1.Differentiation and treatment of urticarial vasculitis based on the theory of Xuanfu-collateral theory

Urticarial vasculitis is a skin disease with urticaria-like lesions and a histopathological pattern of leukocytoclastic vasculitis. It is considered a "hidden rash" in traditional Chinese medicine. Xuanfu is the portal that regulates qi, blood, fluid, and the ascending, descending, exiting, and entering of nutrition qi and defensive qi. Collaterals are the pathways for the circulation of qi and blood. The two accompany each other, connecting zang-fu organs, reaching the surfaces of the skin, hair, and external body, circulating qi and fluid, and moistening and protecting the skin. Based on the theory of Xuanfu-collateral, this study aimed to clarify the etiology, pathogenesis, and treatment method of urticarial vasculitis. External assault by wind and Xuanfu blockage are believed to be the initiating factors of this disease. The malnutrition of Xuanfu and collaterals and accumulated dampness-heat are important links in the occurrence and development of urticarial vasculitis. It spreads from Xuanfu to the collaterals, and blockage of the collaterals is the immanent trend of this disease. Clinically, by closely adhering to the core pathogenesis of blockage of Xuanfu-collateral, treatment method such as using wind medicinals to open Xuanfu with pungent and dispersing properties, using the method of supplement deficiency and removing the blockage, and using medicinals to promote blood circulation and remove blood stasis to unblock the blocked collaterals. The herbs are flexibly added or subtracted to unblock Xuanfu and collaterals, harmonize qi and blood, thus all symptoms can be relieved. We hope that this study will provide new ideas for the treatment of urticarial vasculitis with traditional Chinese medicine.
2.Exploration and practice of knowledge management system empowering high-tech development in a prefecture-level hospital

Medical technology proficiency is a key indicator of a municipal-level hospital′s core competitiveness. To continuously enhance its medical capabilities, a tertiary prefecture-level hospital in Zhejiang province launched the " 100 high-tech projects" in 2010, designating it as a priority technical development program. The hospital established a hospital-level knowledge management team in 2020 to build a knowledge management system for high-tech projects. This system fostered a virtuous cycle of synergistic advancement in medical technology innovation, knowledge management, and talent development by extracting both explicit and tacit knowledge from the hospital′s high-tech projects, absorbing tacit knowledge from benchmark hospitals′ relevant technologies, and establishing a knowledge-sharing platform. From January 2021 to October 2024, the hospital secured approval for 9 national and provincial-level clinical key specialties, established 7 municipal key technological innovation teams, and enrolled 46 provincial/municipal-level talents. A the same time, talent attrition remained low. The implementation of the knowledge management system to empower high-tech development had achieved phased results. This practice could provide references for other hospitals to enhance their medical technology capabilities.
3.Survey on the current situation of management of sterilization and supply centers in response to emerging and outbreaks of infectious diseases in 442 hospitals and analysis of influencing factors

Objective To investigate the current situation of emergency response management during new outbreaks of infectious diseases in central sterile supply department(CSSD)of hospitals in 18 provinces(autonomous regions and municipalities),and to provide a reference basis for the establishment of a sound emergency response management system.Methods A convenience sampling method was used to investigate and study the CSSDs in 447 hospitals from June to December 2023 using a self-developed questionnaire.The questionnaire included 3 aspects,including general information,emergency disposal management situation(including environment management,personnel management,material management,and process management),and satisfaction with emergency disposal management,and the relevant factors affecting the satisfaction with the emergency disposal management of CSSDs responding to new outbreaks of infectious disease devices were analyzed.Results A total of 447 questionnaires were recovered,with 442 valid questionnaires and a valid questionnaire recovery rate of 98.88%.In terms of environmental management,CSSDs in 88(19.91%)hospitals set up independent instrument pre-sterilization rooms for infected device disposal.In terms of material management,CSSDs in 42(9.50%)hospitals conducted daily emergency material management.In terms of personnel management,CSSDs in 236(53.39%)hospitals conducted emergency response knowledge assessments.In terms of process management,CSSDs in 225(50.90%)hospitals conducted information traceability system full-process closed-loop management.The results of multiple linear regression analysis showed that whether or not to carry out pre-treatment,emergency supplies stockpiling and management,emergency disposal knowledge assessment,establishment of standard operating procedures,and full-process closed-loop management of the traceability system were the influencing factors affecting the satisfaction of CSSD's emergency disposal management(P<0.05).Conclusion The current situation of CSSD's emergency disposal management of new outbreaks of infectious diseases needs to be improved,and the environmental management of the decontamination area should be strengthened to improve the staff's ability of emergency disposal,guarantee the reserve of emergency supplies,and strengthen the informatization of the whole process management,to reduce the risk of hospital infections and safeguard the quality of medical care.
4.Analysis of changes in ocular surface biology and visual quality in diabetic dry eye patients under different influencing factors

