To explore the correlation between MTHFR C677T gene polymorphism and hypertension, hyperhomocysteinemia(Hcy), and hyperlipidemia in the Tibetan population of Tibet. Using a cluster sampling method, participants from high-altitude regions including Ngari Prefecture, Lhasa City, and Nyingchi City in Tibet were enrolled. Differences in MTHFR C677T genotype distribution among individuals with hypertension, HHcy, and hyperlipidemia were analyzed, and multivariate logistic regression was performed to assess the association between these conditions and the TT genotype. A total of 574 eligible subjects were included, with a mean age of 40.64±12.67 years. Males accounted for 46.7%(268/574) and females 53.3%(306/574). Regional distribution was 34.8%(200/574) from Nyingchi City, 33.1%(190/574) from Lhasa City, and 32.1%(184/574) from Ngari Prefecture. Mean systolic and diastolic blood pressures were 117.89±18.98 mm Hg and 79.74±14.88 mm Hg, respectively. The frequency of the TT genotype was significantly higher in the hypertension group than in the non-hypertension group(12.32% The MTHFR C677T TT genotype is significantly associated with hypertension and hyperhomocysteinemia in the Tibetan population, suggesting that this polymorphism may be a genetic risk factor for these diseases in high-altitude regions.

3.Analysis of Current Practices and Exploration of Alternative Technologies in Use of Laboratory Animals for Minimally Invasive Surgery Education and Training
Laboratory Animal and Comparative Medicine 2026;46(2):279-287
Currently, the rapid development of minimally invasive surgical techniques poses significant challenges to the traditional surgical education and training system. The teaching model relying on laboratory animals is facing the need for transformation due to ethical controversies, high costs, and limitations of the model. Through a combined approach of systematic literature analysis and summarization of practical experience at the Da Vinci Surgical Robot Training Center at the First Affiliated Hospital of Naval Medical University, this study systematically evaluated the multidimensional efficacy of laboratory animals in training for the Da Vinci robot operating system and laparoscopic techniques. The results indicate that, under current technological conditions, laboratory animals—leveraging their advantages of authentic tissue tactile feedback, dynamic physiological responses, and the ability to be trained under complex surgical field exposure—remain the core modality for cultivating advanced surgical skills (such as vascular dissection and mobilization, organ suturing, etc.). Their irreplaceability lies primarily in key training aspects such as simulating intraoperative dynamic physiological responses and handling intraoperative emergencies. Based on interdisciplinary technical assessment and practical observation, the authors propose and construct an "Ethics-Technology Synergy" development framework. They further explore the innovative value of the virtual-real interaction mechanism in extended reality technology, the construction of pathological simulation platforms using organoid models, and the resource coordination mechanism of digital sharing platforms. Using application cases from the training center, the study verifies the potential of these technologies to form a "stepwise replacement" pathway for laboratory animals. It also highlights their key breakthroughs in standardized scenario replication, improved tactile simulation accuracy, and reduced laboratory animal usage. This study emphasizes that the transformation of the minimally invasive surgical education and training system is not only a technological innovation but also an advancement of ethical concepts. The "Ethics-Technology Synergy" pathway requires focusing on three key implementation areas: constructing a multi-modal hybrid training system from the perspective of technological integration; establishing a collaborative biological specimen sharing network from the perspective of resource management; and refining standards for assessing the necessity of animal use from the perspective of ethical practice. This provides an actionable pathway for building a new training paradigm that balances technical efficacy with ethical compliance, thereby promoting the transformation of surgical education towards precision and minimal harm.

