Objective:To construct a competency evaluation indicator system for infection prevention and control nurses in Operating Rooms (hereinafter referred to as "IPC") and provide an objective basis for the management of IPC nurses.Methods:From June to November 2022, an initial competency evaluation indicator system for IPC nurses was developed through literature review and semi-structured interviews. The Delphi method was employed to consult 20 experts from 11 provinces and municipalities across the country. Analytical Hierarchy Process (AHP) and mean distribution method were applied to quantify and determine the weight of each level of indicators within the system.Results:Nineteen experts were finally included, with two rounds of questionnaire recovery rates of 95.00% (19/20) and 100.00% (19/19), respectively. The authority coefficients of the experts were 0.858 and 0.861, familiarity coefficients were 0.850 and 0.853, and coefficients of judgment basis were 0.865 and 0.868, respectively. The Kendall's W coefficient of concordance for the two rounds of inquiries were 0.139 and 0.202 ( P<0.05), respectively. The final IPC nurse competency evaluation indicator system included six primary indicators, 22 secondary indicators, and 66 tertiary indicators. Conclusions:The constructed IPC nurse competency evaluation indicator system is scientific, reasonable, objective, and comprehensive, providing a valuable reference for the capability training, assessment, entry standards, and qualification certification of IPC nurses.

Stroke is a major disease that endangers the health of human beings.Since 2015,mechanical thrombectomy has emerged as the primary therapeutic method for acute ischemic stroke.Present guidelines lean towards treating patients with mechanical thrombectomy within a therapeutic window,while the efficacy of thrombectomy beyond 24-hour remains a matter of contention.The authors manuscript undertook a retrospective review of extant clinical research,scrutinizing its safety and efficacy from the standpoint of research design,and offering a detailed synthesis concerning on predictive factors and imaging modalities for selection.

Ischemic stroke is characterized by high rates of prevalence,mortality and recurrence.Atrial fibrillation(AF)is the most common arrhythmia in adults.AF significantly increases the risk of ischemic stroke and stroke recurrence.This review summarized the current research progress on AF screening in ischemic stroke patients,the relationship between AF characteristics,biomarkers and ischemic stroke,as well as the research progress on secondary prevention strategies for patients with concomitant AF,aiming to assist clinicians detect AF at an early stage and prevent stroke recurrence.

Studies have shown that vertebral artery hypoplasia(VAH)is associated with posterior circulation ischemic stroke,but it is difficult to distinguish VAH from atherosclerotic stenosis in cerebrovascular angiography and other imaging examinations,and there is currently no unified diagnostic standard for intracranial VAH.The authors reviewed the definition and incidence of intracranial VAH from the aspects of autopsy and imaging,and summarized its relationship with posterior circulation ischemic stroke,in order to improve the clinicians'understanding of intracranial VAH.

Objective To investigate value and risk factors of pelvic floor three-dimensional ultrasound parameters in single spontaneous delivery primipara with bladder neck funnel formation at 6-8weeks after delivery.Methods A total of 108 cases of single spontaneous delivery primipara were selected as research objects according to three-dimensional ultrasound indexes of pelvic floor,they were assigned into two groups:Observation group(40 cases)with bladder neck funnel formation and control group(68 cases)without bladder neck funnel formation.Three-dimensional ultrasound indexes of pelvic floor and clinical symptoms were compared between two groups.Results The incidence of stress urinary incontinence(SUI),anterior vaginal wall prolapse,bladder neck distance(BND),posterior urethrovesical angle(PUA)in Valsalva state,levator hiatus area(LHA)in resting and Valsalva states,levator hiatus length(LHL),and changes in LHL(△ LHL)in observation group were all higher than those in control group(P＜0.05).Logistic regression analysis revealed that SUI and anterior vaginal wall prolapse are independent risk factors for bladder neck funnel formation,adjusted OR were 11.255 and 3.643;Resting state and Valsalva state LHA,LHL,and △ LHL were also independent risk factors for bladder neck funnel formation(P＜0.05).Receiver operating characteristic(ROC)curve analysis revealed that area under the curve(AUC)for diagnosing bladder neck funnel formation in SUI and anterior vaginal wall prolapse were 0.744 and 0.615,respectively;In ultrasound indicators,when the LHL of Valsalva state is＞63.5mm,the AUC is 0.706;The diagnostic sensitivity of the combination factor 1(resting state LHA+Valsalva state LHA)is 95.0％;The diagnostic specificity of the combination factor 2(Valsalva state LHL+△LHL)is 80.9％.Conclusion In patients of single spontaneous delivery primipara with bladder neck funnel formation,occurrence of SUI and vaginal anterior wall prolapse,increased LHA and LHL under two states and △LHL were independent risk factors.The diagnostic sensitivity of the combination factor 1 was higher than single index,the diagnostic specificity of the combination factor 2 was higher.

