1.Short- to medium-term safety and efficacy of the implantable Corheart 6 left ventricular assist system in patients with end-stage heart failure

Objective To investigate the efficacy and safety of the Corheart 6 left ventricular assist system in patients with end-stage heart failure. Methods A retrospective study was conducted on patients with end-stage heart failure who were treated with Corheart 6 left ventricular assist system from March 2022 to June 2024 in 4 hospitals in Jiangsu Province. The efficacy of the device was evaluated by comparing changes in clinical indicators at preoperative, discharge, 3-month postoperative, and 6-month postoperative timepoints, including the New York Heart Association (NYHA) functional classification, left ventricular ejection fraction (LVEF), and left ventricular end-diastolic diameter (LVEDD). The safety of the device was assessed by analyzing the intraoperative position and orientation of the blood pump inlet cannula, as well as the incidence of adverse events. Results In this study, 39 patients were collected, including 34 males and 5 females with a mean age of (56.4±12.5) years, ranging from 20 to 75 years. There was no operative death. There was no death in postoperative 3 months with a survival rate of 100.0%. There were 3 deaths in 6 months postoperatively, with a survival rate of 92.3%. All patients had a preoperative NYHA cardiac function classification of class Ⅳ. The NYHA cardiac function class of the patients improved (P<0.05) at discharge, 3 and 6 months after surgery when compared to the preoperative period. LVEF was significantly higher at 3 months after surgery than that during the preoperative period (P<0.05). LVEDD was significantly smaller at discharge, 3 and 6 months after surgery than that during the preoperative period (P<0.05). The safety evaluation's findings demonstrated that all 39 patients' intraoperative blood pump inlet tubes were oriented correctly, the artificial blood vessel suture sites were appropriate, there were no instances of device malfunction or pump thrombosis, or instances of bleeding or hemolysis, and the rate of the remaining adverse events was low. Conclusion With a low rate of adverse events and an excellent safety profile, the Corheart 6 left ventricular assist system can efficiently enhance cardiac function in patients with end-stage heart failure. It also has considerable clinical uses.
2.Effect of the Otago Exercise Program on Activities of Daily Living in Older Adults With Sarcopenia

Objective To explore the effects of the Otago Exercise Program(OEP)on activities of daily living,muscle strength,balance,and physical function in older adults with sarcopenia,to compare OEP with conventional exercise training,and to provide a basis for clinical rehabilitation programs for older adults with sarcopenia.Methods In this randomized controlled trial,120 older adults clinically diagnosed with sarcopenia were enrolled.The participants were randomly assigned to the OEP intervention group(experimental group)and the conventional exercise intervention group(control group),with 60 in each group.The experimental group underwent 12 weeks of OEP training,three times a week,with each session lasting 45 minutes.The control group underwent conventional exercise training following the same schedule.The Modified Barthel Index was used as the primary outcome measure to assess activities of daily living.Secondary outcome measures included muscle strength,gait stability,dynamic balance,and physical function status,evaluated using grip strength,6-meter walking speed,the Timed Up and Go Test(TUGT),and the Short Physical Performance Battery(SPPB).Results A total of 120 older adults with sarcopenia were included.The mean age of the participants was(80.17±8.48)years.Baseline data before treatment showed no statistically significant differences between the two groups.Both groups completed the treatment within 12 weeks without experiencing any adverse events.The baseline data for the experimental group were as follows,MBI at(67.00±22.76)points,hand grip strength at(15.29±4.94)kg,gait speed at(0.61±0.26)m/s,TUGT time at(15.05±6.74)s,and SPPB score at(6.17±1.40)points,while the corresponding post-intervention findings were as follows,(78.72±15.83)points,(17.67±5.83)kg,(0.77±0.28)m/s,(13.49±6.16)s,and(9.25±1.71)points,respectively.The experimental group showed improvements in all measures from baseline to post-intervention(P<0.05 for all measures).As for the control group,the baseline data for the corresponding measures were as follows,(67.20±22.12)points,(15.00±5.35)kg,(0.58±0.23)m/s,(17.29±6.90)s,and(6.00±1.24)points,respectively.The post-intervention findings increased to(71.13±20.28)points,(15.47±5.72)kg,(0.64±0.28)m/s,(16.50±6.99)s,and(6.73±1.61)points,respectively,but the changes were not statistically significant(P>0.05).Furthermore,an intergroup comparison of intervention effects(post-intervention minus preintervention)revealed significant differences in mean changes from baseline.The experimental group demonstrated improvements of(+11.72±6.32)points in modified Barthel Index,(+11.72±6.32)kg in grip strength,(+0.16±0.09)m/s in gait speed,(—1.56±1.32)s in TUGT time,and(—1.56±1.32)points in SPPB score.In contrast,the control group showed smaller changes of(+3.93±5.65)points,(+0.47±1.37)kg,(+0.06±0.07)m/s,(—0.79±1.54)s,and(+0.73±1.12)points,respectively(all P<0.05).Intergroup comparisons revealed superior outcomes in the experimental group across all measures.Conclusion OEP significantly enhances activities of daily living,improves muscle strength,balance,and physical function in older adults,and is more effective than conventional rehabilitation exercise programs,making it suitable for extensive clinical application.
3.Efficacy of Transfer Energy Capacitive and Resistive Therapy Combined With β-Hydroxy-β-Methylbutyrate Nutritional Supplementation in Older Adults With Sarcopenic Obesity

