1.The predictive value of gastric tumor markers for pathological complete response fol-lowing neoadjuvant therapy in gastric cancer

Objective:To investigate the risk factors of pathological complete response(pCR)after neoadjuvant therapy for locally advanced gastric cancer(LAGC)and assess the value of gastric tumor markers for predicting pCR in LAGC patients.Methods:We retrospectively ana-lyzed the clinical and pathological characteristics of 213 patients who underwent radical gastrectomy and gastric tumor marker analysis after neoadjuvant therapy at The Chinse PLA General Hospital First Medical Center,between January 2020 and April 2024(20 and 193 cases in the pCR and non-pCR groups,respectively).The interrelationships among pCR,tumor markers,and clinicopathological features were compared,and independent risk factors for pCR were analyzed.A nomogram was constructed to predict the pCR.Results:Among 213 patients,20(9.4% )achieved pCR.Univariate analysis showed that age(P=0.067),tumor bed diameter(P<0.001),gastrin-17 levels(P=0.005),CA72-4 levels(P=0.073),pepsinogen ratio(P=0.024),and neoadjuvant immunotherapy(P=0.022)were strongly associated with pCR in LAGC pa-tients.Multivariate analysis showed that neoadjuvant immunotherapy,CA72-4 levels<2.5 U/mL,gastrin-17 levels<1.48 pmol/L,and tumor bed diameter<2.85 cm were independent predictive factors for pCR in LAGC patients(P<0.05).These indicators were incorporated into a nomogram prediction model;an receiver operating characteristic curve(ROC)was plotted with an AUC(95% CI)of 0.863(0.785-0.942).The calibration and decision curves suggested that the nomogram was well calibrated and had a good net benefit.Conclusions:Gastric tumor markers can effectively predict pCR after neoadjuvant therapy in LAGC patients.Our nomogram showed a good predictive ability for pCR.Thus,our findings can serve as a useful reference for clinical decision making for LAGC patients.
2.Clinical results of 100 cases of modified eversion carotid endarterectomy

Objective:The primary goal of this study is to explore the safety and effectiveness of a new modified eversion carotid endarterectomy (MECEA).Methods:This is a retrospective case series study. One hundred patients were consecutively treated with MECEA by the same operator at Department of Vascular Surgery,Peking Union Medical College Hospital from January 2019 to December 2023. There were 77 males and 23 females. The age was (66.0±8.6)years (range: 39 to 85 years). Twenty-four (24.0%) patients were symptomatic with the degree of carotid stenosis over 50%,76 patients (76.0%) were asymptomatic with the degree of stenosis over 70%. All these patients meet the indication of carotid endarterectomy. The main difference between MECEA and traditional eversion carotid endarterectomy was the anterior,lateral,and posterior walls of the internal carotid artery were incised obliquely from the origin of the internal carotid artery toward the common carotid artery,leaving the wall of internal carotid artery intact at the bifurcation. The surgical process,cardiovascular and cerebrovascular complications and other surgical complications were recorded. The incidences of complications,restenosis of intraoperative target lesions and re-intervention were collected during follow-up.Results:All procedures were performed successfully under general anesthesia. The total operation time was (36.5±10.1)minutes (range: 22 to 65 minutes),and carotid clamping time was (15.0±6.3)minutes (range: 7 to 31 minutes). One patient (1.0%) occurred postoperative cerebrovascular accident,1 patient (1.0%) developed cerebral hyperperfusion syndrome (CHS),and another 1 patient (1.0%) suffered myocardial infarction. All these patients were recovered after medical treatment within a week. The follow-up time( M(IQR)) was 24 (28) months (range: 6 to 62 months). Two patients (2.0%) were reported to have hemodynamically significant restenosis within 2 years,with one patient requiring intervention. No patient suffered from ipsilateral ischemic stroke. Conclusions:MECEA is a safe and effective surgical method of treating carotid artery stenosis. This method can reduce carotid clamping time and lowers the risk of ischemic stroke. Meantime,it preserves the integrity of the adventitia at the bifurcation of carotid artery,reduces the chance of restenosis. Moreover,it might be helpful to prevent postoperative CHS due to reducing damage to the carotid body and carotid sinus nerve.

