1.Effects of adalimumab treatment on miR-141,miR-335-5p and bone metabolism in patients with rheumatoid arthritis

Objective To analyze the effects of adalimumab treatment on serum micro ribonucleic acid-141(miR-141),micro ribonucleic acid-335-5p(miR-335-5p),and bone metabolism indexes in patients with rheumatoid arthritis(RA),and to provide a basis for the rational development of clinical treatment plans.Methods 82 cases of patients with RA admitted to our hospital from April 2021 to March 2024 were selected as study subjects,and they were divided into the methotrexate group(41 cases)and the adalimumab group(41 cases)by randomized numerical table method.The methotrexate group was treated with methotrexate tablets orally,and the adalimumab group was treated with adalimumab injection subcutaneously on the basis of methotrexate group,and both groups continued the treatment for 3 months.The efficacy after 3 months of treatment,symptom improvement and adverse reactions during the treatment period were counted;enzyme-linked immunosorbent assay detected the levels of inflammatory response,serum ACPA,RF and bone metabolism indexes before and after 3 months of treatment;and fluorescence quantitative PCR assay detected serum miR-141,miR-335-5p before and after 3 months of treatment.Results The total effective rate after 3 months of treatment was higher in the adalimumab group(90.24%)than in the methotrexate group(73.17%)(P<0.05).Compared with before treatment,the duration of morning stiffness was shorter in both groups after 3 months of treatment,and it was shorter in the adalimumab group than in the methotrexate group(P<0.05);pressure pain,number of swollen joints,scores of visual analog score(VAS),28 joints disease activity(DAS28),levels of serum interleukin-22(IL-22),interleukin-17(IL-17),C-reactive protein(CRP),tumor necrosis factor α(TNF-α),miR-141,anti-cyclic citrullinated peptide antibody(ACPA),rheumatoid factor(RF)decreased in both groups,and decreased in the adalimumab group compared with the methotrexate group(P<0.05);levels of serum miR-335-5p,25 hydroxyvitamin D,osteocalcin(BGP),osteocalcin N-terminal intermediate molecular fragment(N-MID),and bone alkaline phosphatase(B-ALP)were elevated in both groups,and increased in the adalimumab group compared with the methotrexate group(P<0.05).The overall incidence of adverse reactions during treatment was lower in the adalimumab group(7.32%)than in the methotrexate group(24.39%)(P<0.05).Conclusion Adalimumab could regulate the levels of inflammatory response indicators,serum miR-141,miR-335-5p,ACPA,RF,and bone metabolism indicators in patients with RA,and promote the improvement of symptoms,which in turn contributes to the improvement of therapeutic efficacy and had a high safety profile.
2.Impact of changes in tear cytokines on the ocular surface of patients with advanced refractory glaucoma after surgery

Objective To investigate the postoperative changes of tear cytokines in patients with advanced refractory glaucoma and their impact on ocular surface-related indicators.Methods In total,120 patients(240 eyes)with advanced refractory glaucoma admitted from January 2023 to June 2024 were selected as the observation group,all of whom underwent glaucoma drainage device(GDD)implantation.Another 85 healthy individuals(170 eyes)who underwent ophthalmic examinations during the same period were selected as the control group.The changes in tear cytokine levels[interleukin-1α(IL-1α),interleukin-2(IL-2),tumor necrosis factor-α(TNF-α),transforming growth factor-β(TGF-β),interleukin-10(IL-10),vascular endothelial growth factor(VEGF)]and ocular surface-related indicators[tear film break-up time(BUT),Schirmer I test(SIt)values,and corneal fluorescein staining(CFS)scores]were observed and recorded at 1 d before surgery and at 1,7,30,and 60 d after surgery in the observation group,and the levels of the above indicators were compared with those in the control group.Pearson correlation analysis was used to investigate the correlation between changes in IL-1α,IL-2,TNF-α,TGF-β,IL-10,VEGF levels and SIt values,CFS scores,and BUT.Results There were significant differences in the levels of IL-1α,IL-2,TNF-α,TGF-β,IL-10,and VEGF in the observation group when compared at 1 d before surgery and at 1,7,and 30 d after surgery(P<0.05).There was no significant difference in the levels of IL-1α,IL-2,TNF-α,TGF-β,IL-10,and VEGF in the observation group when compared at 1 d before surgery and at 60 d after surgery(P>0.05),nor significant difference compared with the control group(P>0.05).The differences in BUT,SIt values,and CFS scores in the observation group at 1 d before surgery and at 1,30,and 60 d after surgery were statistically significant(P<0.05),and the levels of the above indicators in the observation group were comparable to those in the control group at 60 d after surgery(P>0.05).Pearson correlation analysis showed that IL-1α,IL-2,TNF-α,TGF-β,IL-10,and VEGF in the observation group were positively correlated with SIt value and CFS score,and negatively correlated with BUT(P<0.05).Conclusion In the early postoperative period(1-30 d),the levels of IL-1α,IL-2,TNF-α,TGF-β,IL-10,and VEGF in the tears of patients with advanced refractory glaucoma are significantly increased,and returned to preoperative levels at 60 d after surgery.The above cytokines are positively correlated with SIt values and CFS scores,and negatively correlated with BUT.

