1.Multicenter study on the prediction of microvascular invasion in hepatocellular carcinoma using multiphase ultrasound imaging radiomics models

Objective:To construct and evaluate the predictive performance of a multiphase ultrasound radiomics model for microvascular invasion(MVI)in hepatocellular carcinoma(HCC).Methods:A total of 126 patients with pathologically confirmed HCC were retrospectively enrolled from 4 medical centers between May 2018 and July 2025,including the First Hospital of Shanxi Medical University,Shanxi Province Third People's Hospital,Changzhi People's Hospital,and the Organ Transplant Center of the Affiliated Hospital of Qingdao University. A total of 630 ultrasound images of the lesions in different phases were collected,from which 1 561 radiomic features were extracted. The patients from medical institutions in Shanxi Province were chosen as the training set( n=91),and the patients from the Organ Transplant Center of the Affiliated Hospital of Qingdao University were chosen as the validation set( n=35). In the training set,37.4%(34/91)patients presented MVI(+),whereas in the validation set,54.3%(19/35)patients presented MVI(+). Radiomics features were extracted from ultrasound images,and features related to the MVI(+)were selected through dimensionality reduction analysis. Five multiple machine learning algorithms were used to construct predictive models,which were then evaluated using an external validation set. The Radscore was calculated,and a nomogram was constructed combining Radscore with ultrasound and clinical characteristics to predict MVI. Results:The model combining radiomics features from the portal venous phase and the delay phase showed the best predictive performance in both the training and validation sets,with area under curve(AUC)values of 0.835 and 0.727,respectively. The prediction model developed using radiomics Radscore and clinical indicators could be represented and presented as a nomogram.Conclusions:The radiomics model based on multi-phase ultrasound offers a novel approach for non-invasive preoperative prediction of MVI in liver cancer. Furthermore,its integration with clinical features aids in optimizing clinical treatment strategies.
2.Application of dexmedetomidine combined with esketamine nasal drops in pediatric flexible bronchoscopy surgery with spontaneous respiration retained

Objective:To observe the application effect of dexmedetomidine combined with esketamine nasal drops in pediatric flexible bronchoscopy (referred to as bronchoscopy) surgery with spontaneous respiration retained.Methods:A total of 96 children undergoing bronchoscopy in the Changsha Hospital for Maternal & Child Health Care from May 2022 to December 2023 were selected and divided into the dexmedetomidine nasal drop group (group D, 47 cases) and the dexmedetomidine combined with esketamine nasal drop group (group DE, 49 cases) by random number table method. The mean arterial pressure (MAP), heart rate (HR), blood oxygen saturation (SpO 2) and respiratory rate (RR) of the two groups were observed before nasal drops (T 0), 30 minutes after nasal drops (T 1), 5 minutes after surgery started (T 2), and 10 minutes (T 3), 30 minutes (T 4), 60 minutes (T 5) and 120 minutes (T 6) after surgery. The separation anxiety and mask cooperation scores at T 1, modified Aldrete scores and Pediatric Anesthesia Emergence Delirium Scale (PEAD) scores at T 3-T 6 were evaluated; the number of children with PEAD score>10 was recorded, as well as the time to postoperatively take food and clear water. Results:There was no statistically significant difference in MAP and HR at T 0 between the two groups (all P>0.05); the MAP and HR of Group DE at T 1-T 6 were significantly higher than those of group D (all P<0.05). There was no statistically significant difference in SpO 2 and RR between the two groups within the group or between the groups at each time point (all P>0.05), while the incidence of hypoxemia (SpO 2≤92%) in group D at T 2 was significantly higher than that in group DE ( P<0.05). The separation anxiety and mask cooperation scores of group DE at T 1 were significantly better than those of group D ( P<0.05). The modified Aldrete scores of group DE at T 3, T 4 and T 5 were significantly higher than those of group D (all P<0.05). The PEAD score of group DE at T 3 was significantly lower than that of group D ( P=0.001); there was no statistically significant difference in PEAD scores between the two groups at T 4, T 5 and T 6 (all P>0.05), and no child in either group had PEAD scores>10 at T 4, T 5 and T 6. The time to take clear water and food in group DE was significantly earlier than that in group D (all P<0.05). Conclusions:In pediatric bronchoscopy surgery with spontaneous respiration retained, dexmedetomidine combined with esketamine nasal drops can more effectively relieve children′s separation anxiety, meet surgical needs, improve recovery quality, shorten the postoperative time to take food, and better implement the concept of enhanced recovery after surgery (ERAS) compared with dexmedetomidine alone.
3.Blood sugar management in aged patients with Type Ⅱ diabetes based on theoretical domain framework:a qualitative study