Objective To investigate the comprehensive intervention effects of transfer energy capacitive and resistive(TECAR)therapy combined with β-hydroxy-β-methylbutyrate(HMB)nutritional supplementation in older patients with sarcopenic obesity(SO).Methods We conducted a randomized controlled trial,enrolling 140 older patients who met the Asian diagnostic criteria for SO.Participants were randomly assigned to 4 groups,including a double-placebo group(Group A),TECAR+placebo group(Group B),sham TECAR+HMB group(Group C),and TECAR+HMB group(Group D),with 35 patients in each group.The intervention lasted 12 weeks.The primary outcome measure was the total score of the Short Physical Performance Battery(SPPB).Secondary outcome indicators included the modified Barthel Index(MBI),scores of the Mini Nutritional Assessment-Short Form(MNA-SF),handgrip strength,body mass,and body mass index(BMI).A two-way analysis of variance(ANOVA)was used to assess the interaction effects between TECAR and HMB.Results After the intervention,Group D(TECAR+HMB)demonstrated significant improvements across all metrics.The SPPB total score increased from 6.29±1.34 to 8.06±1.51(P<0.001),with notable enhancements in walking speed(2.71±0.86 vs.1.97±0.82),chair stand(2.60±0.55 vs.2.11±0.47),and balance(2.74±0.74 vs.2.20±0.76).MBI improved from 71.74±14.41 to 79.91±10.52(P<0.001).Handgrip strength increased from(13.65±5.05)kg to(15.72±4.89)kg(P=0.001).Body mass decreased from(81.78±9.02)kg to(76.95±9.89)kg(P<0.001),and BMI reduced from(30.14±1.68)kg/m2 to(28.34±2.33)kg/m2(P<0.001).Interaction analysis revealed significant synergistic effects between TECAR and HMB in improving the SPPB total scores(F=16.374,P<0.001,η2=0.107)and reducing BMI(F=14.328,P<0.001,η2=0.095).Conclusion TECAR therapy combined with HMB supplementation significantly enhances physical function,activities of daily living,and body composition in elderly patients with sarcopenic obesity,demonstrating a synergistic effect.

