1.Component Analysis of Anmeidan and Its Mechanism in Regulating ERK1/2/MNK/ELF4E Signaling Pathway to Improve Circadian Rhythm in Insomnia Rats

ObjectiveTo identify the main chemical constituents of Anmeidan (AMD) and to explore the mechanism of AMD in regulating the extracellular signal-regulated kinase 1/2 (ERK1/2)/mitogen-activated protein kinase (MAPK)-interacting serine/threonine-protein kinase (MNK)/eukaryotic translation initiation factor 4E (eIF4E) signaling pathway to improve circadian rhythm disturbances in insomnia rats. MethodsThe main chemical constituents of AMD were identified using ultra-high-performance liquid chromatography-linear ion trap-electrostatic orbital trap mass spectrometry (UPLC-LTQ/Orbitrap/MS) in combination with reference standards. Sixty male Sprague-Dawley (SD) rats were randomly divided into control, model, melatonin, and AMD low-, medium-, and high-dose groups, with 10 rats in each group. Except for the control group, all rats were administered p-chlorophenylalanine via intraperitoneal injection to establish an insomnia model. The activity-rest rhythm of rats was assessed using the open field test and circadian rhythm test. Hematoxylin-eosin (HE) staining and Nissl staining were used to observe structural changes in hypothalamic neurons. Immunofluorescence, real-time quantitative polymerase chain reaction (Real-time PCR), and Western blot analysis were employed to detect mRNA and protein expression levels of ERK1/2, MNK, and eIF4E in the hypothalamus. ResultsA total of 50 chemical components, including flavonoids, phenylpropanoids, triterpenoid saponins, alkaloids, and lignans, were identified in AMD. Compared with the control group, the model group exhibited significantly increased total distance traveled, average speed, central area residence time, and cumulative rearing time (P<0.01), as well as prolonged cumulative activity time and total activity time in both light and dark phases (P<0.01). Hypothalamic neurons in the model group were sparsely arranged, reduced in number, and exhibited nuclear disappearance or nucleolar rupture, with a significantly increased apoptosis index (P<0.01). The cytoplasm appeared turbid, Nissl body staining was lighter, and the Nissl body apoptosis index was significantly increased (P<0.01). The mRNA expression levels of ERK1/2, MNK, and eIF4E were significantly decreased (P<0.01), along with a significant reduction in protein expression levels of ERK1/2, phosphorylated ERK1/2 (p-ERK1/2), MNK, phosphorylated MNK (p-MNK), eIF4E, and phosphorylated eIF4E (p-eIF4E) (P<0.01). Compared with the model group, the total distance, average speed, central area residence time and body upright cumulative time of the AMD high-dose group were significantly reduced (P<0.01). The total distance, average speed and body upright cumulative time of the AMD medium-dose group were significantly reduced (P<0.01). The cumulative time of light activity and total time of activity in each dose group of AMD were significantly shortened (P<0.01). The cumulative time of dark activity in the high-dose group of AMD was prolonged (P<0.01). The neurons in the middle and high dose groups of AMD were closely arranged, the number of neurons increased, and the apoptosis index of hypothalamic cells decreased significantly (P<0.05, P<0.01). The cytoplasm of the low, middle and high dose groups of AMD was clear, the color of Nissl body became darker, and the apoptosis index of Nissl body decreased significantly (P<0.01). The expression of ERK1/2, MNK and eIF4E mRNA and protein in the hypothalamus of the middle and high dose groups of AMD increased significantly (P<0.05, P<0.01). ConclusionAMD primarily contains 50 chemical constituents, including flavonoids, phenylpropanoids, and triterpenoid saponins. It exhibits a "synergistic enhancement" effect through multiple components and multiple pathways to improve insomnia. AMD ameliorates circadian rhythm disturbances in p-chlorophenylalanine-induced insomnia rats by upregulating ERK1/2/MNK/eIF4E signaling pathway-related proteins.
2.Visual acuity and corrected visual acuity of children and adolescents in Shanghai City

