2.Research advances in traditional Chinese medicine for the treatment of hepatocellular carcinoma by regulating immune cells

Hepatocellular carcinoma (HCC) is a common malignant tumor with a high mortality rate, an insidious onset, and complex pathological mechanisms. In the tumor microenvironment, tumor-promoting immune cells protect tumor cells from immune attacks, while dysfunction of anti-tumor immune cells causes the inhibition of immune response, thereby leading to the continuous deterioration of cancer. In recent years, traditional Chinese medicine has shown good efficacy in the treatment of HCC, and it can inhibit the proliferation and metastasis of cancer cells by regulating immune cells. By analyzing related articles in China and globally, this article summarizes how immune cells affect the progression of HCC through the immunosuppressive pathway and how traditional Chinese medicine exerts an anti-HCC effect by regulating immune cells, in order to provide theoretical basis and reference for optimizing the treatment of HCC.
3.Analysis of the Development and Implementation of the Environmental Damage Compensation System in Japan

In the 1950s and 1960s,Japan's implementation of policies prioritizing economic develop-ment caused a lack of effective supervision over the discharge of industrial wastewater and exhaust gases,which led to the occurrence of the"Four Major Pollution Diseases",including Minamata disease,causing serious social and public health problems.To more effectively address public nuisances and pro-vide compensation to victims,the Japanese government gradually established an environmental damage compensation system with administrative relief characteristics since the 1970s.Through long-term prac-tice and system optimization,this system has evolved into a mature institutional framework with a clear division of labor and efficient collaboration.This paper systematically reviews the development process of Japan's environmental damage compensation system and deeply analyzes its legal frame-work and supporting policies,aiming to provide useful references for the construction and improve-ment of China's environmental damage compensation system.Meanwhile,through the case analysis of Minamata disease,the paper explores the specific mechanisms and effects in the compensation practices,further revealing the system's operational characteristics and implications,and providing a reference ba-sis for the construction of China's environmental governance legal system.
4.Chinese herbal prescription combined with head acupuncture exercise therapy improves limb spasticity in rats with ischemic stroke

BACKGROUND:Chinese herbal prescription with head acupuncture exercise therapy has significant clinical efficacy in the treatment of limb spasticity after ischemic stroke;however,the exact mechanism is not clear.OBJECTIVE:To explore the possible mechanism of the combination of Chinese herbal prescription with head acupuncture exercise therapy in improving limb spasticity after stroke.METHODS:Seventy Sprague-Dawley rats were divided into blank,model,exercise therapy,head-acupuncture,Chinese herbal prescription,Chinese herbal prescription with head acupuncture exercise therapy,and sham-operation groups using the random number method,with 10 rats in each group.Except for the blank group and the sham-operation group,a rat model of cerebral ischemia/reperfusion injury was made by using the middle cerebral artery cable embolism method in the rest of the groups.Interventions in each group started on the 3rd day after modeling.In the Chinese herbal prescription group,rats were given 1.5 g/kg Yi'nao Fujian herbal prescription by gavage;in the head acupuncture group,acupuncture was applied to Baihui and 2 mm on the left and right sides of Baihui,and was left in place for 30 minutes;in the exercise therapy group,rats were exercised on a treadmill for 30 minutes;in the Chinese herbal prescription with head acupuncture exercise therapy group,1.5 g/kg Yi'nao Fujian herbal prescription was given by gavage followed by acupuncture treatment(identical to that in the head acupuncture group,and then the animals were exercised for 30 minutes on the treadmill while the acupuncture needles remained in place.Rats in the model and sham-operation groups were subjected to grasping under the same environmental conditions and placed on a rat immobilizer for 30 minutes,and the blank group did not have any intervention.The frequency of intervention in each group was once a day for 7 consecutive days.The neurological deficit score and muscle tone were evaluated in each group before and after treatment.Pathological changes in rat brain tissue were observed.Protein and mRNA expressions of glutamic acid decarboxylase 67,succinate semialdehyde dehydrogenase,and γ-aminobutyric acid transaminase in the cerebral cortex of the rats were detected.RESULTS AND CONCLUSION:(1)Compared with the model group,the Chinese herbal prescription with head acupuncture exercise therapy group had lower neurological deficit scores and modified Ashworth spasticity scale(MAS)muscle tone scores(P<0.01),decreased expression of succinate semialdehyde dehydrogenase andγ-aminobutyric acid transaminase at protein and mRNA levels,and increased expression of expression of glutamic acid decarboxylase 67 at protein and mRNA levels(P<0.01);the neuronal cell structure was relatively intact,with only some irregular cavities visible and reduced nuclear consolidation.(2)Compared with the Chinese herbal prescription with head acupuncture exercise therapy group,the neurological deficit scores and MAS muscle tone scores were higher in the exercise therapy group(P<0.05),and the MAS muscle tone scores were higher in the head-acupuncture group and the Chinese herbal prescription group(P<0.05,P<0.01),while the expressions of glutamic acid decarboxylase 67 protein and succinate semialdehyde dehydrogenase andγ-aminobutyric acid transaminase mRNAs were increased in all the other intervention groups(P<0.05,P<0.01).Neuronal cells in the Chinese herbal prescription with head acupuncture exercise therapy group were more orderly arranged and clearly outlined,with less nuclear consolidation and shape irregularity compared with the other intervention groups.(3)The neurological deficit score and MAS muscle tone score in the Chinese herbal prescription with head acupuncture exercise therapy group were significantly lower after treatment(P<0.01).To conclude,Yi'nao Fujian herbal prescription combined with head-acupuncture exercise therapy can improve post-stroke spasticity by affecting the expression of γ-aminobutyric acid transaminase in rats,which is more effective than Chinese herbal prescription alone,exercise therapy alone,and head-acupuncture alone.
5.Monitoring and countermeasures analysis of 367 medical device adverse events in a children's hospital

