2.Discussion on the mechanism of Ganwei Baihe Decoction in treating gastric ulcer based on bioinformatics and experimental validation

Objective:To study the potential mechanism of Ganwei Baihe Decoction in the treatment of gastric ulcer (GU) based on bioinformatics and validate it through animal experiments.Methods:TCMSP, DisGeNET, and GeneCards databases were used to retrieved active components and action targets of Ganwei Baihe Decoction. After obtaining the intersection, protein interaction data of the intersection genes were obtained through the STRING database. A PPI network was constructed by Cytoscape 3.10.0 software and the key genes and key components were obtained. DAVID online analysis database was used for GO functional enrichment and KEGG pathway enrichment analysis of key targets. Animal experiments were used for verification. Totally 36 SD rats were divided into blank group, model group, Omeprazole group and Ganwei Baihe Decoction group according to the random number table method, with 9 rats in each group. After 7 days of gavage of the corresponding drugs to each group of rats, they fasted and but with water for 24 hours, and then re-gavaged once. After 1 hour of administration, a gastric ulcer rat model was prepared by gavage of 80 mg/kg of indomethacin. After 3 hours of administration, anesthesia was used to extract the sample. The expression level of Caspase-3 protein in the gastric tissue of rats was to be determined by Western blot method.Results:There were 234 effective active components with 290 targets in Ganwei Baihe Decoction, and 6 496 therapeutic targets for GU. 213 potential targets for GU were screened out. There were 437 GO function and 153 KEGG pathway enriched entries. Compared with the model group, the protein expression of Caspase-3 in the Ganwei Baihe Decoction group and Omeprazole group decreased ( P<0.05). Conclusion:The mechanism of Ganwei Baihe Decoction in treating GU may be through key components such as quercetin and β-sitosterol acting on key targets such as AKT1 and CASP3, regulating the Apoptosis pathway, PI3K-Akt signaling pathway, MAPK signaling pathway, etc. to exert inhibitory effects on apoptosis.
3.Research on the application of monitoring and regulation of intrapelvic pressure in supermicropercutaneous nephrolithotomy

Objective To investigate the application of intra pelvic pressure(IPP)in ultramicro-channel percutaneous nephrolithotripsy.Methods From January 2022 to January 2023,60 patients with urinary calculi who needed Super mini-PCNL(SMP)in The First Hospital of Jiaxing selected as the study objects.According to random number method,the patients were divided into control group and experimental group,with 30 cases.Both groups were treated with ultra-micro channel percutaneous nephrolithotripsia,while the experimental group was monitored and regulated IPP in real time during the operation,and observed and compared clinical indicators,IPP,fever,urinary protein,renal function,hemoglobin(Hb)and adverse reactions between the two groups.Results Compared with the control group,the hospitalization time of experimental group was shortened and the stone clearance rate was increased(P<0.05).The IPP levels of experimental groups at 6min,12min,24min and 36min were lower than those of control group(P<0.05).The fever of experimental group was lower than that of control group at 2d,3d,4d and 5d after operation(P<0.05).The urinary protein level of experimental group was lower than that of control group at 1d,2d,3d and 4d after operation(P<0.05).Compared with control group,blood urea nitrogen(BUN)and serum creatinine(SCr)levels of experimental group were decreased,and Hb levels were increased(P<0.05).The incidence of adverse reactions in experimental group was lower than that in control group(P<0.05).Conclusion Monitoring and adjusting intrapelvic pressure during super-mini percutaneous nephrolithotomy is beneficial in reducing postoperative fever in patients with urolithiasis,reducing urinary protein expression and kidney function damage,and controlling the occurrence of adverse reactions.It is worth recommending.
4.The reliability and validity of cervical movement sense measurement in patients with chronic non-specific neck pain

Objective:To observe the validity of the transversal 8(T8)graphs and zigzag(ZZ)graphs tests in the measure-ment of cervical movement sense(CMS),and to study their effectiveness in evaluating patients with chronic non-specific neck pain.Method:A total of 71 subjects were recruited,including 35 with chronic non-specific neck pain and 36 healthy subjects.Two groups of researchers separately conducted tests(T8 and ZZ)to the subjects twice,re-corded the test process and scored the test by slow playback.The intra-and inter-rater reliability was quanti-fied in terms of inter-class correlation coefficients(ICCs)and the difference in index between the two groups was also determined for validity.Result:The intra-and inter-rater ICC for ZZ graph and T8 graph tests were 0.813/0.832 and 0.892/0.908,the corresponding standard error of measurement(SEM)values were 1.15/1.10,1.24/1.37,while the minimal detect-able change(MDC)values were 3.18/2.73,3.44/3.05.There was a significant difference in the results of the ZZ graphs and the T8 graphs tests between patients with chronic non-specific neck pain and healthy individu-als(P<0.05).The ideal cut-off values using to distinguish normal individuals from patients with chronic non-specific neck pain are 10.5 for ZZ graphs test and 16.3 for T8 graphs test.Conclusion:The T8 graphs and ZZ graphs tests showed good intra-and inter-raters reliability in assessing cervi-cal movement sense,which can help distinguish patients with chronic non-specific neck pain and the healthy in-dividuals.They can be considered as a cervical movement sense assessment method in the patients with chron-ic non-specific neck pain.