Objective To investigate the requirement of blood sugar management in elderly patients with Type Ⅱ diabetes and to provide a reference for development of a program in management of blood sugar.Methods Using interpretive decriptive phenomenological research,an interview outline based on the theoretical domain framework was developed and face-to-face semi-structured interviews were conducted with 16 aged patients who had Type Ⅱ diabetes in the Department of Endocrinology and Metabolism of a Grade ⅢA hospital in Jiangxi Province.The participants were selected by purposive sampling.Colaizzi 7-step analytical method,in conjunction with Nvivo 11 software,was used to analyse,summarise and extract the themes.Results Seven themes related to the theoretical domain framework were extracted,which included knowledge,skills,environmental context and resources,behavioural adjustment,motivation and goals,emotions and self-efficacy.After refinement,nine sub-themes on major requirement for blood glucose management were extracted,which included the requirements for knowledge and skills in blood sugar management,support from the community and healthcare resources,support from the family,changes in maladaptive behaviours,individualised blood sugar management,motivation for self-management,clarity in goals of blood sugar control,emotional support and psychological adjustment,and support for strengthened beliefs(9 sub-themes).Conclusion Elderly patients with Type Ⅱ diabetes have diverse and individualised needs in management of blood sugar.In the future,healthcare staff should develop appropriate strategies or programs to meet the individual needs of patients by addressing the actual needs of patients,improving the self-management behaviours of patients so as to improve the effects of the blood sugar control.
4.Differential Analysis on Chemical Composition and Pharmacodynamic Effect Between Combined Decoction and Single Decoction of Famous Classical Formula Huaganjian

ObjectiveThrough qualitatively and quantitatively analysis of the differences in chemical composition between the combined decoction and single decoction of Huaganjian and comparison of their core efficacy, to explore the rationality of the flexible clinical application of Huaganjian compound preparations and single-flavored dispensing granules. MethodsUltra performance liquid chromatography-quadrupole-electrostatic field orbitrap high resolution mass spectrometry(UPLC-Q-Exactive Orbitrap MS) was used to qualitatively analyze the combined decoction and single decoction samples of Huaganjian, and meanwhile, the contents of four index components(geniposide, paeoniflorin, hesperidin and paeonol) were quantitatively analyzed by high performance liquid chromatography(HPLC). Nonalcoholic fatty liver disease(NAFLD) rat model induced by high-fat diet was applied to compare the efficacy of combined decoction and single decoction of Huaganjian. A total of 30 male SD rats were randomly divided into the control group, model group, lovastatin group(1.8 mg·kg-1), combined decoction group(1.26 g·kg-1) and single decoction group(1.18 g·kg-1). After successful modeling, lovastatin group, combined decoction group and single decoction group were given corresponding doses of drugs by intragastric administration every day, and the control group and model group were given equal amounts of normal saline by intragastric administration, after 4 weeks of administration, the serum and liver tissues were collected, and the contents of alanine aminotransferase(ALT), aspartate aminotransferase(AST), total cholesterol(TC), triglyceride(TG), low-density lipoprotein cholesterol(LDL-C) and high-density lipoprotein cholesterol(HDL-C) in serum of rats were detected, and the liver pathological examination was carried out by hematoxylin-eosin(HE) staining and oil red O staining, so as to compare differences of their efficacy. ResultsSeventy chemical components were initially identified and attributed from the lyophilized powder of the combined decoction and single decoction samples of Huaganjian, and there was no obvious difference in composition between the two. Further quantitative analysis showed that the contents of geniposide, paeoniflorin, hesperidin and paeonol in the combined decoction samples were significantly increased when compared with those of the single decoction samples(P<0.01). The pharmacodynamic results showed that compared with the model group, both the combined and single decoction groups of Huaganjian could improve the liver index of NAFLD rats, reduce the serum levels of AST, ALT, TC, TG and LDL-C, increase the serum level of HDL-C, and ameliorate the pathological changes of liver cell steatosis and fat accumulation. However, there was no significant difference in pharmacodynamic effects between the combined decoction group and the single decoction group. ConclusionThere is no significant difference between the combined decoction and single decoction of Huaganjian in terms of chemical composition, but the contents of the four index components show significantly difference. Both of them can significantly improve the fat accumulation and liver function in NAFLD rats. This study provides a reference basis for the rational clinical application and evaluation of famous classical formula compound preparations and single-flavored dispensing granules.
5.Ultrasound-guided PRP and silver needle thermal synergy in early osteonecrosis of the femoral head

