1.Quality evaluation of Heat-clearing and symptom-relieving formula based on multi-component quantification and screening of marker components

OBJECTIVE To systematically evaluate the quality of the Heat-clearing and symptom-relieving formula and screen potential marker components that influence the quality of the formula. METHODS The contents of 11 components (calycosin-7- O - β -D-glucoside, ononin, hyperoside, isoquercitrin, baicalin, baicalein, cryptotanshinone, tanshinone Ⅱ A , tanshinone Ⅰ, senkyunolide A, ferulic acid) in the Heat-clearing and symptom-relieving formula were determined by high-performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS). Using the contents of the aforementioned components as variables, cluster analysis (CA), principal component analysis (PCA), and orthogonal partial least squares-discriminant analysis (OPLS-DA) were conducted using OriginPro 2024 software and SIMCA 14.1 software; marker components affecting the quality of the Heat-clearing and symptom-relieving formula were then screened based on the criteria of variable importance in the projection (VIP) value>1 and P <0.05. The comprehensive evaluation of 20 batches of samples was carried out using the entropy weight-technique for order preference by similarity to ideal solution(TOPSIS) and grey correlation analysis (GCA) methods. RESULTS The contents of the above 11 components were 7.993-72.866, 4.542-31.228, 727.666-1 901.884, 496.846-1 293.279, 1 995.501-6 779.150, 54.500-241.280, 150.302-304.339, 79.698-189.206, 257.118-682.418, 5.498-21.687, 7.524-26.935 μg/g. CA, PCA and OPLS-DA results showed that 20 batches of samples were grouped into 2 categories. Q1, Q3, Q4, Q7-Q9, Q12, Q15, Q16 were grouped into one category, and the rest were grouped into another category; VIP values of ferulic acid, tanshinone Ⅱ A , baicalin, cryptotanshinone, calycosin-7- O - β -D-glucoside and ononin were all greater than 1 ( P <0.05). Both the entropy weight-TOPSIS and GCA methods showed that the samples ranked in the top 11 according to the euclidean distance and relative correlation degree were Q2, Q5, Q6, Q10, Q11, Q13, Q14, Q17-Q20. CONCLUSIONS The established HPLC-MS/MS method is rapid, accurate and highly sens itive. Combined with chemical pattern recognition analysis, entropy weight-TOPSIS and GCA methods, this method can be used to evaluate the quality of the Heat-clearing and symptom-relieving formula. Ferulic acid, tanshinone Ⅱ A , baicalin, cryptotanshinone, calycosin-7- O - β -D-glucoside and ononin may be the marker components that affect the quality of this formula. The overall quality of 11 batches of the Heat-clearing and symptom-relieving formula, including Q17, is relatively superior.

Oral squamous cell carcinoma (OSCC) is a common head and neck malignancy. Approximately 50% to 60% of patients with OSCC are diagnosed at a locally advanced stage (clinical staging III-IVa). Even with comprehensive and sequential treatment primarily based on surgery, the 5-year overall survival rate remains below 50%, and patients often suffer from postoperative functional impairments such as difficulties with speaking and swallowing. Programmed death receptor-1 (PD-1) inhibitors are increasingly used in the neoadjuvant treatment of locally advanced OSCC and have shown encouraging efficacy. However, clinical practice still faces key challenges, including the definition of indications, optimization of combination regimens, and standards for efficacy evaluation. Based on the latest research advances worldwide and the clinical experience of the expert group, this expert consensus systematically evaluates the application of PD-1 inhibitors in the neoadjuvant treatment of locally advanced OSCC, covering combination strategies, treatment cycles and surgical timing, efficacy assessment, use of biomarkers, management of special populations and immune related adverse events, principles for immunotherapy rechallenge, and function preservation strategies. After multiple rounds of panel discussion and through anonymous voting using the Delphi method, the following consensus statements have been formulated: 1) Neoadjuvant therapy with PD-1 inhibitors can be used preoperatively in patients with locally advanced OSCC. The preferred regimen is a PD-1 inhibitor combined with platinum based chemotherapy, administered for 2-3 cycles. 2) During the efficacy evaluation of neoadjuvant therapy, radiographic assessment should follow the dual criteria of Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1 and immune RECIST (iRECIST). After surgery, systematic pathological evaluation of both the primary lesion and regional lymph nodes is required. For combination chemotherapy regimens, PD-L1 expression and combined positive score need not be used as mandatory inclusion or exclusion criteria. 3) For special populations such as the elderly (≥ 70 years), individuals with stable HIV viral load, and carriers of chronic HBV/HCV, PD-1 inhibitors may be used cautiously under the guidance of a multidisciplinary team (MDT), with close monitoring for adverse events. 4) For patients with a poor response to neoadjuvant therapy, continuation of the original treatment regimen is not recommended; the subsequent treatment plan should be adjusted promptly after MDT assessment. Organ transplant recipients and patients with active autoimmune diseases are not recommended to receive neoadjuvant PD-1 inhibitor therapy due to the high risk of immune related activation. Rechallenge is generally not advised for patients who have experienced high risk immune related adverse events such as immune mediated myocarditis, neurotoxicity, or pneumonitis. 5) For patients with a good pathological response, individualized de escalation surgery and function preservation strategies can be explored. This consensus aims to promote the standardized, safe, and precise application of neoadjuvant PD-1 inhibitor strategies in the management of locally advanced OSCC patients.
3.The magnetic compression technique in the treatment of severe mid-ureteral stricture