Objective To explore the impact and prevention strategies of medical device adverse events in a children's hospital,and to improve the monitoring and management capabilities of medical device adverse events.Methods We retrieved 367 medical device adverse events that occurred in a children's hospital from 2017 to 2022.Using Excel software,we analyzed,organized,and statistically summarized the annual reports of adverse events,the distribution of event grades,the age distribution of involved patients,and the main medical devices involved.Results The annual reports of medical device adverse events showed an overall upward trend.Grade Ⅲ and Ⅳ adverse events accounted for the majority,representing 97.28%of the total re-ports.The main age group affected was 0-3 years old,accounting for 43.60%of the total adverse event reports.Disposable ster-ile syringes,disposable vacuum blood collection tubes,disposable gastric tubes,disposable infusion sets(with needles),dispos-able intravenous catheters,and other polymer products accounted for a high proportion of adverse events.Adverse events related to infusion,nursing,and protective equipment also accounted for a high proportion.Conclusion Adopting measures such as continuous monitoring and analysis of medical device adverse event reports,improving the construction of adverse event systems,strengthening professional skill training for medical staff,and enhancing quality management are conducive to improving the man-agement level of medical device adverse events in hospitals and ensuring patient safety.
6.ED50 of 0.375% ropivacaine for supraclavicular brachial plexus block with L-shaped pressure baffle intervention based on cross sectional area

Objective To explore median effective dose(ED50)of 0.375%ropivacaine based on the cross sectional area(CSA)of supraclavicular brachial plexus block(SCBPB)with L-shaped baffle intervention.Methods Patients scheduled for upper limb surgery from September 2023 to May 2024 at Ningbo NO.6 Hospital were enrolled.Patients were randomly divided into two groups:L-shaped baffle compression group(group L)and non-compression group(group C).CSA of supraclavicular brachial plexus was measured by ultrasound,and 0.375%ropivacaine was administered based on the CSA.The ED50 was determined by using the Dixon up-and-down sequential method,with an initial dose of 0.4 ml/mm2 and an incremental difference of 0.04ml/mm2.If the block was effective within 30 minutes,the next patient received a lower dose;If ineffective,a higher dose was administered.The process continued until seven cross-over points(ineffective to effective)were observed.ED50 and its 95%CI were calculated by using the Probit method.Adverse reactions,such as phrenic nerve paralysis,nerve injury,dyspnea were recorded.Results The ED50 of 0.375%ropivacaine for SCBPB in group C was 0.254 ml/mm2(95%CI:0.228-0.278),while in group L,it was 0.239 ml/mm2(95%CI:0.215-0.262),with no statistically significant difference between two groups(P>0.05).The incidence of phrenic nerve paralysis in group L was significantly lower than that in group C(14.29%vs.41.67%,P<0.05).No significant nerve injuries,dyspnea,or local anesthetic toxicity were observed in either group.Conclusion The ED50 of 0.375%ropivacaine for SCBPB with L-shaped baffle compression,based on the CSA of the brachial plexus,was 0.239 ml/mm2(95%CI:0.215-0.262).L-shaped baffle compression significantly reduced the incidence of phrenic nerve paralysis without notable side effects.
7.Hemolysis rates of three red blood cell components at the end of storage: a 5-year retrospective study

