Objective:To investigate the effectiveness of gastric-soleal turndown flap and knotless anchor bridging fixation via a modified incision in the treatment of chronic Achilles tendon rupture of Myerson type Ⅲ.Methods:A retrospective case series study was conducted to analyze the clinical data of the 18 patients who had been treated at Department of Minimally Invasive Orthopedics, Zhengzhou Orthopaedic Hospital from February 2020 to July 2023 for chronic Achilles tendon rupture of Myerson type Ⅲ by means of gastric-soleal turndown flap and knotless anchor bridging fixation via a modified incision. There were 15 males and 3 females, with an age of (37.2±6.8) years. All patients suffered from unilateral injury, involving 13 left sides and 5 right sides. Their body mass index was (22.6±2.5) kg/m 2. The operation time, blood loss, incision length, wound healing and complications were documented. The ankle dorsiflexion and plantar flexion at the last follow-up were compared between the affected side and the healthy side. The American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot score and Achilles tendon total rupture score (ATRS) before operation were compared with those at the last follow-up. Results:All the 18 patients were followed up for (18.0±5.5) months. Their operation time was (69.1±5.2) minutes, blood loss (71.6±9.2) mL, incision at the broken end of the Achilles tendon (12.4±2.6) cm and incision at the proximal end (2.5±0.4) cm. At the last follow-up, the ankle dorsiflexion was 12.7°±1.9° and the ankle plantar flexion 35.2°±2.0° at the affected side, showing no significant difference from those at the healthy side (13.0°±2.1° and 34.7°±1.8°) ( P>0.05). The AOFAS ankle-hindfoot score was (89.4±3.4) points and the ATRS (85.3±3.2) points for the affected side at the last follow-up, showing significant improvements compared with the preoperative values [(54.2±4.2) points and (51.1±4.6) points] ( P<0.05). All the incisions healed at one stage after operation, with no such complications as incision infection or re-rupture of the Achilles tendon. One patient experienced mild pain at the anchor insertion site, but the pain disappeared 6 months after operation without any treatment. One patient had the symptoms of sural nerve injury which responded to 3 months of oral neurotrophic medication. Conclusion:In the treatment of chronic Achilles tendon rupture of Myerson type Ⅲ, gastric-soleal turndown flap and knotless anchor bridging fixation via a modified incision can result in limited invasion, a low incidence of complications, and definite effectiveness.

Objective:
To understand the association between beverage and snack intake and insufficient serum 25 hydroxyvitamin D [25(OH)D] among primary and secondary school students, so as to provide a scientific basis for targeted intervention measures.
Methods:
From October to December 2021, a stratified random sampling method was used to select 2 477 primary and secondary school students aged 8 to 15 years old from 9 counties in Yunnan Province implemented the Nutrition Improvement Plan for Rural Compulsory Education Students. The intake of beverages and snacks was investigated using the Rural Student Nutrition Monitoring Questionnaire from Chinese Center for Disease Control and Prevention. The snack intake intensity was calculated and classified into no intake, extremely low, low, medium, and high intensity. Serum 25(OH)D levels were measured in the laboratory, and levels <20 ng/mL were defined as insufficient. Chi square tests, LASSO regression, random forest and binary Logistic regression were used to analyze the association between 20 types of beverages and snacks and serum 25(OH)D insufficiency.
Results:
Insufficient serum 25(OH)D was detected in 564 boys (45.9%) and 855 girls (68.5%), with a total of 1 419 cases (57.3%). Binary Logistic regression results showed that extremely low intake intensity of carbonated beverages ( OR =1.51), plant protein beverages ( OR =1.61), and milk tea beverages ( OR =1.39) increased the risk of insufficient serum 25(OH)D, while protective factors were fruits and vegetables ( OR =0.77) and pure milk and yogurt ( OR =0.74) (all P <0.05). Subgroup analysis showed that extremely low intake intensity of carbonated beverages, milk containing beverages, tea beverages, fruit and vegetable juices, and plant protein beverages increased the risk of insufficient serum 25(OH)D in girls ( OR =2.22, 1.72, 1.67, 1.74, 1.92), and high intake intensity increased the risk of insufficient serum 25(OH)D in boys ( OR =1.73, 1.48, 1.52, 1.49, 1.97) (all P <0.05). Extremely low intake intensity of carbonated beverages, plant protein beverages, and milk tea beverages in junior high school students ( OR =1.92, 2.54, 1.68) and low intake intensity in primary school students ( OR =1.40, 1.33, 1.45) increased the risk of insufficient serum 25(OH)D (all P <0.05).
Conclusions
Frequent intake of beverages and highly processed snacks increases the risk of insufficient serum 25(OH)D in primary and secondary school students, while natural foods such as fruits, vegetables, pure milk and yogurt can reduce the risk. Girls and junior high school students are more susceptible to these effects.

