1.Primary Central Nervous System Lymphoma

Background:
Primary central nervous system lymphoma (PCNSL) is a rare form of extranodal non-Hodgkin lymphoma that is confined to the brain, spinal cord, leptomeninges, or eyes.
Case:
Our patient was a 52-year-old immunocompetent male who presented with progressive ataxia, dizziness, weakness and pain in both legs, imbalance, nausea, bladder/bowel retention and fever for over 18 months. After repetitive brain and spinal MRI, he was diagnosed with encephalomeningomyelitis and received four courses of steroid therapy, but his clinical symptoms continued to worsen. He was unable to undergo brain tissue analysis due to his persistent fever. We present a case of PCNSL based on the history, clinical presentation, repeated brain imaging, cerebrospinal fluid analysis, and clinical treatment.
Conclusion
Early diagnosis through accurate interpretation of imaging and timely management is crucial for improved outcomes in PCNSL.
2.Cerebral malaria: rare case

Background:
Malaria is caused by parasite of the genus Plasmodium and considered one of the biggest public health
issues because almost half of the world’s population is at risk of contracting malaria. It causes 2% of the world’s total
deaths and millions of clinical infections. In 2022, 94% of cases and 95% of deaths occurred in the WHO African Region.
Cerebral malaria the most severe neurological complication of infection with Plasmodium falciparum malaria. It is a
clinical syndrome characterized by coma and asexual forms of the parasite on peripheral blood smears. The neurological
complication, induced by cerebral malaria is irreversible and lethal, therefore it is of great significance to unravel its exact
etiology, which may be beneficial for the effective management of this severe disease.
In Mongolia, malaria normally not present unless the disease was contracted abroad. Considerable attention in malaria
control and elimination is needed, yet, increasingly, domestic hospitals are unfamiliar with it, and so there is a risk of
being overlooked. The following is the second case, to our knowledge, of cerebral malaria in Mongolia.
Conclusion
Although Mongolia is not a malaria-endemic region and routine malaria testing is not commonly performed,
the number of imported cases is increasing due to the growing mobility of the population—travel, study, tourism, and
peacekeeping missions to Africa and Southeast Asia. Therefore, neurologists should be aware of the possibility of cerebral
malaria when evaluating patients with neurological deficits who have a history of travel to malaria-endemic areas. This
case highlights important clinical, imaging, and laboratory considerations for suspecting and diagnosing malaria in such
patients.
3. ATHEROGENIC DYSLIPIDEMIA IN MONGOLIAN OLDER PEOPLE

Atherogenic dyslipidemia comprises a triad of increased blood concentrations of small, dense low density lipoprotein (LDL) particles, decreased high-density lipoprotein (HDL) particles, and increased triglycerides. A typical feature of obesity, the metabolic syndrome, atherogenic dyslipidemia has emerged as an important risk factor for cardiovascular disease. We have determined levels of serum lipid profiles in 1861 older people who lives 5 regions in Mongolia. The concentrations of total cholesterol, triglycerides and high density lipoprotein cholesterol (HDL-C) were measured using a biochemical reagents by biochemical fully automated analyzer. The levels of LDL-C were calculated by the Friedewald equation. Overall prevalence of dyslipidemia was 4.3% in men and 3.0% in women. Logistic regression showed that Odds ratio of the atherogenic dyslipidemia was OR=1.3, p=0.001 (CI 95% 0.93-2.47) for body mass index, OR=1.6, p=0.02 (CI 95% 1.0-2.88) for waist circumference, OR=1.76, p=0.03 (CI 95% 1.12-3.54) for waist hip ratio. Odds ratio of the atherogenic dyslipidemia was OR=0.98, p=0.001 (CI 95% 0.34-1.05) for gender and OR=1.0 p=0.001 (CI 95% 0.65-1.03) for age. Overall, 3.3% of older people had atherogenic dyslipidemia and 4.3% of men and 3.0% of women had atherogenic dyslipidemia. An increase of physical parameters are getting a risk factor of atherogenic dyslipidemia.
4. PHARMACOLOGICAL EFFECTS OF SILLICHOL ON BILE SECRETION AND