Objective:
To investigate the visual acuity and correction conditions of children and adolescents in Shanghai, so as to provide a scientific basis for developing intervention measures to prevent myopia and protect vision among children and adolescents.
Methods:
From October to December 2022, a stratified cluster random sampling survey was conducted, involving 47 034 students from 16 municipal districts in Shanghai, covering kindergartens (≥5 years), primary schools, middle schools, general high schools and vocational high schools. According to the Guidelines for Screening Refractive Errors in Primary and Secondary School Students, the Standard Logarithmic Visual acuity Chart was used to examine naked vision and corrected vision of students, and general information was collected. The distribution and severity of visual impairment in different age groups were analyzed, and χ 2 tests and multivariate Logistic regression were used to explore factors associated with visual impairment.
Results:
The detection rate of visual impairment among children and adolescents was 76.2%, with a higher rate among females (78.8%) than males ( 73.8 %), higher among Han ethic students ( 76.2 %) than minority students (71.2%), and higher among urban students (76.7%) than suburban students (75.8%), all with statistically significant differences ( χ 2=162.6, 10.4, 5.5, P <0.05). The rate of visual impairment initially decreased and then increased with age, reaching its lowest at age 7 (53.8%) and peaking at age 17 (89.6%) ( χ 2 trend = 3 467.0 , P <0.05). Severe visual impairment accounted for the majority, at 56.6%, and there was a positive correlation between the severity of visual impairment and age among children and adolescents ( r =0.45, P <0.05). Multivariate Logistic regression showed that age, BMI, gender, ethnicity and urban suburban status were associated with visual impairment ( OR =1.18, 1.01, 1.38 , 0.79, 0.88, P <0.05). Among those with moderate to severe visual impairment, the rate of spectacle lens usage was 62.8%, yet only 44.8 % of those who used spectacle lens had fully corrected visual acuity. Females (64.9%) had higher spectacle lens usage rates than males (60.6%), and general high school students had the highest spectacle lens usage (83.9%), and there were statistically significant differences in gender and academic stages ( χ 2=57.7, 4 592.8, P <0.05).
Conclusions
The rate of spectacle lens usage among students with moderate to severe visual impairment is relatively low, and even after using spectacle lens, some students still do not achieve adequate corrected visual acuity. Efforts should focus on enhancing public awareness of eye health and refractive correction and improving the accessibility of related health services.
3.Efficacy and Safety of Acupuncture Combined with Levodopa in the Treatment of Parkinson's Disease: A Randomized Controlled Trial

ObjectiveTo evaluate the clinical efficacy and safety of acupuncture combined with levodopa in the treatment of Parkinson's disease(PD). MethodsA total of 60 patients with PD were enrolled and randomly assigned to test group or control group, with 30 patients in each group. The control group received levodopa only, starting at 100 mg per dose, three times daily, with gradual increases not exceeding a maximum daily dose of 800 mg. The test group received acupuncture three times per week in addition to levodopa. Both groups were treated for 12 weeks. Assessments were conducted before treatment, after 6 and 12 weeks treatment, using the Unified Parkinson's Disease Rating Scale(UPDRS), Wearing-Off Questionnaire-9(WOQ-9), Montreal Cognitive Assessment(MoCA), Mini-Mental State Examination(MMSE), Depression Rating Scale(DRS), Hamilton Depression Scale(HAMD), Hamilton Anxiety Scale(HAMA), PD Questionnaire-39(PDQ-39), and Pittsburgh Sleep Quality Index(PSQI). Repeated measures ANOVA was utilized to evaluate the effects of time, group, and their interaction on each index. Spearman correlation analysis was conducted to examine the relationships between combined treatment and outcome scores. Adverse events in both groups were recorded throughout the study. ResultsBoth groups showed significant improvements after 6 and 12 weeks treatment, with decreases in UPDRS total score, WOQ-9 total score, DRS score, HAMD score, HAMA score, PDQ-39 score, and PSQI score, and increases in MoCA and MMSE scores(P<0.05). Compared with the control group, the test group demonstrated significantly greater improvements in all the above indicators after 6 and 12 weeks (P<0.05). Repeated measures ANOVA showed significant time main effects, group main effects, and their interaction across all outcome measures(P<0.01). Spearman correlation analysis revealed that combined therapy was significantly negatively correlated with UPDRS, WOQ-9, DRS, HAMD, HAMA, PDQ-39, and PSQI scores, while positively correlated with MoCA and MMSE scores after 12 weeks of treatment(P<0.05). Both groups did not experience any serious adverse events and did not affect treatment. ConclusionAcupuncture combined with levodopa is more effective than levodopa alone in improving motor function, non-motor symptoms, cognitive function, depression and anxiety, quality of life, and sleep quality in patients with PD, with good safety.
4.Mechanism of Huazhuo Jiedu Prescription in Alleviating Renal Fibrosis in 5/6 Nephrectomy Rats Based on AMPK/mTOR Pathway

