Mycophenolic acid (MPA), the active moiety of both mycophenolate mofetil (MMF) and enteric-coated mycophenolate sodium (EC-MPS), serves as a primary immunosuppressant for maintaining solid organ transplants. Therapeutic drug monitoring (TDM) enhances treatment outcomes through tailored approaches. This study aimed to develop an evidence-based guideline for MPA TDM, facilitating its rational application in clinical settings. The guideline plan was drawn from the Institute of Medicine and World Health Organization (WHO) guidelines. Using the Delphi method, clinical questions and outcome indicators were generated. Systematic reviews, Grading of Recommendations Assessment, Development, and Evaluation (GRADE) evidence quality evaluations, expert opinions, and patient values guided evidence-based suggestions for the guideline. External reviews further refined the recommendations. The guideline for the TDM of MPA (IPGRP-2020CN099) consists of four sections and 16 recommendations encompassing target populations, monitoring strategies, dosage regimens, and influencing factors. High-risk populations, timing of TDM, area under the curve (AUC) versus trough concentration (C₀), target concentration ranges, monitoring frequency, and analytical methods are addressed. Formulation-specific recommendations, initial dosage regimens, populations with unique considerations, pharmacokinetic-informed dosing, body weight factors, pharmacogenetics, and drug‍-‍drug interactions are covered. The evidence-based guideline offers a comprehensive recommendation for solid organ transplant recipients undergoing MPA therapy, promoting standardization of MPA TDM, and enhancing treatment efficacy and safety.

Objective To investigate the effect of combination of rope-assisted proprioceptive neuromuscular facilitation(PNF)training and rope-assisted brain-computer interface(BCI)training on upper limb function in stroke patients with hemiplegia. Methods From March,2022 to February,2023,96 inpatients with stroke hemiplegia from the Second Affiliated Hospital of Guangxi Medical University were randomly divided into conventional group(n=32),PNF group(n=32)and combined group(n=32).All the groups received routine rehabilitation treatment.The conventional group re-ceived upper limb PNF training,the PNF group received upper limb rope-assisted PNF training,and the com-bined group received both upper limb rope-assisted PNF training and upper limb rope-assisted BCI training,for four weeks.They were assessed with Functiongal Test for the Hemiplegic Upper Extremity-Hong Kong version(FTHUE-HK),Fugl-Meyer Assessment-Upper Extremities(FMA-UE)and modified Barthel Index(MBI)before and after treatment. Results The intra-group effect(F>341.219,P<0.001),inter-group effect(F>21.705,P<0.001)and interaction effect(F>3.171,P<0.05)were significant in the scores of FTHUE-HK and MBI.The intra-group effect(F=520.472,P<0.001)and inter-group effect(F=41.939,P<0.001)were significant in the scores of FMA-UE,and the interaction effect was not(P>0.05).After treatment,the FTHUE-HK,FMA-UE and MBI scores were the best in the combined group(P<0.05). Conclusion The combination of rope-assisted PNF training with rope-assisted BCI device training could further improve the motor function of the upper limbs in stroke patients with hemiplegia,and enhance their activities of daily liv-ing.

ObjectiveTo investigate the effect of combination of proprioceptive neuromuscular facilitation (PNF) rope training and repetitive transcranial magnetic stimulation (rTMS) on upper limb motor function of stroke patients with hemiplegia. MethodsFrom March, 2021 to February, 2022, 90 stroke inpatients with hemiplegia in the Department of Rehabilitation Medicine, the Second Affiliated Hospital of Guangxi Medical University were divided into control group (n = 30), rope group (n = 30) and combination group (n = 30) randomly. All the groups received routine rehabilitation, while the control group received routine PNF training, the rope group received PNF rope training, and the combination group received PNF rope training and rTMS, for four weeks. They were assessed with Functional Test for the Hemiplegic Upper Extremity-Hong Kong version (FTHUE-HK), Fugl-Meyer Assessment-Upper Extremities (FMA-UE) and modified Barthel Index (MBI) before and after treatment. ResultsThe scores of FTHUE-HK, FMA-UE and MBI increased in all the three groups after treatment (t > 2.167, P < 0.05), and they were more in the combination group than in the rope group (P < 0.05), and in the rope group than in the control group (P < 0.05). ConclusionThe combination of PNF rope training and rTMS is more effective on motor function of upper limbs of stroke patients with hemiplegia than PNF training alone.

