OBJECTIVE To evaluate the efficacy and safety of first-line treatments regimens for human epidermal growth factor receptor 2 (HER2)-positive advanced gastric cancer. METHODS Computer searches were conducted on databases such as Web of Science, Embase, CNKI, and VIP in both Chinese and English, and abstracts of papers from the annual meetings of the European Society of Oncology and the American Society of Clinical Oncology were screened. Collected randomized controlled trials (RCTs) on first-line treatment for HER2-positive advanced unresectable or metastatic gastric cancer or gastroesophageal junction adenocarcinoma patients, with a retrieval period from database establishment until April 1, 2025. A network meta-analysis was conducted by two researchers who independently screened literature, extracted data, and evaluated the risk of bias in the study. RESULTS A total of 5 RCTs involving 2 797 patients and encompassing 6 treatment regimens were ultimately included. In the assessment of overall survival, there was a trend towards survival benefit for trastuzumab combined with chemotherapy (Tra_chemo), pertuzumab combined with trastuzumab and chemotherapy (Per_Tra_chemo), high-dose trastuzumab combined with chemotherapy (TraHD_chemo), lapatinib combined with chemotherapy (Lap_chemo), and pembrolizumab combined with trastuzumab and chemotherapy (Pem_Tra_chemo) compared to chemotherapy alone (chemo); however, the differences were not statistically significant (P>0.05); the top two treatment regimens in terms of the surface under the cumulative ranking curve (SUCRA) were Pem_Tra_chemo (77.8%) and TraHD_chemo (74.2%). For progression-free survival, there was statistical significance between Per_Tra_chemo and chemo, Pem_Tra_chemo and chemo (P<0.05); the top two treatment regimens in terms of SUCRA were Per_Tra_chemo (83.0%) and Pem_Tra_chemo (82.8%). Regarding objective response rate, there was statistical significance between Pem_Tra_chemo and chemo (P<0.05); the top two treatment regimens in terms of SUCRA were Pem_Tra_chemo(87.4%)and Per_Tra_chemo(72.2%). In terms of safety, there was statistical significance in the incidence of any level of adverse events between Lap_chemo and chemo (P<0.05); the top two treatment regimens in terms of SUCRA were chemo(87.1%)and Pem_Tra_chemo(53.8%). Lap_chemo exhibited a higher incidence of grade ≥3 adverse events compared to chemo, and Per_Tra_chemo showed a higher incidence compared to Tra_chemo (P<0.05); the top two treatment regimens in terms of SUCRA were Tra_chemo (79.0%) and chemo (77.6%). CONCLUSIONS In the first-line treatment of HER2-positive advanced gastric cancer, Pem_Tra_chemo and Per_Tra_chemo regimens have relatively good efficacy, but the safety risks are relatively high, requiring close attention and whole- process management.

Objective To investigate the expression of neutrophil elastase (ELANE) mRNA in peripheral blood mononuclear cells (PBMCs) under different infection states of Mycobacterium tuberculosis (MTB), and analyze the feasibility of ELANE as a diagnostic marker for MTB infection. Methods PBMCs were isolated from peripheral blood of active tuberculosis patients (ATB), latent infection of Mycobacterium tuberculosis (LTBI) and healthy control (HC) Luanzhou People's Hospital, Hebei Province, February-November 2023 and cell RNA was extracted. After reversing into cDNA, the difference of ELANE mRNA level in PBMCs in each group was detected by fluorescence quantitative PCR. The whole cell lysate of Mycobacterium tuberculosis H37Rv was used to stimulate PBMCs in each group, and total RNA was extracted. Fluorescence quantitative PCR was also used to detect the expression differences of ELANE gene before and after cell stimulation in each group; these expression differences were analyzed for identifying the feasibility of ATB. Results The expression of ELANE gene in peripheral blood mononuclear cells of ATB patients was significantly higher than that of LTBI and healthy individuals (t=4.550, P<0.001; t=4.540, P<0.001). ROC analysis showed that the area under the curve for ELANE differential diagnosis of ATB and LTBI, ATB and HC were 0.802 and 0.805, respectively. H37Rv whole cell lysate can upregulate the expression of ELANE gene in PBMCs of ATB patients, with a significant difference before and after stimulation (t=4.717, P<0.001). However, there was no significant difference in ELANE gene expression in PBMCs of LTBI and HC populations before and after H37Rv bacterial lysate stimulation. Conclusion Compared with LTBI and healthy individuals, the expression level of ELANE mRNA in peripheral blood mononuclear cells of ATB was significantly upregulated; After stimulation with MTB whole cell lysate antigen, the expression level of ELANE in PBMCs of ATB population significantly increased. ELANE can serve as a diagnostic biomarker for active pulmonary tuberculosis to differentiate ATB from LTBI and healthy individuals.