Objective: To evaluate the suitability of the existing hemolysis rate standards for locally processed red blood cell components by retrospectively analyzing 5-year hemolysis rate data at the end of storage. Methods: A total of 720 blood samples of three types of red blood cell components from our blood station from January 2019 to December 2023 were collected. Parameters included hemoglobin concentration (Hb), hematocrit (Hct), and free hemoglobin concentration (fHb). Hemolysis rate were taken as the control standard of 0.8% in accordance with the national standard. The hemolysis rates were compared against the national standard threshold of 0.8% (GB18469-2012), and annual trends of the detection parameters were observed. Results: The hemolysis rates (x-+s,%) of leukocyte-depleted whole blood at the end of storage were (0.038±0.023 8) in 2019, (0.049±0.039 5) in 2020, (0.043±0.040 7) in 2021, (0.049±0.030 7) in 2022, and (0.058±0.054 8) in 2023, respectively; The hemolysis rates (x-+s" />,%) of leukocyte-depleted suspended red blood cells at the end of storage were (0.093±0.050 2) in 2019, (0.086±0.049 5) in 2020, (0.123±0.072 3) in 2021, (0.122±0.052 1) in 2022, and (0.106±0.058 6) in 2023, respectively; The hemolysis rates (x-+s,%) of washed red blood cells at the end of storage were (0.127±0.038 2) in 2019, (0.150±0.066 5) in 2020, (0.121±0.052 2) in 2021, (0.124±0.038 9) in 2022, and (0.128±0.044 3) in 2023, respectively. Conclusion: Hemolysis rates at the end of blood storage of three red blood cell components were significantly lower than the limits specified in Quality Requirements for Whole Blood and Components (GB18469-2012), as well as standards from the EU, AABB and the United States. The results demonstrate excellent product quality control. A regional internal control standard of <0.2% is proposed for hemolysis rates at the end of storage.

T7 RNA polymerase (T7 RNAP) is one of the simplest known RNA polymerases. Its unique structural features make it a critical model for studying the mechanisms of RNA synthesis. This review systematically examines the static crystal structure of T7 RNAP, beginning with an in-depth examination of its characteristic “thumb”, “palm”, and “finger” domains, which form the classic “right-hand-like” architecture. By detailing these structural elements, this review establishes a foundation for understanding the overall organization of T7 RNAP. This review systematically maps the functional roles of secondary structural elements and their subdomains in transcriptional catalysis, progressively elucidating the fundamental relationships between structure and function. Further, the intrinsic flexibility of T7 RNAP and its applications in research are also discussed. Additionally, the review presents the structural diagrams of the enzyme at different stages of the transcription process, and through these diagrams, it provides a detailed description of the complete transcription process of T7 RNAP. By integrating structural dynamics and kinetics analyses, the review constructs a comprehensive framework that bridges static structure to dynamic processes. Despite its advantages, T7 RNAP has a notable limitation: it generates double-stranded RNA (dsRNA) as a byproduct. The presence of dsRNA not only compromises the purity of mRNA products but also elicits nonspecific immune responses, which pose significant challenges for biotechnological and therapeutic applications. The review provides a detailed exploration of the mechanisms underlying dsRNA formation during T7 RNAP catalysis, reviews current strategies to mitigate this issue, and highlights recent progress in the field. A key focus is the semi-rational design of T7 RNAP mutants engineered to minimize dsRNA generation and enhance catalytic performance. Beyond its role in transcription, T7 RNAP exhibits rapid development and extensive application in fields, including gene editing, biosensing, and mRNA vaccines. This review systematically examines the structure-function relationships of T7 RNAP, elucidates the mechanisms of dsRNA formation, and discusses engineering strategies to optimize its performance. It further explores the engineering optimization and functional expansion of T7 RNAP. Furthermore, this review also addresses the pressing issues that currently need resolution, discusses the major challenges in the practical application of T7 RNAP, and provides an outlook on potential future research directions. In summary, this review provides a comprehensive analysis of T7 RNAP, ranging from its structural architecture to cutting-edge applications. We systematically examine: (1) the characteristic right-hand domains (thumb, palm, fingers) that define its minimalistic structure; (2) the structure-function relationships underlying transcriptional catalysis; and (3) the dynamic transitions during the complete transcription cycle. While highlighting T7 RNAP’s versatility in gene editing, biosensing, and mRNA vaccine production, we critically address its major limitation—dsRNA byproduct formation—and evaluate engineering solutions including semi-rationally designed mutants. By synthesizing current knowledge and identifying key challenges, this work aims to provide novel insights for the development and application of T7 RNAP and to foster further thought and progress in related fields.
9.Investigation on ventilator use and management in secondary and tertiary medical institutions in Sichuan Province

