1.Effect of Xiangsha Liu Junzitang Combined with Phlegm-removing and Detoxifying Traditional Chinese Medicine on Immune Escape in Lewis Lung Cancer Mice

ObjectiveTo study the effect and mechanism of Xiangsha Liu Junzitang combined with phlegm-removing and detoxifying traditional Chinese medicine on immune escape in Lewis lung cancer mice. MethodA total of 60 specific-pathogen-free (SPF)-grade C57BL/6J male mice were injected subcutaneously with 0.2 mL of Lewis cell suspension (containing 2×106 cells·mL-1) in the right mid-axillary line. After 7 days, the mice that had been successfully modeled were randomly divided into six groups: the model group, the cisplatin group, the Xiangsha Liu Junzitang low-, medium-, and high-dose groups, and the combined group, with 10 mice in each group. The Xiangsha Liu Junzitang low-, medium- and high-dose groups were gavaged with 17.88, 35.75, 71.50 g·kg-1 Xiangsha Liu Junzitang solution once a day, respectively, and the dosage of cisplatin intraperitoneally injected into the mice was converted to 5 mg·kg-1 twice a week, and the tumour volumes of each group were measured every two days. The intervention lasted for 14 consecutive days. At the end of treatment, the tumour mass of mice in each group was weighed and the tumour inhibition rate was calculated. The morphological characteristics of tumours in each group were observed by hematoxylin-eosin (HE) staining. Fluorescent quantitative real-time polymerase chain reaction (Real-time PCR) assay was used to detect messenger ribonucleic acid (mRNA) contents of the natural killer group 2 member D (NKG2D) receptor, ribonucleic acid export-1 (RAE-1), and γ interferon (IFN-γ) in the tumour tissues of each group. NKG2D, RAE-1, and IFN-γ mRNA in tumour tissues of each group. Immunohistochemistry (IHC) and Western blot were applied to detect the expressions of RAE-1, NKG2D, and IFN-γ in tumour tissues of each group, and Western blot was used to detect the expressions of interleukin-6 (IL-6), Janus kinase 2 (JAK2), p-JAK2, signal transducer and activator of transcription 3 (STAT3), and p-STAT3 in tumour tissues of each group, as well as the protein levels of NKG2D, and RAE-1 in spleen tissues of each group. ResultCompared with that in the model group, the tumour mass decreased in all dose groups of Xiangsha Liu Junzitang, with no statistically significant difference. The tumour volume was reduced (P<0.05, P <0.01). The pathological morphology was improved. The mRNA contents of NKG2D, RAE-1 and IFN-γ were increased in the medium-dose group (P<0.05, P<0.01), and the protein expressions of NKG2D, RAE-1, and IFN-γ in tumour tissues were elevated (P<0.05, P<0.01), and p-JAK2 and p-STAT3 protein expressions were decreased (P<0.05, P<0.01). In spleen tissues, the protein expressions of NKG2D and RAE-1 in all dose groups of Xiangsha Liu Junzitang were significantly elevated (P<0.01). Compared with those in the cisplatin group, NKG2D, RAE-1 and IFN-γ mRNA contents were elevated in the middle-dose group of Xiangsha Liu Junzitang, and the difference was not statistically significant. IHC showed that the protein expressions of NKG2D and IFN-γ in the combined group were significantly elevated (P<0.01), and Western blot results showed that the protein expressions of RAE-1, NKG2D and IFN-γ were elevated (P<0.05, P<0.01). p-JAK2 and p-STAT3 protein expressions were decreased in the combined group (P<0.05, P<0.01). NKG2D and RAE-1 protein expressions were significantly increased in spleen tissues of the medium-dose groups and the combined group (P<0.01). ConclusionXiangsha Liu Junzitang combined with phlegm-removing and detoxifying traditional Chinese medicine can inhibit the growth of tumours in Lewis lung cancer mice by up-regulating the expressions of RAE-1/NKG2D, promoting the activation of NK cells, and inhibiting immune escape, the mechanism of which may be related to down-regulation of the JAK2/STAT3 pathway.
2.Clinical application of magnetic resonance imaging-guided radiation therapy in the treatment of pancreatic cancer