Objective To assess the efficacy and the survival rate of the hip joint following the treatment of osteonecrosis of the femoral head(ONFH)with ultrasound-guided percutaneous intra-articular platelet-rich plasma(PRP)injection in combination with percutaneous silver needle thermalolysis.Methods Fifty-six patients diag-nosed with ARCO(Association Research Circulation Osseous)stage Ⅱ ONFH were randomly allocated into two groups.The first group,designated as the PRP injection combined with silver needle thermalolysis group(Group R,n=28),and the second group,the steroid injection combined with silver needle thermalolysis group(Group S,n=28).Both groups underwent three injections(administered at 4-week intervals)and one session of silver needle thermalolysis.Outcome measures,including the Numerical Rating Scale(NRS)for pain assessment,Harris Hip Score(HHS),daily consumption of non-steroidal anti-inflammatory drugs(etoricoxib)and tramadol,progression to ARCO stage Ⅲ,and the rates of surgical intervention,were recorded at baseline,3 days,and 1,3,6,and 12 months after the treatment.Results When compared to the baseline,both groups manifested significant decreases in NRS scores and enhancements in HHS at all follow-up time points(P<0.05).Specifically,Group R demonstrated more favorable outcomes compared to Group S.At 3,6,and 12 months,Group R had lower NRS scores(P<0.01)and more notable improvements in HHS(P<0.01).Additionally,the daily analgesic consumption(etoricoxib and tramadol)in Group R was significantly lower than that in Group S at 3,6,and 12 months(P<0.01).Moreover,Group R exhibited a significantly lower progression rate to ARCO stage III and a lower hip replacement surgery rate(P<0.05).Conclusions Ultrasound-guided intra-articular PRP injection in combination with silver needle therma-lolysis expedites pain alleviation,enhances hip function,reduces analgesic dependence,and retards the progression of ONFH.This approach thus represents a clinically viable option for early-stage intervention.
6.Evaluation and Analysis of the Effectiveness of the Reform of Teaching Mode in Health Statistics by Postgraduate Students

Objective To investigate the effectiveness of the curriculum design and teaching mode reform in Health Statistics through the assessment by postgraduate students so as to enhance the teaching performance of the course.Methods A questionnaire survey was conducted among the postgraduate students of grade 2023 at a certain medical university.The survey covered such aspects as students'mastery and application of the course learning content,their evaluation and satisfaction with the course,etc.Descriptive approaches were employed to analyze and summarize the data.Results Students achieved a good command of theoretical knowledge and its application.They highly evaluated the teacher,the course content and its practicality,demonstrated a strong interest in learning,expressed a high level of satisfaction with the course,and manifested a strong willingness to continue studying the course.The learning of the course met the expectations of the students.The final exam scores in the later stage of curriculum reform(78.60±10.58)was higher than that before the reform(75.78±7.97,P<0.05);the excellent rate after the reform was 53.6%,which was higher than the 33.5%before the reform(P<0.05).Conclusion The construction of a course system that integrates knowledge,skills the mixed teaching mode of case-based teaching and the combination of theory and statistical software package operation are beneficial for enhancing postgraduate students'learning and application of the course in health statistics.It also strengthens the design and training of course application aspects for students in clinical medicine and dental medicine disciplines.
7.Investigation on status quo of indwelling needle use in 77 hospitals of Chongqing city