ObjectiveBased on the AMP-activated protein kinase (AMPK)/mammalian target of rapamycin (mTOR) signaling pathway, this study aimed to observe the effect of the Huazhuo Jiedu prescription on renal fibrosis in 5/6 nephrectomy rats and explore its underlying mechanism. MethodsA total of 67 SPF-grade male SD rats were used, of which 11 were randomly selected as the normal group. A chronic renal failure (CRF) model was established using 5/6 nephrectomy. The successfully modeled rats were randomly assigned to the model group, losartan potassium group (4.5 mg·kg-1), and low- (1.175 g·kg-1), medium- (2.35 g·kg-1) and high-dose (4.7 g·kg-1) Huazhuo Jiedu prescription groups, with 9 rats per group. Each group received an equivalent volume of saline or the corresponding concentration of Huazhuo Jiedu prescription by gavage once daily for 8 weeks. Hematoxylin-eosin (HE) and Masson staining were used to observe renal tissue pathological changes. Transmission electron microscopy examined renal ultrastructure. Immunohistochemistry (IHC) detected expressions of α-smooth muscle actin (α-SMA) and transforming growth factor-β1 (TGF-β1). Western blot analyzed expression levels of microtubule-associated protein Ⅰ light chain 3Ⅱ (LC3Ⅱ), Beclin1, p62, AMPK, phosphorylated AMPK (p-AMPK), mTOR, and phosphorylated mTOR (p-mTOR). ResultsCompared with the normal group, the model group exhibited glomerular shrinkage, mesangial and interstitial thickening, and tubular vacuolar degeneration, with no evident autophagosomes or autophagolysosome structures. Expression levels of α-SMA and TGF-β1 were significantly increased (P0.01), while p-AMPK/AMPK, Beclin1, and LC3Ⅱ were significantly decreased (P0.01), and p-mTOR/mTOR and p62 were significantly increased (P0.01). Compared with the model group, the medium- and high-dose Huazhuo Jiedu prescription groups and the losartan potassium group showed varying degrees of pathological improvement. Autophagosomes with double- or multiple-layer membranes and autophagolysosomes with monolayer membranes containing undegraded organelles were observed. Renal α-SMA and TGF-β1 protein expression levels were markedly reduced (P0.05, P0.01), p-mTOR/mTOR and p62 were significantly decreased (P0.05, P0.01), and p-AMPK/AMPK, Beclin1, and LC3Ⅱ expression levels were significantly increased (P0.05, P0.01). ConclusionHuazhuo Jiedu prescription may improve renal fibrosis in 5/6 nephrectomy rats by regulating the AMPK/mTOR signaling pathway and enhancing autophagy.
5.Qualitative study of active management dilemmas in young and middle-aged postoperative patients with lumbar disc herniation

Objective:To understand the dilemma of active management in young and middle-aged postoperative patients with lumbar disc herniation, in order to provide reference for the formulation of active intervention program of self-health management meeting the needs of patients.Methods:Using the phenomenological research method, a total of 16 middle-aged and young patients with lumbar disc herniation were selected for semi-structured in-depth interviews, and the themes were analyzed and extracted by Colaizzi 7-step analysis method.Results:Three themes and nine sub-themes were extracted namely, inadequate disease response capacity and response resources (lack of disease symptom management capacity, limited access to disease management information, difficult use of disease management information, difficulty in maintaining positive self-management for a long time (forced to stop behavior change due to stress, lack of motivation to achieve good behavior change, doubts about the effectiveness of self-management) and multi-dimensional negative emotions (contrast between maintaining independence and dependence on others, fear of being misunderstood, fear of relapse) .Conclusions:Medical staff should provide comprehensive health information services for young and middle-aged postoperative patients with lumbar disc herniation, improve the out-of-hospital follow-up mechanism with the help of information technology, further improve family and social support strategies, reduce the difficulties faced in the process of active management and achieve long-term active disease management.
6.Application of speech repetition task in the detection of mild cognitive impairment

Mild cognitive impairment(MCI)is often regarded as a prodromal stage of dementia,primarily characterized by mild decline in cognitive function.Due to the mild nature of its symptoms,many MCI cases miss the opportunity for intervention.Research indicates that linguistic changes in MCI may precede other cognitive symptoms,providing an opportunity for early identification and intervention.These linguistic changes include abnormalities in speech rate,pronunciation,and intonation.Speech repetition tasks,such as the five-word test,delayed digit matching,and sentence repetition tests,are effective methods for assessing the speech characteristics of MCI patients.These tasks involve asking patients to repeat specific content,and analyzing the accuracy of their repetitions to assess their speech function.The application of machine learning and deep learning techniques enables automatic extraction of MCI-related features from speech data,improving diagnostic accuracy.The combined application of these techniques contributes to the early detection of MCI,providing a basis for timely intervention.
7.Research on clinical application of urine sediment score in the diagnosis of acute kidney injury

