1.Effects of resistance exercise therapy on pain and function in patients with cervical spondylotic radiculopathy:a meta-analysis

OBJECTIVE:In recent years,resistance exercise therapy as a means of rehabilitation has attracted much attention,but its therapeutic effect on cervical radiculopathy is still controversial.This study evaluated the clinical efficacy of resistance exercise in the treatment of cervical radiculopathy through a systematic meta-analysis.METHODS:A comprehensive search was conducted in CNKI,VIP,WanFang,Chinese Biomedical Literature Database,PubMed,Web of Science,Cochrane Library and Embase for clinical randomized controlled trials related to resistance exercise in the treatment of cervical radiculopathy.The search time was from the establishment of each database to November 30,2024.The resistance training group(experimental group)used resistance exercise or resistance exercise as the main intervention method.The control group received other rehabilitation therapy except resistance exercise.Outcome measures included total effective rate,visual analog scale score,Neck Disability Index,Tanaka Symptom Scale for Cervical Spondylosis,cervical joint motion,and recurrence rate.Cochrane bias risk assessment tool and physical therapy evidence database scale were used to determine the quality of the included references.Meta-analysis was performed using RevMan 5.4 software and Stata 17.0.RESULTS:(1)Finally,9 RCTs were included,including a sample size of 673 cases.GRADE evidence quality evaluation showed that the total effective rate was medium level of evidence,while visual analog scale score,Neck Disability Index,Tanaka Symptom Scale for Cervical Spondylosis score,joint motion,and recurrence rate were low level of evidence.(2)The results of meta-analysis showed that the total effective rate in the experimental group was higher than that in the control group(RR=1.22,95%confidence interval[CI]=1.06-1.41,P=0.005).(3)The visual analog scale score in the experimental group was better than that in the control group(mean difference[MD]=-0.72,95%CI=-0.98 to-0.46,P<0.000 01).The Neck Disability Index in the experimental group was better than that in the control group(MD=-2.57,95%CI=-4.13 to-1.02,P=0.001).The Tanaka Symptom Scale for Cervical Spondylosis score in the experimental group was better than that in the control group(MD=3.83,95%Cl=3.55-4.10,P<0.000 01).Cervical joint motion:the anterior flexion joint motion of the experimental group was better than that of the control group(MD=2.86,95%CI=2.30-3.43,P<0.000 01);the posterior extension joint motion of the experimental group was better than that of the control group(MD=5.23,95%CI=3.81-6.64,P<0.000 01).The range of motion of the left rotator joint in the experimental group was better than that in the control group(MD=7.15,95%CI=5.43-8.87,P<0.000 01);the range of motion of the right rotator joint in the experimental group was better than that in the control group(MD=5.45,95%CI=3.59-7.31,P<0.000 01).The range of motion of the left flexion joint in the experimental group was better than that in the control group(MD=3.35,95%CI=1.98-4.72,P<0.000 01);the range of motion of the right flexion joint in the experimental group was better than that in the control group(MD=3.91,95%CI=2.65-5.17,P<0.000 01).The recurrence rate in the experimental group was lower than that in the control group(RR=0.22,95%CI=0.06-0.80,P=0.02).CONCLUSION:Resistance exercise can effectively improve the pain of patients with cervical radiculopathy,promote the recovery of cervical vertebra function and reduce the recurrence rate.However,the evidence level of the methodological quality and outcome indicators of the included studies is low,and more high-quality and large-sample randomized controlled trials are needed for further research and confirmation in the future.

