1.Key Information Research and Modern Clinical Application of Xiaofengsan

Employing bibliometric methods and adhering to principles of textual research, this study systematically investigated prescription source, formula name, composition evolution, dose evolution, origin, processing, ancient and modern applications of Xiaofengsan. Xiaofengsan, also known as Renshen Xiaofengsan and Chantui Xiaofengsan, was first recorded in the Taiping Huimin Hejijufang(hereafter referred to as Jufang) of the Southern Song dynasty. The formula composition included Schizonepetae Spica, Glycyrrhizae Radix et Rhizoma, Chuanxiong Rhizoma, Notoptery Rhizoma et Radix, Bombyx Batryticatus, Saposhnikoviae Radix, Poria, Cicadae Periostracum, Pogostemonis Herba, Ginseng Radix et Rhizoma, Magnoliae Officinalis Cortex and Citri Reticulatae Pericarpium, a total of 12 medicinal materials. In terms of the evolution of formula composition, formulas across dynasties largely aligned with those recorded in Jufang, with only minor variations in application. The results of the formula dosage research indicated that one dose of medication in Jufang corresponded to the following modern dosages:Schizonepetae Spica of 82.6 g, Glycyrrhizae Radix et Rhizoma of 82.6 g, Chuanxiong Rhizoma of 82.6 g, Notoptery Rhizoma et Radix of 82.6 g, Bombyx Batryticatus of 82.6 g, Saposhnikoviae Radix of 82.6 g, Poria of 82.6 g, Cicadae Periostracum of 82.6 g, Pogostemonis Herba of 82.6 g, Ginseng Radix et Rhizoma of 82.6 g, Magnoliae Officinalis Cortex of 20.65 g and Citri Reticulatae Pericarpium of 20.65 g, the origins of all the constituent drugs were consistent with the 2020 edition of Pharmacopoeia of the People's Republic of China. The results of the investigation into the decoction method indicated that the aforementioned drugs should be finely ground into powder(pass through the No.5 sieve), and 8.26 g was taken for each dose, which was taken with the clear liquid obtained by steeping tea leaves in boiling water for several minutes. This mixture was administered three times daily, 30 min after meals. The ancient functional indications of this formula mainly involved dispelling wind-heat, eliminating pathogenic factors and regulating the middle Jiao. It primarily treated all wind-heat syndromes manifesting as skin diseases, predominantly affecting the upper body, especially the head and face. The diseases involved in modern applications were mostly dermatological diseases, including urticaria, eczema, atopic dermatitis and others. In this paper, by combing the relevant ancient literature, the key information of Xiaofengsan was textual researched, in order to provide reference for the modern application and development of this formula.
2.Parecoxib sodium alleviates pain in rats with femoral fractures by modulating TLR4/p38MAPK pathway