5.The effect and safety of monoclonal antibodies to calcitonin gene-related peptide and its receptor on migraine: a systematic review and meta-analysis

Objective:To evaluate the effect and safety of monoclonal antibodies to calcitonin gene-related peptide and its receptor (CGRP-mAbs) on migraine.Methods:Database of PubMed, Embase, Cochrane, CNKI, Wangfang digital journals were searched for randomized controlled trials (RCTs) of CGRP-mAbs in treatment of migraine. Quality of enrolled literature was assessed by the software of Review Manager 5.3 and software of StataMP14 was employed to conduct meta analysis.Results:A total of 13 RCTs were included, including 6 218 adult migraine patients (experimental group: 2 679 patients, placebo group: 3 539 patients). Meta analysis suggested that CGRP-mAbs for preventive treatment of migraine significantly reduced the monthly migraine days from baseline (standardized mean difference (SMD)=-0.35, 95% CI-0.4--0.3) and monthly acute migraine-specific medication consumption from baseline (SMD=-0.38, 95% CI-0.43--0.32), as compared with placebo group. CGRP-mAbs for preventive treatment of migraine significantly increased the ≥50% reduction from baseline in migraine days per month ( RR=1.65, 95% CI 1.54-1.76). The adverse events were similar between the CGRP-mAbs group and placebo group ( RR=1.06, 95% CI 1.01-1.10). Conclusion:CGRP-mAbs are effective and safe for preventive treatment of migraine.
6.Research progress on electroencephalogram characteristics of anti-N-methyl-D-aspartate receptor encephalitis.

Anti-N-methyl-d-aspartate (NMDA) receptor encephalitis is a kind of autoimmune disease aiming at NR1 subunit of NMDA receptor. In the early stage, functional damage is the main cause. Electroencephalogram (EEG) can reflect the abnormal brain function by recording the changes of EEG signals. The common EEG patterns of anti NMDA receptor encephalitis are slow wave abnormality, epileptic discharge, a large number of β activity, extreme delta brush, etc. Here we review the waveform characteristics, origin, pathogenesis and clinical value of EEG in patients with NMDA receptor encephalitis.

7.Epidemiological analysis of Plague in Dehong Dai and Jingpo Autonomous Prefecture of Yunnan province, 1950-2019

Objective:To analyze the epidemic situation and influencing factors of plague in Dehong Dai and Jingpo Autonomous prefectures (Dehong) from 1950 to 2019, for the improvement of strategies on prevention and control of the disease.Methods:The epidemic situation and surveillance data of animal and human plague in Dehong prefecture from 1950 to 2019 were collected and analyzed by descriptive epidemiological methods. Multiple linear regression equations were established by SPSS 20.0 software to analyze the relationship between plague among animals and rat density and flea index, as well as the relationship between human plague and plague among animals.Results:In Dehong prefecture, plague experienced a phenomenon of epidemic-resting-epidemic-resting cycle, between 1950 and 2019. During this period, 614 epidemic spots were identified in 36 townships of five counties/cities,. Human plague had been prevalent for 15 years, with 1 153 human cases and 379 deaths involved. Between 1982 and 2019, 261 319 rodents were captured, of which Rattus flavipectus accounted for 70.95%(185 421/261 319). A total of 70 124 fleas were seized with 76.65%(53 752/70 124), xenopsylla cheopis as fleas. The index of fleas and free fleas were 0.57 and 0.22 respectively. A total of 1 577 strains of Yersinia pestis were isolated from 285 091 animal specimens and 418 strains of Yersinia pestis were isolated from 60 119 groups of fleas, with 255 F1 antibody positive samples were detected from 64 157 sera samples. Neither Yersinia pestis nor the outbreak of plague had been noticed since 2008. Correlations between both animal plagues and rat density, as well as human plague and animals plague were noticed from the regression analysis. Conclusions:The epidemics of plague were with long-term and stable nature, in Dehong prefecture. Affected by many factors, plague seemed in a resting period currently. However, we should be reminded that the fact that outbreak of plague may revive at some uncertaint point. Hence in order to prevent human plague, strategies as careful surveillance programs on animals, need to be strenthened.
8.Therapeutic effect of mild hypothermia on the inflammatory response and outcome in perioperative patients with acute Stanford type A aortic dissection: A randomized controlled trial

