1.Correlation between leisure crafting and post competence among anesthesia specialist nurses

Objective:To investigate the status of leisure crafting and post competence and their correlation among anesthesia specialist nurses.Methods:A convenience sampling method was used to conducted a questionnaire survey on 280 anesthesia specialist nurses at grade A tertiary hospitals in 19 provinces of China. Data were collected using a general information questionnaire, the Chinese version of Leisure Crafting Scale, and the Post Competency Scale for Anesthesia Specialist Nurses. SPSS 24.0 was used for t test and one-way analysis of variance. A Pearson correlation analysis was used to assess relationships between variables. Results:The total score of leisure crafting was (30.41±6.69) and the average score was (3.38±0.74). The total score of post competency was (141.76±21.76) and the average score was (3.73±0.57). A positive correlation was observed between leisure crafting and post competency ( r=0.215, P<0.01). Conclusions:Managers should encourage anesthesia specialist nurses to actively engage in leisure crafting and provide diverse opportunities for leisure crafting to improve their post competence and ultimately the quality of anesthesia nursing.
2.Comparison of the medication rules for treating two types of non-puerperal mastitis with traditional Chinese medicine

Objective To compare the medication rules of traditional Chinese medicine in the treatment of granulomatous lobular mastitis(GLM)and plasma cell mastitis(PCM).Methods The relevant literatures on GLM and PCM therapy published by CNKI,VIP,Wanfang Data Knowledge Service Platform and SinoMed from the establishment of the database to June 2024 were searched.The prescription was screened,and the frequency,nature and flavour,channel tropism were analyzed,and association rule analysis and cluster analysis were carried out.Results The top four most frequently used traditional Chinese medicines in the treatment of GLM and PCM were Chaihu,Pugongying,Zaojiaoci and Danggui.In the two-item association,the highest confidence levels for the treatment of the two were Huzhang→Muli and Ezhu→Sanleng.In the three-item association,the highest confidence levels for the treatment of the two were Huzhang→Jinyinhua-Pugongying and Chaihu→Yujin-Danggui.In the cluster analysis,the drugs for both could be clustered into four categories.Conclusion This study compares the medication rules of traditional Chinese medicine in the treatment of GLM and PCM,reflecting similarities and differences,and provides data reference for clinical compound research and development.

5.Paraventricular hypothalamic nucleus projects directly to the preB?tzinger complex and is involved in regulation of respiration in mice

Objective To probe the spatial distribution of respiratory-related nuclei throughout the whole brain,and to investigate the circuit and projection links between the paraventricular hypothalamic nucleus (PVN)and the preB?tzinger complex (preB?tC)in mice.Methods Using a retrograde tracing strategy from peripheral to central,pseudorabies viruses (PRV)were microinjected into the diaphragm of mice to retrogradely label the upstream inputs brain regions from spinal respiratory phrenic motor neurons. The anterograde non-transsynaptic tracing viruses (AAV2/9-EGFP)were microinjected into the PVN to label the downstream brain region of the PVN;And using the Cre/loxP gene expression strategy,Dio-EGFP and Retro-Cre viruses were injected in the PVN and preB?tC,respectively,to validate the PVN-preB?tC circuit and the projection relationship between the two.Results The results of PRV-mediated retrograde tracing showed that PRV-EGFP-positive neurons were distributed in several brain regions such as PVN,preB?tC,nucleus tractus solitarii (NTS),locus coeruleus (LC)and periaqueductal grey (PAG).Except for the medulla oblongata region,the PVN brain region had the highest percentage of PRV-EGFP-positive neurons (P<0.0001 ).After AAV2/9-EGFP viruses were injected in the PVN,green neurofibrils were observed in the preB?tC brain region after viral infection;Dio-EGFP and Retro-Cre viruses were injected in the PVN and preB?tC,respectively,and green neuronal cells were seen in the brain region of the PVN after viral infection.Conclusion PVN is an important upstream input nucleus of spinal respiratory phrenic motor neurons,and has the potential to regulate respiration through direct projections from PVN-preB?tC circuit.
6.Impact of the third lumbar skeletal muscle index on drug therapy of patients with inflammatory bowel disease