Objective To investigate the effects of parecoxib sodium on pain and Toll-like receptor 4(TLR4)/p38 mitogen activated protein kinase(p38MAPK)pathway in rats with femoral fracture.Methods Sixty rats were randomly separated into sham operation group,model group,TLR4 inhibitor(TAK-242)group(3 mg/kg),parecoxib sodium group(10 mg/kg),and parecoxib sodium+TLR4 activator lipopolysaccharide group(10 mg/kg parecoxib sodium+15 mg/kg LPS),with 12 rats in each group.Except for the sham operation group,rats in all other groups were used to establish a femoral fracture model by transverse cutting of the mid femur.After 28 days of treatment in each group,X-rays were used to detect the degree of fracture healing in rats.The mechanical pain threshold(PWMT)and thermal pain threshold(PWTL)of rats were measured.ELISA was applied to detect the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and IL-10 in serum.Micro-CT method was applied to detect changes in femoral bone density(BMD),bone volume fraction(BV/TV),trabecular thickness(Tb.Th),and trabecular quantity(Tb.N)in rats.HE staining was applied to observe the pathological changes of bone tissue at the fracture site in rats.Western blot was applied to detect the expression of TLR4/p38MAPK pathway related proteins in bone tissue at the fracture site.Results Compared with the sham operation group,the fracture lines of rats in the model group were obvious,with a small amount of callus growth,the PWMT,PWTL,femoral BMD,BV/TV,Tb.Th,and Tb.N decreased,while the levels of serum IL-6,TNF-α,and IL-10,and the levels of TLR4 and p-p38MAPK/p38MAPK proteins in the bone tissue at the fracture site increased(P<0.05).Compared with the model group,the fracture lines in the TAK-242 group and the parecoxib sodium group were blurred,and there was an increase in callus growth,the PWMT,PWTL,femoral BMD,BV/TV,Tb.Th,and Tb.N,the serum IL-10 level increased,while the serum IL-6,TNF-αlevels,the TLR4 and p-p38MAPK/p38MAPK protein levels in bone tissue at the fracture site decreased(P<0.05).LPS could upregulate the phosphorylation levels of TLR4 and p38MAPK,and weaken the relieving effects on anti-inflammatory and pain of parecoxib sodium on fracture rats.Conclusion Paracoxib sodium may alleviate pain and accelerate fracture healing in rats with femoral fractures by inhibiting the TLR4/p38MAPK pathway and suppressing inflammatory responses.

The above herbs are ground into a fine powder with a particle size passing through a No. 5 sieve. For each dose, take 8.26 g of the powdered formula, add 300 mL of water, along with 3 g of Zingiberis Rhizoma Recens and 3 g of Jujubae Fructus, and decoct until reduced to 140 mL. The decoction should be administered hot, with three times daily. To induce sweating, the patient should be kept warm under a quilt, and an additional dose should be prepared and taken if needed. This formula is traditionally used to relieve the exterior and resolve dampness, regulate Qi and harmonize the middle, which is mainly used to treat a series of diseases of digestive and respiratory systems. However, potential adverse reactions, including allergies, purpura and disulfiram-like reactions, should be considered during clinical use. Huoxiang Zhengqisan features a rational composition, extensive clinical application, and strong potential for further research and development.

5.Clinical manifestations and prognostic analysis of four patients with thyroid peroxidase gene mutations

Objective:To examine the clinical features and genetic profiles of patients with thyroid peroxidase(TPO) gene mutations and provide diagnostic guidance for clinicians.Methods:A retrospective review of four patients with TPO mutations treated at Ruijin Hospital, Shanghai Jiao Tong University School of Medicine, from January 2014 to December 2023. Data on demographics, clinical manifestation, genotypes, treatment, and prognosis of these patients were analyzed.Results:Two males and two females, aged 13 to 27 years at diagnosis, presented with goiter as the initial symptom, with three cases menifesting during puberty. Laboratory findings showed mildly elevated or upper-limit-normal serum thyroid-stimulating hormone(TSH) levels, significantly increased free triiodothyronine/free thyroxine(FT 3/FT 4) ratios, and elevated thyroglobulin(TG) levels. Genetic testing revealed compound heterozygous pathogenic or likely pathogenic TPO mutations. Despite regular levothyroxine(L-T 4) therapy, goiter persisted. Three patients required thyroidectomy due to cosmetic or compressive symptoms, with pathology showing follicular hyperplasia. Conclusion:TPO mutations are characterized by adolescent-onset goiter, elevated FT 3/FT 4 ratios, and normal to high TSH. Genetic testing confirms the diagnosis. L-T 4 offers limited improvement, and surgery is often needed.