Objective To investigate the current situation of the ventilator use and management in secondary and tertiary medical institutions in Sichuan Province to provide references for management and training of ventilator users.Methods A questionnaire survey involving in 235 nurses from 13 secondary and tertiary medical institutions in Sichuan Province was conducted in terms of general information of the respondents,pipeline connection and parameter setup of ventilators,cleaning,sterilization and maintenance of ventilators,identification and treatment of ventilator alarms and hospital-acquired infection prevention and control.Results The results showed 20.4%of the respondents were nurses from critical care departments,and 79.6%of the respondents underwent the training for using the ventilator in their department;91.9%of the respondents could conduct ventilator self-testing by connecting the simulated lungs after the ventilator was switched on;66.4%of the respondents indicated that the initial parameters of the ventilator were set by the doctor;60.9%of the respondents proved that the performance monitoring and routine maintenance of the ventilator were carried out by the nurse;63.4%of the respondents showed disposable lines were commonly used in the departments.There were 89.8%of the respondents said the daily sterilization and management of the ventilator were performed by the nurse;46.0%of the respondents expressed the external surface of the ventilator was disinfected mainly with gamma disinfectant wipes;44.7%of the respondents indicated the external surface of the ventilator was sterilized every day;48.5%of the respondents said the internal airway of the ventilator was disinfected;57.0%of the respondents proved the disinfection was conducted after the expiratory flow sensors were used by any patient.There were 74.5%of the respondents that had paid attention to the ventilator waveform,and by the ventilator waveform only 21.3%were correct in determining whether there was secretion or fluid in the circuit and 22.1%in clarifying whether there was air leakage in the circuit.There were 59.1%of the respondents indicated that the closed suction tube was replaced once every 24 h;71.1%of the respondents could perform airbag pressure monitoring by the special pressure gauge.Conclusion Most of the nurses from secondary and tertiary medical institutions in Sichuan Province can use ventilators correctly,who have problems in disinfection specifications,infection prevention and control or recognition of ventilation waveform.It's suggested the training be strengthened to enhance the clinical nursing staffs in ventilator management.[Chinese Medical Equipment Journal,2025,46(3):75-80]

Bletilla striata polysaccharide(BSP)exhibits a wide range of pharmacological activities,including antibacterial,antitumor,antioxidant,immunomodulatory,and wound healing-promoting effects.In drug delivery systems,BSP can significantly enhance therapeutic efficacy while reducing adverse side effects,showing great potential in the treatment of cancer,infectious diseases,and tissue regeneration.This review systematically collected and analyzed recent studies from domestic and international literature databases concerning the pharmacological effects of BSP and its applications in drug delivery systems.The pharmacological properties of BSP and the progress in BSP-based drug delivery system development are comprehensively summarized.Owing to its unique pharmacological activities and excellent biocompatibility,BSP has been increasingly utilized in drug delivery platforms,often synergizing with targeted delivery,controlled release systems,and tissue repair technologies,thereby offering innovative strategies for the treatment of complex diseases.