Varieties of plants and lyophilized bovine bile have been used for increase secretion of bile in traditional systems of medicine of various countries. Following many articles note on the benefi cial effects of lyophilized bovine bile particularly on the wound healing and gastric protection effects, there is paucity of reports in literature on its effects on a bile secretion, a bile bilirubin, bile cholesterol and a plasma cholesterol levels. Sillichol contains lyophilizedbovine bile, liver hydrolisate, yarrow extract and silymarin. The aim of this study was to find out the effect of bile fl ow, bile bilirubin concentration bile cholesterol level and hepatoprotective of Sillichol. Sixteen adult male wistar rats (weighing between 200-250 gr) were used in the study. They were randomly assigned into control and sillichol group comprising 4 in each group. Thereafter, they were weighed and anaesthetized with ketamine (2ml/200gr body weight) muscle leg and quickly pinned to a dissecting board. Laparotomy was performed and liver lobes were defl ected anterolaterally to expose the common bile duct. The common bile duct was cannulated with a portex cannula (0.5 mm diameter) after a semitransection was made on the bile duct. The cannuls was held in place with thread tied over it and around the bile duct.The bile was collected for 8 hours from each rat studied according to method of Rozuet Jousse. The rate of bile fl ow was noted, the volume of bilirubin, bile cholesterol levels were determined in the control and test groups. Moreover, total of 18 wistar rats (200-250 gr) were obtainedfrom laboratory house of Drug research institute and acclimated for 10 days before starting the experiment. Liver toxicity was induced by the subcutaneous injection of carbon tetrachloride (CCL4, 0.4 ml/100gr), 1:1 diluted with paraffi n oil, for four successive days of the experiment (N.P.Scakun et al, 1983). The rats were divided randomly into 3 groups comprising 6 rats in each group and fed the same diet throughout the experimental period. Mean values of bile cholesterol and bilirubinlevels, rate of bile secretion in the control and sillichol group. Bile cholesterol levels were signifi cantly decreased in the sillichol group compared with the control group (60.3±0.88 mg/dl vs 62.6±1.21mg/dl, p<0.05). Rate of bile secretion was signifi cantly increased in the experimental compared with the control group(10.21±0.25 ml/8hr vs 4.18±0.25 ml/8hr, p<0.05). Total bilirubin, conjugated and unconjugatedbilirubin concentrations in both sillichol and control groups were not signifi cantly different (p<0.01). The activities of GOT, GPT and ALP were estimated in serum samples as the liver function biomarkers using biochemical diagnostic test. The CCL4 treatment markedly affected the liverspecifi c enzymes. It was found that a signifi cant (p<0.05) increase in serum GOT, GPT and ALP activities of CCL4 treated rats. After the treats, hepatic biomarkers were elevated in the serum due to release of the enzymes from damaged liver. GOT (69.8±1.5), GPT (103.9±1.2), ALP (23.8±0.2) and Cholesterol (67.7±13.6) andtriglyceride (64.0±3.3) levels weredecreasedsignifi cantly (p<0.05) in the sillichol groupcompared with the control group. Silichol is decreasing concentration of cholesterol and bilirubin’s level in bile, constantly after administration of drug. Also, liverpreparation is increasing bile acid secretion in hepatocytes and a speed of secretion.From the results of pharmacological study concluded that involves CCL4 induced acute toxic hepatitis, liver preparation has hepatoprotective effect by protecting the liver cells from injury, improving the regeneration process and by correcting metabolic functions of the liver.
5. Correlation between the blood glucose level and food consumption of Elder people

