1.Study on the new workflow of PIVAS based on intelligent auxiliary devices

OBJECTIVE To build a new workflow of pharmacy intravenous admixture services (PIVAS), effectively connect intelligent equipment, and promote the intelligent development of PIVAS. METHODS Based on intelligent auxiliary equipment, PIVAS workflow was optimized, and a process-oriented model was established. This model integrated intelligent prescription review (automatic prescription review+manual intervention mode), intelligent labeling, intelligent allocation, intelligent sorting, and finished infusion quality inspection system. Furthermore, an assessment was conducted to examine unreasonable medical order rate of intelligent prescription review, the working efficiency and error rate of intelligent labeling machine and intelligent sorting machine, and the dispensing efficiency and accuracy of intelligent dispensing robot. RESULTS Under the intelligent prescription review mode, the rate of unreasonable medical orders decreased from 0.157% to 0.050% (P<0.05); automatic labeling efficiency reached 21.7 sheets/min, surpassing the manual labeling efficiency of 13.8 sheets/min (P<0.05), and the daily labeling error rate decreased from 6.1‰ to 2.5‰ (P<0.05). Simultaneously operating two dispensing robots significantly improved the efficiency of batch dispensing and reduced the residual amount of liquid medicine (P<0.05); additionally, a quality testing system for finished infusion was established, involving appearance, Tyndall effect, insoluble particles, turbidity, absorbance, pH and osmotic pressure, to ensure the quality of finished infusion and reduce the risk of infusion. CONCLUSIONS The new process of PIVAS connected with intelligent devices in our hospital can improve work efficiency, reduce dispensing errors, ensure the quality of finished infusion, and improve the level of pharmaceutical care.
2.Prediction model of tacrolimus blood concentration based on population pharmacokinetics information and machine learning

OBJECTIVE To construct a prediction model for tacrolimus trough concentration based on population pharmacokinetics (PPK) information and machine learning, evaluate its predictive performance in clinical therapeutic drug monitoring (TDM) data, and provide evidence-based support for individualized dose adjustment of tacrolimus after renal transplantation. METHODS A retrospective cohort study design was adopted, and consecutive TDM records of kidney transplant recipients treated in the First Affiliated Hospital of University of Science and Technology of China (Anhui Provincial Hospital) from January to June 2025 were included to construct the final analysis set. Based on 29 input variables including previous TDM concentration, previous concentration/dose ( C / D ) ratio, and individual predicted exposure (IPRE), 13 commonly used regression algorithm models and 1 weighted ensemble model were systematically compared. The dataset was split into training set and test set in an 80∶20 ratio by patient. Five-fold cross-validation based on patient grouping was performed on the training set, and the model performance was evaluated on the obtained evaluation points using the coefficient of determination ( R 2 ), mean absolute error (MAE), root mean square error (RMSE), Pearson correlation coefficient (Corr) and ±20%/±30% accuracy. The SHapley Additive exPlanations (SHAP) method was used for model interpretability analysis to identify core predictive features. RESULTS A total of 809 consecutive TDM records from 83 kidney transplant recipients were included, with 66 cases in the training set and 17 cases in the test set. The SVR model showed the best performance, with R 2 of 0.649 8, MAE of 1.126 0 ng/mL, RMSE of 1.478 6 ng/mL, Corr of 0.819 1, ±20% accuracy of 59.68%, and ±30% accuracy of 79.03%. The Weighted Ensemble model and Extra Trees model ranked second and third, respectively. SHAP interpretability analysis showed that the top 6 features contributing to the prediction were IPRE, previous TDM concentration, previous C/D ratio, dose-normalized concentration, hematocrit (HCT), and concentration-dose coefficient ( θ cd ), among which IPRE was the core predictive feature (SHAP mean value 0.915 4). CONCLUSIONS The machine learning model incorporating PPK information demonstrates good predictive ability for tacrolimus concentration in the current TDM cohort. SVR is the optimal model, IPRE is the core predictive feature of the model, and a few core PK-derived features (IPRE, dose-normalized concentration, θ cd ) together with clinical features (previous TDM concentration, HCT, etc.) constitute the main information sources for model prediction.
3.Effects of various prone ventilation positions in patients with acute respiratory distress syndrome: a network Meta-analysis