Objective To investigate the status quo of indwelling needles use in the hospitals at all lev-els in Chongqing city,and to evaluate the normalization and safety of indwelling needles use by referring to Nursing Practice Standards for Intravenous Therapy.Methods A self-designed questionnaire was used in July 2023 to conduct a cross-sectional survey on the use of indwelling needles in the hospitals at all levels in Chongqing city.Results A total of 30 807 patients in 77 hospitals were included,among them 62.67%used indwelling needles.The closed type indwelling needles accounted for 88.87%and the safety type indwelling needles accounted for 41.74%.Non-needle infusion connectors accounted for 37.78%,and steel needle con-nected with heparin caps accounted for 51.69%.95.66%of puncture site was in the upper limb,the non-cor-rosive drug infusion accounted for 87.57%,and the complications occurrence rate was 2.46%.Among the 6 419 surveyed nurses,75.51%removed indwelling needles based on clinical indications.There were statistically significant differences in the usage rates of closed type indwelling needles and non-needle infusion connectors among hospitals at different levels(P<0.05).There were also statistically significant differences in the inci-dence rates of complication and phlebitis(P<0.05).Conclusion The overall application of indwelling needles in the hospitals at all levels in Chongqing city is generally standardized,but the use of safety type indwelling needles and needle-free infusion connectors still needs to be improved.
8.Best evidence summary of perioperative fluid management in hepatectomy patients with liver cancer under the enhanced recovery after surgery concept

Objective:To comprehensively retrieve and evaluate the best available evidence on perioperative fluid management in patients undergoing hepatectomy for liver cancer, and to provide references for clinical practice.Methods:A systematic search was conducted across multiple platforms, including UpToDate, BMJ Best Practice, Guidelines International Network, National Institute for Health and Care Excellence, PubMed, Web of Science, China National Knowledge Infrastructure, and Wanfang Data, for clinical decisions, guidelines, expert consensus, systematic reviews, evidence summaries, and randomized controlled trials related to perioperative fluid management in patients undergoing hepatectomy for liver cancer. The search period was from database inception to March 13, 2024. The 2014 version of the Joanna Briggs Institute evidence pre-grading and recommendation grading system was used to classify the evidence.Results:A total of 14 articles were included: 3 clinical decision articles, 3 guidelines, 1 evidence summary, 5 expert consensuses, 1 systematic review, and 1 randomized controlled trial. A total of 27 best evidence items were extracted, covering seven aspects: principles of fluid therapy, preoperative fluid management, intraoperative fluid management, postoperative fluid management, fluid type selection, assessment and monitoring, and training and education.Conclusions:This study summarizes the best evidence on perioperative fluid management for hepatectomy in liver cancer patients and provides an evidence-based reference for clinical nursing practice. Healthcare professionals should apply the evidence in a personalized manner based on the actual clinical context to promote scientific fluid management in the perioperative period.
9.Transvaginal two-dimensional ultrasonography combined with three-dimensional ultrasonography OmniView for diagnosing intrauterine adhesion

Objective To observe the value of transvaginal two-dimensional ultrasonography(2D-TVS)combined with three-dimensional ultrasonography OmniView for diagnosing intrauterine adhesion(IUA).Methods Totally 200 women with suspected IUA were retrospectively enrolled,including 172 cases who were ultimately diagnosed IUA and the rest 28 without IUA.The efficacy of 2D-TVS combined with OmniView for diagnosing IUA was analyzed.Results Among 200 cases,150 cases were diagnosed IUA with 2D-TVS(135 cases were diagnosed correctly),167 cases were diagnosed IUA with OmniView(159 cases were diagnosed correctly),and 170 cases were diagnosed with combination of the above two(165 cases were diagnosed correctly).The area under the curve(AUC)of 2D-TVS,OmniView and 2D-TVS combined with OmniView for diagnosing IUA was 0.625,0.819 and 0.890,respectively,and 2D-TVS combined with OmniView had the highest AUC(both P<0.05),with sensitivity of 95.93%,specificity of 82.14%,positive predictive value of 97.06%and negative predictive value of 76.67%.Conclusion 2D-TVS combined with OmniView could be helpful for diagnosing IUA.