Objective:To evaluate the clinical application of urine sediment score (USS) in early diagnosis, etiological differentiation, staging and prognosis of acute kidney injury (AKI), and to investigate the diagnostic efficacy of independent USS and its combination with blood urea nitrogen(Bun) serum creatinine(sCr) and uric acid(UA) in AKI.Methods:From August 23 to September 28, 2023, 9 020 morning urine samples of hospitalized patients in the First Hospital of Jilin University were detected by Sysmex UF5000.A total of 3 226 ssamples with small and round cell (SRC) > 1/μl and/or CAST>1/μl were screened for microscopic examination, and 404 cases with positive renal tubular epithelial cells and/or cast were enrolled in this study. There were 218 males and 186 females, aged 59.5 (49.0, 71.0) years. The 404 cases were divided into the USS AKI group (345 cases) and the USS non-AKI group (59 cases) according to the USS results based on the microscopic findings. According to Kidney Disease: Improving Global Outcomes (KDIGO) criteria, they were divided into KDIGO criteria AKI group (63 cases) and KDIGO criteria non-AKI group (341 cases), and the AKI group was divided into renal AKI group (33 cases) and non-renal AKI group (30 cases). According to the clinical diagnosis recorded in the medical records, they were divided into clinically diagnosed AKI group (29 cases) and clinically diagnosed non-AKI group (375 cases).The χ 2 test or Fisher exact test was used to compare USS in different AKI causes and stages. Logistic regression was used to calculate the odds ratio of renal AKI and stage 3 AKI. The area under the receiver operating characteristic curve was used to evaluate the sensitivity and specificity of USS, sCr, UA and Bun alone and in combination in the diagnosis of AKI, and the best cut-off value, sensitivity and specificity in the diagnosis of AKI were calculated. P < 0.05 was considered statistically significant. Results:The USS was used to identify the etiology of KDIGO standard AKI group,and there were significant differences in USS between renal AKI group and non-renal AKI group (χ 2=11.070, P<0.001). Compared to USS=1, the odds ratio of renal AKI was 8.125 when USS≥2 (95% CI 2.208—29.901). There was a statistically significant difference in the comparison of USS between groups in each stage of the AKI staging study based on USS (χ 2=15.724, P<0.05). Compared to USS=1, the odds ratio of stage 3 AKI was 9.714 when USS≥2 (95% CI 1.145-82.390). The AUC of independent USS in the diagnosis of AKI was 0.687 (95% CI 0.618-0.757, P<0.001), the specificity was 65.7% and the sensitivity was 61.9%. The AUC of USS combined with Bun, sCr, UA in the diagnosis of AKI was 0.794 (95% CI 0.608-0.980, P<0.05), the specificity was 82.4%, and the sensitivity was 88.9%. Conclusions:There wasan increased likelihood of renal AKI or stage 3 AKI while USS≥2,and whose combination with Bun, sCr and UA will improve the diagnostic efficiency of AKI.
8.Investigation of different sterilization methods of Astragali Radix decoction pieces and research on the best sterilization process

Objective To investigate the effects of different sterilization methods on the quality of Astragali Radix decoction pieces by comparing the properties,odor,fingerprint,sterilization effect,extract and active component contents of Astragali Radix decoction pieces,and to further optimize the best sterilization process parameters of Astragali Radix decoction pieces.Methods The effects of moist heat sterilization,irradiation sterilization and dry heat sterilization on the quality of Astragali Radix decoction pieces were compared,and the best sterilization method was selected.Based on this,taking the content of astragaloside IV,the content of isoflavone glucoside,the total content of the two and the sterilization rate as the assessment indicators,taking the sterilization temperature,sterilization time and material thickness as the assessment factors,on the basis of the single-factor test,the response surface method was used to optimize the optimal dry heat sterilization process of Astragali Radix decoction pieces.Results The dry heat sterilization method was selected to sterilize Astragali Radix decoction pieces.The best process for dry heat sterilization was as follows:the sterilization temperature of 113℃,sterilization time of 5.1 h,and the material thickness of 19 mm.Conclusion Considering the sterilization effect,the influence on the content of active ingredients,operability and cost,selecting dry heat sterilization is the best sterilization method for Astragali Radix decoction pieces,and the optimal sterilization process is stable and feasible,with good repeatability,which can provide scientific basis for its industrial production application.
9.Diagnostic Efficacy of Platelet-Related Parameters on Anxiety and Depression in Patients Undergoing Peritoneal Dialysis

