1.Mining and characterization of new enzymes based on Phi29 DNA polymerase.

In recent years, the bacteriophage Φ29 (Phi29) DNA polymerase has garnered increasing attention due to its high-fidelity amplification capacity at constant temperatures. To advance the industrial application of this type of isothermal polymerases, this study mined and characterized new enzymes from the microbial metagenome based on the known Phi29 DNA polymerase sequence. The results revealed that a new enzyme, Php29 DNA polymerase, was identified in the microbial metagenome with plants as the hosts. This enzyme exhibited higher strand displacement activity, with a 59.5% similarity to bacteriophage Φ29. Experimental validation demonstrated that the enzyme had 3'→5' exonuclease activity, and its amplification products can serve as substrates for further catalytic reactions. The discovery and validation of Php29 DNA polymerase gives insights into the future industrial application of isothermal polymerases.

Objective The purpose of writing the"expert consensus on humanistic care for patients in hospice care"(hereinafter referred to as the"consensus")aims to standardize the practice of humanistic care in the field of hospice care,ensuring that humanistic care is integrated throughout the entire service process for hospice care patients and their families.Methods A systematic search was conducted in domestic and foreign databases for literature related to hospice care and humanistic care,including guidelines,expert consensuses,systematic reviews or Meta-analyses,and evidence summaries.High-quality evidence was evaluated,extracted,and summarized to form the initial draft of the"consensus".From June to October 2024,20 experts from the fields of hospice care,nursing humanities,and evidence-based nursing were invited to participate in 1 round of expert consultation.Among them,13 experts were selected for 2 rounds of expert demonstration meetings.After collating and analyzing the experts' opinions,the initial draft was revised and refined,ultimately resulting in the final version of the"consensus".Results The effective response rate of the consultation questionnaire was 100%,with expert authority coefficient of 0.880,judgment coefficient of 0.935,and familiarity level of 0.825.The Kendall harmony coefficient of the expert consultation was 0.134(P<0.05).The"consensus"consisted of 13 aspects,including the targets and objectives,principles,institutional guarantees,environmental requirements,etc.Conclusion This"consensus"possesses strong scientific rigor and practicality,which can provide guidance and references for the practice of humanistic care in the field of hospice care,promoting the standardization and humanization of hospice care services.

4.Application of sodium bicarbonate solution for flushing to prevent fibrin sheath blockage of peripherally inserted central catheter

Objective:To explore the feasibility and safety of sodium bicarbonate solution for flushing in preventing fibrin sheath occlusion in peripherally inserted central catheters (PICC), providing a reference for PICC maintenance.Methods:A randomized controlled study was conducted from June 2022 to September 2023. Convenience sampling was used to select 96 patients who underwent PICC at the Affiliated Tumor Hospital of Guangxi Medical University. The patients were randomly divided into a control group and an observation group. Both groups performed grip strength exercises using a grip ball on the catheterized upper limb. During catheter maintenance, both groups used 10 ml of 0.9% sodium chloride solution for pulsed flushing. The control group used 10 ml of 0.9% sodium chloride solution for positive pressure flushing, while the observation group used 3 ml of 5% sodium bicarbonate solution for positive pressure flushing. The incidence of "mechanical valve" and catheter occlusion, as well as blood bicarbonate concentration and interleukin-6 (IL-6) levels, were recorded on the first day and at 1, 2, 3, 4, and 5 months after catheterization.Results:A total of 92 PICC patients were included, with 45 in the control group, 28 males and 17 females, aged (43.80 ± 2.21) years old, and 47 in the observation group, 29 males and 18 females, aged (43.80 ± 2.25) years old. The total incidence of "mechanical valve" in the observation group was 2.1% (1/47), lower than the control group′s 26.7% (12/45), the difference was statistically significant ( χ2=11.41, P<0.05). The incidence of catheter occlusion in the observation group was 2.1% (1/47), lower than the control group's 15.5% (7/45), the difference was statistically significant ( χ2=5.22, P<0.05). The IL-6 levels in the observation group at 2, 3, 4, and 5 months after catheterization were (4.53 ± 0.62), (4.68 ± 0.59), (4.76 ± 0.57), and (4.83 ± 0.53) ng/L, respectively, lower than the control group (4.83 ± 0.59), (4.95 ± 0.54), (5.21 ± 0.48), and (5.63 ± 0.57) ng/L, the differences were statistically significant ( t values were 2.26-6.86, all P<0.05). There were no significant differences in blood bicarbonate concentration between the two groups at different time points (all P>0.05). Conclusions:Sodium bicarbonate solution for flushing can effectively prevent fibrin sheath blockage of PICC and reduce serum IL-6 levels, making it a safe and effective flushing method.
5.Clinical study on the prevention of chemotherapy-induced nausea and vomiting using Erchen decoction acupoint application

