1.Value of spectral CT multi-parameter imaging in preoperative prediction of lymphovascular invasion in invasive lung adenocarcinoma

Objective:To investigate the value of quantitative parameters of dual-layer detector spectral CT (DLCT) in preoperative prediction of lymphovascular invasion (LVI) in invasive lung adenocarcinoma.Methods:This study employed a cross-sectional design. The data of 91 patients with primary lung adenocarcinoma confirmed by postoperative pathology who underwent DLCT enhanced scanning in the First Affiliated Hospital of Zhengzhou University from March 2022 to July 2024 were retrospectively collected. The patients were divided into LVI positive group and LVI negative group according to the postoperative pathology. Gender, age, smoking status, and lesion location were recorded,the clinical T-stage and N-stage were evaluated. Spectral base data images were generated based on the spectral reconstruction algorithm. Conventional CT features such as the size and density of the lesions were analyzed. The DLCT quantitative parameters such as 40 keV single-energy CT value (CT 40 keV), iodine density (ID), standardized iodine density (NID), and effective atomic number (Z eff) were measured. The differences of parameters between the two groups were compared using t-test, Mann-Whitney U test, or χ2 test. Multivariate logistic regression analysis was used to identify independent factors for predicting invasive lung adenocarcinoma LVI. The predictive performance of individual parameters and their combinations for LVI in invasive lung adenocarcinoma was evaluated using receiver operating characteristic curve analysis and area under the curve (AUC) measurements. Results:There were significant differences in age, T stage, N stage, maximum diameter, focal density, and arteriovenous stage NID and Z eff between the LVI positive group and the LVI negative group ( P<0.05). Multiple logistic regression analysis showed that age ( OR=0.890, 95% CI 0.821-0.966), N stage( OR=12.721, 95% CI 2.676-60.473) and venous stage Z eff( OR=0.012,95% CI 0-0.668) were independent factors for predicting invasive lung adenocarcinoma LVI ( P<0.05). The combination of age, N stage, and venous-phase Z eff values demonstrated the highest predictive efficacy for LVI in invasive lung adenocarcinoma, with an AUC of 0.916. Conclusions:The quantitative parameters of DLCT are helpful for preoperative prediction of LVI status of invasive lung adenocarcinoma. The Z eff in the venous stage is an independent predictor, the combination of multiple parameters can further improve the diagnostic efficiency.
2.Correlation between short chain fatty acids in saliva and salivary microbiota in patients with laryngopharyngeal reflux disease

Objective:This study aimed to compare short-chain fatty acid (SCFA) levels in saliva between patients with laryngopharyngeal reflux disease (LPRD) and healthy controls, and to explore the relationship between these SCFAs and the salivary microbiota.Methods:A retrospective case-control study was conducted, enrolling 36 patients with laryngopharyngeal reflux disease (LPRD) who visited the Department of Otorhinolaryngology Head and Neck Surgery, the Eighth Medical Center, Chinese PLA General Hospital between February and November 2023. All patients were diagnosed via pharyngeal pH monitoring. The LPRD group included 30 males and 6 females, aged 20-53 years (30.61±7.83 years). In addition, 39 healthy volunteers were recruited as the control group, comprising 25 males and 14 females, aged 18–58 years (28.64±7.97 years). Unstimulated mixed saliva samples were collected from all participants. Concentrations of eight SCFAs (acetic acid, propionic acid, isobutyric acid, butyric acid, valeric acid, isovaleric acid, hexanoic acid, and heptanoic acid) in saliva were quantified using gas chromatography-mass spectrometry (GC-MS). Salivary DNA was extracted, followed by amplification and sequencing of the 16S rRNA gene to analyze the microbiota composition at the genus level. The SCFA concentrations and the differences in bacterial species between the LPRD and control groups were compared, and the correlation between SCFA concentrations and the relative abundance of different bacterial genera in the salivary microbiota was analyzed. All statistical analyses were performed using R version 3.6.1 and SPSS version 26.0, while, microbiome analyses were conducted using R language.Results:Salivary hexanoic acid concentration was significantly higher in the LPRD group than in the control group [(29.50±19.61) ng/ml vs. (10.15±3.65) ng/ml; t=-2.72, P<0.05]. Significant differences in the relative abundance of 17 bacterial genera were observed between the two groups ( P<0.05), including Prevotella, Butyrivibrio, Streptococcus, and Actinomyces. Correlation analysis revealed that hexanoic acid concentration was significantly positively correlated with the abundance of Butyrivibrio (γ=0.73, P<0.05) and Streptococcus (γ=0.78, P<0.05), while showing a significant negative correlation with Actinomyces (γ=-0.73, P<0.05). Conclusion:Elevated salivary hexanoic acid levels may be associated with the development of LPRD. Dysbiosis of the salivary microbiota might contribute to LPRD pathogenesis by altering the concentrations of SCFA, particularly hexanoic acid.
3.Analysis of clinical applicability and implementation of expert consensus on the implementation and removal of protective restraints in psychiatry

