1.Analysis of the incidence and mortality trends of type 2 diabetic nephropathy in China from 1990 to 2021

ObjectiveTo analyze the long-term trend of incidence and mortality of type 2 diabetic kidney disease (DKD) in China from 1990 to 2021. MethodsThe Joinpoint regression model was used to analyze the average annual percentage change (AAPC) of standardized incidence rate and standardized mortality rate, and the age-period-cohort (APC) model was constructed to analyze the longitudinal age change, period and cohort effect risk ratio (RR). ResultsFrom 1990 to 2021, the standardized incidence rate of type 2 DKD in males and females showed an overall upward trend, with AAPC of 0.08% and 0.36%, respectively. The age-standardized mortality rate of the total population and female showed a downward trend, with AAPC of -0.61% and -1.03%, respectively. However, there was no significant difference in males. APC model showed that the age effect existed: the peak age was 75-79 years old, the mortality rate of females increased, and the mortality rate of males decreased after 80-84 years old. For the effect of time period, the risk of type 2 DKD incidence in females in 2017—2021 was 1.05 times that in 2002—2006, and the risk of death in males and females in 2017—2021 was 0.84 and 0.71 times that in 2002—2006, respectively. For cohort effects, the highest risk of disease was seen in men and women born in 1967—1971, and the highest risk of death was seen in men born in 1952—1956 and women born in 1912—1916. ConclusionFrom 1990 to 2021, the standardized incidence rate of type 2 DKD in China shows an upward trend, and the standardized mortality rate shows a downward trend. It is necessary to strengthen the health behavior publicity and education of type 2 DKD, and actively carry out early screening to reduce the disease burden.

Objective:
To explore the distribution of traditional Chinese medicine (TCM) syndromes in children with hepatolenticular degeneration (Wilson disease, WD) based on factor analysis and cluster analysis.
Methods:
From November 2018 to November 2023, general information (gender, age of admission, age of onset, course of disease, clinical staging, Western medicine clinical symptoms, and family history) and TCM four-examination informations (symptoms and signs) were retrospectively collected from 757 cases of children with WD at the First Affiliated Hospital of Anhui University of Chinese Medicine, and factor analysis and cluster analysis were used to investigate TCM syndromes in children with WD.
Results:
A total of 757 children with WD were included, of which 483 were male and 274 were female; the median age at admission was 12.58 years, the median age at onset was 8.33 years, and the median course of disease was 24.37 months; clinical typing result indicated 506 cases of hepatic type, 133 cases of brain type, 99 cases of mixed-type, and 19 cases of other type; 36.46% of the children had no clinical symptoms (elevated aminotransferases or abnormalities in copper biochemistry); a total of 177 cases had a definite family history, and 10 cases had a suspected family history. Forty-three TCM four-examination information were obtained, with the top 10 in descending order being feeling listless and weak, brown urine, slow action, inappetence, dim complexion, slurred speech, angular salivation, body weight loss, hand and foot tremors, and abdominal fullness. In children with WD, the syndrome element of disease location was primarily characterized by the liver, involving the spleen and kidney, and the syndrome elements of disease nature were characterized by dampness, heat, and yin deficiency. Based on factor analysis and cluster analysis, five TCM syndromes were derived, which were, in order, syndrome of dampness-heat accumulation (265 cases, 35.01%), syndrome of yin deficiency of the liver and kidney (202 cases, 26.68%), syndrome of liver hyperactivity with spleen deficiency (185 cases, 24.44%), syndrome of qi and blood deficiency (79 cases, 10.44%), and syndrome of yang deficiency of the spleen and kidney (26 cases, 3.43%).
Conclusion
The TCM syndromes of children with WD were primarily syndromes of dampness-heat accumulation, yin deficiency of the liver and kidney, and liver hyperactivity with spleen deficiency. The liver was the main disease location, and the disease nature was characterized by deficiency in origin and excess in superficiality, excess and deficiency mixed. These findings suggest that treating children with WD should be based on the liver while also considering the spleen and kidney.