4.Effect of metformin on proliferation and differentiation of HaCaT cells and secretion of inflammatory factors by HaCaT cells

Objective To evaluate the effect of mefformin on the human keratinocyte line HaCaT,and to explore its molecular mechanism.Methods HaCaT cells were divided into several groups to be treated with mefformin at different concentrations of 1,2,5,10,20,50 mmol/L for 24,48 and 72 hours.Cell counting kit-8 (CCK-8) assay was performed to evaluate the effect of metformin on the survival rate of HaCaT cells.After 48-hour treatment with metformin at concentrations of 0 (control group),0.5,1,2,5,10 mmol/L,flow cytometry was conducted to evaluate the effect of metformin on cell cycle and apoptosis.Western blot analysis was performed to determine the expression of cell proliferation-and differentiation-related proteins (keratin-16 [K16],K17,K1,involucrin),apoptosis-related proteins (Bax,Bcl-2) and AKT/mTOR/STAT3 pathway proteins.Enzyme-linked immunosorbent assay (ELISA) was conducted to detect the levels of interleukin (IL)-8,tumor necrosis factor (TNF)-α and IL-23 (inflammatory factors) in the culture supernatant of HaCaT cells.Statistical analysis was carried out with SPSS19.0 software using one-way analysis of variance for comparison of the above indices among the 0.5-,1-,2-,5-,10-mmol/L metformin groups and control group,repeated measures analysis of variance for comparisons among different time points or different metformin groups,least significant difference (LSD)-t test for multiple comparisons.Results CCK-8 assay showed that mefformin had inhibitory effects on the proliferation of HaCaT cells (F =116.87,P < 0.05),and the cell survival rates gradually decreased along with the increase in the concentrations of mefformin.After 48-hour treatment with mefformin at concentrations of 0,0.5,1,2,5,10mmol/L,the proportion of HaCaT cells in G2/M phase gradually increased (5.55% ± 1.03%,6.37% ±0.93%,8.57% ± 1.18%,10.05% ± 0.60%,10.76% ± 0.87%,13.63% ± 1.41%,respectively,F =24.98,P <0.05),and the early apoptosis rate also gradually increased (0.78% ± 0.71%,19.18% ± 1.41%,25.67% ±1.34%,28.45% ± 0.92%,34.97% ± 2.12%,40.41% ± 1.49%,respectively,F =296.08,P < 0.05).Along with the increase in the concentrations of metformin,there were increasing trends in the expression of K1 and the pro-apoptotic protein Bax (F =8.86,5.38 respectively,both P < 0.05),while there were decreasing trends in the expression of K16,K17 and the apoptotic protein Bcl-2 (F =8.02,4.82,12.10 respectively,all P < 0.05).There was no significant difference in the expression of involucrin among different metformin groups (F =0.57,P > 0.05).After 48-hour treatment with mefformin at different concentrations,the levels of IL-8 and TNF-α in HaCaT cells significantly decreased (F =33.89,14.99 respectively,both P <0.05),while there was no significant change in the IL-23 level (F =2.12,P > 0.05).Along with the increase in the concentrations of metformin,the expression of p-AKT,p-mTOR and p-STAT3 significantly decreased (F =11.38,0.35,4.38 respectively,all P < 0.05),but there were no significant changes in the protein expression of AKT,mTOR and STAT3 (F =0.66,0.35,4.24 respectively,all P > 0.05).Conclusion Metformin can inhibit the proliferation,promote the differentiation and apoptosis of HaCaT cells,and inhibit the secretion of inflammatory factors by regulating the AKT/mTOR/STAT3 signaling pathway.
5. Application of suspension technique between dermal fascia and frontalis in upper lid lift and eyebrow augmentation