Objective To explore the feasibility of magnetic compression technique (MCR) in the treatment of severe single mid-ureteral stricture in a rabbit model. Methods After 10 female New Zealand rabbits were selected, the distal end of the right ureter was ligated with silk thread. Two weeks after the operation, when the right ureter was dilated, the mid-ureter was ligated again with silk thread to create a severe single mid-ureteral stricture animal model. Longitudinal incisions were made at the upper and lower ends of the ligation site, and cylindrical NdFeB magnets with central holes (sub-magnets and main magnets) were placed to anastomose and recanalize the mid-ureteral stricture. The magnets were adjusted to the appropriate position to automatically align and attract each other. The longitudinal incision sites of the ureter were sutured with 6-0 Prolene thread, and the abdominal cavity was closed layer by layer. The operation time was recorded. X-ray films were taken every two days to confirm the position of the magnets and record the time of magnet detachment. Three weeks after the operation, the anastomotic specimens were obtained, and the formation of the anastomosis was evaluated with gross observation and histological staining. Results Two weeks after ligation, all experimental rabbits showed obvious dilation of the right ureter. The mid-ureteral ligation and placement of the male and female magnets were successfully completed in all 10 experimental rabbits, with an operation time of (24.00±5.15)minutes. The male and female magnets attracted each other well. X-ray examination showed that the magnets detached and entered the distal ureter (14.0±1.9) days after magnetic compression. The recanalization of the severe single mid-ureteral stricture was successfully achieved. The gross specimens and histological examination of the anastomosis showed that the anastomosis remained patent and the mucosal healing was good. All experimental rabbits survived well after the operation without serious complications. Conclusion MCT provides a new idea and novel method for the treatment of mid-ureteral stricture. This study confirmed the feasibility of this technique in treating severe single mid-ureteral stricture in a rabbit model. The operation is relatively simple and has potential for clinical application.

By analyzing the current application of multi-modal data in the diagnosis of gastric "inflammation-cancer" transformation, this study explored the feasibility and strategies for constructing a disease-syndrome integrated diagnosis and treatment system. Based on traditional Chinese medicine (TCM) phenomics, we proposed utilizing multi-modal data from literature research, cross-sectional studies, and cohort follow-ups, combined with artificial intelligence technology, to establish a multi-dimensional diagnostic and treatment index system. This approach aims to uncover the complex pathogenesis and transformation patterns of gastric "inflammation-cancer" progression. Additionally, by dynamically collecting TCM four-diagnostic information and modern medical diagnostic information through a long-term follow-up system, we developed three major modules including information extraction, multi-modal phenotypic modeling, and information output, to make it enable real-world clinical data-driven long-term follow-up and treatment of chronic atrophic gastritis. This system can provide technical support for clinical diagnosis, treatment evaluation, and research, while also offering insights and methods for intelligent TCM diagnosis.
5.Report of 4 cases of IgG4-related urinary diseases and literature review