Objective:To investigate the clinical effects of Erchen decoction acupoint application on the prevention of chemotherapy-induced nausea and vomiting. Methods:A prospective study was conducted involving 120 patients with solid tumors who visited Wenzhou Central Hospital between January 2022 and July 2023. The patients were randomly divided into an observation group and a control group using a random number table method. Ultimately, 55 patients in the observation group and 51 patients in the control group completed the study (due to dropouts or rescue treatment, 5 patients were excluded from the treatment group and 9 patients were excluded from the control group). The observation group received Erchen decoction acupoint application in addition to the antiemetic regimen of tropisetron and dexamethasone, while the control group received an acupoint application of pigment powder alongside the same antiemetic regimen as the observation group. The clinical efficacy, pre- and post-treatment grading of nausea and vomiting, adverse reactions, and quality of life scores were compared between the two groups. Results:There was no statistically significant difference in the effective rate of treating vomiting between the two groups ( P > 0.05). However, the total effective rate for delayed vomiting in the observation group was significantly higher at 98.18% (54/55) compared with 84.31% (43/51) in the control group ( χ2 = 4.75, P < 0.05). The proportion of patients in the observation group with a nausea grade of 0 was 76.36% (42/55), which was significantly higher than the 37.25% (19/51) in the control group ( Z = 4.12, P < 0.001). Furthermore, the observation group had higher scores in social functioning [(72.83 ± 6.67)], cognitive functioning [(78.91 ± 5.83)], emotional functioning [(70.65 ± 6.64)], role functioning [(71.70 ± 6.68)], physical functioning [(76.23 ± 7.28)], and overall health [(73.56 ± 6.21)]compared with the control group [(64.57 ± 7.28), (70.87 ± 6.67), (63.24 ± 5.57), (65.36 ± 5.73), (68.82 ± 6.71), and (66.56 ± 5.74), t = 6.10, 6.62, 6.20, 5.23, 5.44, 6.01, all P < 0.05]. There was no statistically significant difference in the incidence of adverse reactions between the two groups ( P > 0.05). Conclusions:Erchen decoction acupoint application can effectively alleviate chemotherapy-induced delayed nausea and vomiting and improve the quality of life of patients.
6.Comparison of ethinyl-estradiol/drospirenone tablets alone or in combination with orlistat on ovulation recovery rate and metabolic indices in overweight/obese patients with polycystic ovary syndrome