At present, pancreatic cancer is a solid tumor with the worst prognosis. Compared with surgery and chemotherapy, radiotherapy plays an auxiliary role in the treatment of pancreatic cancer. In recent years, significant advances have been achieved in radiotherapy technology, which have been gradually applied in the diagnosis and treatment of pancreatic cancer. In this article, the progress in radiation therapy in the treatment of pancreatic cancer was reviewed, especially the clinical application of magnetic resonance imaging-guided radiation therapy in the treatment of pancreatic cancer, aiming to deepen the understand of the progress in radiation therapy for pancreatic cancer, and providing reference for improving the survival rate of pancreatic cancer patients.
3.Analysis on correlation between body components at T4 thoracic vertebra plane on chest CT in patients with multiple myeloma and prognosis

Objective:To automatically segment four body components at the T4 thoracic veertebra plane on chest CT in the newly diagnosed multiple myeloma(MM)patients by deep learning model,and to discuss the correlation between the four body components and the prognosis of the MM patients.Methods:The retrospective analysis was conducted on the clinical data of the MM patients diagnosed in our hospital from January 2017 to December 2021.The clinical informations such as age,gender,weight,height,and body mass index(BMI)of the patients were collected.The laboratory data of the patients were collected,including serum levels of lactate dehydrogenase(LDH),calcium(Ca),creatinine(Scr),albumin(Alb),hemoglobin(Hb),β2-microglobulin(β2-MG),and serum free light chains.The chest CT images of 79 regularly evaluated MM patients detected by deep learning model were divide into four body components:pectoralis major,pectoralis minor,subcutaneous fat,and mediastinal fat.Image J software was used to detect the areas of the four body components at the T4 thoracic vertebra plane,and their correlation with the prognosis of the MM patients was analyzed by Kaplan-Meier survival analysis.Results:The univariate analysis results showed that the area of subcutaneous fat,serum Ca levels,Scr levels,and International Staging System(ISS)stage were related to the overall survival(OS)of the MM patients(HR=2.260,95%CI:1.116-4.578,P=0.024;HR=2.088,95%CI:1.007-4.327,P=0.048;HR=2.209,95%CI:1.105-4.414,P=0.025;HR=1.730,95%CI:1.040-2.879,P=0.035).The multivariate analysis results showed that the area of subcutaneous fat among the four body components was an independent risk factor affecting the prognosis of the MM patients(95%CI:1.228-5.782,P=0.013).The Log-Rank test results showed that compared with high subcutaneous fat area group,the OS of the patients in low subcutaneous fat area group was decreased(P=0.018).There was no significant difference in OS of the patients with different genders between high subcutaneous fat area group and low subcutaneous fat area group(P>0.05).In the patients without hematopoietic stem cell transplantation,compared with high subcutaneous fat area group,the OS of the patients in low subcutaneous fat area group was decreased(P=0.037).Conclusion:Among the four body components at the T4 thoracic vertebra plane,the area of subcutaneous fat is related to the OS of the MM patients and it is an independent risk factor for the prognosis of the MM patients,while the areas of mediastinal fat,pectoralis major,and pectoralis minor have no predictive value for the prognosis of the MM patients.
4.Effect of Asari Radix et Rhizoma-Zingiberis Rhizoma Herb Pair on Lung and Liver Lipid Metabolism in Rats with COPD Based on Metabolomics and Network Pharmacology