5.Machine-learning-based models assist the prediction of pulmonary embolism in autoimmune diseases: A retrospective, multicenter study

Background::Pulmonary embolism (PE) is a severe and acute cardiovascular syndrome with high mortality among patients with autoimmune inflammatory rheumatic diseases (AIIRDs). Accurate prediction and timely intervention play a pivotal role in enhancing survival rates. However, there is a notable scarcity of practical early prediction and risk assessment systems of PE in patients with AIIRD.Methods::In the training cohort, 60 AIIRD with PE cases and 180 age-, gender-, and disease-matched AIIRD non-PE cases were identified from 7254 AIIRD cases in Tongji Hospital from 2014 to 2022. Univariable logistic regression (LR) and least absolute shrinkage and selection operator (LASSO) were used to select the clinical features for further training with machine learning (ML) methods, including random forest (RF), support vector machines (SVM), neural network (NN), logistic regression (LR), gradient boosted decision tree (GBDT), classification and regression trees (CART), and C5.0 models. The performances of these models were subsequently validated using a multicenter validation cohort.Results::In the training cohort, 24 and 13 clinical features were selected by univariable LR and LASSO strategies, respectively. The five ML models (RF, SVM, NN, LR, and GBDT) showed promising performances, with an area under the receiver operating characteristic (ROC) curve (AUC) of 0.962-1.000 in the training cohort and 0.969-0.999 in the validation cohort. CART and C5.0 models achieved AUCs of 0.850 and 0.932, respectively, in the training cohort. Using D-dimer as a pre-screening index, the refined C5.0 model achieved an AUC exceeding 0.948 in the training cohort and an AUC above 0.925 in the validation cohort. These results markedly outperformed the use of D-dimer levels alone.Conclusion::ML-based models are proven to be precise for predicting the onset of PE in patients with AIIRD exhibiting clinical suspicion of PE.Trial Registration::Chictr.org.cn: ChiCTR2200059599.
6.The alterations and significance of amino acid metabolism in cancers

Recently, the understanding of metabolic reprogramming in cancer is expanding continuously.Besides glycolysis, researchers are now paying more attention to amino acid metabolism, especially glutamine metabolism.Glutamine is one of the most abundant non-essential amino acids in human body, but it is a “conditionally essential” amino acid in various types of cancer cells.Thus, targeting glutamine metabolism is becoming a promising strategy for the development of novel cancer therapeutic agents.In addition, another gradually clarified metabolic pathway is serine and glycine metabolism.In this review, we will focus on the alterations, the underlying mechanisms, and significance of glutamine as well as serine and glycine metabolism.We will also discuss cancer therapy opportunities through targeting amino acid metabolism.
7.Study on expression of long non-coding RNA in rheumatoid arthritis

Objective:To investigate the expression profile variation of long non-coding RNA( lncRNA) in the peripheral blood mononuclear cells of rheumatoid arthritis ( RA ) and healthy controls, and explore the role of lncRNA in the pathogenesis of RA.Methods:A total of 12 RA patients and 11 age-matched healthy controls from University Hospital of Hubei University for Nationalities were recruited.Using lncRNA microarray technology to detect differently expressed lncRNAs in 3 cases of RA PBMCs and 3 cases of healthy controls.GO and Pathway analysis was performed.The coding-non-coding gene co-expression networks of lncRNA and mRNA was constructed based on the correlation analysis,and then searched lncRNA in pathogenesis of RA through the cis-analysis and trans-analysis.Results:A total of 1 615 deregulated lncRNAs and 878 deregulated mRNAs were detected in RA patients.GO analysis of different expressed mRNA may involve in metal ion binding,protein kinase binding,nucleotide binding,regulation of transcription,et al.Pathway analysis of different expressed mRNA may involve in TNF signaling pathway,B cell receptor signaling pathway,pancreatic cancer,system lupus erythematosus endometrial cancer,et al.REL,SMAD3 and ETS1 may play an important role in the pathogenesis and development of RA through cis-analysis and trans-analysis.As the NONHSAG027875,FR378506 and NONHSAT031501 also had the similar function,and they may be related to the pathogenesis and development of RA.Conclusion:Differentially expressed lncRNAs may exert a partial role in RA,and may provide potential targets for future treatment of RA.