Objective:
To explore the application of the technique of eyelid lift combined with eyebrow arch augmentation in the correction of upper eyelid skin ptosis.
Methods:
The incision was taken below or above the eyebrow, the superficial epithelium removed from the skin area with subcutaneous dermis and fascia preserved; the eyebrow skin flap was separated from the underlying muscles plane through the incision near the eyebrow and to expose the muscle superior to the eyebrow arch. The reserved dermis and fascia flap was folded and fixed to the muscle near the eyebrow arch area.
Results:
Using this method we corrected 186 patients with upper eyelid skin relaxation from August 2008 to October 2018, including 178 females and 8 males. 52 patients were followed up from 6 to 50 months post operation. All the incision healed well with improved skin lift and augmented eyebrow arch, the incision scar were almost invisible. 45 patients were very satisfied with their appearance; asymmetry was encountered in 3 cases and 2 of them experienced revision surgery. 3 patients were not satisfied with the insufficient skin lift. 1 case was complaint of her unilateral dog ear in the early stage post operation.
Conclusions
This technique can augment the low eyebrow arch while improve the upper eyelid ptosis. The preserved dermis and fascia folding technique can also reduce the incision tension and obtain a invisible incision scar as a result.
6. Retrospective analysis of the clinical features and prognostic factors of 370 patients with advanced-stage diffuse large B-cell lymphoma

Objective:
The clinical features and prognosis of diffuse large B-cell lymphoma (DLBCL) were analyzed to optimize the treatment.
Methods:
We retrospectively collected the clinical data of patients with advanced-stage DLBCL from January 2006 to December 2012 in National Cancer Center/Cancer Hospital. The demographic characteristics, clinical stage, histological diagnosis, treatment and prognostic characteristics of these patients were analyzed.
Results:
A total of 370 patients with median age of 55 years old were recruited in the study. The male-to-female ratio was 1.3∶1. Among the 361 patients who underwent therapy, 280 cases received chemotherapy alone, 65 cases received chemoradiotherapy, and 16 cases received chemotherapy combined with autologous hematopoietic stem cell transplantation (AHSCT). The median follow-up period was 89 months, the 5-year overall survival (OS) rate of the entire cohort was 42.9%. The 5-year OS rate of chemotherapy alone, chemoradiotherapy and chemotherapy combined with AHSCT were 36.8%, 58.5%, 87.5%, respectively. Objective :
To compare the wear of Ceramage and IPS e.max Press offering foundation for nonmetallic materials selection in clinical.
Methods :
20 class I cavities were prepared in the approximal flattened sides of the extracted human maxillary first molars. The cavities were divided into 2 groups randomly, and then restored with Ceramage or IPS e.max Press. Inlays were prepared according to the manufactures’ instructions with indirect methods and cemented with Varolink N. The non-wear extracted maxillary third molars were used as antagonistic enamel cusps after standardized prepared. After the self-made wear-testing device concluded 200,000 cycles under a load of 20 N, the wear depth of specimens and antagonistic enamel cusps was assessed by Kavo PREP assistant.
Results :
The wear in Ceramage group was (298.30 ± 40.02) μm, while the wear of normal enamel was (206.60 ± 49.17) μm. The wear in IPS e.max Press group was (212.90 ± 61.21) μm, while the wear of normal enamel was (217.00 ± 25.10) μm. The wear of IPS e.max Press inlays was higher than Ceramage inlays (P < 0.05). Ceramage inlays showed higher wear than normal enamel (P < 0.05), while IPS e.max Press inlays was not statistically different from the normal enamel (P > 0.05).
Conclusions
Ceramage has more wear than IPS e.max Press. Ceramage shows higher wear compared with normal enamel, while IPS e.max Press has lower antagonistic war compared with normal enamel.