[Objective] To explore the clinical features of IgG4-related urinary diseases so as to provide reference for the diagnosis and treatment of such diseases. [Methods] The clinical data of 4 cases of IgG4-related urinary system diseases diagnosed and treated in Xijing Hospital of Air Force Medical University during Aug.2019 and Dec.2023 were retrospectively collected.Here, we report on the diagnosis and treatment of these patients, analysing their symptoms, serology, imaging and pathology as well as their treatment and outcomes. [Results] The patients included 2 male and 2 female.The lesions were involved with the retroperitoneum and urinary system.Three patients had symptoms of lumbar pain.The imaging manifestations were complex, including retroperitoneal mass involving urinary system organs in 2 cases, tabdense shadow of the right kidney in 1 case, and simple cystic mass of kidney in 1 case.Serum IgG4 value was not detected before surgery.All patients underwent radical surgical treatment.Postoperative pathology showed fibrous tissue hyperplasia with a large number of plasma cells, lymphocytes, a few neutrophil infiltrates, and lymphoid follicles and obliterated vasculitis in some specimens.The number of IgG4+ plasma cells was more than 10 in all tissues under high power microscope.After surgery, 3 patients had symptoms improved, and serum IgG4 value was within the normal range; 1 patient (patem 3) had elevated IgG4 value during follow-up, received subsequent hormone therapy, and the serum IgG 4 level remained stable. [Conclusion] The symptoms of IgG4-related diseases involving the urinary system are non-specific, and the imaging findings are various, easily confused with other diseases.Early detection of serum IgG4 and biopsy pathology can help clinicians make correct diagnosis in the early stage.
6.The virtue ethics construction of the red doctor spirit

People’s health is an essential requirement of Chinese-style modernization. The red doctor spirit serves as the fundamental safeguard for achieving the goal of people’s health. What is the red doctor spirit? What is the relationship between the red doctor spirit and virtue ethics? How can virtues shape the red doctor spirit and seek theoretical support for this spirit from extensive and profound ethical resources? From the perspective of virtue ethics, the red doctor spirit refers to the virtue requirements that the Communist Party of China (CPC) puts forward for medical workers during the process of leading the Chinese people to create and develop healthcare undertakings with Chinese characteristics, including political firmness, respecting life and healing the wounded and rescuing the dying with superb medical skills, and serving the people. This spirit is passed down and developed in line with the times, characterized by political, people-centered, practical, and inheritable traits. The integration of virtue ethics elements with the red doctor spirit not only puts forward new virtue ethics requirements for this spirit in the new era but also provides corresponding pathways for the virtue ethics cultivation of the red doctor spirit.
7.The effects of exercise based on the sports and medical care integration model on the community-dwelling persons with type 2 diabetes mellitus

Objective:To explore the effect of exercise on glycemic control and physical indicators among community-dwelling persons with type 2 diabetes mellitus (T2DM).Methods:A total of 42 T2DM patients undergoing regular follow-up at a community health center were divided into a control group and an exercise group. To minimize confounding factors, 1∶1 propensity score matching was performed, resulting in 21 patients in each group. Both groups received standardized diabetes management, including medication, dietary guidance and diabetes health education. The exercise group additionally performed exercises which integrated sports and medical care models. The experiment lasted 8 weeks. Before and after the intervention, glycosylated hemoglobin A1c (HbA1c) levels, body weight, body mass index (BMI), fat mass, visceral fat area (VFA), fat mass index (FMI) and appendicular skeletal muscle mass index (ASMI) were recorded and compared between the two groups. Moreover, the actual exercise compliance of the exercise group was observed, along with any adverse events such as hypoglycemia or exercise-related injuries, as well as changes in antidiabetic medication dosage.Results:After the intervention, significant improvement was observed in both groups in all of the measurements, but with the average HbA1c, BMI, fat mass, VFA and FMI values of the exercise group then significantly better than the control group′s averages. The average compliance rate in the exercise group was 60%. No cases of hypoglycemia or exercise-related injuries were reported during the intervention, and there were no changes in the use of anti-diabetes medications.Conclusions:Exercise effectively improves glycemic control and physical indicators among community-dwelling T2DM patients. However, exercise compliance needs improving.
8.Academic Research of ZHANG Zhicong and the Traditional Chinese Medical Academy Education

