1.Transient Gastric Pressure Elevation Synergizing With Impaired Esophagogastric Junction Barrier Function Plays a Pivotal Role in the Refractory Gastroesophageal Reflux Disease

Background/Aims:
The pathophysiology of refractory gastroesophageal reflux disease (RGERD), which differs from proton pump inhibitor dependent gastroesophageal reflux disease (DGERD), remains incompletely elucidated. This study aims to compare esophageal motility patterns, transdiaphragmatic pressure gradients (TPG), and reflux profiles between RGERD and DGERD patients, and to delineate dynamic pressure gradient-esophagogastric junction (EGJ) interactions in these patients.
Methods:
In this retrospective study, 274 patients who underwent 24-hour impedance-pH monitoring and high-resolution manometry, along with an assessment of proton pump inhibitor responsiveness, were classified as RGERD (32.5%), DGERD (54.4%), or non-GERD (13.1%). Clinical characteristics, TPG, esophageal motility, and reflux metrics were compared between RGERD and DGERD patients. Subgroup analysis excluding hiatal hernia (HH) was conducted to investigate the pathophysiology of RGERD.
Results:
The RGERD group exhibited a significantly higher proportion of chest pain compared to the DGERD group. Regarding reflux profiles, RGERD patients without HH (RGERDHH- group) experienced increased weakly acidic reflux (P < 0.001) and prolonged bolus exposure (P = 0.006) compared to their counterparts (DGERDHH- group). Mechanistically, the RGERDHH- group showed reduced lower esophageal sphincter basal pressure (P = 0.010) and EGJ contractile integral (P = 0.005). Notably, following a wet-swallow, the RGERDHH- group experienced the significant elevation in gastric pressure and TPG. Correlation analyses revealed weakly acidic reflux and bolus exposure were positively correlated with gastric pressure variation, and inversely correlated with lower esophageal sphincter basal pressure.
Conclusions
Transient gastric pressure elevation and compromised EGJ barrier function drive weakly acidic reflux and esophageal bolus exposure. This pressure gradient-barrier mismatch underpins the refractoriness of RGERD.
2.Efficacy of low temperature plasma in treatment of rats with Pseudomonas aeruginosa wound infection

OBJECTIVE To explore the efficacy of low temperature plasma(LTP)in treatment of the rats with Pseudomonas aeruginosa(PA)wound infection.METHODS Totally 20 SD rats were assigned to establish the PA wound infection models and were randomly divided into the LTP group with 10 rats and the air control group with 10 rats.The wound healing dynamics,bacterial loading capacity,histopathology,levels of cytokines and microen-vironment features were observed during the healing process.RESULTS LTP accelerates the healing of PA infec-tion wound,the wound area reduced after the treatment for 3 days,and the inflammatory reactions alleviated;the infection symptoms subsided after the treatment for 7 days,and the wounds became dry and scabbed;the wounds reduced in size by day 15,showing favorable healing trends.Repeated measurement analysis of variance(ANOVA)indicated that there was significant difference in the impact of LTP on the covering rate of wounds(Ftime=10.230,P<0.001;Fbetween-group=11.340,P=0.001;Finteraction=2.890,P=0.042).The result of quantitative bacterial loads analysis showed that the bacterial loads of the LTP group were lower than those of the control group after the treatment for 3,7 and 15 days(P<0.05).The result of histopathology indicated that the infiltra-ted wound inflammatory cells of the LTP group reduced,the collagen fibers were well-aligned,and the neovascu-larization and formation of granulation tissues were evident.In addition,the pH value of wound tissues and reac-tive oxygen species(ROS)level were improved after the treatment with LTP,which was matched with the process of wound healing.The result of cytokines test showed that LTP could upregulate the levels of VEGF and TGF-β1,downregulate the levels of IL-1β and TNF-α,and promote the wound healing,with statistically signifi-cant differences observed(P<0.05).CONCLUSION LTP may facilitate the control of infections by reducing the bacterial colony counts,establish the effective microenvironment for tissue healing,and accelerate the healing of wounds with PA infection by regulating the release of healing-related cytokines.
3.Diagnostic value of immunohistochemical and molecular markers in diffuse pleural mesothelioma