Objective To investigate the factors that influence ocular surface biology and visual acuity in individuals with diabetic dry eye(DDE)and analyze how these factors contribute to changes in visual quality.Methods Based on the disease duration,fasting blood glucose(FBG),and glycated hemoglobin(HbA1c)levels of patients with type 2 diabe-tes mellitus(T2DM),the DDE patients were divided into different groups.Logistic regression analysis was used to identify influencing factors related to ocular surface biology and visual quality in each group of DDE patients.Tear film stability was evaluated based on the tear film rupture time(BUT),Schirmer I test(SIt),and ocular surface disease index(OSDI).Lip-iview? Surface interferometers were used to measure tear film lipid layer thickness(LLT),meibomian gland loss rate(MGP),meibomian gland opening number(MGYLS),and meibomian gland secretion score(MGYSS).Wavefront aber-rometry was used to measure corneal wavefront aberration values at 4 mm and 6 mm pupil diameters.Ocular response ana-lyzer(ORA)was adopted to analyze corneal hysteresis(CH)and corneal resistance factor(CRF).Moreover,ELISA ex-periment to evaluate the trend of changes in inflammatory factors in tears.Results Logistic regression analysis revealed that T2DM duration,smoking history,FBG,HbA1c,total cholesterol(TC),triglycerides(TG),OSDI score,LLT,BUT,SIt,MGP,MGYLS,MGYSS,total higher-order aberrations,spherical aberration,coma aberration,trefoil aberration,tumor necrosis factor-α,interleukin-6,matrix metalloproteinase-9,receptor for advanced glycation end products,and insu-lin were all influencing factors for the risk of DDE(all P<0.05).As the T2DM course prolonged and FBG or HbA1 c levels rose,tear film-related indicators(LLT,BUT,and SIt)and meibomian gland-related indicators(MGYLS and MGYSS)inpa-tients gradually decreased,while OSDI scores and MGP gradually increased(all P<0.05).As the T2DM course prolonged and FBG or HbA1c levels rose,the total higher-order aberrations,spherical aberration,coma aberration,and trefoil aber-ration in DDE patients under 4 mm and 6 mm pupil diameters gradually increased;Meanwhile,best corrected visual acuity,corneal hysteresis,and corneal resistance factor gradually decreased;The contents of tumor necrosis factor-α,interleukin-6,matrix metalloproteinase-9,receptor for advanced glycation end products,and insulin in tears all gradually increased,while mucin-5AC gradually decreased(all P<0.05).Conclusion With the prolongation of T2DM duration and the in-crease of FBG or HbA1c,the ocular surface inflammatory response in DDE patients gradually worsens,corneal biological function decreases,and visual quality deteriorates.Timely systemic and local interventions are of great significance for im-proving dry eye symptoms and visual quality in DDE patients.
5.Efficacy of endoscopic ultrasound-guided paraesophageal varices puncture cyanoacrylate selective seal for liver cirrhosis complicated with esophageal varices (with video)