Objective To understand the clinical applicability and implementation of expert consensus on the implementation and removal of protective restraints in psychiatry,and to provide references for promoting the standardized practice of psychiatric protective restraints and updating the consensus.Methods By the convenience sampling method,a questionnaire survey was conducted among nurses from 480 hospitals in 30 provinces from June 15 to July 15,2024.The survey was conducted using the instrument for evaluating clinical applicability of guide-lines(version 2.0)and a self-compiled questionnaire on the clinical implementation of the restraint consensus.Results A total of 7,844 valid questionnaires were collected,with a valid questionnaire recovery rate of 93.78%.The results of clinical applicability scoring showed that the consensus had the lowest availability score(64.72%)and the highest acceptability score(76.74%).The results showed that nurses' receiving training and the level of their hospitals were the main influencing factors for scores in various dimensions(P<0.05).4,774 participants(87.42%)believed that the application of consensus could enhance the standardization of nurses' restraint operations.The safety rate of the restraint consensus was 79.51%,and the economic ratio was 76.87%.Among the evaluators,1,739(22.17%)believed that there were implementation obstacles in the consensus.Conclusion The clinical applicability of the consensus is relatively good,and the application of the consensus helps to improve the standardization of clinical operations.In the future,efforts should be made to strengthen the promotion and training of the consensus,develop hierarchical promotion strategies according to the characteristics of medical institutions,and improve the quality of evidence for the consensus,so as to further enhance the clinical application effect of the consensus.
4.Proportions and clinical significance of myeloid-derived suppressor cell subsets in the peripheral blood of ovarian cancer pa-tients

Objective To investigate the proportions of myeloid-derived suppressor cells(MDSCs)and their subsets,including poly-morphonuclear MDSCs(PMN-MDSCs),early-stage MDSCs(e-MDSCs),monocytic MDSCs(M-MDSCs),and lectin-like oxidized low-density lipoprotein receptor-1(LOX-1)positive PMN-MDSCs,in the peripheral blood of ovarian cancer(OC)patients and ana-lyze their correlations with clinicopathological parameters of the patients.Methods The proportions of MDSCs and their subsets in the peripheral blood of 38 OC patients(OC group)and 46 healthy individuals(healthy control group)were detected by flow cytometry.The levels of serum IL-10 and TGF-β were detected using ELISA.The OC group was further divided into LOX-1 high and low expres-sion subgroups based on the median proportion of LOX-1+PMN-MDSCs in MDSCs.Results The proportions of MDSCs,PMN-MDSCs,and LOX-1+PMN-MDSCs in the peripheral blood mononuclear cells(PBMCs)of the OC group were significantly higher than those in the healthy control group(U=492,P<0.001;t=8.741,P<0.000 1;U=223,P<0.000 1).The proportions of M-MDSCs and e-MDSCs in the OC group were significantly lower than those in the healthy control group(t=4.366,P<0.000 1;t=6.927,P<0.000 1).The proportion of LOX-1+PMN-MDSCs in the lymph node metastasis group of OC patients was significantly higher than that in the non-metastasis group(t=2.249,P<0.05).The levels of serum IL-10 and TGF-β in the OC group were significantly higher than those in the healthy control group(P<0.05).In addition,the level of serum TGF-β in the LOX-1 high expression group was significantly higher than that in the LOX-1 low expression group(t=2.302,P<0.05).Conclusion The proportion of LOX-1+PMN-MDSCs in the peripheral blood of OC patients is significantly increased and closely related to lymph node metastasis.
5.Efficacy and mechanism of Fuke Yangrong capsule combined with letrozole in treating anovulatory infertility based on network pharmacology and clinical observation