Space Medicine & Medical Engineering 2025;36(1):27-31
Objective In this study, the gas exchange performance and impact patterns of microalgae culture based on hollow fiber membrane technology were investigated for the culture of space microalgae. Methods The effects of parameters and conditions such as gas flow rate, membrane area and liquid flow rate on the CO2 fixation efficiency and fixation rate of hollow fiber membrane modules were studied, and the gas exchange performance of different parameters were compared by fitting formula. Results The gas flow rate and membrane area have a significant effect on the gas exchange performance of the module. With the increase of gas flow rate, the fixation rate of CO2 increases at first and then stabilizes, and the fixation efficiency of CO2 shows a continuous downward trend;with the increase of membrane area, the fixation rate and fixation efficiency of CO2 increase significantly, while the liquid flow rate has no significant effect on the gas exchange performance of the module. The highest CO2 fixation rate was 168.24 mg/(L·h)when the membrane area was 0.3m2 and the gas flow rate was 2.0 L/min.Conclusion The use of hollow fiber membrane technology can solve the problem of two-phase flow management in the gas exchange between algae culture and atmosphere, which has a better effect on gas exchange, and can provide reference for the design of gas exchange module of space microalgae photobioreactor.

Objective:To explore the potential molecular mechanism of modified Bushen Huoxue decoction in the treatment of lumbar disc herniation (LDH) based on Gene Expression Omnibus (GEO) database and network pharmacology analysis. Methods:LDH samples were retrieved from GEO, and LDH targets were obtained from Gene Cards, CTD databases combined with GEO chips. The components and targets of modified Bushen Huoxue decoction were obtained by searching the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP). STRING was used to draw the drug-component-target interaction network of modified Bushen Huoxue decoction in the treatment of LDH, so as to obtain the potential action targets of modified Bushen Huoxue decoction in the treatment of LDH. Gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses were used to systematically analyze and elaborate the action targets. Results:A total of 23 LDH-related differential genes were identified after screening from GEO dataset GSE146904. A total of 652 potential targets of modified Bushen Huoxue Decoction were obtained from the database, and 45 were selected after screening. A total of 1 121 target genes related to LDH were obtained. By taking the intersection of the targets of modified Bushen Huoxue decoction and LDH disease targets, 8 potential targets of modified Bushen Huoxue decoction in the treatment of LDH were obtained, namely TNF, CASP3, PTGS2, ESR1, ACHE, BCL2, WNT3A, and CCL11. Modified Bushen Huoxue decoction in the treatment of LDH involves key processes such as regulation of metabolism and cell proliferation in the body; among them, cell nucleus, extracellular matrix, and mitochondria are important structures for the body to exert main functions; molecular functions mainly include protein and ion binding, cytokine activity, etc. KEGG pathway enrichment analysis results showed that classical WNT signaling pathway, cytokine signaling in immune system, interleukin-4/interleukin-13 signaling pathway, and apoptosis signaling pathway were key pathways. Conclusions:Modified Bushen Huoxue decoction can mediate immune and inflammatory responses of LDH through multiple targets and pathways, thereby improving LDH symptoms.