Purpose To investigate the diagnostic value of immunohistochemical(IHC)and molecular markers in diffuse pleural mesothelioma(DPM).Methods A total of 114 cases of DPM were retrospectively analyzed for clinical and imaging manifestations,histologic subtype and tumor grade.The positivity rates of Calretinin,WT-1,CK5/6,MC,D2-40,UPK3B,and GATA3 were assessed by IHC,and the loss rates of BAP-1 and MTAP were determined.The concordance between MTAP IHC and p16 gene fluorescence in situ hybridization(FISH)status was calculated,a-long with the sensitivity and specificity of MTAP IHC relative to p16 FISH.Results Among the 114 DPM patients,66(57.9%)were male and 48(42.1%)were female,with a mean age of 58.1 years(range 16-85 years).Imaging predominantly demonstrated pleural effusion and multiple pleural nodules(55.3%,63/114).Histologically,epitheli-oid,sarcomatoid and biphasic subtypes accounted for 88(77.2%),17(14.9%)and 9(7.9%)cases,respectively.Within the epithelioid group,low and high-grade tumors numbered 69(78.4%)and 19(21.6%),respectively.In epithelioid DPM,the highest IHC positivity rates were observed for Calretinin(92.4%,81/88),D2-40(90.0%,79/88)and WT-1(90.0%,79/88).In sarcomatoid DPM,D2-40(76.5%,13/17),WT-1(64.7%,11/17),and Cal-retinin(29.4%,5/17)showed the greatest positivity.UPK3B was positive in epithelioid(59.1%,39/66)and bi-phasic cases(66.7%,4/6),but was absent in sarcomatoid tumors(0/12).Among all DPM cases,loss rates were 47.3%(53/112)for BAP-1 and 19.2%(20/104)for MTAP by IHC,p16 gene deletion by FISH was 31.5%(34/108);Concordance between MTAP IHC and p16 FISH was 81.0%(81/100);MTAP IHC had a specificity of 95.5%(64/67)and sensitivity of 51.5%(17/33)relative to p16 FISH.Additionally,GATA3 was highly expressed in sarco-matoid DPM(76.5%,13/17).UPK3B positivity differed significantly between thoracoscopic DPM(59.2%,32/54)and percutaneous biopsy samples(36.7%,11/30)in epithelioid DPM(P<0.05).WT-1 positivity was higher in thoracoscopic than percutaneous samples of sarcomatoid DPM(90.0% vs 28.6%,P=0.009).Conclusion Calreti-nin,D2-40,and WT-1 are highly sensitive mesothelial markers and should serve as first-line IHC stains in DPM diag-nosis.UPK3B is diagnostically valuable in epithelioid DPM,GATA3 may complement the diagnosis of sarcomatoid DPM,and MTAP IHC can be used as a surrogate or adjunct to p16 FISH.
4.Qualitative study on the experience of fall alertness in elderly patients with rheumatoid arthritis

Objective To understand the experience of fall alertness in elderly patients with rheumatoid arthritis and to provide references for the development of targeted nursing intervention strategies.Methods 13 elderly patients with rheumatoid arthritis,who were admitted to the Department of Rheumatology and Immunology of a tertiary hospital in Guizhou Province from February to April 2024,were selected through purposive sampling.Phenomenological qualitative research methods were used,with semi-structured in-depth interviews.Data were analyzed using Colaizzi's seven-step method,and themes were extracted.Results 3 main themes and 9 sub-themes were identified:the process of fall risk perception(reshaping fall risk cognition,establishing emotional coping mechanism,enhancing fall alertness effect),behavioral responses to fall alertness(creating a safe living environment,regularizing daily routine plan),and needs for fall prevention support(need for fall prevention knowledge,home rehabilitation care needs,intelligent device usage needs,psychosocial support needs).Conclusion Clinical medical staff should pay attention to the transformation process of fall risk perception in elderly rheumatoid arthritis patients,accurately identify patients'fall support needs in practice,and guide patients to adopt appropriate fall alert response behaviors through innovative fall demand support interaction paths and the construction of social support network systems,in order to improve patients'fall alertness and reduce their fall risk.
5.Translate the nursing time management Scale into Chinese version and test its reliability and validity