Objective To explore the clinical efficacy and mechanism of FuKe Yangrong capsule(FKYRC)in treating anovulatory infertility(AI)using network pharmacology and clinical observation meth-ods.Methods A total of 110 AI patients who visited the hospital from January 2023 to July 2024 were select-ed as the research subjects.They were divided into three groups according to the treatment method:traditional Chinese medicine treatment group(n=30,treated only with FKYRC),western medicine treatment group(n=40,treated only with letrozole),and combination treatment group(n=40,treated only with FKYRC+letrozole).After 4 cycles of treatment,the total effective rate,maximum follicle diameter,pre-ovulation endo-metrial thickness,ovulation rate and pregnancy rate,and embryo survival rate of each group were compared af-ter treatment.Using the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP)and GeneCards database,the core active ingredients,target proteins,and AI related targets of FKYRC were obtained,and common targets were screened.Then,a protein-protein interaction(PPI)network was constructed,and gene ontology(GO)function and Kyoto Gene and Genome Database(KEGG)pathway enrichment analysis were performed using a bioinformatics platform for visualization.Finally,three-dimen-sional visualization analysis was performed.Results The total clinical efficacy of the traditional Chinese medi-cine group,western medicine group,and combination therapy group were 90.00%,92.50%,and 97.50%,re-spectively.The ovulation rates were 60.00%,70.00%,and 87.50%,respectively.The pregnancy rates were 33.33%,37.5%,and 60.00%,respectively.The survival rates of embryos were 60.00%,73.30%,and 95.80%,respectively,and the differences between the groups were statistically significant(P<0.05).The maximum follicle diameter and pre-ovulation endometrial thickness in the combination therapy group were higher than those in the traditional Chinese medicine group and the western medicine group,and the difference was statistically significant(P<0.05).Network pharmacology discovered 239 active ingredients and 3 977 target genes in FKYRC.After screening,200 active ingredients and 299 target genes were identified.A total of 478 disease target genes,38 potential interaction targets,and 19 core targets were obtained.Molecular func-tions,cellular components,and biological processes mainly involved steroid protein binding,estrogen response elements,estrogen receptor activity,nuclear chromatin,and other aspects.KEGG pathway enrichment analysis showed that FKYRC anti AI core targets were mainly enriched in estrogen signaling pathway,P53 signaling pathway,advanced glycation end product receptor(AGE-RAGE)of diabetes complications and other signaling pathways.The key targets of FKYRC against AI were two estrogen receptors(ESR1 and ESR2),steroid re-ceptor(AR),and peroxisome proliferator activated receptor-γ(PPARG).Conclusion The combined treat-ment of FKYRC and letrozole can improve AI patients' clinical symptoms,increase ovulation rate,pregnancy rate,and embryo survival rate.The active ingredients in FKYRC can comprehensively regulate the core targets of AI,and may promote follicular development and maturation in infertile patients with ovulation disorders through signaling pathways such as estrogen,P53,and AGE-RAGE,thereby increasing pregnancy rate.
6.Clinical prognosis of patients with CKD complicated with coronary heart disease based on thyroxine and thyroid-binding globulin