8.Thyroid dosimetry evaluation of two intensity-modulated radiotherapy techniques after modified radical mastectomy for breast cancer

Objective To compare the dosimetric characteristics of thyroid between intensity-modulated radiotherapy (IMRT) and volumetric modulated arc therapy (VMAT) in patients with left-sided breast cancer after modified radical mastectomy. Methods Thirty patients with left-sided breast cancer who underwent adjuvant radiotherapy after modified radical mastectomy at the Huangpu Branch of the Ninth People’s Hospital, Shanghai Jiao Tong University School of Medicine, from December 2021 to May 2023, were selected as the study subjects, and IMRT and VMAT were used with a target prescription dose of 50 Gy/25 times. The target dose parameters of the two plans, including the mean dose (Dmean) and the dose-volume limit for organs at risk (Vx, x represents dose), were compared. Results The conformity index of VMAT was higher than that of IMRT (P<0.001). Compared with VMAT, IMRT plan significantly reduced the V5、V10、Dmean of heart, the V5 of right lung and the Dmean of right breast, while it significantly increased the V20 of left lung (P<0.001). There was no statistically significant difference in V5 and V10 of the left thyroid between IMRT and VMAT. However, the V20, V30, V40, and Dmean of left and whole thyroid, and the V10 and Dmean of right thyroid were significantly lower in IMRT than those in VMAT (P<0.001). Conclusions Both IMRT and VMAT plans can meet the clinical dosimetric requirements, among which IMRT has lower thyroid exposure dose and is more suitable for patients with high cardiac requirements.

Osteoporosis is an emerging threat characterized by systemic damage to bone mass and microarchitecture leading to fragility fractures.Exosomes are nanosized vesicular particles secreted by cells into the extracellular compartment with biological activities similar to those of their cell of origin and play an important role in intercellular communication processes.Exosomes from multiple cell sources are involved in the regulation of bone-related cell proliferation and differentiation during bone metabolism,and have the advantages of high stability,non-immunogenicity and strong targeting ability,which make up for the shortcomings of traditional drug and stem cell therapies.Exosomes secreted from bone marrow mesenchymal stem cells(MSCs)can promote bone regeneration and improve morphology,biomechanics and histological damage,and exosomes derived from bone marrow mesenchymal stem cells(BMSCs)in the mechanical microenvironment are more effective in inducing osteogenesis,significantly enhancing the osteogenic effect of BMSCs and promoting bone regeneration.Therefore,this article provides a review on the mechano-sensitivity of MSCs,mechanical responsive functionalized exosomes of MSCs,and explores their potential role in the treatment of osteoporosis.
10.Nutritional status of type 2 diabetic patients after gastric bypass and its influencing factors

Objective To investigate the nutritional status of patients with type 2 diabetes after gastric bypass and its influencing factors.Methods The clinical data of 129 patients with type 2 diabetes who underwent gastric bypass in Shanghai Sixth People's Hospital from May 2018 to April 2021 were retrospectively analyzed.The nutritional status of the patients was evaluated by the control the nutritional status(CONUT)scoring 3 months after surgery.According to the nutritional status,they were divided into malnutrition group and normal nutrition group.Multivariate logistic regression analysis was used to assess the influencing factors of malnutrition 3 months after gastric bypass in patients with type 2 diabetes mellitus.Results Three months after gastric bypass,36 patients(27.91%)were cured,68 patients(52.71%)were relieved,and 25 patients(19.38%)were ineffective,with a total effective rate of 80.62%.Of the 129 patients,68(52.71%)had normal nutrition and 61(47.29%)had malnutrition,including 45 cases(73.77%)of mild malnutrition,13 cases(21.31%)of moderate malnutrition,and 3 cases(4.92%)of severe malnutrition.The age and course of disease in the malnutrition group were higher than those in the normal nutrition group(P<0.05),while the circumference of the upper arm,triceps skinfold thickness,preoperative hemoglobin(Hb),glycosylated hemoglobin and triacylglycerol(TG)levels in the malnutrition group were lower than those in the normal nutrition group(P<0.05).Multivariate logistic regression analysis showed that age,course of disease,preoperative Hb and TG were influencing factors of malnutrition in type 2 diabetic patients 3 months after gastric bypass(P<0.05).Conclusion Gastric bypass is effective in patients with type 2 diabetes,but postoperative nutritional status still needs to be improved.Age,course of disease,preoperative Hb and TG levels will affect the nutritional status of patients with type 2 diabetes after gastric bypass.