Objective:To investigate the prevalence of sarcopenia as defined by the third lumbar skeletal muscle index (L3-SMI) and explore its impact on drug therapy in patients with inflammatory bowel disease (IBD).Methods:The baseline abdominal CT images of 230 patients with IBD hospitalized in the First Hospital of Shanxi Medical University from December 2018 to December 2022 were retrospectively analyzed to obtain L3-SMI. The optimal cut-off value of L3-SMI to define sarcopenia was determined using receiver operating characteristic curve analysis. The IBD group included 164 cases of ulcerative colitis (UC) and 66 cases of Crohn's disease (CD), compared with 100 cases of the normal control. The clinical data and biochemical indicators, such as body mass index (BMI), albumin (ALB), pre-albumin (PA), disease activity and therapeutic drugs, were collected. Outcomes were followed up, including the optimization of baseline drug therapy, the initiation of additional biological agents or combined drug therapy. Multivariate logistic regression analysis was used to identify risk factors for the escalation of drug therapy, and the nomogram was constructed and evaluated for the prediction of drug regimen escalation in patients with IBD.Results:The prevalence of sarcopenia in the IBD population in this study was 39.6%, among whom 36.0% were with ulcerative colitis and 48.5% with Crohn's disease. The prevalence of malnutrition (BMI<18.5) in IBD patients was 18.7%, whereas 69.2% of patients with concurrent IBD and sarcopenia had a BMI≥18.5 . The median L3-SMI of IBD patients was significantly lower than that of normal controls (36.30 cm 2/m 2vs. 39.37 cm 2/m 2, P=0.004). Sarcopenia ( OR=3.33, 95% CI: 1.06 to 10.50, P=0.040) was associated with the escalation of drug therapy in patients with IBD. Based on the patient's age, presence or absence of sarcopenia, albumin and BMI, the nomogram prediction model was established, with the area under the curve (AUC) of 0.762 (95% CI: 0.691 to 0.834), showing good discriminating performance. The calibration curve and Hosmer-Lemeshow goodness-of-fit test demonstrated good consistency between the predicted and observed data ( χ2 =11.906, P=0.156). Decision curve analysis showed potential benefits of the prediction model in clinical settings. Conclusion:Sarcopenia is common in patients with IBD, which is related to the escalation of drug therapy in patients with IBD.
7.Study on the determination of compound levonorgestrel sustained release microspheres injection by HPLC

Objective:To establish a high-performance liquid chromatography (HPLC) for determining the content of levonorgestrel (LNG) and ethinylestradiol (EE) in compound levonorgestrel sustained-release microsphere injection.Methods:C18 column Inertsil ODS-3 (4.7 mm×250 mm, 5 μm) was used. The mobile phase was acetonitrile-water (50∶50, V/V). The flow rate was 1.0 mL/min. The column temperature was 35 ℃. The detection wavelength was 265 nm, and the injection amount was 20 μL. The content of microspheres was determined after validation of HPLC detection methodology.Results:The linear relationship was good in the range of 5.03-201.20 μg/mL LNG, 1.55-61.92 μg/mL EE ( r2=0.999 8). The specificity, precision, recovery, repeatability, stability, etc, met the requirements. The content of LNG in the compound LNG sustained-release microspheres injection was 16.26%, and the content of EE was 2.58%. Conclusion:This HPLC can determine the content of compound levonorgestrel sustained-release injection microspheres, and the method is simple, stable, and has good reproducibility, providing a reference for the determination of levonorgestrel and ethinylestradiol in compound sustained-release preparations.