Objective:To compare the effects of 5 prone ventilation positions in patients with acute respiratory distress syndrome (ARDS) with network Meta-analysis.Methods:The randomized controlled trials on various prone ventilation positions in patients with ARDS were retrieved from Cochrane Library, EMbase, CINAHL, PubMed, Web of Science, SinoMed, CNKI, Chinese Medical Association Guideline Database, Wanfang Data, China Biology Medicine disc, VIP Information and Chinese Clinical Trial Registry from their inception to December 31, 2023. Literature screening, quality assessment and data extraction were done following the inclusion and exclusion criteria. Network Meta-analysis of outcome indicators was done using Stata 18.0.Results:A total of 19 articles, covering 1 284 patients and 5 prone ventilation positions (0°, 10°, 30°, 25° and 45°) were included. Cochrane risk assessment results showed that 15 articles were grade B and 4 articles were grade C. In direct comparisons, heterogeneity was acceptable ( I2≤50% and P≥0.1). In network analysis, the global and local consistency test result showed good consistency ( P>0.05). Network Meta-analysis result showed that the incidences of pressure injury in patients with 10°, 25°, 30° and 45° prone ventilation were significantly lower than that in patients with 0° prone ventilation, the incidence of pressure injury in patients with 25° prone ventilation was significantly lower than that in patients with 10° prone ventilation, and there were statistical differences ( P<0.05); the oxygenation index in patients with 25° prone ventilation was significantly better than that in patients with 0°, 30° and 45° prone ventilation, and there was statistical difference ( P<0.05). Conclusions:The 25° prone ventilation can both effectively improve oxygenation and maximally reduce the incidence of pressure injury in patients with ARDS.
4.Network pharmacological analysis of berberine inhibiting breast can-cer cell proliferation and in vitro cell validation

AIM:To explore the mechanism of berberine on breast cancer cells based on network pharmacology and in vitro cell experiments.METH-ODS:Firstly,berberine and breast cancer were tak-en as the research objects,the intersection targets of the two were screened by VEEN diagram,GO function and KEGG enrichment analysis were per-formed by R language,and molecular docking and visualization were carried out by Autodock Vina and Pymol software.Then,berberine treated breast cancer MCF-7 cells for 24 h,and then in vi-tro cell experiments were performed.CCK-8 was used to detect cell viability,Edu and plate cloning were used to detect cell proliferation and cloning,and apoptosis was detected by An-nexin V-FITC/PI double staining and Western blot.Laser confocal and CETSA were used to verify the binding effect of berberine and AKT1 protein.RESULTS:The results of network pharmacology showed that berberine had a good binding to the core targets AKT1,AKT2 and MAPK3.Berberine(20,40,80 μmol/L)signifi-cantly inhibited the proliferation and cloning ability of MCF-7 cells in a concentration-dependent man-ner(P<0.05,P<0.01).The results of laser confocal and CETSA experiments showed that berberine and AKT1 had a binding effect,and the stability of the two was enhanced after the combination.CONCLU-SION:Berberine inhibits MCF-7 cell proliferation and induces apoptosis in human breast cancer cells by targeting binding to AKT1 protein.
5.Mechanism of pirfenidone inhibiting cell pyroptosis and reduceing myocardial fibrosis

