Objective:To investigate the preventive and therapeutic effects and mechanisms of Dachaihu decoction(DCD)on dextran sodium sulfate(DSS)-induced ulcerative colitis(UC)and liver injury in mice,as well as the association between DCD benefits and gut microbiota modulation.Methods:Mice were treated with DCD(20.10 and 10.05 g/kg)for 2 weeks,with free access to drinking water containing 3%DSS in the second week to induce UC.Histopathological examination,RT-qPCR and 16S rRNA sequencing were used to investigate the effect of DCD on UC mice.Results:DCD pretreatment significantly alleviated weight loss,bloody diarrhea with mucus,histopathological abnormalities of the colon,and colon shortening in mice with DSS-induced UC.In addition,DCD pretreat-ment significantly upregulated the levels of Occludin,ZO-1,and MUC-2 in the colon and protected the intestinal barrier of mice.DCD pretreatment also alleviated inflammatory cell infiltration in the colon and the liver and significantly reduced the expression levels of the proinflammatory factors such as IL-1β,IL-6,TNF-α,iNOS,COX-2,and NLRP3,thereby exerting a protective effect against UC and liver injury.It should be noted that DCD corrected gut micro-biota imbalance in UC mice by enriching probiotic bacteria such as Lactobacillus and Bifidobacterium and reducing harmful bacteria such as Norank_f_Desulfovibrionaceae and Escherichia-Shigella.Conclusion:DCD can alleviate DSS-induced UC and exert a liver-protecting effect by protecting intestinal barrier,inhibiting inflam-mation,and regulating gut microbiota.

OBJECTIVE: To investigate the clinical and immunological characteristics, treatment and prognosis of common variable immune deficiency (CVID) in adult patients. METHODS: We retrospectively analyzed the clinical data of 13 adult patients hospitalized in our hospital for CVID diagnosed according to the criteria in International Consensus Document (2016), and analyzed their clinical manifestations, laboratory test results, imaging findings, pathological examinations and treatments. RESULTS: The mean age of onset was 24.46±16.82 years in these patients, who had a mean age of 32.54±14.86 years at diagnosis with a median diagnostic delay of 5 years (IQR: 2-15 years). The main manifestation of the patients was repeated infections, including repeated respiratory tract infection (10 cases; 76.9%) and repeated diarrhea (3 cases; 23.1%). Three (23.1%) of the patients had autoimmune disease and 10 (76.9%) had chronic pulmonary disease. IgG, IgA and IgM were decreased in all the patients. The proportion of CD4+T cells decreased in 10 patients (76.9%), CD8+T cells increased in 11 patients (84.6%), and CD4/ CD8 decreased in 10 patients (76.9%). Complement C3 decreased in 58.3% (7/12) and C4 decreased in 33.3% (4/12) of the patients. Twelve patients (92.3%) were treated with intravenous infusion of gamma globulin with symptomatic treatments. One patient died due to massive gastrointestinal hemorrhage, and the other patients showed improve ments after the treatments and were discharged. CONCLUSIONS The clinical manifestations of CVID are diverse, and recurrent respiratory tract infection is the most common manifestation. Decreased IgG often accompanied by lowered IgA and IgM levels is a common finding in laboratory tests. The treatment of CVID currently relies on gamma globulin with symptomatic treatments for the complications.

Objective To describe the characteristics of organizational culture,to identify the level of quality of work life,and to explore the relationship between each dimension of organizational culture and quality of work life among nurses in tertiary hospitals in Yunnan Province,China.Methods The subjects were 416 nurses from six tertiary hospitals in Yunnan Province.Research instruments were the Organizational Culture Profile (OCP) and the Quality of Work Life Scale (QWLS).Results The results of this study showed that four dimensions of organizational culture including competitiveness (3.56 ± 0.49);performance orientation (4.08 ± 0.22);stability (3.68 ± 0.43) and social responsibility (3.95 ± 0.37) were prominent.The other 3 dimensions,namely supportiveness (3.19 ± 0.79);innovation (3.21 ±.0.62) and emphasis on rewards were perceived (2.42 ± 0.79) as less prominent.The overall mean score of quality of work life was at a moderate level (57.46 ± 15.17).Conclusion There are positive significant correlation between each dimension of organizational culture and quality of work life (P＜ 0.01).Improvement of organizational culture can enhance quality of work life among nurses,and then guarantee the nursing quality.

Objective To apply quality of control circle ( QCC) to reduce the rate of low back pain (LBP) happened on medical staff. Methods The QCC activity were applied in medical staff with LBP in Orthopedic Department of West China Hospital of Sichuan University. The reasons was analyzed, and the corresponding measurements were made and implemented. The result of activity were evaluated. Results The rate of LBP were decreased significantly from 146 to 92 with a progress rate of 37. 23% and achieved rate106. 17%. This activity standardized the process of low back pain protection. Conclusions QCC activity play a positive role in reducing the rate of low back pain and it definitely can improve the health status of medical staff and their life quality.

Objective To apply quality control circle ( QCC) to improve antimicrobial use correct rate treated type I incision during perioperative orthopedic. Methods The QCC group was established to grasp the current status of antimicrobial use correct rate for type I incision, set goals, analyze major factors, and form and implement countermeasures. Results The unreasonable use incidences of antimicrobial agents were decreased from 69 to 22 cases with a progress of 70%, and improved the nurse′s ability of finding and solving problems and the ability of team cooperation after conducting QCC activities. Conclusions QCC activities plays a positive role in management of orthopedic perioperative antimicrobial use, and ensures the safety of clinical application and cure rates, prevents the incision infection and postoperative nosocomial infection, increases the operation safety, and reduces the drug resistant strains.