Objective To compare ovulation recovery rate and metabolic indicators between ethinyl-estradiol/drospirenone(EE/DRSP)combined with orlistat and EE/DRSP alone in overweight/obese patients with polycystic ovary syndrome(PCOS).Methods This study was a randomized controlled clinical trial conducted based on the 2004 Rotterdam criteria.From October 2020 to December 2023,180 overweight/obese PCOS patients aged 20-40 were recruited from the Department of Department of Gynecological Endocrinology,Beijing Obstetrics and Gynecology Hospital,Capital Medical University.The patients were randomly divided into two groups in a 1∶2 ratio.Among them,60 patients received treatment with EE/DRSP(EE20 μg,DRSP 3 mg),while 120 patients received a combination treatment of EE/DRSP and orlistat(360 mg/d).The height,weight,waist circumference,hip circumference,and blood pressure of the patients were measured before treatment and after 12 weeks of treatment.Laboratory tests included measurements of follicle-stimulating hormone(FSH),luteinizing hormone(LH),fasting insulin(FINS),homeostasis model assessment of insulin resistance(HOMA-IR),free androgen index(FAI),alanine transaminase(ALT),aspartate transaminase(AST),gamma-glutamyl transferase(GGT),fasting plasma glucose(FPG),total cholesterol(TC),triglycerides(TG),high density lipoprotein-cholesterol(HDL-C),low density lipoprotein-cholesterol(LDL-C),lipoprotein(a)[Lp(a)],sex hormone-binding globulin(SHBG),total testosterone(T),and free testosterone(FT).After 12 weeks of treatment,the medication was discontinued,and natural ovulation was observed.Results After 12 weeks of treatment,the ovulation rate of the EE/DRSP combined with orlistat group reached 70.8%,while the natural ovulation rate of the EE/DRSP group alone was only 35%,indicating that the ovulation rate was significantly increased after EE/DRSP combined with orlistat treatment.After 12 weeks of treatment,both groups showed a significant decrease in total testosterone,free testosterone,and low-density lipoprotein levels(all P<0.05),and the decrease in BMI,waist circumference,fasting insulin,and HOMA-IR in the EE/DRSP combined with orlistat group was greater than that in the EE/DRSP group alone(P<0.05).After treatment,both groups showed a significant increase in high-density lipoprotein and triglyceride levels(all P<0.05),with no significant changes in total cholesterol and fasting blood glucose(all P>0.05).Conclusion After 12 weeks of treatment,EE/DRSP combined with orlistat can significantly improve the ovulation rate of PCOS patients.It is superior to EE/DRSP alone in reducing androgen levels,body weight,insulin resistance,and low-density lipoprotein levels.
7.Effect of individualized lifestyle intervention on sexual function in obese patients with polycystic ovary syndrome

Objective To investigate the effect of individualized lifestyle intervention on sexual function in obese patients with polycystic ovarian syndrome(PCOS).Methods The study was conducted on obese patients with PCOS in the Department of Endocrinology,Beijing Obstetrics and Gynecology Hospital,Capital Medical University from October 2022 to December 2023.There were 160 cases in both the experimental group and the control group,progesterone was administered to both groups for 3 months to regulate menstruation.For women in the experimental group received individualized comprehensive lifestyle intervention combining diet,exercise,and behavior modifications,along with dedicated follow-up and weight management.The control group received routine clinical education for lifestyle intervention aimed at weight loss.Female Sexual Function Index(FSFI)and 12-item Short-Form Health Survey(SF-12)scale were applied to evaluate the sexual function and quality of life.The clinical data that may affect sexual function were collected and sex hormone levels were measured,including anthropometric indicators,estradiol,sex hormone-binding globulin,total testosterone,bioactive testosterone,and dehydroepiandrosterone sulfate in serum.Results After 3 months of intervention,the body mass index,waist circumference,body fat percentage,insulin resistance index,total testosterone,bioactive testosterone and dehydroepiandrosterone sulfate in experimental groups were significantly lower(P<0.05),and the sex hormone binding globulin level was significantly higher(P<0.001),compared with the control group.SF-12 mental health scores,FSFI total scores,sexual desire,orgasm and sexual satisfaction scores were significantly increased in experimental groups(P<0.05),however,there were no significant differences in SF-12 physical health scores,as well as the scores of sexual arousal,vaginal lubrication,and coital pain.Conclusions Individualized lifestyle intervention can better improve the sexual function and mental health of obese patients with PCOS.
8.Exploration of factors influencing abnormal iodine nutrition and pregnancy outcome in pregnant women during pregnancy based on thyroid function and thyroid autoantibody analysis