Objective To observe the effect of pirfenidone on myocardial fibrosis in rats and inves-tigate its underlying mechanism.Methods Twenty-four SD rats were randomly divided into sham-operation group,model group,low-and high-dose pirfenidone groups,with 6 rats in each group.Rat model of myocardial fibrosis was established by injecting isoprenaline into the tail vein,while normal saline was given to the sham operation group.Pirfenidone of 150 and 300 mg/(kg·d)were infused gastrically to the rats of low-and high-dose pirfenidone groups after modeling.Mas-son staining was used to observe the severity of myocardial fibrosis,immunohistochemical assay was employed to detect the expression of Collagen-1,NOD-like receptor thermal protein domain associated protein 3(NLRP3)inflammasome,cysteinyl aspartate-specific protease-1(Caspase-1),and Western blotting was performed to detect the protein levels of Collogen-1 and atrial desmo-plakin D(gasdermin D,GSDMD).Results The model group showed obvious myocardial fibrosis,and elevated expression of Collogen-1,NLRP3,Caspase-1 and GSDMD when compared with the sham operation group(P<0.05).Low-and high-dose pirfenidone treatment resulted in signifi-cantly reduced myocardial fibrosis and reduced expression of Collogen-1,NLRP3,Caspase-1 and GSDMD[(8.14±1.40)%,(6.56±0.75)%vs(22.15±2.57)%,P<0.05;0.14±0.03 vs 0.33±0.05,0.42±0.13,P<0.05;(10.34±1.40)%,(10.33±3.40)%vs(23.22±1.99)%,P<0.05;(15.67±0.56)%,(17.33±0.78)%vs(22.87±1.92)%,P<0.05;0.43±0.06,0.46±0.11 vs 0.65±0.03,P<0.05].Conclusion Pirfenidone inhibits cardiomyocyte pyroptosis and attenuates myocar-dial fibrosis through the NLRP3/Caspase-1/GSDMD signaling axis.

objective:Thirty-three batches of Baohe Pill were subjected to quality analysis and exploratory study based on the test results.Methods:According to the methods in Chinese Pharmacopoeia,2020 Edition,Part Ⅰ and Ⅳ,the full-item chemical tests were conducted on Baohe Pill,and the exploratory study on the identification(2)method of Baohe Pill was conducted based on the method of Forsythia identification(2)in Chinese Pharmaco-poeia,2020 Edition,Part Ⅰ.Results:All the tested items in the 33 batches of samples were qualified,and the exploratory study on Baohe Pill based on the Forsythia herb also yielded satisfactory results.Conclusion:The qual-ity of Baohe Pill produced by various manufacturers can meet the requirements of Chinese Pharmacopoeia,ensuring the safe and effective use of drugs by the people.Through more extensive and thorough investigation,it is proposed to explore whether the identification(2)method of Baohe Pill in Chinese Pharmacopoeia needs to be optimized.

9.Effects of miR-483-3p on hypoxia/reoxygenation-induced apoptosis and pyroptosis in cardiomyocytes

Objective To investigate the effects of miR-483-3p on hypoxia/reoxygenation(H/R)-induced apoptosis and pyroptosis of H9c2 cardiomyocytes and its possible mechanism.Methods Rat H9c2 cardiomyocytes were cultured in vitro,adeno-associated virus-infected H9c2 and the H/R model were constructed by triple-air incuba-tor,and the cells were randomly divided into blank control(Sham)group,model(H/R)group,AAV-miR-483-3p mimic+H/R(AAV-miR-483-3p)group,AAV-miR-483-3p negative control+H/R(AAV-NC)group.The growth status of cells in each group was observed using an inverted microscope;cell proliferation activity was detected by cell counting kit-8(CCK-8);LDH release by lactate dehydrogenase(LDH)kit;apoptosis rate by flow cytometry;apoptosis by notched end labeling(TUNEL).Western blot(WB)was used to detect the expression levels of IL-1β and GSDMD proteins in each group.Results Compared with the Sham group,the H/R group showed abnormal cell status and increased cell death,decreased cell activity,increased LDH release,increased apoptosis rate and apopto-sis level,and increased expression levels of IL-1β and GSDMD proteins(P<0.05);compared with the H/R group,the AAV-miR-483-3p group showed improved cell status and less cell death,increased cell proliferation activity,increased LDH release,and increased IL-1β and GSDMD protein expression levels(P<0.05).Compared with the H/R group,the AAV-miR-483-3p group showed improved cell status and less cell death,increased cell proliferation activity,decreased LDH release,decreased apoptosis rate and apoptosis level,and decreased expression of IL-1β and GSDMD proteins(P<0.05).Conclusion Over-expression of miR-483-3p can improve H/R-inducedH9c2 cardiomyocyte injury by enhancing cell activity and cell metabolism,and inhibiting apoptosis and cell charring.