3.Feasibility study of low-dose chest CT with deep learning reconstruction algorithm combined with axial scan in children with mycoplasma pneumoniae pneumonia

Objective To explore the diagnostic value of deep learning image reconstruction(DLIR)com-bined with low-dose chest computed tomography(CT)with axial scan in the diagnosis of mycoplasma pneumoniae pneumonia(MPP)in children,and to provide reference for clinical practice.Methods 160 cases MPP children from February 2024 to June 2025 were selected as study subjects,and low-dose chest CT with axial scan was performed on all patients.DLIR and conventional adaptive iterative reconstruction-V(ASIR-V)were used for image reconstruction.The objective image quality[background noise(SD),signal-to-noise ratio(SNR),and contrast-to-noise ratio(CNR)],subjective image quality,and CT sign detection rate were compared,and the consistency of DLIR and ASIR-V in the diagnosis of MPP severity and clinical diagnosis was compared.Results As the intensity of DLIR and the weight of ASIR increasd,SD gradually decreased,while SNR and CNR gradually increased.The high-strength DLIR(DLIR-H)SD was lower than that of ASIR with a blending level of 80%(ASIR-V80%).The SNR and CNR were higher than those of ASIR-V80%,showing statistical significance(P<0.05).Ridit test showed that DLIR-H had the best subjective image quality score under different DLIR intensities,and ASIR-V80%had the best subjective image quality score under different ASIR weights.Furthermore,the subjective image quality score of DLIR-H was higher that of ASIR-V80%,and the differences were statistically significant(P<0.05).Using DLIR-H,the detection rates of air bronchogram,pulmonary consolidation,and interstitial infiltration(69.38%,86.88%,20.63%,respectively)were higher than those using ASIR-V80%(50.00%,71.88%,7.50%,respectively),and the differences were statistically significant(P<0.05).Consistency analysis showed that the Kappa value between the diagnostic results of MPP severity using DLIR-H and clinical diagnosis was 0.856(95%CI:0.771~0.996),while that between the diagnostic results of MPP severity using ASIR-V80%and clinical diagnosis was 0.498(95%CI:0.346~0.650).ROC analysis showed that the area under the curve(AUC)for diagnosing MPP severity was 0.925(95%CI:0.872~0.960)for DLIR-H and 0.729(95%CI:0.653~0.796)for ASIR-V80%,and the diagnostic value of DLIR-H was superior to that of ASIR-V80%(Z=3.952,P<0.001).Conclusion DLIR can effectively improve image quality.DLIR-H combined with low-dose chest CT with axial scan has high diagnostic value for the severity of MPP,and can serve as a feasible solution for clinical diagnosis of MPP severity and reducing radiation dose.

Chronic hepatitis B(CHB)is one of the major challenges in the global public health field. As of 2022,approximately 254 million people worldwide were infected with the hepatitis B virus(HBV). CHB is one of the main causes of liver cirrhosis and hepatocellular carcinoma(HCC). Nucleos(t)ide analogs(NAs)and interferon therapy can delay the progression of liver fibrosis by inhibiting viral replication,but they cannot completely avoid the problem of heterogeneous treatment responses. Some patients are in a state of low-level viremia(LLV)during treatment. The persistent LLV state can induce chronic inflammation and the progression of liver fibrosis,ultimately increase the risk of HCC. In patients with poor treatment responses,the continuous active viral replication can induce immune disorders,accelerate the evolution of fibrosis to the decompensated stage of liver cirrhosis,and increase the risk of patient death. This article aims to review the definition,mechanisms,and impact on treatment outcomes of LLV and suboptimal response based on the latest research,provide a basis for optimizing antiviral therapy for CHB.