ObjectiveTo investigate the effects of Asari Radix et Rhizoma-Zingiberis Rhizoma herb pair (XGHP) on lung and liver lipid metabolism in rats with chronic obstructive pulmonary disease (COPD). MethodForty SD male rats were divided into a normal group (10 rats) and a model group (30 rats). The method of cigarette smoke + tracheal injection of lipopolysaccharide(LPS) + cold stimulation was used to replicate COPD model with the syndrome of cold phlegm obstruction in lung. A COPD group, a XGHP group (5.4 g·kg-1·d-1), and an aminophylline group (0.5 g·kg-1·d-1) were established after successfully inducing the model, with 10 rats in each group. After treatment, the serum triglyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) levels of rats in each group were measured. Gas chromatography-mass spectrometer (GC-MS) was used to detect the differential metabolites in the lung and liver tissues of rats in each group, and the relevant targets of the differential metabolites were predicted by network pharmacology. Molecular docking was used to verify the binding ability of key components in XGHP to the relevant targets in network pharmacology. The mRNA and protein expression levels of peroxisome proliferator-activated receptor α (PPARα) and fatty acid binding protein 4 (FABP4) in lung and liver tissues of rats in each group were detected by real-time polymerase chain reaction (PCR) and Western blot. ResultXGHP significantly increased the levels of TG, TC, and LDL-C in serum (P<0.05), and decreased the level of HDL-C (P<0.05) in rats with COPD. GC-MS results showed that there were 8 lung differential metabolites and 17 liver differential metabolites in the COPD group and XGHP group. Network pharmacology predicted 59 common targets for the two differential metabolites, mainly enriched in the PPAR signaling pathway. Molecular docking results showed that the main components in XGHP were well combined with both PPARα and FABP4. Real-time PCR showed that XGHP effectively up-regulated the expression levels of PPARα and FABP4 mRNA (P<0.05), and Western blot showed that XGHP effectively up-regulated the expression levels of PPARα and FABP4 proteins (P<0.05) in lung and liver tissues of rats with COPD. ConclusionXGHP effectively improves the blood lipid levels of rats with COPD, which may be related to the increase of the expression levels of PPARα and FABP4 mRNA and proteins in the PPAR signaling pathway, thus regulating lung and liver lipid metabolism.
5.Misdiagnosis of angiosarcoma of foot as diabetic foot ulcer: One case report and review of literature

Angiosarcoma is a rare malignant tumor of soft tissue in adults. However, tumor ulcers caused by foot angiosarcoma are rarely reported in the literature. This paper outlines a case of foot angiosarcoma misdiagnosed as diabetic foot ulcer. It was treated as diabetic foot ulcer in other hospitals without notable improvement. In consideration of its special clinical characteristics and treatment process, pathological biopsy was performed after admission, and angiosarcoma was confirmed. The purpose of this article is to emphasize that foot angiosarcoma or other tumor ulcers in diabetes patients are easily misdiagnosed as diabetic foot ulcers. If there is no obvious improvement in treatment, pathological tissue biopsy should be performed as early as possible.
6. Early efficacy evaluation of the treatment of complex proximal humeral fractures in the elderly with reverse shoulder arthroplasty