BackgroundFamily functioning is one of the factors influencing non-suicidal self-injury (NSSI) behaviors in adolescents with depressive disorders. Previous studies have treated family functioning as a unitary construct, which may obscure the differential impacts of specific dimensions on NSSI behaviors. ObjectiveTo explore the relationships between various dimensions of family functioning and NSSI behaviors in adolescents with depressive disorders, aiming to provide precise targets for family-based interventions for adolescents with depressive disorders who exhibit NSSI behaviors. MethodsIn this cross-sectional study, 217 adolescent patients who were treated at the outpatient or inpatient department of The First Psychiatric Hospital of Harbin from January to July 2025 and met the diagnostic criteria for depressive disorders as stipulated in the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) were included as the research subjects. Assessments included a self-designed questionnaire, the Hamilton Depression Scale-17 item (HAMD-17), and the Family Assessment Device (FAD). Univariate Logistic regression analysis was employed to investigate the association between each dimension of family functioning and the NSSI behaviors, and multivariate Logistic regression was used to test the independent effect of each dimension of family functioning on the NSSI behaviors. ResultsA total of 204 cases (94.01%) of adolescent patients with depressive disorders completed the valid questionnaire survey. Among them, 134 cases (65.69%) exhibited NSSI behaviors (NSSI group), and 70 cases (34.31%) did not exhibit NSSI behaviors (non-NSSI group). Compared with the non-NSSI group, the NSSI group had a higher HAMD-17 score [(20.97±7.50) vs. (17.79±6.95), t=8.705, P=0.004], a higher FAD total score [(155.68±21.84) vs. (148.87±22.72), t=4.348, P=0.038], and a higher problem-solving dimension score [(2.54±0.49) vs. (2.34±0.51), t=7.399, P=0.007]. All the differences were statistically significant. The results of the Logistic regression analysis showed that the FAD total score (OR=1.014, 95% CI: 1.001–1.028, P=0.041) and the problem-solving dimension score (OR=2.241, 95% CI: 1.228–4.090, P=0.009) were both risk factors for NSSI behaviors. After adjusting for gender, age, residence, educational level, monthly family income, and whether being an only child, the correlation between the FAD total score and NSSI behaviors was not statistically significant (OR=1.010, 95% CI: 0.995–1.025, P=0.185), while the correlation between the FAD problem-solving dimension score and NSSI behaviors remained statistically significant (OR=2.000, 95% CI: 1.028–3.889, P=0.041). ConclusionImpaired problem-solving capacity within family functioning may constitute a risk factor for NSSI behaviors in adolescents with depressive disorders. Chinese Journal of Tissue Engineering Research 2026;30(3):553-560
BACKGROUND:Lumbar degenerative disease is a disease of the musculoskeletal system that primarily affects the intervertebral structures, and the disease is treated with lumbar fusion and disc replacement.OBJECTIVE:To conduct comparative analysis of the differences between lumbar fusion, mechanical lumbar disc prosthesis replacement, and viscoelastic lumbar disc prosthesis replacement.METHODS:The computerized tomography images of a healthy adult were used to construct a finite element model of the L2-L5 normal lumbar spine with Mimic,Geomagic, and Ansys software. The equipment required for lumbar fusion and lumbar spine replacement was constructed using modeling software, and the L3-L4 segment of the lumbar spine was processed according to the surgical requirements for lumbar fusion and intervertebral disc replacement, creating the corresponding finite element model.Specific boundary conditions were applied to extract the mobility of each lumbar spine segment, the stresses on the neighboring intervertebral discs, and the stresses on the prosthesis lining.RESULTS AND CONCLUSION:(1)Compared with the preoperative period, the maximum stress in the upper neighboring discs increased by 64.09%and 39.79%in the forward flexion and lateral bending states if the original mobility was maintained after lumbar fusion surgery. The maximum stress in the lower neighboring discs increased by 24.39%and 20.98%in forward flexion and lateral bending. This suggested that the upper adjacent discs would suffer greater stress changes than the lower adjacent discs during heavy physical labor.(2)Disc replacement did not show significant changes in adjacent disc stress, with mechanical prosthesis replacement slightly reducing adjacent disc stress, while viscoelastic prosthesis replacement was more in line with the biological properties of the disc, with maximum adjacent disc stress similar to that of the preoperative period.(3)In terms of stability, the mechanical prosthesis replacement segment showed a 51.67%increase in range of motion in the lateral bending state and a 53.27%increase in range of motion in the posterior extension state, whereas the viscoelastic prosthesis was better able to maintain mobility in the replacement segment.(4)In addition, the stresses in the liner of the mechanical prosthesis were mainly concentrated in the edge region, and this stress distribution may lead to edge wear and thus affect the service life of the prosthesis.