6.Development of a scoliosis rehabilitation robot and a preliminary study of its effectiveness in treating coronal deformity

Objective:To explore the appropriate use of a self-developed scoliosis rehabilitation robot in treating adolescent idiopathic scoliosis (AIS), and also its safety and effectiveness.Methods:The scoliosis rehabilitation robot consists of a closed-loop pneumatic control system and multi-segment torso, pelvis and upper extremity fixation devices. It provides three-dimensional synchronous correction. Eighteen AIS subjects first received 30min of robot-assisted treatment using the maximum tolerable orthotic force. The angles of their spinal processes were evaluated using ultrasound before the treatment and after 30 seconds, 5 minutes and 30 minutes of treatment, then 5min later. In a second 30sec course of treatment the transverse orthotic force was 10%, 15%, 20%, 25%, and 30% of the patient′s body weight. Any adverse effects were observed and recorded.Results:The robot ran smoothly and could apply intelligent and precise correction. No severe adverse effects were reported. The mean correction of the spinal process angles showed a significant cumulative effect with treatment time, reaching 104% at 30min. The mean process angle correction increased with the applied force. Force at 25% of the patient′s weight produced an average correction of 104% in patients with mild AIS and 65% in those whose AIS was moderate.Conclusion:The scoliosis rehabilitation robot is safe and immediately effective. Setting the transverse force at 25% of a patient′s weight gives the best corrective effect with mild AIS. Moderate AIS requires more force.
7.A Survival Prediction Model of Pulmonary Sarcomatoid Carcinoma Based on SEER Database

Objective To analyze the factors affecting the prognosis of patients with pulmonary sarcomatoid carcinoma (PSC) and construct a nomogram prediction model for the prognosis of PSC patients. Methods Based on the SEER database, 1671 patients diagnosed as PSC from 1988 to 2015 were collected and divided into modeling group and validation group according to the ratio of 7:3. Univariate and multivariate Cox regression analysis were performed in the modeling group to explore independent risk factors affecting the prognosis and construct a nomogram survival prediction model. The consistency index and calibration curve were used for verification in the modeling group and the test module respectively. Results Age, gender, histological type, TNM stage, tumor diameter > 50mm, surgery, radiotherapy and chemotherapy were independent factors that affected the prognosis of PSC patients. Objective::Observational studies indicate that insomnia may increase the risk of developing and/or dying from cardiovascular diseases, especially coronary artery disease (CAD). Our purpose is to explore the underlying causal relationship between genetic variants susceptible to insomnia and the risk of CAD by Mendelian randomization analysis.Methods::The study was conducted using publicly available statistical data on genetic variants identified from a genome-wide association meta-analysis of insomnia ( n = 113,006 individuals) and a genome-wide association meta-analysis of CAD ( n = 184,305 individuals), which consisted of both cases and non-cases. The genetic association between variants and CAD was assessed by the variants’ association with insomnia, and estimations were integrated by an inverse-variance weighted meta-analysis. Results::Among the Mendelian randomized analytical sample, 8 variants were associated with insomnia complaints and CAD. And there was no pleiotropic association with the latent confounders. In addition, in the inverse-variance weighted meta-analysis (the estimations combined from the 8 variants), the odds ratio was 1.15 (95% CI: 1.05-1.25; P= 0.002) for CAD, and in the weighted method analysis, the odds ratio was 1.14 (95% CI: 1.03-1.27; P= 0.015) for CAD. Conclusions::All of the data indicated that some valuable variants might involve in the development of CAD by leading the insomnia. Therefore, insomnia might be a causal factor for CAD, and improving the quality of sleep might be a new way for populations with insomnia to prevent CAD.

10.Effects of Ligustrazine on miR- 20b/VEGF and BMP 2/Smad1 Pathways in Subchondral Bone of Knee Osteoarthritis Model Rats

OBJECTIVE: To study the effects of ligustrazine on miR-20b/VEGF and BMP2/Smad1 pathways in subchondral bone of knee osteoarthritis (KOA) model rats, and to investigate the mechanism of ligustrazine for KOA prevention and treatment. METHODS: Totally 18 healthy male SD rats were randomly divided into normal control group, model group and ligustrazine group, with 6 rats in each group. The rats in the latter two groups were used to establish KOA model by intra-articular injection of 4% papain solution. From the 2nd day after the last injection, ligustrazine group was given intragastrical administration of Ligustrazine suspension (100 mg/kg) 2 mL; normal control group and model group were given intragastrical administration of isometrical normal saline, once a day, for consecutive 6 weeks. After the last after medication, the situation of bilateral knee articular cartilage of rats were observed after exposure. The knee joints of rats were sectioned and stained with HE. The pathological change of articular cartilage were observed by microscope and scored by modified Mankin’s score. mRNA expression of VEGF, BMP2 and Smad1, and the expression of miR-20b were detected by RT-PCR; the protein expression of VEGF, BMP2 and Smad1 were detected by Western blot assay. RESULTS: Model group and ligustrazine group suffered from cartilage injury of knee joint at varying degrees. Compared with normal control group, Mankin’s scores of knee joint and cartilage tissue were increased significantly in model group (P<0.01); mRNA and protein expression of BMP and Smad1, the expression of miR-20b in subchondral bone of model group were decreased significantly, while mRNA and protein expression of VEGF were increased significantly (P<0.01). Compared with model group, Mankin’s score of cartilage tissue were decreased significantly in ligustrazine group (P<0.01); mRNA and protein expression of BMP and Smad1, the expression of miR-20b in subchondral bone were increased significantly, while mRNA and protein expression of VEGF were decreased significantly (P<0.05 or P<0.01). CONCLUSIONS: Ligustrazine can repair damaged articular cartilage in KOA model rats, the mechanism of which may be associated with inhibiting the protein expression of VEGF and activating BMP-2/Smad1 signaling pathway via up-regulating the expression of miR-20b, and promoting the degradation of VEGF mRNA in subchondral bone.