Objective To explore the therapeutic effect of mild hypothermia on the inflammatory response, organ function and outcome in perioperative patients with acute Stanford type A aortic dissection (AAAD). Methods From February 2017 to February 2018, 56 patients with AAAD admitted in our department were enrolled and randomly allocated into two groups including a control group and an experimental group. After deep hypothermia circulatory arrest during operation, in the control group (n=28), the patients were rewarmed to normal body temperatures (36 to 37 centigrade degree), and which would be maintained for 24 hours after operation. While in the experimental group (n=28), the patients were rewarmed to mild hypothermia (34 to 35 centigrade degree), and the rest steps were the same to the control group. The thoracic drainage volume and the incidence of shivering at the first 24 hours after operation, inflammatory indicators and organ function during perioperation, and outcomes were compared between the two groups. There were 20 males and 8 females at age of 51.5±8.7 years in the control group, 24 males and 4 females at age of 53.3±11.2 years in the experimental group. Results There was no obvious difference in the basic information and operation information in patients between the two groups. Compared to the control group, at the 24th hour after operation, the level of peripheral blood matrix metalloproteinases (MMPs) was lower than that in the experimental group (P=0.008). In the experimental group, after operation, the awakening time was much shorter (P=0.008), the incidence of bloodstream infection was much lower (P=0.019). While the incidence of delirium, acute kidney injury (AKI), hepatic insufficiency, mechanical ventilation duration, intensive care unit (ICU) stays, or hospital mortality rate showed no statistical difference. And at the first 24 hours after operation, there was no difference in the thoracic drainage volume between the two groups, and no patient suffered from shivering. Conclusion The mild hypothermia therapy is able to shorten the awakening time and reduce the incidence of bloodstream infection after operation in the patients with AAAD, and does not cause the increase of thoracic drainage volume or shivering.

We investigated the distribution characteristics of Yersinia enterocolitica in Ningxia ,China .In accordance with the requirements of the National Yersinia enterocolitica Disease Monitoring Scheme ,Y .enterocolitica were isolated from differ‐ent kinds of specimens collected in Ningxia in 2008 to 2013 .Then they were serotyped and detected for virulence gene and ana‐lyzed the pulsed‐field gel electrophoresis (PFGE) in Chinese CDC .It was found that 173 strains were isolated from various types of 9 643 specimens ,and the detection rate was 1 .79% .There were statistical differences among detection rates in differ‐ent years and in different specimens (P<0 .01) .Pathogenic serotypes O∶3 and O∶9 carried ail gene and ystA gene were de‐tected from specimens of pigs and diarrhea patient .Non‐pathogenic serotypes O∶5 and O∶8 and non‐typeable strains didn't carry ail gene and ystA gene ,and also can't be detected from swine ,cattle ,sheep ,chickens and dogs .In conclusion ,Y .en‐terocolitica was widely distributed in Ningxia and pigs were the dominant animal host .In all pathogenic serotypes ,the highest proportion was O∶3 following by O∶9 .It was no time and regional difference in the distribution of that in Ningxia ,China .
10.Psychological states of chronic hepatitis B patients during family disinfection and isolation:a qualitative study

Objectives To explore the psychological states of chronic hepatitis B(CHB)patients during family disinfection and isolation:a qualitative study.Method Using random sampling,10 CHB patients from the Eighth People’s Hospital of Guangzhou during Sep 2012 and Feb 2013 were selected for the investigation by way of semi-structured in-depth interviews.Results Ten patients participated in the interview and five themes were extracted:1)no special mental burden because CHB patients believed they should conduct family disinfection and isolation;2)group psychology for their lack of knowledge;3)suspecting the effect of disinfection and isolation;4)worrying about involving their families because of guilty;5)feeling isolated,self-abased,sensitive and longing for being accepted and understood.Conclusions The psychological states are complex during disinfection and isolation.So,a feasible domestic disinfection and isolation plan should be worked out.Meanwhile,enhanced mental guidance and family supports are essential for them to get rid of worries and adapt themselves into family disinfection and isolation.