7.Mugwort pollen-induced mouse allergic asthma and endotyping

Objective:To construct a mouse asthma model induced by mugwort pollen and to explore endotyping,providing methods for subsequent precision treatment.Methods:BALB/c mice were intraperitoneally injected with mugwort pollen extract(MPE)to sensitize,following MPE intranasal challenge to construct MPE allergic asthma murine model.Mice were randomly divided into PBS sensitization and PBS challenge(P-P),MPE sensitization and PBS challenge(M-P),MPE sensitization and MPE challenge model(M-M)groups.24 h after final challenge,mice were performed to examine airway responsiveness;bronchoalveolar lavage fluid(BALF)was harvested for cell counting and statistical classification of inflammatory cells through flow cytometry analysis.Pulmonary slides were collected for pathological examination,including HE,PAS,Masson and α-SMA immunohistochemical staining.ELISA was used to detect levels of IFN-γ,IL-4,IL-5,IL-13,IL-17A in lung tissue and serum,as well as serum total IgE and MPE-specific IgE,IgG1,IgG2a levels.Results:Pathological examination showed higher airway reactivity,more inflammatory cells infiltration around airway,obvious goblet metaplasia,thickening of airway smooth muscles and dramatical fibrin deposition around airway in model group.Total cell numbers of BALF were increased from<1×105 cells/ml in P-P group to>5×105 cells/ml in model group,in which eosinophils were predominant cellular type,levels of IL-4,IL-13,IL-17A in lung and IL-5,IL-13 levels in serum were significantly increased,as well as significant increasing levels of total IgE and MPE-specific IgE,IgG1,IgG2a.Conclusion:MPE-sensitized and challenged mice induces typical eosinophilic asthma featured with elevated eosinophils,as well as secretion of inflammatory factors of type 2 and type 17,IgE,IgG1 and IgG2a subtypes soars to high levels.
8.Analysis of research trends and hotspots in patient navigation based on CiteSpace

Objective To analyse the hotspots and trends in research of patient navigation. Methods Literature on patient navigation were retrieved from China National Knowledge Infrastructure (CNKI) and Web of Science Core Collection from 1 January,2000 to 31 December,2023. CiteSpace 6.1.R6 was used to analyse the co-citation literature,institutions and key words. Results A total of 483 articles were included,covering 444 in English and 39 in Chinese. The overall number of annual publications showed a fluctuating upward trend. Domestic research started later than abroad. The research hotspots included navigation model,navigation nurse,breast cancer and social service. The development of navigation model,training of navigation nurses,and identification of standardised navigation outcome indicators are expected to become the trend in future research. Conclusion Domestic research in patient navigation is at a preliminary stage. It is recommended that researchers should learn from foreign studies by strengthen communications and cooperation,further explore and improve theories,and develop a scientific and localised patient navigation model and a training system for navigation nursing talents.

10.The clinical and radiological features of children with first attack of inflammatory demyelinating central nervous system disorders and seropositivity to myelin-oligodendrocyte glycoprotein

Objective To investigate the clinical and radiological features of children with first attack of inflammatory central nervous system disorders and seropositivity to myelin-oligodendrocyte glycoprotein (MOG).Methods The clinical course,cerebrospinal fluid (CSF),MRI studies,MOG status and outcomes were retrospectively analyzed in children with first attack of inflammatory central nervous system disorders and seropositivity to MOG who were hospitalized in Children's Hospital of Fudan University from January 2016 to April 2017.Results Thirteen patients including 8 males and 5 females were included in this study,the ratio of male/female was 1.6∶ 1,and the median age was six years.Ten patients were diagnosed with acute disseminating encephalomyelitis,and three with clinically isolated syndromes.Seven patients had elevated CSF lymphocyte cells,and five patients had elevated CSF protein.All the patients' sera were tested for the anti-MOG IgG,which ranged from 1∶10 to 1∶100 with cell-based assay.MRI results showed that multiple anatomical areas were involved.Twelve patients had brain lesions,in which 10 patients had multiple lobes involved and four had tumefactive demyelinating lesions.The affected anatomical areas included white matters in 11 cases,thalamus/basal ganglias in nine,corpus callosums in three,brainstems in 10,spinal cord in five.The MRI features were characterized by hazy,bilateral lesions without clear boundaries.Clinical symptoms were fully restored in all the patients after treated with intravenous globulin and methyl prednisone.The average follow-up time was 8.9 months,and none of the patients had clinical recurrence.Conclusions MOG was associated with many kinds of inflammatory demyelinating diseases of central nervous system in children.Most of them were diagnosed with acute disseminating encephalomyelitis which has an acute or sub acute clinical course.The clinical manifestations of patients showed diversity.Multiple anatomical areas were involved,and treatment with intravenous globulin and methyl prednisone was effective in the acute phase.All of the patients had a favorable outcome.