The increasing proportions of aged persons have been accompanied in the world. NCDs are often associated with older age groups. High blood glucose levels and unhealthy diet increase the risk of or cause most NCDs. In this study we aimed to determine correlation between the older people (60<) blood glucose level and food consumption. 1563 healthy elder people participated in this research. We measured blood glucose level in all subjects at the Nursing school’s Training and Research Center of health science university of Mongolia. Ulaanbaatar city, Orkhon aimag, Khovd aimag, Khentii aimag, Bulgan aimag, Dornogovi aimag, Tov aimags represented urban areas, while the rest of aimags and soums represented rural areas. The questionnaire was used to collect data on respondent’s social-economic status, fruit and vegetable consumption, physical activity, and their causes. In order to assess the diet pattern of the surveyed population, the respondents were asked about frequency of fruit and vegetable consumption, type of oil used in food, and amount of salt consumed daily. Simple regression analysis was performed to shown that significantly positive correlations between blood glucose and salt intake (р<0.001), The other composition are no significantly changes.
6. DEGREE OF CORONARY ATHEROSCLEROSIS IS ASSOCIATED WITH CHANGING CIRCULATING LEVELS OF ENDOTHELIN RECEPTOR TYPE A IN MONGOLIAN ADULTS

Background:The cardiovascular disease especially coronary artery disease is the leading cause of mortality in worldwide. There is lack of research study which evaluated stenosis of coronary atherosclerosis. It is known that coronary stenosis is highly connected to the levels of biomarkers. Coronary atherosclerosis correlated with endothelin receptor type A (EDNRA) levels in a group of patients suspected of having coronary artery disease.Objective:The goal of this study was to evaluate the relation between the coronary atherosclerosis and levels of EDNRAMethods:A total of 311 participants were involved in this study. A case-control study was used in the study. The baselines data were collected from the department of Angiography at the National Third Central Hospital and National lilood Transfusion and Research center. We have determined the degree of coronary atherosclerosis using the Angiography machine and Elisa were used for detecting the blood endothelin levels in all groups.Results:98 participants were diagnosed with stenosis and occlusion. The blood endothelin levels were estimated to 6.32±0.64 pg/ml which refer to () degree of coronary stenosis, the first degree of stenosis of coronary atherosclerosis is estimated to 5.56±0.22pg/ml, the second degree of stenosis of coronary atherosclerosis is estimated to 5.42J0.34 pg/ml, the third degree of stenosis of coronary atherosclerosis is estimated to 5.87 H). 13 pg/ml, the fourth degree of stenosis of coronary atherosclerosis is I'SlilllSltll lo 5.69±0.09 pg/ml, it was estimated to 5.88±0.13 pg/ml in control groups. Level of EDNRA (occupying one segment of coronary artery) was estimated to 5.77±0.08 pg/ml. two segment involvement was estimated to 5.72±0.16 pg/ml, three segment involvement 5.73±0.19 pg/ml. four segments involvement 5.50±0.25 pg/ml, respectively.Conclusion:The blood endothelin level and coronary artery stenosis were not correlated statistically significant in control group of the study. However, blood endothelin levels were increased in patients who tend to experience the coronary artery stenosis.
7.Diagnostic Issues Of Shoulder Complaints

Shoulder pain is a common complaint in modern societies which affects on functional abilities and quality of life. Long timefor shoulder pain the diagnosis " shoulder joint periarthritis" was explaining all disorders of shoulder. Modern studies of functional anatomy of shoulder area requires new vision of the problem and new attitude of the diagnosis and treatment. In International classification of diseases - 10 there is classification of shoulder disorders in terms of functional anatomy. Later studies of shoulder disorders described it as three groups of disorders as synovial disorders, shoulder girdle disorders and combination diagnosis. There are examination test for identification of shoulder disorder topography. In Mongolia the shoulder pain is not studied enough, there is no guidelines on diagnostics and treatment. Therefore we did this literature review to find out modern tendencies of shoulder disorder diagnostic approaches which could become a basis for shoulder examination guideline.