Parkinson's disease(PD) is the second most common neurodegenerative disease in the world, which seriously affects the lives of patients. With the acceleration of aging process, the number of patients continues to rise. Its main pathological features are aggregation of α-synuclein and degenerative death of dopaminergic neurons in the substantia nigra. However, the pathogenesis of PD is still unclear. According to reports, the brain-derived neurotrophic factor(BDNF)/tyrosine kinase receptor B(TrkB) signaling pathway is highly expressed and activated in dopaminergic neurons in the substantia nigra, which is closely related to neurophysiological processes such as neurogenesis, synaptic plasticity, neuroinflammation, and oxidative stress. It plays an important role in the occurrence and development of PD. At present, the treatment methods of Western medicine for PD are mainly based on drugs such as levodopa and dopamine agonists to alleviate motor symptoms, but with the increase of dose, the adverse reactions are significantly enhanced. Traditional Chinese medicine(TCM) has attracted people to explore its therapeutic effects on PD due to its characteristics of homology of medicine and food, economy, minor adverse reactions and multi-target action. Therefore, this paper systematically reviews the role of BNDF/TrkB pathway in the pathogenesis of PD and the mechanism of TCM formulas, extracts and monomers in the treatment of PD by regulating the BNDF/TrkB pathway according to retrieving the latest research reports at home and abroad, so as to provide a reference for the clinical application of related TCM and the development of new drugs for PD.
5.Correlation among picky eating levels in preschool children, parenting self-efficacy and parenting stress

Objective:
To explore the correlation among picky eating levels in preschool children, parental self-efficacy and parenting stress.
Methods:
A convenience sampling method was employed to conduct an electronic questionnaire survey among 459 children aged 3-6 years and their parents from five kindergartens in Urumqi in November 2023. The survey included a general information questionnaire, the Children s Eating Behavior Questionnaire (CEBQ), the Parenting Sense of Competence Scale (PSOC), and the Parenting Stress Index-Short Form (PSI-SF). The Mann-Whitney U-test was used for twogroup comparisons, and the Kruskal-Wallis H-test was applied for multi-group comparisons. Spearman correlation analysis was conducted to examine the relationships between children s picky eating levels and parenting selfefficacy as well as parenting stress.
Results:
The picky eating score of preschool children was 10.00 (4.00), and the parenting self-efficacy score was 58.00 (12.00), both indicating a moderate level. The parenting stress score was 75.00 (16.00), reflecting a moderately low level. Spearman correlation analysis showed that children s picky eating levels were negatively correlated with the total score of parenting self-efficacy ( r =-0.28) and positively correlated with the total score of parenting stress( r =0.25)( P <0.01).
Conclusions
Picky eating levels of preschool children are closely associated with parenting self-efficacy and parenting stress. Picky eating behaviors in children can be reduced by implementing various effective measures to enhance parenting self-efficacy and alleviate parenting stress.
6.Predictive value of preoperative ultrasound for restenosis of arteriovenous fistula after percutaneous transluminal angioplasty:a report of 225 cases

Objective To explore the value of preoperative ultrasound examination in the prediction of restenosis of arteriovenous fistula(AVF)after percutaneous transluminal angioplasty(PTA)in hemodialysis patients.Methods A case-control trial was conducted on 225 hemodialysis patients who undergoing PTA due to AVF in our hospital January 2023 to May 2024.After 3 months of follow-up,they were divided into a patency group(n=204)and a restenosis group(n=21)according to the occurrence of postoperative restenosis.The preoperative clinical data and ultrasound parameters were compared between the groups.Binary logistic regression analysis was used to identify the independent factors for AVF restenosis after PTA.Receiver operating characteristic(ROC)curve was drawn to evaluate the value of preoperative stenosis length in the prediction of the restenosis after PTA.Results There were significant differences in preoperative internal diameter at the site of stenosis,stenosis length,stenosis number,intimal thickness,and brachial artery flow between the 2 groups(P<0.05).Preoperative stenosis length(OR=1.856,95%CI:1.350~2.552,P<0.001)was an independent factor of AVF restenosis in hemodialysis patients after PTA.ROC curve analysis showed that the area under the curve of preoperative stenosis length in predicting restenosis after PTA was 0.868(95%CI:0.784~0.953,P<0.001),with a sensibility and specificity of 85.7%and 80.4%,respectively.Conclusion Preoperative stenosis length may be an independent factor for AVF restenosis after PTA in hemodialysis patients.