Objective To investigate the value of thyroid hormone in predicting the prognosis of patients with chronic kidney disease(CKD)complicated with coronary heart disease.Methods The clinical data of 254 patients with coronary heart disease who underwent coronary angiography in the hospital directly under Hubei Province from January to December 2018 were analyzed.The serum levels of thyroid stimulating hormone(TSH),thyroxine(T4),free thyroxine(fT4),triiodothyronine(T3),free triiodothyronine(fT3)and thyroid-binding globulin(TBG)were measured by automatic chemiluminescence immunoassay.The estimated glo-merular filtration rate(eGFR)of each patient was calculated by the kidney disease formula.After the baseline survey,patients were followed until the end of the study or the end of the observation period.Multivariate Lo-gistic regression was used to analyze the influencing factors of CKD progression.Receiver operating character-istic(ROC)curve was used to analyze the value of serum T4,TBG levels and their combination in predicting the progression of CKD.Results Among the 254 patients enrolled,the incidence of renal function decline was 15.75%,and they were included in the progression group(n=40),and the rest were included in the non-pro-gression group(n=214).Compared with the non-progression group,the progression group had significantly lower levels of hemoglobin,T4,and TBG(P<0.05)and significantly higher levels of triglyceride,low-density lipoprotein,and proteinuria(P<0.05).Multivariate Logistic regression analysis showed that both serum T4 and TBG were risk factors for renal function decline in patients with CKD complicated with coronary heart disease(P<0.05).Compared with patients with eGFR≥90 mL/(min·1.73 m2),patients with eGFR<30,30-<60,60-<90 mL/(min·1.73 m2)had lower serum TBG levels(P<0.05).Patients with eGFR<30 mL/(min·1.73 m2)and 30-<60 mL/(min·1.73 m2)had lower serum T4 levels(P<0.05).Serum T4 and TBG levels were positively correlated with eGFR(r=0.178,0.300,P<0.01).The area under the curve of serum T4 combined with TBG for predicting CKD progression was 0.849(95%CI 0.800-0.899).Conclu-sion Low serum T4 and TBG levels are closely related to the progression of CKD in patients with CKD com-plicated with coronary heart disease.
7.Retrospective epidemiological analysis of fungal infection of a hospital from 2018 to 2024

Objective To analyze the main epidemiological characteristics of fungal infection in this hospital in the past 7 years,and to provide reference for clinical treatment and prevention and control strategies of fun-gal infection.Methods The fungal data and clinical data of related patients isolated from clinical samples in Peking Union Medical College Hospital from early January 2018 to the end of May 2024 were selected,and the main epidemiological characteristics of fungal infection in this hospital were identified and described through multi-angle statistical analysis.Results A total of 4 479 patients with filamentous fungal infection were en-rolled.The proportion of male patients[57.5%(2 576/4 479)]was higher than that of female patients[42.5%(1 903/4 143)],mainly distributed in internal medicine,Intensive Care Unit(ICU)and emergency de-partment,among which internal medicine accounted for the highest proportion[50.0%(2 241/4 479)].About 90.0%of the specimens were from the lower respiratory tract,in addition to specimens from skin and soft tis-sue,tissue,ear and blood culture.In terms of seasonal distribution,there are more patients in winter.The fun-gi were mainly composed of Aspergillus,Mucor,Cerdosporium,Fusarium and Penicillium,among which As-pergillus was the most abundant,accounting for 74.6%of the total.Aspergillus fumigatus was the most a-bundant Aspergillus,accounting for 42.5%of the total Aspergillus(1 418/3 340).Among the related infec-tions caused by mold,Aspergillus was the most common in the lower respiratory tract,accounting for 76.8%.Among them,Aspergillus fumigatus accounted for the highest proportion(33.6%).98.6%of the molds infected the ear were Aspergillus,of which Aspergillus niger and Aspergillus terreus were the most common.Skin infections are mainly caused by Sporothrix schenckii,Trichophyton rubrum,Microsporum ca-nis.The results of in vitro drug sensitivity test showed that the four common Aspergillus isolated in this hos-pital were sensitive to voriconazole,and amphotericin B had better antifungal activity against Mucorales in vitro.Conclusion Based on the main epidemiological characteristics of fungal infections in this hospital,it is recommended that special attention be paid to the admission of patients in the respiratory department during the peak infection period in autumn and winter.In the treatment of fungal infections in different regions and on different body parts,attention should be paid to the differences in the distribution of bacterial species.