9.Analysis of tumor recurrence factors of the patients with intermediate risk papillary thyroid carcinoma after ra-dioactive iodine treatment

Objective To analyze the recurrence factors of the patients with intermediate risk papillary thyroid carcinoma(PTC)after radioactive iodine treatment(RAIT).Methods A total of 254 intermediate risk PTC patients after RAIT from January 2018 to May 2020 in our hospital were enrolled into this study.Chi square test and multivariate analysis were applied to analyze the main recurrence factors after RAIT.Results Thirty four(13.4%)patients with intermediate risk PTC relapsed after RAIT.The primary tumor size,lymph node metastasis staging,and pre-ablation stimulated thyroglobulin(psTg)were associated with tumor recurrence.According to the receiver operating characteristic(ROC)curve,psTg 25.6 μg/L was the best cutoff value for predicting recurrence.Further multivariate analysis revealed that lymph node metastasis staging and psTg levels were independent factors of predicting PTC recurrence.When the patients with N1b and psTg≥25.6 μg/L,the incidence of tumor recurrence after RAIT was 58.3%.While the patients with N1a and psTg<25.6 μg/L,the incidengce of tumor recurrence was only 2.3%.Conclusions Lymph node metastasis staging and psTg level are the important indicators of PTC recurrence,and combining these two indicators can help predict tumor recurrence after RAIT.
10.Study on the antipyretic mechanism of large pushing Tianheshui for young rabbits with endotoxin-induced fever

Objective:To observe the effect of large pushing Tianheshui manipulation intervention on the body temperature of young rabbits with endotoxin-induced fever and discuss its antipyretic mechanism.Methods:Thirty-two young rabbits meeting the standards were selected from 40 ordinary young male New Zealand rabbits after being adapted for 7 d,and randomly divided into a normal group,a model group,a large pushing Tianheshui group,and an ibuprofen group according to the random number table method,with 8 rabbits in each group.Rabbits in the model group,the large pushing Tianheshui group,and the ibuprofen group were subjected to preparing the endotoxin-induced fever model by intravenous lipopolysaccharide from the marginal ear vein.Rabbits in the large pushing Tianheshui group received Tuina(Chinese therapeutic massage)manipulation intervention 1.5 h and 2.5 h after modeling,respectively.Rabbits in the ibuprofen group were intragastrically given ibuprofen suspension 1.5 h after modeling.The dynamic changes in body temperature were observed for the young rabbits after fever modeling.Enzyme-linked immunosorbent assay was used to determine the content changes in positive mediators of hypothalamic body temperature,including prostaglandin(PG)E2 and cyclic adenosine monophosphate(cAMP),as well as negative mediators of hypothalamic body temperature,including arginine vasopressin(AVP)and α-melanocyte stimulating hormone(α-MSH).Results:The body temperature of the young rabbits in the model group was significantly higher than that in the normal group at 0.5-4.0 h,5.0 h,and 5.5 h after modeling(P<0.01),showing two obvious fever peaks in the model group at 1.5 h and 3.0 h after modeling,respectively,with the highest peak at 1.5 h.Compared with the model group,body temperatures of the large pushing Tianheshui group and the ibuprofen group decreased significantly after 0.5 h of intervention(P<0.05).Compared with the normal group,the contents of PGE2 and cAMP were significantly increased(P<0.05),and the contents of AVP and α-MSH were significantly decreased(P<0.01)in the hypothalamus of the model group.Compared with the model group,the contents of PGE2 and cAMP were significantly decreased(P<0.01),and the levels of AVP and α-MSH were significantly increased(P<0.01)in the hypothalamus in the large pushing Tianheshui group and the ibuprofen group.There were no significant differences in the PGE2,cAMP,AVP,and α-MSH contents in the hypothalamus between the ibuprofen group and the large pushing Tianheshui group(P>0.05).Conclusion:Large pushing Tianheshui manipulation has a significant antipyretic effect on endotoxin-induced fever in young rabbits.The mechanism may be related to inhibiting the positive regulators(PGE2 and cAMP)and promoting the negative regulators(AVP and α-MSH)of hypothalamic control of body temperature.
