High myopia has become a major public health concern worldwide, particularly in China and Southeast Asia. It is associated not only with a variety of fundus diseases but also with earlier onset and greater severity of cataracts, resulting in significant visual impairment. Phacoemulsification is currently the main surgical treatment for cataracts. However, intraoperative fluctuations in perfusion pressure and exposure to ultrasonic energy may affect the vitreoretinal structures, potentially accelerating the progression of fundus pathology after surgery in highly myopic eyes. This article summarizes current evidence on the progression of posterior scleral staphyloma, myopic maculopathy, and retinal detachment following phacoemulsification in highly myopic eyes.

One of the significant hurdles in telemedicine, particularly in ophthalmology, is the absence of direct physical examination. This specialty depends extensively on specialized instruments that typically require proficient operators. Visual function tests are crucial for both outpatient and inpatient ophthalmic services, playing a vital role in screening, diagnosing, monitoring treatment effectiveness, and managing follow-ups for various eye conditions. The progress in mobile technology has paved the way for expanding these tests beyond traditional clinic settings, promoting the creation of patient-focused, straightforward, cost-effective, and efficient measurement tools. In light of the swift advancement of digital technologies, this article reviews the characteristics, and reliability of self-administered visual function tests tools, including visual acuity, refractive error assessment, visual field, contrast sensitivity, and color vision, along with other pertinent diagnostic tools that have been developed and validated for accuracy and repeatability through research, with a view to providing ophthalmologists and patients with scientific and practical references when selecting and using these tools, further promoting efficiency and efficacy of teleophthalmology.

Autism spectrum disorder(ASD),characterized by unknown etiology and high heterogeneity,ne-cessitates precise diagnostic and intervention strategies.Neuroimaging techniques have shown great promise in un-covering the neural mechanisms of ASD,providing a foundation for aided diagnosis and transcranial magnetic stim-ulation(TMS)interventions.This review highlights that integrating multimodal neuroimaging and developing indi-vidualized indices with developmental specificity can significantly improve the accuracy of ASD diagnosis and clas-sification.Furthermore,TMS interventions guided by functional connectivity derived from functional magnetic reso-nance imaging(fMRI)offer a personalized approach to ASD treatment.

Lens injury is an important etiological factor in the reduction of visual function following ocular trauma.Currently, there are no clear standards for the classification of lens injury, and comprehensive diagnostic tools are lacking. This deficiency leads to numerous controversies and challenges in critical areas, such as diagnosis and preoperative evalution, timing of surgery, surgical strategy, and assessment of postoperative prognosis.Tomographic imaging technology, such as computed tomography, magnetic resonance imaging, optical coherence tomography, has introduced a new dimension to the evaluation of lens injury, which is crucial for assessing the transparency, texture, location, morphology, and integrity of the lens, as well as the zonules and nearby intraocular structures. However, the use of tomographic imaging technology is somewhat limited due to the limitations of relying on a single method. With the ongoing advancement of imaging technologies and the rapid development of big data and artificial intelligence, tomographic imaging will become an increasingly essential tool in the future management of lens injury. Our expert group reviewed the epidemiological characteristics and classification of lens injury and the major challenges currently faced in the diagnosis and treatment of lens injury, and provided expert recommendations mainly focusing on the application, shortcomings and limitations of current tomographic imaging technology in the diagnosis and treatment of lens injury, and future development directions.

The incidence rate of drug-induced liver injury (DILI) is increasing year by year with unknown mechanisms, and the treatment methods for DILI mainly include drugs, liver support systems, and liver transplantation, all of which have certain limitations. Therefore, the search for safer and more effective treatment methods has become a research hotspot at present. Studies have shown that mesenchymal stem cells and their exosomes can alleviate liver injury by reducing liver inflammation, promoting hepatocyte proliferation and regeneration, inhibiting the apoptosis of hepatocytes, improving oxidative stress, and regulating immunity. This article briefly reviews the role of mesenchymal stem cells and their exosomes in the treatment of DILI, so as to provide a reference for further research.

Autoimmune hepatitis(AIH)is chronic hepatitis caused by the attack of live cells by the immune system, and at present, the pathogenesis of AIH remains unclear.Inflammasomes are important components of innate immunity and are involved in a variety of pathophysiological processes.Studies have shown that inflammatory response associated with nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)plays an important role in the pathogenesis of AIH,which mainly mediates the release of proinflammatory factors and pyroptosis, thereby participating in the pathophysiological process of AIH.Therefore, the development and progression of AIH can be delayed by inhibiting the activation of NLRP3 inflammasomes, which provides new ideas for the prevention and treatment of AIH.

Background Sleep disturbances in children and adolescents have become a global public health concern, with pronounced issues in the Western China.Despite this, research on the prevalence of insomnia and its influencing factors among children and adolescents in Chongqing is still lacking. Objective To understand the prevalence of insomnia and analyze its influencing factors among children and adolescents in Chongqing,so as to provide references for clinical interventions and preventive strategies of insomnia. Methods A stratified random sampling method was used to select 9 969 children and adolescents in Chongqing from November to December 2021.Insomnia Severity Index(ISI)and a self-designed questionnaire on awareness of core mental health knowledge were administered.Binary Logistic regression analysis was used to identify the influencing factors of insomnia in this population. Results A total of 3 578 children and adolescents(35.89%)were found to have insomnia symptoms. There were statistically significant differences in the detection of insomnia symptoms among gender, ethnicity, education level,domicile,only-child status, parental education level,introversion/extroversion,parental relationship, average monthly per capita income, family history of mental illness, medical insurance coverage, living situation and understanding of mental health knowledge(P<0.05 or 0.01).Binary Logistic regression analysis identified several risk factors for insomnia, including female gender(OR=1.301,95%CI:1.192~1.419),ethnic minority status(OR=1.163,95%CI:1.015~1.333),junior high school for education level(OR=1.985,95%CI:1.774~2.220),senior high school for education level(OR=3.085,95%CI:2.749~3.462),non-only-child status(OR=1.127,95%CI:1.013~1.253),degree of harmony between parents is not high or not harmonious[(OR=1.846,95%CI:1.669~2.041 for relatively harmonious;OR=2.524,95%CI:2.214~2.877 for generally harmonious;OR=2.452,95%CI:1.999~3.007 for not very harmonious;OR=2.926,95%CI:2.307~3.710 for very discordant)]and incomplete medical insurance coverage(OR=1.218,95%CI:1.093~1.358).Protective factors included an extroverted personality(OR=0.838,95%CI:0.766~0.917),absence of family history of mental illness(OR=0.719,95%CI:0.549~0.941),junior high school for mother's education level(OR=0.822,95%CI:0.734~0.920),senior high school or secondary specialized school for mother's education level(OR=0.862,95%CI:0.752~0.988),college and above for mother's education level(OR=0.748,95%CI:0.633~0.884)and knowledge of mental health(OR=0.854,95%CI:0.778~0.937).Conclusion The problem of insomnia among children and adolescents in Chongqing is quite serious.Risk factors for insomnia include female gender, ethnic minority status, higher education levels, being a non-only child, poor family relationships and incomplete medical insurance.Conversely,an extroverted personality, absence of family history of mental illness, higher maternal education and knowledge of mental health serve as protective factors against insomnia.