Turning to critical illness is a common stage of various diseases and injuries before death. Patients usually have complex health conditions, while the treatment process involves a wide range of content, along with high requirements for doctor′s professionalism and multi-specialty teamwork, as well as a great demand for time-sensitive treatments. However, this is not matched with critical care professionals and the current state of medical care in China. Telemedicine, which shortens the distance of medical professionals and the gap of disease diagnosis and treatments in various regions through electronic information, can effectively solve the current problem. Therefore, there is an urgent need to develop a standardized, high-quality visualization telemedicine round system .Therefore, experts have been organized to search domestic and foreign literature on telemedicine round for critically ill patients and to form this consensus based on clinical experiences so as to further improve the level of critical care treatments in regions.
9.Qualitative study on the experience of fall alertness in elderly patients with rheumatoid arthritis

Objective To understand the experience of fall alertness in elderly patients with rheumatoid arthritis and to provide references for the development of targeted nursing intervention strategies.Methods 13 elderly patients with rheumatoid arthritis,who were admitted to the Department of Rheumatology and Immunology of a tertiary hospital in Guizhou Province from February to April 2024,were selected through purposive sampling.Phenomenological qualitative research methods were used,with semi-structured in-depth interviews.Data were analyzed using Colaizzi's seven-step method,and themes were extracted.Results 3 main themes and 9 sub-themes were identified:the process of fall risk perception(reshaping fall risk cognition,establishing emotional coping mechanism,enhancing fall alertness effect),behavioral responses to fall alertness(creating a safe living environment,regularizing daily routine plan),and needs for fall prevention support(need for fall prevention knowledge,home rehabilitation care needs,intelligent device usage needs,psychosocial support needs).Conclusion Clinical medical staff should pay attention to the transformation process of fall risk perception in elderly rheumatoid arthritis patients,accurately identify patients'fall support needs in practice,and guide patients to adopt appropriate fall alert response behaviors through innovative fall demand support interaction paths and the construction of social support network systems,in order to improve patients'fall alertness and reduce their fall risk.
10.Efficacy and safety of fractional CO 2 laser combined with topical application of radix scutellariae ointment in treatment of hyperplastic scar

Objective:To evaluate the clinical efficacy and safety of fractional CO 2 laser combined with topical application of radix scutellariae ointment in the treatment of hyperplastic scar. Methods:This randomized controlled trial prospectively enrolled 90 patients with hyperplastic scar treated with fractional CO 2 laser at the Affiliated Hospital of Nanjing University of Chinese Medicine (Jiangsu Province Hospital of Chinese Medicine) from January 2020 to December 2021. Participants were randomly divided into a trial group and a control group using a random number table. The trial group ( n=45, 22 males, 23 females, aged 18-54 years) received topical radix scutellariae ointment post-laser treatment, while the control group ( n=45, 23 males, 22 females, aged 19-55 years) received recombinant bovine basic fibroblast growth factor gel. Both groups underwent treatment every 3 months, with scar assessments conducted at 1-month post-treatment. Outcomes included Vancouver scar scale (VSS) scores, scar cosmesis assessment and rating (SCAR) scores, adverse reaction rates, and patients' satisfaction. Results:After the first, second, and third treatments, the trial group showed significantly lower VSS and SCAR scores compared to the control group (all P<0.05). Patients' satisfaction in the trial group was 91.1% (41/45), significantly higher than 77.8% (35/45) in the control group ( P=0.036). The adverse reaction rate in the trial group was 15.6% (7/45), and 28.9% (13/45) in the control group, there was no statistical difference ( P=0.813). Conclusion:Fractional CO 2 laser combined with topical radix scutellariae ointment demonstrate superior clinical efficacy, higher patient satisfaction, and lower adverse reaction rates in the treatment of hyperplastic scar.