Objective:
To evaluate the short-term efficacy of one-stage reverse shoulder arthroplasty in the treatment of complex proximal humeral fractures in the elderly.
Methods:
Data of 43 elderly patients with complex proximal humeral fractures who were treated by reverse shoulder arthroplasty from July 2017 to January 2019 were retrospectively analyzed. There were 12 males and 31 females with an average age of 72 years (range, 66-78 years). All fractures were fresh and close which caused by trauma. The average time from injury to operation was 8.0 days (range, 6-11 days). According to Neer classification, 21 cases (48.8%, 21/43) were three-part fractures, 22 cases (51.2%, 22/43) four-part fractures. Visual analogue scale (VAS), Neer shoulder replacement evaluation system and Constant-Murley score were used to evaluate the postoperative results.
Results:
All operations were successfully completed. The average operation time was 141.3 minutes (range, 120-170 minutes. The mean blood loss was 407 ml (range, 250-700 ml) and intraoperative blood transfusion was 446.5 ml (range, 400-800 ml). All patients were followed up for 10.9 months (range, 6 to 16 months). All patients were discharged within 7 days after operation, and no wounds related complications occurred. The bigger and lesser tuberosities of all patients healed completely within 8 weeks. At the latest follow-up, no loosening or dislocation of prosthesis occurred, and the forward elevation was 133.0° (range, 100°-165°); external rotation was 29.5° (range, 20°-35°); internal rotation was 46.7° (range, 30°-60°). VAS score was 0.8 (range, 0-3). The Neer score was 87 (range, 73-98), with 20 cases (46.5%, 20/43) excellent, 16 cases (37.2%, 16/43) good, and 7 cases (16.3%, 7/43) fair. Constant-Murley score was 88.7 (range, 70-98). A 71-year-old patient had symptoms of axillary nerve injury after operation and recovered completely 6 weeks after the operation, which did not affect the functional rehabilitation exercise or the stability of the prosthesis. During the follow-up, no other complications such as infection, acromial stress fracture and scapular notching were found in all patients.
Conclusion
The short-term effect of one-stage reverse shoulder arthroplasty for the treatment of complex proximal humeral fractures in the elderly is satisfactory.
7.An analysis of 445 lawsuits of medical damage liability disputes in Beijing

Objective To identify departments involved, and the appraisal and compensation of such disputes in Beijing, for preventive recommendations. Methods A retrospective descriptive statistical analysis was carried out to retrieve and extract keywords from the first-instance judgments totaling 445 cases of medical damage liability disputes from 2014 to 2016 in Beijing. This analysis covered the age, disease, departments involved, liability judgment among other information. These cases were published at the website of China judgment documents. Results Patients with musculoskeletal connective tissue diseases, circulatory system disease and tumor accounted for the most lawsuits. The departments involved with more disputes were the department of gynecology and obstetrics (80/445, 18. 0%), orthopedics (69/445, 15. 5%), and emergency department(45/445, 10. 1%). 83. 8 percent of the disputes were found with medical quality problems;and 84. 3% were lost with compensations. Conclusions It is imperative to elevate the diagnosis and treatment level on musculoskeletal connective tissue diseases, circulatory system disease and tumors, and take precautions against disputes at such key departments as gynecology and obstetrics department, orthopedics department, and emergency department. Other precautions include normalized medical record writing, medical risk disclosure, higher medical quality and patient safety.
8. Study on the toxicology of traditional medicine Nao-de-kang

According to the digital table, SD rats were randomly divided into blank group and Nao-de-kang small dose group(3.85g/kg), high dose group(7.71g/kg) and maximum tolerated dose group(77.14g/kg), 20 rats in each group, half male and half female.All groups were treated for 12 weeks, and the maximum tolerated dose group was treated for 1 week.The animal activity during treatment, the blood routine indicators, blood biochemical index and the organs for pathological examination were recorded and compared.
Results:

9.Effect of nebulized hypertonic saline solution for bronchiolitis in infant

Objective To assess the efficacy and safety of nebulized hypertonic saline solution in infant with bronchiolitis.Methods From January 2014 to January 2016,95 patients at 3-13 months old in our hospital who diagnosed as bronchiolitis were randomly divided into three groups.On the basis of conventional suit support treatment,the observation group A was given nebulized 3% hypertonic saline (n =32),the observation group B was given nebulized 3% hypertonic saline and salbutamol (n =32),and the control group C was given nebulized normal saline (0.9%) and salbutamol (n =31),this therapy was repeated every 6 hours until discharge.The Lowell score,clinical symptoms remission time,days of hospitalization and incidence rate of adverse reaction of the three groups were compared.Results After treatment,the symptoms and signs of the three groups were all improved.The Lowell scores were lower in the two observation groups compared to the control group C in 24,48,72 hours after treatment[(5.81 ±1.53) points and (5.85 ± 1.37) points vs.(6.61 ± 1.54) points,(4.75 ± 1.34) points and (4.72 ± 1.30) points vs.(5.52 ± 1.29) points,(3.19 ± 1.15) points and (3.22 ± 1.16) points vs.(3.90 ± 1.01) points,Z =-1.999,-2.241,-2.518 and-2.002,-2.335,-2.316,all P < 0.05).And the cough,wheezing remission time and pulmonary rales disappearance time,days of hospitalization in the two observation groups were also shorter,there were statistically significant differences [(6.63 ± 1.41) d and (6.56 ± 1.37) d vs.(7.35 ± 1.25) d,(5.19 ± 1.03) d and (5.25 ± 1.05)d vs.(5.87 ± 1.09)d,(5.75 ±1.34)d and (5.72 ± 1.51)d vs.(6.68 ± 1.60)d,(7.25 ± 1.37)d and (7.16±1.48)d vs.(8.10±l.47)d,Z=-2.498,-2.469,-2.359,-2.213 and-2.982,-2.405,-2.373,-2.222,P <0.05,or P <0.01)].There were no significant differences in the Lowell score,the length of time of cough,wheeze,lung rales disappears and the length of hospital stay between the observation group A and observation groupB[(5.81 ± 1.53) points vs.(5.85 ± 1.37) points,(4.75 ± 1.34) points vs.(4.72 ±1.30) points,(3.19± 1.15) points vs.(3.22 ± 1.16) points,(6.63 ± 1.41) d vs.(6.56 ± 1.37) d,(5.19 ±1.03)d vs.(5.25 ± 1.05)d,(5.75 ± 1.34) d vs.(5.72 ± 1.51) d,(7.25 ± 1.37) d vs.(7.16 ± 1.48) d,Z =-0.164,-0.021,-0.140,-0.295,-0.167,-0.374,-0.233,all P > 0.05].Children in three groups had no serious adverse events (all P > 0.05).Conclusion Nebulized hypertonic saline in the treatment of bronchiolitis can relieve symptoms and signs,shorten the hospitalization time,and has less adverse reaction,it is worthy of clinical use.
10.Systemic Reactions to Dust Mite Subcutaneous Immunotherapy: A 3-Year Follow-up Study.

PURPOSE: The incidence of allergen specific immunotherapy-related systemic reactions (SRs) varies among different studies, and many factors are likely to contribute to SRs. This study aims to investigate the incidence, characteristics, and risk factors of SRs to standardize dust mite-specific subcutaneous immunotherapy (SCIT) in Central China. METHODS: All patients receiving standardized dust mites (100-100,000 SQ-U/mL; Alutard SQ, Hørsholn, Denmark) immunotherapy were followed up. Recorded data included demographics, diagnosis, patient status, pulmonary function testing results before and after each injection, allergen dosage, and details of SRs. RESULTS: From June 2011 to August 2014, a total of 208 patients received 4,369 injections; 27 (13.0%) patients experienced 48 (1.1%) systemic reactions. Most of the SRs were grade 2 reactions (n=30, 62.5%), followed by grade 1 (n=11, 22.9%), grade 3 (n=7, 14.6%), and no fatal reactions occurred. Forty-six SRs (95.8%) occurred within 30 minutes. Higher SR rates were associated with high concentration extracts (100,000 SQ-U/mL), injections with concomitant local reactions (LRs), children, asthma and high sensitivity (skin prick test 3+/4+ and/or sIgE≥17.5 kUA/L) (P<0.05). The estimated odds of SRs increased in children (OR=6.57; 95% CI: 1.88-22.97, P=0.003), asthmatic patients (OR=4.10; 95% CI: 1.72-9.80, P=0.002), and injections with LRs (OR=2.41; 95% CI: 1.33-4.36, P=0.004). CONCLUSIONS: The incidence of SRs to dust mite SCIT was low, and multiple factors were associated with the increased incidence of SRs. Children, asthmatics and patients with concomitant LR may be prone to develop SRs.