Objective To translate the nursing time management scale(NTMS)into Chinese and test its reliability and validity,hence to provide a tool for evaluating the time management among nurses in China.Methods Authorised by the author on June 30th,2023,the English version of NTMS was translated into a Chinese version through forward translation,reverse translation,cross-cultural debugging and pre-test by using Brislin translation model.Between September 2023 and January 2024,a convenience sampling method was employed to select a total of 409 nurses as the test subjects from five hospitals of Grade IIA and above in Dongguan.Then the reliability and validity of the translated version of NTMS were tested.Results All 409 nurses completed the tests.The Chinese version of NTMS included three dimensions comprising 15 items:6 in nursing work planning and goal setting,3 in organisation of nursing work and 6 in coordination of nursing work.It was found that the mean scale content validity index(S-CVI/Ave)was 0.920,and the item-level CVI(I-CVI)ranged from 0.840 to 1.000.The overall Cronbach's α coefficient of the scale was 0.894,with 0.700 for the split-half reliability coefficient of and 0.942 for the retest reliability.The Cronbach's α coefficient of each of the 3 dimensions ranged from 0.781 to 0.901,with the split-half reliability coefficient from 0.823 to 0.870 and the retest reliability from 0.692 to 0.965.In the exploratory factor analysis,the KMO value was 0.904.The statistic test of Bartlett's sphericity was 3941.503(P<0.05),with the factor load range of each entry from 0.629 to 0.838.The cumulative variance contribution rate was 67.672%.Conclusion The translated Chinese version of the nursing time management scale demonstrates good reliability and validity.It can be applied as an effective evaluation tool in the nursing time management.
6.The clinical efficacy of two types of exoskeleton robots for training balance after stroke

Objective:To compare the clinical efficacy of a fixed exoskeleton robot with that of an exoskeleton walker in training the balance of stroke survivors in the subacute stage.Methods:Thirty persons with subacute stroke were divided at random into a fixed robot group (15 cases) and a walking robot group (15 cases). In addition to conventional rehabilitation treatment, the fixed robot group underwent daily 20-minute treadmill-based fixed exoskeleton robot training, 5 days a week for 2 weeks. The walking group was trained with an exoskeleton walking robot on the same schedule. Before and after the treatment, the Berg balance scale, the sitting modified functional stretch test (SMFRT), an isokinetic muscle strength test and the modified Barthel index (MBI) were used to evaluate the subjects′ global balance, self-dynamic balance, knee proprioception and ability in the activities of daily living (ADL).Results:After the treatment, both groups′ average Berg score, SMFRT extension difference, proprioceptive absolute error angle in the affected knee and MBI had improved significantly. The average SMFRT extension difference after treatment in the walker group was then significantly better than the fixed robot group′s average. However, the fixed robot group′s proprioception in the affected knee joint was then significantly better, on average, than among the walking robot group.Conclusions:Both fixed exoskeleton robot training and exoskeleton walker training can improve the balance, proprioception and ADL ability after a stroke. The former better improves dynamic balance; the latter better improves proprioception in the affected knee joint.

10.Research on the trinity path of integrating narrative education into the cultivation of humanistic care a-bility for higher vocational nursing students

Objective To explore the effectiveness and practical pathways of integrating narrative education into higher vocational nursing courses for cultivating students' humanistic care ability.Methods A total of 156 nursing students from a higher vocational college were randomly divided into an observation group(narrative education intervention)and a control group(traditional teaching).Quantitative evaluations were conducted using the Caring Ability Inventory(CAI)and a self-developed Humanistic Care Quality Scale,supplemented by semi-structured interviews for qualitative thematic analysis.Results The ob-servation group demonstrated significantly higher total caring ability scores(180.1±10.9 vs 145.2±14.3),humanistic care quality(97.5±3.8 vs 77.7±5.9),and course satisfaction(9.5±0.6 vs 7.0±1.3)compared to the control group(P<0.01).Qualitative findings revealed that narrative education enhanced professional beliefs and practical skills through empathy training,reflective writing,and team collaboration.Conclusion Narrative education systematically improves the humanistic care ability and professional of higher vocational nursing students,providing empirical support for nursing education reform.