Objective:To study the characteristics of metabolites in lower resPiratory tract between coal workers' pneumoconiosis patients and dust exposure patients, and compare the differences of metabolites and their main metabolic pathways. Methods:From December 2020 to February 2021, through a prospective cross-sectional study, a total of 26 patients with coal workers' pneumoconiosis (metabolic group of coal workers' pneumoconiosis) were selected from the bronchoalveolar lavage treatment of coal workers' pneumoconiosis and dust exposure in the Respiratory and Critical Care Medicine Department of the 920th Hospital of the Joint Logistics Support Force during the same period. With 19 cases of dust exposure as the control group (dust exposure metabolic group), samples of alveolar lavage fluid were collected from 2 groups. Metabolites of the two groups were quantitatively analyzed by metabonomics technology, and the characteristics of metabolites and their metabolic pathways were compared. The metabolites with potential predictive value were screened by receiver operating characteristic curve (ROC curve) .Results:Through metabolomic analysis of alveolar lavage fluid in the coal workers' pneumoconiosis group and the dust contact group, a total of 28 different metabolites were screened, including trihydroxybutyric acid, alanine, ethanolamine, L-osan, proline (carboxyl), leucine, 2-hydroxyglutaric acid, proline, lactic acid, serine, valine and threonine in the coal workers' pneumoconiosis group. The levels of differential metabolites such as ornithine, isoleucine, threitol, glucose and lysine were higher ( P<0.05). The levels of different metabolites such as sarcoine, pelanoic acid, palmitic acid, heptadecanoic acid, n-butylamine, tetradecanoic acid, isobutylamine, aminoadipic acid, phosphate, uracil and cytosine were higher in the dust exposure group ( P<0.05). Two major metabolic pathways include glycine, serine and threonine metabolism, arginine and proline metabolism, biotin metabolism, and aminoacyl biosynthesis metabolism. Among the 17 metabolites increased in the coal workers' pneumoconiosis group, the AUC of threitol and lactic acid was greater than 0.8, and the specificity and sensitivity of the working characteristic curves of the two metabolites were 80% and 70%, respectively. Conclusion:There were significant differences in the metabolites of lower respiratory tract between patients with coal workers' pneumoconiosis and those exposed to dust, and the differences were related to multiple metabolic pathways. Threitol and lactic acid may have potential predictive value for pneumoconiosis.

Objective:To investigate the potential characteristic manifestations and application value of the Dynamic Visual Acuity Test(DVAT) in vestibular migraine(VM). Methods:A total of 50 VM patients(case group) and 50 healthy subjects(control group) diagnosed at the Department of Otorhinolaryngology Head and Neck Surgery, First Hospital of Shanxi Medical University between November 1, 2023, and December 31, 2024, were enrolled. The case group underwent DVAT, video head impulse test(vHIT), caloric test, and Dizziness Handicap Inventory(DHI) assessment, whereas the control group only received DVAT. Group-based analyses were conducted to examine the effect of age on Dynamic Visual Acuity Loss(DVALoss), as well as the correlations of DVALoss with vestibular function tests and DHI scores. Results:DVALoss in the case group was significantly higher than that in the control group(P<0.001). In both groups, age was significantly and positively correlated with DVALoss(P<0.001). Within the case group, DVALoss was strongly and positively correlated with DHI scores(r=0.807, P<0.001); it was negatively correlated with the vestibulo-ocular reflex(VOR) gain in vHIT, though without clinical significance, and showed no significant association with the caloric test. Age and DVALoss collectively accounted for 71.3% of the variance in DHI scores(R²=0.713), with age exerting a relatively minor actual impact. Conclusion:DVAT can sensitively identify the core functional impairments of VM. DVALoss, as a direct functional reflection of the pathological mechanism of VM, is strongly correlated with DHI scores. Incorporating DVALoss into standardized assessments may provide an objective basis for the diagnosis and management of VM.

Acute respiratory distress syndrome (ARDS) is the leading cause of respiratory failure with high morbidity and mortality. Pulmonary surfactant (PS)-based complementary therapies have exhibited potential for ARDS healing and applied as an adjunctive therapy strategy. Coacervate (Coac) has the characteristics of softness, deformability and excellent molecular enrichment properties, and has attracted extensive attention in the biomedical field. Here PS and coacervate were combined for the potential ARDS treatment. The Coac, fabricated from polyallylamine hydrochloride (PAH) and adenosine triphosphate (ATP) by simple mixing, exhibited soft droplet property and high enrichment for dexamethasone sodium phosphate (DSP). To avoid the fusion effect of membraneless coacervate and endow it with biological functions of PS, liposomes with PS-biomimetic lipid components (PS-lipo) were further introduced to construct PS-biomimetic membranized coacervate (DSP@PS-Coac). The DSP@PS-Coac demonstrated high lung targeting effect and significant penetration efficiency after intravenous injection. Furthermore, PS-lipo replenished the endogenous PS pool and facilitated the distribution of DSP in inflammatory cells in the lung. In the ARDS mouse model, PS-Coac and DSP exerted synergetic anti-inflammatory functions, via reducing the recruitment of inflammatory neutrophils and modulating macrophages into anti-inflammatory phenotype. The overall results confirmed that DSP@PS-Coac may provide a promising delivery option for the treatment of ARDS.