Objective To analyze the factors influencing iodine nutritional status in pregnant women dur-ing pregnancy,based on thyroid function and thyroid autoantibody levels,and to explore the association between iodine nutritional abnormalities and pregnancy outcomes.Methods A total of 838 pregnant women who underwent routine prenatal checkups at Qinghai Provincial People's Hospital between January 2021 and June 2023 were pro-spectively enrolled in this study.All participants were followed until delivery.Seven cases were lost to follow-up,resulting in a final sample size of 831 participants.Among them,276 were in the first trimester,384 in the second trimester,and 171 in the third trimester.Data on urinary iodine concentration(UIC),urinary creatinine(UCr),thyroid function indicators,and thyroid autoantibodies were collected.Based on their iodine nutritional status,the participants were categorized into either the iodine-sufficient group or the iodine-abnormal group(including iodine-deficient,iodine-hyper-sufficient,and iodine-excessive subgroups).This study analyzed the iodine nutritional sta-tus of pregnant women during different gestational periods,compared thyroid function indices,prevalence of thy-roid diseases,and the positivity rates of thyroid peroxidase antibody(TPOAb),thyroglobulin antibody(TGAb),and thyroid-stimulating hormone receptor antibody(TRAb)among different iodine status groups.Additionally,ad-verse pregnancy outcomes were compared across groups.Multivariate logistic regression analysis was conducted to identify risk factors associated with iodine abnormalities during pregnancy,and a predictive model was developed to assess its potential predictive value.Results Among the 831 pregnant women included in the study,373 cases(44.89%)exhibited iodine sufficiency,while 458 cases(55.11%)presented with iodine abnormalities,including 282 cases of iodine deficiency,144 cases of iodine hypersufficiency,and 32 cases of iodine excess.No statistically significant differences were observed in the iodine nutritional status across different trimesters(P>0.05).The se-rum level of thyroid-stimulating hormone(TSH)was significantly higher in the iodine abnormal group compared to the iodine sufficient group(P<0.05).Additionally,the iodine abnormal group demonstrated higher positivity rates of TPOAb alone,TGAb,and TRAb,as well as increased incidence of thyroid dysfunction and total adverse pregnancy outcomes compared to the iodine sufficient group(all P<0.05).These adverse indicators were also sig-nificantly elevated in the iodine-deficient,iodine super-sufficient,and iodine overdose subgroups compared to the iodine sufficient group(P<0.05).Elevated serum TSH levels and the presence of TPOAb,TGAb,and TRAb were identified as risk factors for iodine abnormalities during pregnancy(P<0.05).The predictive model con-structed for identifying iodine abnormalities in pregnant women demonstrated an area under the curve(AUC)of 0.876,with a sensitivity of 72.27%and a specificity of 89.01%.Conclusions The prevalence of iodine nutritional abnormalities among pregnant women during pregnancy was high,with most cases presenting iodine deficiency.These abnormalities were associated with thyroid function,thyroid autoimmunity,and pregnancy outcomes,but showed no significant correlation with gestational age.Furthermore,the prediction model developed based on iden-tified risk factors demonstrated effective performance in predicting iodine nutritional abnormalities during preg-nancy.

10.Exploration, practice, and implications of evaluating the implementation quality of scenario-based simulation courses

Objective:To explore the construction of an assessment tool for the implementation of scenario-based simulation courses, and provide a reference for the standardized implementation of scenario-based simulation courses and the key direction of teacher training.Methods:Based on the elements of the implementation of scenario-based simulation courses, the Scenario-Based Simulation Course Implementation Quality Scale was developed through expert interviews and expert panel meetings. This scale was used to rate the implementation of scenario-based simulation courses by 12 teaching teams in Shanghai, China. The evaluation scores were collected and analyzed using R language.Results:The reliability of the evaluation scale was assessed using Cronbach's α (0.893) and inter-rater consistency was assessed using the intraclass correlation coefficient (0.226). Among the 12 teaching teams, the lowest score rate was 67.07% for "debriefing". A correlation analysis between scenario-based simulation teaching design and course implementation showed a Pearson correlation coefficient r of -0.08 ( P=0.809), which indicated no significant correlation between them. Conclusions:The assessment of the implementation of scenario-based simulation courses should include all aspects of course implementation. The rater's understanding of scenario-based simulation courses can affect the results. Debriefing remains a challenge in implementing scenario-based simulation courses. A comprehensive assessment of the teaching ability of simulation teaching teams should include both teaching design and course implementation.