Methods: From March 2014 to September 2018, ESCC patients from three hospitals (the Affiliated Hospital of Medical College of Shantou University, Shantou Central Hospital in Southern Chaoshan area and First Affiliated Hospital of Zhengzhou University in Northern Taihang Mountain) were selected as a case group; non-esophageal patients who had a physical examination were selected as a control group. The case and control group were paired by age (±5 years) and a 1:1 ration. A total of 1 528 subjects were enrolled including 764 patients in the case group and 764 patients in the control group. About 3-5 ml venous blood samples were collected, and the erythrocyte glutathione reductase activity coefficient (GRAC) was measured to assess the whole blood riboflavin level. A multivariate conditional logistic regression model was used to analyze the association between the GRAC and the risk of ESCC. The association between the GRAC and the prognosis of ESCC was analyzed by using Cox proportional risk regression model based on 288 patients with complete survival data. They were divided into two groups, the high GRAC group (GRAC≥7.87) group and the low GRAC group (GRAC<7.87) according to the strongest correlation between the total survival time, survival outcome and GRAC (GRAC=7.87). Results: Among the 1 528 patients, 958 patients were from Southern Chaoshan area, including 479 patients in the case group with an average age about (59.90±9.34) years and 479 patients in the control group with an average age about (59.55±8.77) years. Other 570 patients were from Northern Taihang Mountain area, including 285 patients in the case group with an average age (58.39±5.19) years and 285 patients in the control group with an average age about (58.74±4.57) years. The multivariate conditional logistic regression showed that the OR (95%CI) of the GRAC and the risk of ESCC was 1.009 (0.998-1.019). The Cox proportional hazard regression model analysis showed that the HR (95%CI) of the high GRAC group was 1.712 (1.034-2.824) compared with the low GRAC group in the 50-70 years group. Conclusion The whole blood riboflavin level might not be associated with the occurrence of ESCC. The high whole blood riboflavin level would be more beneficial to the prognosis of ESCC patients aged 50-70 years.

Objective To analyze the relationship between planning factors of intensity-modulated radiation therapy (IMRT) and gamma index and investigate the effect of each parameter upon the γ passing rate of IMRT.Methods Gamma analysis was performed using 3％/3 mm acceptance criteria for 457 IMRT beams with different planning factors.During multi-factor ANOVA analysis of planning factors and gamma passing rate,the control variables primarily included the minimum segment area,minimum number of monitor unit (MU),number of segment,segment conversation,and the spatial resolution in the measured dose distribution.Results The percentage of pixels with passingγsignificantly differed under different minimum segment area,segment conversation and the spatial resolution in the measured dose distribution (all P＜ 0.05).No significant correlation was observed between the passing rate and the minimum number of MU and the number of segment (P＞ 0.05).Conclusions According to the actual situation of the equipment,the minimum segment area should be determined during IMRT planning.Direct machine parameter optimization should be performed.Appropriate resolution of the plane dose images can be chosen according to the minimum detector interval of dose matrix device,

The relationship of medical papers and growth tendency with medical and health funds in different countries of the world was studied by analyzing SCI-covered medical papers published in 2007-2016,which showed that medical and health field was one of the research hotspots in the world,the number of papers published by the top 10 countries showed a growth tendency,their medical and health funds were closely related with the production of scientific papers (r=0.99),and the compound growth rate of medical and health funds was closely related with that of the number of scientific papers (r=0.91).

Purpose A systematic analysis of the articles published in the Chinese Journal of Medical Imaging from year 2013 to 2015 was performed to investigate its features of acceptance and publication for future decision making.Methods The printed journal and retrieved data from the Chinese Hospital Knowledge Database was analyzed using biometrical index,including the number of the papers,subspecialties,publication types,medical imaging modalities,MeSH frequency,citation and download.Results From year 2013 to 2015,there were 770 papers published,21.4 articles per issue and 0.267 articles per page in average.The top three subspecialty were abdominal imaging,chest imaging and reviews/comments,accounting for 17.27％,9.61％,9.48％,respectively.The top three modalities were CT,ultrasound and MRI,accounting for 39.61％,38.31％,29.87％,respectively.The original research papers were 584,accounting for 75.84％.The frequent MeSH subjects were differential diagnosis,angiography,pathology,computer-assisted image processing and contrast agents.Totally,there were 2395 citations and averagely 3.11 citation for each paper.The top citation was 36.There were 71317 downloads and 92.62 for each paper in average,the top download was 499.Conclusions The publications and its density per page increased annually from 2013 to 2015.The subspecialty were fully covered,the papers focus on original researches and clinical applications.The publication theme could follow the tendency of the imaging technology and had good and steady effects in the field.

〔Abstract〕 Medical and health sci-tech novelty assessment plays a key role in information supporting in the hospital scientific research and management work of health sector.Combining with the current status of medical and health sci-tech novelty assessment work station in the Military Hospital of Beijing PLA, the paper puts forward countermeasures acoording to the existing problems, namely, weak management and personnel and so on, in order to promote comprehensive development of medical and health sci-tech novelty assessment work.
