1.Effect of mechanism-based secondary preventive scheme on recurrence of intracranial arterial stenosis-related stroke cerebrovascular disease event and related factors analysis

Objective To investigate the effect of secondary preventive scheme on recurrence rate of ce-rebrovascular event based on China ischemic stroke subclassification(CISS)of intracranial arterial stenosis is-chemic stroke(IS).Methods A total of 192 patients with intracranial arterial stenosis IS were prospectively included and the secondary preventive scheme was formulated according to whether or not based on the patho-genesis.Then the patients were divided into the personalized treatment group and conventional treatment group.The personalized group conducted the classification and was given different the secondary preventive schemes the artery-artery embolism group was given the active lipid-lowering scheme to make the low density lipoprotein(LDL)reaching the standard;the low hypoperfusion group was given the smooth pressure reduc-tion program;the carrier artery occlusion perforating artery group was given the routine secondary prevention program;the mixture mechanism group was given the corresponding schemes superposition according to dif-ferent mechanisms].The conventional treatment group was given the conventional secondary preventive scheme.The difference in the recurrence rate of cerebrovascular events on 90 d was compared between the two groups.Meanwhile the univariate and multivariate logistic regression were used to analyze the influencing fac-tors of recurrence of cerebrovascular events on 90 d in intracranial arterial stenosis IS.Results Among 192 study subjects,there were 90 cases in the personalized treatment group(20 cases of vector artery occlusion and perforator artery,44 cases of arterial-arterial embolization,6 cases of hypoperfusion and 20 cases of mixed mechanism).There were 102 cases in the conventional treatment group(16 cases of perforator artery occlu-sion of the vector artery,52 cases of arterial-arterial embolism,8 cases of hypoperfusion and 26 cases of mixed mechanism).The occurrence rate of cerebrovascular events on 90 d in the personalized treatment group was significantly decreased compared to the conventional treatment group(7.8%vs.17.6%,χ2=4.112,P=0.043).The Logistic regression analysis revealed that the active lipid-lowering scheme for LDL reaching the standard was the independent protective factor of the cerebrovascular event recurrence on 90 d(OR=0.128,95%CI:1.150-71.170).Conclusion The personalized secondary prevention scheme based on pathogenesis reduces the recurrence rate of cerebrovascular event in intracranial arterial stenosis IS,in which making the LDL reaching the standard by the active lipid-lowering scheme is the independent protective factor for cerebro-vascular event recurrence in intracranial arterial stenosis IS.
3.The investigation of the effect of hospital -family bed nursing model on patients with post -stroke depression

Objective To investigate the improving efficacy of hospital -family bed nursing model on pa-tients with post -stroke depression.Methods 80 patients with post -stroke depression were randomly divided into two groups,40 cases in each group.In -hospital comprehensive rehabilitation intervention was applied in the control group,while comprehensive rehabilitation intervention of hospital -family bed was conducted in the observation group.Hamilton Depression Scale (HAMD)was adopted to evaluate the depressive condition before and after inter-vention.Non -conditional logistic regression analysis was applied to analyze the correlation between the patients'age, sex,scores before intervention,hospital -family bed nursing model and the improvement level of HAMD scores. Results The HAMD scores after nursing intervention in observation group and control group were significantly decreased [(control group from (29.57 ±5.69)points to (19.22 ±7.21)points,t =10.329,P =0.000;observation group from (28.60 ±4.12)points to (12.02 ±5.81)points,t =14.302,P =0.000)].After intervention,HAMD score in the observation group was significantly lower than the control group (t =0.786,P =0.000).The effective rate of the observation group was higher than the control group(97.5% vs.70.0%,χ2 =11.114,P =0.001).The results of multivariate logistic regression analysis showed scores before intervention (95%CI:0.788 -0.995,P =0.040)and hospital -family bed nursing model (95%CI:2.501 -23.309,P =0.000)were the independent factors of the reduction of HAMD score.Conclusion Rehabilitation nursing using hospital -family bed nursing model has a markedly effect on patients with post -stroke depression and it deserves the clinical expansion.
4.Application of single solution focused approach on nursing intern learning team in emergency department

Objective To explore the application of single-solution focused approach on nursing intern team in Emergency Department.Methods We selected ninety nursing interns from the period of January to December 2013, and intervene by single-solution focused approach.The nursing scale of learning organization had been utilized and compared and analyzed the differences between before and after intervention.The intervention model had been assessed.Results After intervention, ninety interns acquired (80.31 ±4.68) score for nursing scale of learning organization, which was beyond the score of before intervention (74.30 ±3.92) (t=6.26,P <0.05).The global consciousness and strategic leadership, team cooperation and innovation, advocate dialogue and communication and investigation achieved [(23.12 ±4.28),(24.54 ±3.12),(26.05 ± 2.55) and (11.03 ±2.40) respectively] after intervention, which all of them were higher than those score of before intervention [(21.36 ±3.90),(20.79 ±2.56),(23.44 ±3.24) and (9.78 ±2.34) respectively] (t=2.44,4.98,3.19,2.12, respectively; P <0.05).Conclusions The model of single-solution focused approach on nursing intern team in Emergency Department could improve nurses’ team learning capability and construct learning organization better.