Metal-organic frameworks(MOFs)are a class of organic-inorganic hybrid materials formed by the self-assembly of metal ions or metal clusters with organic ligands through coordination,and possess high specific surface area,tunable pore size and diverse structures.In recent years,MOFs and their composites have shown great application potential in the field of drug detection,especially in selective recognition,enhancing detection sensitivity and on-site rapid detection.This paper summarized the structural characteristics,synthesis methods and detection principles of MOFs and their composites,and reviewed the latest research progresses in detection of various drugs such as opioids,amphetamines,cannabinoids,cathinones,cocaine,ketamine,fentanyls and psychotropic drugs.The advantages and challenges of MOFs materials in the pretreatment of complex biological samples,sensor construction and on-site rapid detection were discussed,and the prospects for future development were analyzed,with the aim of providing theoretical support and technical references for promoting the applications of MOFs in anti-drug practice.
8.Distribution law of TCM syndrome types of diabetes peripheral neuropathy and its correlation with neuroelectrophysiological indexes

Objective:To investigate the distribution law of TCM syndromes in DPN patients with different TCM syndromes and their correlation with neuroelectrophysiological indexes.Methods:The general data of 252 DPN patients in Nanchong Traditional Chinese Medicine Hospital from October 2021 to October 2023 were retrospectively analyzed, and they were divided into 139 cases of primary deficiency syndrome group and 113 cases of excess in superficiality group according to TCM syndrome type. The distribution of TCM syndromes in DPN patients was observed, nerve electrophysiological indicators were detected, the differences of nerve electrophysiological indicators among patients with different TCM syndromes were compared, and the correlation between TCM syndromes distribution and nerve electrophysiological indicators in DPN patients was analyzed.Results:Among 139 patients of DPN with primary deficiency syndrome, there were 56 cases of qi deficiency and blood stasis syndrome, 44 cases of yin deficiency and blood stasis syndrome, 39 cases of liver and kidney deficiency syndrome; among 113 cases of excess in superficiality syndrome group, 42 cases of phlegm-dampness blocking collaterals syndrome, 37 cases of blood stasis blocking collaterals syndrome, and 34 cases of damp-heat blocking collaterals syndrome. There was no significant difference in the general data of patients with different TCM syndrome types ( P>0.05). The MCV and SCV of the median nerve, common peroneal nerve, and ulnar nerve in patients with primary deficiency syndrome group were lower than those in the excess in superficiality syndrome ( P<0.001), and the MCV and SCV of the median nerve, common peroneal nerve, and ulnar nerve in patients with qi deficiency and blood stasis syndrome, yin deficiency and blood stasis syndrome, and liver and kidney deficiency syndrome sequentially decreased ( P<0.01); there was no statistical significance ( P>0.05) in MCV and SCV between the three types of patients in the excess in superficiality group. Point-biserial correlation analysis results showed that DPN patients with primary deficiency syndrome had significant correlation with MCV and SCV ( P<0.05), while DPN patients with excess in superficiality syndrome had no significant correlation with MCV and SCV ( P>0.05). Conclusions:Qi deficiency and blood stasis syndrome is the most common among DPN patients with primary deficiency syndrome. Compared with patients with excess in superficiality group syndrome, MCV and SCV of patients with primary deficiency syndrome are at a lower level, and the TCM syndrome types of DPN patients are closely related to nerve conduction velocity.

Abelmoschi Corolla, the dried corolla of Abelmoschus manihot, has anti-inflammatory, antioxidant, and anti-fibrosis activities. Its chemical constituents mainly include flavonoids, organic acids, steroids, and polysaccharides. This study reviewed the research progress in the chemical constituents and pharmacological activities of Abelmoschi Corolla in recent 20 years. According to the concept of quality marker(Q-marker), the Q-markers of Abelmoschi Corolla were predicted from plant phylogeny, chemical constituent specificity, traditional efficacy, chemical constituent measurability, and absorbed constituents. The primary Q-markers for Abelmoschi Corolla were anticipated to include quercetin-3'-O-β-D-glucopyranoside, gossypetin-8-O-β-D-glucuronide, isoquercetin, myricetin,quercetin, and hyperoside, with the aim of providing reference data for improving the quality evaluation system of Abelmoschi Corolla.
