±2.0) min] (
10.The evaluation of peri-implant bone defects with bioelectrical impedance in canine

Objective:To measure the bioelectrical impedance around the dental implants with bone defects.Methods:Bilateral maxillar second premolar were extracted in 6 native mongrel dogs,implants were placed immediately with or without bone defect of corresponding alveolar bene.Impedance value(IV)between the implant and contralateral mouth corner was measured by LCR TEST-ER.IVs were compared and analyzed by SPSS.Results:IVs of bone defect group were bigger than that of control group(P <0.05), in largger defect group were smaller than that in smaller group(P <0.05).Conclusion:Bioelectrical impedance can be used as an evaluation of peri-implant bone defects and the size of bone defect.