Objective To study the relationship between bronchial asthma and smoking status in Chinese people.Methods Asthma epidemiological survey and stratified-cluster-random method survey were performed in residents over 14 years in 8 provinces (cities) of China from February 2010 to August 2012.Asthma was diagnosed based upon case history,clinical signs and lung function test.Smoking status was investigated by questionnaire.Results Sampling population was 180 099 and 164 215 were valid.A total of 2 034 subjects were diagnosed as asthma including 79 692 men and 84 523 women.The overall prevalence rate of asthma was 1.24% (2 034/164 215).Smokers were 23.8% (39 137/164 215) in the whole population.Smokers were 34.5% (702/2 034) in asthmatic patients,compared with 23.7% (38 435/ 162 181) in no-asthmatic population.The incidence of asthma was 1.79% and 1.06% in smokers and nonsmokers respectively (P <0.001),suggesting that OR of smoking was 1.70 (95% CI 1.55-1.86,P < 0.001).According to asthma control test (ACT) score,the level of asthma control in non smoking group was higher than that in smoking group(43.2% vs 35.3%).The times of hospitalization due to acute exacerbations (0.51 vs 0.41 events/person/year),total hospitalization rate (27.35 % vs 20.12%),annual emergency room visits (0.80 vs 0.60 events/person/year) and emergency room visit rate (31.77% vs 24.47%) were all much higher in smoking asthmatic patients than those in non smoking asthmatic patients,indicating that the level of asthma control in smoking patients was significantly worse than in non smoking patients.Conclusions The smoking rate in Chinese people over 14 years is still high.The prevalence rate of asthma in smokers is significantly higher than that of non-smokers.The level of asthma control in smokers is significantly worse than that in non smokers.
9.Observation on the Clinical Effect of Seretide and Noninvasive Positive Pressure Ventilation on the COPD Combined with Respiratory Failure

Objective:To analyze the clinical efficacy of Seretide combined with noninvasive positive pressure ventilation (NIPPV) in the treatment of chronic obstructive pulmonary disease (COPD) complicated with type 2 respiratory failure (RF).Methods:120 patients of COPD complicated with type 2 RF were randomly divided into the observation group and the control group.All patients were treated with routine,symptomatic and supportive treatment,based on that NIPPV was used in the control group,and the observation group was treated with Seretide on the basis of NIPPV.The levels of arterial blood gas,inflammatory factors,pulmonary function and CAPS score before and after treatment were compared and analyzed.Results:After treatment,the pH,PaO2 and SaO2 of both groups were significantly increased,PaCO2 levels weresignificantly decreased,serum IL-33,TNF-oα and sICAM-1 levels were significantly decreased,FVC,PEFR,FEV1% and FEV1/FVC were significantly decreased,and the CAPS scores were significantly decreased;the changes of all the index mentioned above in the observation group were more statistically significant (P<0.05).Conclusion:Seretide com bined with NIPPV could more effectively decrease the levels of inflammatory factors and improve the blood gas and lung function in the patients of COPD with type 2 respiratory failure.
10.Clinical observation of three-dimensional conformal radiotherapy combined with S-1 for elderly patients with esophageal cancer

Objective To evaluate the efficacy and safety of S-1 combined with synchronous three-dimensional conformal radiotherapy in the treatment of elderly patients with esophageal cancer.Methods 80 elderly patients with esophageal cancer were randomly divided into control group and the combination group,40 cases in each group.Three-dimensional conformal radiotherapy was applied in the two groups with a total dose of DT 60-64 Gy/ 30-32f.Patients in the combination group received S-1 chemotherapy concurrent with the radiotherapy on the first day.S-1 was orally given 80mg/m2 daily after meals from d1 to d14,21 days as a cycle and treated for 2 cycles.Results The total effective rate (CR + PR) of the combination group was 87.5%,that was 62.5% of the control group,and there was statistically significant difference between the two groups (x2 =6.667,P =0.01).The one year overall survival rate of the two groups was 77.5 % and 57.5 %,respectively,the difference was not significant (P =0.056).The incidence rate of hematological toxicity was higher in the combination group than that in the control group,but the toxic reaction was mild and tolerable.Conclusion The short-term curative effect of S-1 combined with three-dimensional conformal radiotherapy is feasible for elderly patients with esophageal cancer,and the toxicity is tolerable.