Laboratory Animal and Comparative Medicine 2026;46(3):397-407
ObjectiveTo verify the technical feasibility and safety of partial nephrectomy in experimental pigs using a single-port endoscopic surgical robot in a transcontinental ultra-remote environment spanning 13 000 km with an average network delay of 204 ms, and to explore a dynamic combined anesthesia strategy that can effectively prevent intraoperative body movement in animals. MethodsThe research team used one experimental pig in Shanghai, China, and one in Orlando, the United States, and performed bidirectional remote surgery using the same model of single-port endoscopic surgical robot system. The anesthesia regimen included preoperative administration of 0.02 mg/kg dexmedetomidine hydrochloride and 0.05 mg/kg fentanyl citrate for combined sedation and analgesia. During remote surgery, according to the animal's reflex activity, pain response, and train-of-four stimulation (TOF) count results, vecuronium bromide was continuously infused at an initial rate of 0.04 mg·kg-1·h-1, and the dose was dynamically adjusted. Together with isoflurane inhalation anesthesia, the animal was maintained in a deep neuromuscular block state (TOF=0) to prevent intraoperative body movement. In addition, heart rate, blood pressure, muscle relaxation, and other indicators were monitored throughout the operation. ResultsSurgical operations were successfully performed at both sites, and the network delay remained stable at 202-208 ms. There was no data packet loss and no robot equipment failure. The vital signs of the experimental animal in Shanghai, China, remained stable, with zero body movement throughout the operation. Intraoperative blood loss was controllable, and no abnormal bleeding occurred. ConclusionIn transcontinental ultra-remote robotic surgery with inherent network delay, a dynamically adjusted anesthesia strategy can effectively avoid accidental bleeding caused by animal body movement and reduce the risks associated with network latency, thereby verifying the feasibility of this technical approach. In the future, it will be necessary to build a real-time cross-regional physiological data synchronization platform, standardize anesthesia protocols for animals undergoing remote surgery, and provide a technical basis for clinical translation to human trials.

OBJECTIVE Based on comment texts from the Weibo platform, this study adopts the text mining method to systematically analyze the public’s emotional attitudes and core concerns of “medical insurance reimbursement”, and explore the public’s real experiences and feedback in the implementation of the current system. METHODS With “medical insurance reimbursement” as the keyword, Python web crawler technology was used to collect Weibo comment texts from December 2023 to April 2025. Word frequency analysis, sentiment analysis, semantic network analysis and Latent Dirichlet Allocation (LDA) topic models were adopted to mine the in-depth features of comment texts and investigate the public’s concerns and emotional attitudes toward “medical insurance reimbursement”. RESULTS A total of 3 645 valid comment texts were obtained. The overall emotional of the public toward “medical insurance reimbursement” was weakly positive: 66.4% of the comments contained positive emotions, and 32.4% contained negative emotions. Semantic network analysis indicated that positive public emotions mainly centered on the achievements of medical insurance reform and the improvement of security level, while negative emotions mainly focused on reimbursement rules, economic burden and the implementation of medical insurance policies. The LDA model identified eight core themes, mainly including access of new drugs to medical insurance catalogs, medical insurance support for traditional Chinese medicine and biomedicine, disputes over medical insurance coverage of specific medical items, cross-regional medical treatment and medical insurance settlement, medical insurance reimbursement and medical burden, as well as the approval of innovative drugs and medical insurance payment. CONCLUSIONS The public generally holds a positive attitude toward “medical insurance reimbursement”, yet still has many expectations in terms of policy implementation. On the premise of guaranteeing the sustainability of medical insurance funds, it is suggested to further improve the transparency and fairness of the reimbursement process, and promote the scientific, standardized and efficient development of the medical insurance system. Meanwhile, attention should be paid to public opinion feedback on social media, and the policy communication mechanism should be strengthened, so as to enhance the public’s sense of gain and trust in the medical insurance system.

Medical Journal of Peking Union Medical College Hospital 2026;17(4):1096-1104
As a major challenge in the global field of critical care medicine, sepsis is associated with high morbidity and mortality, driving intensive research into the development of precise risk prediction models. Among these, machine learning and deep learning-based predictive models have garnered particular attention. This article systematically reviews the evolution of sepsis risk prediction models and the challenges associated with their clinical translation, with a focus on their clinical applicability in common septic complications, including acute kidney injury, encephalopathy, and coagulation disorders. Current evidence indicates that machine learning models, by integrating multidimensional dynamic data from electronic health records, significantly enhance early warning capabilities and individualized predictive performance. However, major bottlenecks limiting clinical translation include insufficient data standardization, lack of model interpretability, and inadequate external validation. Future research should prioritize multicenter collaboration, time-series modeling strategies, and explainable artificial intelligence frameworks to optimize clinical translation pathways and facilitate the paradigm shift from traditional empirical approaches to data-driven precision strategies in sepsis risk stratification.