Objective:To investigate the efficacy of endoscopic ultrasound-guided paraesophageal varices puncture cyanoacrylate selective seal for liver cirrhosis complicated with esophageal varices.Methods:Data of 86 patients with liver cirrhosis complicated with esophageal varices undergoing endoscopic ultrasound-guided paraesophageal varices puncture cyanoacrylate selective seal (the treatment group, n=40) and conventional endoscopic tissue glue injection treatment (the control group, n=46) were retrospectively collected from Beijing Ditan Hospital from January 2023 to October 2023. The reduced percentage of severe varicose veins, the amount of tissue glue used during operation, the number of punctures and the rate of rebleeding in six months were compared. Results:There were no significant differences in the baseline data including gender, age, blood routine index, liver and kidney functions, proportion of severity varicose veins between the two groups ( P>0.05). In the treatment group, severe varicose veins accounted for 90.0% (36/40) before treatment while it was 12.5% (5/40) after the treatment, and the proportion of severe varicose veins decreased by 77.5%. In the control group, the proportion of severe varicose veins was 89.1% (41/46) before while it was 43.5% (20/46) after the treatment, and the proportion of severe varicose veins decreased by 45.6%. The proportion of severe varicose veins decreased more in the treatment group than that in the control group ( χ2=13.86, P<0.001). Compared with the control group, the median amount of tissue glue applied in the treatment group was significantly less (2.5 mL VS 3.0 mL, Z=-3.55, P<0.001), and the median number of punctures was also significantly less (2 VS 6, Z=-7.10, P<0.001). There was no significant difference in the rate of rebleeding within six months after the treatment between the two groups [12.5% (5/40) VS 10.9% (5/46), χ2=0.06, P=0.814]. Conclusion:For patients with cirrhosis complicated with esophageal varices, endoscopic ultrasound-guided paraesophageal varices puncture cyanoacrylate selective seal demonstrates superior efficacy over conventional endoscopic treatment, with less tissue glue applied, which is worth of promotion and application in clinical practice.
6.The application of Hydrocoil Azur CX in interventional embolization of pulmonary arteriovenous fistula

Objective To discuss the application of Hydrocoil Azur CX(an electrolysis dehydration gel platinum coil)in interventional embolization of pulmonary arteriovenous fistula(PAVF).Methods Three patients with untreated PAVF,who were admitted to the Affiliated Hospital of Qingdao University of China between May 2023 and July 2023,were selected for this study.Interventional embolization of the responsible vessels with Hydrocoil Azur CX and other spring coils was carried out for all 3 patients.Results Multiple PAVF lesions were effectively obstructed in 3 patients.No recanalization occurred during a follow-up period of 6 months.Conclusion The Hydrocoil Azur CX can effectively and permanently embolize the PAVF.

8.Clinical study on the prevention of chemotherapy-induced nausea and vomiting using Erchen decoction acupoint application

Objective:To investigate the clinical effects of Erchen decoction acupoint application on the prevention of chemotherapy-induced nausea and vomiting. Methods:A prospective study was conducted involving 120 patients with solid tumors who visited Wenzhou Central Hospital between January 2022 and July 2023. The patients were randomly divided into an observation group and a control group using a random number table method. Ultimately, 55 patients in the observation group and 51 patients in the control group completed the study (due to dropouts or rescue treatment, 5 patients were excluded from the treatment group and 9 patients were excluded from the control group). The observation group received Erchen decoction acupoint application in addition to the antiemetic regimen of tropisetron and dexamethasone, while the control group received an acupoint application of pigment powder alongside the same antiemetic regimen as the observation group. The clinical efficacy, pre- and post-treatment grading of nausea and vomiting, adverse reactions, and quality of life scores were compared between the two groups. Results:There was no statistically significant difference in the effective rate of treating vomiting between the two groups ( P > 0.05). However, the total effective rate for delayed vomiting in the observation group was significantly higher at 98.18% (54/55) compared with 84.31% (43/51) in the control group ( χ2 = 4.75, P < 0.05). The proportion of patients in the observation group with a nausea grade of 0 was 76.36% (42/55), which was significantly higher than the 37.25% (19/51) in the control group ( Z = 4.12, P < 0.001). Furthermore, the observation group had higher scores in social functioning [(72.83 ± 6.67)], cognitive functioning [(78.91 ± 5.83)], emotional functioning [(70.65 ± 6.64)], role functioning [(71.70 ± 6.68)], physical functioning [(76.23 ± 7.28)], and overall health [(73.56 ± 6.21)]compared with the control group [(64.57 ± 7.28), (70.87 ± 6.67), (63.24 ± 5.57), (65.36 ± 5.73), (68.82 ± 6.71), and (66.56 ± 5.74), t = 6.10, 6.62, 6.20, 5.23, 5.44, 6.01, all P < 0.05]. There was no statistically significant difference in the incidence of adverse reactions between the two groups ( P > 0.05). Conclusions:Erchen decoction acupoint application can effectively alleviate chemotherapy-induced delayed nausea and vomiting and improve the quality of life of patients.