Objective To analyze the correlations between platelet-related parameters and the incidence of anxiety and depression in the patients undergoing peritoneal dialysis(PD),and evaluate the efficacy of the pa-rameters in the diagnosis of anxiety and depression in PD patients.Methods A total of 245 patients undergoing PD in the First Affiliated Hospital of Hebei North University from September 2022 to February 2023 were enrolled.The gener-alized anxiety scale(GAD-7)and the patient health questionnaire(PHQ-9)were used to evaluate the anxiety and depression of the patients,respectively.The personal information and biochemical indicators of the patients were col-lected,and the platelet count(PLT),mean platelet volume(MPV),and platelet distribution width(PDW)were measured.Logistic regression was adopted to analyze the relationships of platelet-related parameters with anxiety and depression in PD patients.Results Among the 245 patients undergoing PD,the incidences of anxiety and depression were 15.9% and 38.0% ,respectively.There were differences in the dialysis period(Z=-2.358,P =0.018;Z =-3.079,P=0.002),MPV(Z=-4.953,P<0.001;Z=-7.878,P<0.001),and PDW(Z =-4.587,P<0.001;Z=-7.367,P<0.001)between the anxiety group and the non-anxiety group as well as between the de-pression group and the non-depression group.The correlation analysis showed that MPV(r =0.358,P<0.001;r =0.489,P<0.001)and PDW(r =0.340,P<0.001;r =0.447,P<0.001)were positively correlated with anxiety and depression in the patients undergoing PD.The Logistic regression model showed that MPV(P =0.022,P =0.011),PDW(P =0.041,P =0.018),and dialysis period(P =0.011,P =0.030)were independent risk factors for the anxiety and depressive state in PD patients.The areas under the receiver operating characteristic curve of MPV in the diagnosis of anxiety and depression in PD patients were 0.750 and 0.800,respectively,and those of PDW were 0.732 and 0.780,respectively.Conclusion MPV and PDW have high efficacy in the diagnosis of anxiety and depression associated with PD and can be used as objective indicators to evaluate the anxiety and depression in the patients undergoing PD.
10.The relationship between laryngopharyngeal reflux and common pharyngeal diseases and analysis of related risk factors

OBJECTIVE To analyze the relationship between LPR and common pharyngeal diseases and its risk factors.METHODS A total of 280 patients who were admitted to the Inpatient Department of Otolaryngology Head and Neck Surgery or Laryngoscopy Room of the First Affiliated Hospital of Harbin Medical University due to throat discomfort from September 2022 to May 2023 were selected as the study objects.All enrolled patients were required to complete the RSI and RFS scoring scales.According to the 2022 LPRD expert Consensus,RSI>13 and/or RFS>7 were classified as LPR group,and RSI≤13 and RFS≤7 are classified as non-LPR group.Chi-square test was used to analyze the differences in the incidence of common pharyngeal diseases such as upper respiratory tract papilloma,epiglottic cyst,vocal cord polyp,vocal cord leucoplakia,vocal cord cancer and hypopharyngeal cancer among different groups.Univariate Chi-square test analysis and multivariate binary logistic regression analysis were performed for gender,age,BMI,smoking and drinking history.All differences were statistically significant with P<0.05.RESULTS The prevalence of LPR in 280 patients with throat discomfort was 70.0%(196/280).There were statistically significant differences in the incidence of LPR group and non-LPR group in patients with vocal cord polyp(χ2=4.228,P<0.05),vocal cord leukoplakia(χ2=12.283,P<0.05),vocal cord cancer(χ2=4.103,P<0.05)and hypopharyngeal cancer(χ2=4.907,P<0.05),while there was no significant difference in the incidence of LPR group and non-LPR group in patients with papilloma of upper respiratory tract(χ2=0.183,P>0.05)and epiglottic cyst(χ2=0.556,P>0.05).Univariate analysis showed that there were significant differences in the incidence rates of smoking(χ2=20.403,P<0.05)and drinking(χ2=7.704,P<0.05)between the LPR group and the non-LPR group,while gender(χ2=0.01,P>0.05),age(χ2=8.147,P>0.05),BMI(χ2=2.060,P>0.05)had no significant difference.Multivariate logistic regression analysis showed that smoking(OR=3.390,95%CI:1.761-6.526,P<0.05)was an independent risk factor for LPR.CONCLUSION There is a high co-prevalence of LPR with vocal cord polyps,vocal cord leukoplakia,vocal cord cancer and hypopharyngeal cancer.Therefore,LPR should be evaluated when treating throat related diseases,and LPR should be actively treated if necessary.In addition,smoking and drinking are risk factors for LPR,so it is necessary to stop smoking and drinking alcohol when treating LPR.