Objective To analyze the clinical characteristics of organizing pneumonia secondary to inva-sive pulmonary aspergillosis.Methods A total of 13 patients with organizing pneumonia secondary to invasive pulmonary aspergillosis in the Department of Respiratory,the First Affiliated Hospital of Zhengzhou Univer-sity diagnosed by pathology from January 2019 to January 2021 were retrospectively included,and their clinical characteristics,laboratory examination,imaging characteristics,treatment and prognosis were comprehensively analyzed.Results Most cases of pulmonary aspergillosis occurred within 2-3 weeks of diagnosis,with coug-hing,expectoration,fever,and elevated C-reactive protein and erythrocyte sedimentation rate.Chest computed tomography showed patchy shadows around the original infection focus,mostly accompanied by consolidation,halo sign,bronchial inflation sign,and might be accompanied by cavity and pleural effusion;secondary multi lobed involvement of both lungs was observed,mostly with small patchy shadows,nodular shadows,and tree bud signs.After receiving glucocorticoid treatment,the symptoms were controlled within one week,and the glucocorticoid treatment course was mostly 3-6 months.Conclusion The clinical characteristics of organi-zing pneumonia secondary to invasive pulmonary aspergillosis are somewhat similar to those of cryptogenic or-ganizing pneumonia and have a better prognosis.

This article employs a multidisciplinary approach encompassing traditional Chinese medicine, history, philology, and exegetics to comprehensively examine the historical evolution of auricular acupoint therapy and otomedicine. Originating in China, systematized in Europe, and currently advancing through modern research and development led by China, this study traces the historical context of otological practices. It outlines the foundational principles of otomedicine as depicted in the Huangdi Neijing, elucidates the accumulation of ancient otomedical knowledge within this framework, reviews the systematic establishment and theoretical underpinnings of auricular acupoints, and delineates the process of formulating auricular acupoint standards. Through case studies such as " ear-brain interconnection" and " ear treatment of encephalopathy", it illustrates the interdisciplinary nature of ear medicine research and development. The article aims to illuminate the historical transition from ear therapy to ear medicine within the contemporary context and the emergence of auricular point-guided ear medicine. By drawing on historical insights and looking toward the future, it provides a foundation for the ongoing development of the ear medical specialty, offering insights for integrating non-drug therapies into China′s healthcare system and global health initiatives.

The construction of standard system is an important part of standardization work， and also the top-level design of standardization work.Taking the group standards of the China Association of Chinese Medicine as a reference， and following the steps of establishing an expert group， literature research， Delphi method expert consultation and expert meeting， it was clarified that the content of the standard system for clinical diagnosis and treatment techniques of gastroenterology in traditional Chinese medicine， including terms （symptoms and signs， syndromes and syndrome components， and therapeutic principles and methods）， guidelines and standards （traditional Chinese medicine disease name， western medicine disease name， and classical syndromes）， effectiveness evaluation （modern medicine evaluation， traditional Chinese medicine characteristics of diseases， symptoms and signs grading， traditional Chinese medicine syndrome evaluation）， and practical techniques. Meanwhile， the structure chart of the standard system， the table of standard details and the table of statistical standards were compiled to lay the foundation for the standardisation of clinical diagnosis and treatment techniques of gastroenterology， and to provide reference for the construction of the standard system of various disciplines.

  Objective  To promote the development of high-quality health standards in China and summarize the characteristics of global health standards management systems and development processes.  Methods  We conducted a systematic search of relevant databases and the websites of standardization institutions. A descriptive method was used to analyze the basic characteristics of the standard management systems and development processes.  Results  The majority of the investigated countries and organizations primarily implement voluntary standards, with the exception of Chinese and the European Committee for Standardization that enforce compulsory standards. All countries and organizations have health standards at the national, group, and enterprise levels. In China and Japan, standards authorities are subordinate to the government, while in other countries or organizations, they operate as independent civil societies. The development process for health standards generally involves five stages: "Project/Proposal → Draft → Enquiry → Review → Publication". However, the specific steps in the development of health standards vary across different countries and organizations. Furthermore, China ranks high in the number of both standards and health standards issued, but the proportion of health standards is only 2.06%.  Conclusions  There are significant differences between China and other countries in terms of the management system and development process of health standards. To promote the management and development of high-quality health standards in China, we recommend promoting the coordination mechanism for standard development, increasing efforts to publicize standards and evaluate the implementation effect, strengthening the construction of personnel for the development of health standards, optimizing the efficiency of standard development and improving the internationalization level of health standards.

National health standards involve all kinds of technical requirements formulated and numbered in accordance with the procedures and formats stipulated in the standardisation system for the implementation of national health and hygiene laws, regulations and policies, and the protection of human health. The establishment of health standards in China should align with our legal framework, including laws, regulations, departmental rules, and health and hygiene policies. During the development of these standards, a comprehensive approach is advocated, encompassing in-depth investigations, rigorous demonstrations, and extensive stakeholder engagement. However, the process of standard formulation may suffer from insufficient research evidence. The evidence-based concept emphasizes the significance of evidence. Therefore, integrating evidence-based concept into the process of developing health standards can enhance the quality and scientific basis of these standards. This article systematically elucidates the current status and development process of health standards in China, explores the necessity and feasibility of incorporating evidence-based concept into the development of these standards, analyzes the challenges and opportunities, and presents reflections and suggestions.