[Objective]To summarize ZHANG Zhicong's academic experience in his books and medical activities taken place in Lvshantang,and to provide reference for the study of traditional Chinese medical academy education.[Methods]By studying ZHANG Zhicong's works,such as Lvshantang Leibian,Collected Commentaries on Suwen,Collected Commentaries on Lingshu,Collected Commentaries on Treatise on Exogenous Febrile Disease,and referring to relevant literature,this paper analyzes the paradigm of the traditional Chinese medical academy education,discusses mainly from four aspects of the establishment background of the traditional Chinese medical academy education,the specific model of Lvshantang and the academic experience of its master ZHANG Zhicong,the influence of Lvshantang education,and the traditional Chinese medical academy education inheritance.[Results]Lvshantang was born in the context of popularization of academies,the gathering of medical experts in Qiantang,and the joint teaching activities of LU Fu and LU Zhiyi.The teaching modes of Lvshantang,including self-study,teaching research and lecture system,explored the basic mode of academy education integrating teaching,research,diagnosis and treatment for traditional Chinese medicine.The curriculum of Lvshantang reflected ZHANG Zhicong's academic thoughts of respecting the classics by maintaining faithful to the ancient text,the old theory,six-meridian Qi-transformation,difficulties before easy ones and collective annotation.The academy education had a large scope of influence.The development of Lvshantang enriched the Qiantang medical school,promoted the prosperity of maintaining the old theory school,started the collective annotation of medical books and left a series of classical annotation monographs for the later generations.Under the influence of ZHANG Zhicong's lectures,GAO Shizong,ZHONG Xuelu,CHEN Xiuyuan continued the academy-style educational model,which had a profound impact on the establishment of modern Chinese medicine specialized schools.[Conclusion]The traditional Chinese medical academy education began to establish norm in Lvshantang,which was founded by ZHANG Zhicong in Qiantang in the Qing Dynasty.To summarize ZHANG Zhicong's academic views in Lvshantang is helpful to understand the changes in the content and mode of the traditional Chinese medical academy education,and the academy education helps the inheritance and development of the traditional Chinese medicine.
9.Epidemiological characteristics and influencing factors for dermatoses among military personnel in tunnel environments

OBJECTIVE This study aims to investigate the epidemiological characteristics and influencing factors of skin diseases among soldiers performing duties and working in tunnel environments,and to propose targeted pre-ventive and therapeutic measures.METHODS A cross-sectional survey was conducted on 537 soldiers from multi-ple sites within a military unit from Nov.2022 to Oct.2023.The survey collected data on general information,liv-ing habits,and details of current skin diseases,including types,symptoms,duration and treatment status.Logis-tic regression analysis was applied to identify risk factors for the skin diseases observed.RESULTS A total of 21 types of skin diseases were identified,with dermatophyte infections being the most prevalent(59.96%,322/537),followed by acne(26.82%,144/537).Further analysis revealed that the incidence of dermatophyte infec-tions was closely associated with the region where the soldiers were stationed[OR(95%CI)=1.694(1.062,2.693),P=0.032]and the frequency of sock washing[OR(95%CI)=1.734(1.023,2.988),P=0.043],but no significant correlation was found between the prevalence of dermatophyte infections and the frequency of washing feet[OR(95%CI)=1.520(0.836,2.824),P=0.175].CONCLUSIONS This study highlights the epidemiological char-acteristics and key risk factors for skin diseases among soldiers in tunnel environments.Targeted prevention strategies are proposed,providing valuable scientific evidence for the prevention and control of skin diseases in similar environments.
10.Epidemiological and clinical characteristics of respiratory adenovirus infection in hospitalized children of a maternal and child health hospital in Hubei Province in 2023

OBJECTIVE To analyze the subtyping,clinical and epidemiological characteristics of respiratory adeno-virus(ADV)infections in hospitalized children,and to provide a scientific basis for the prevention and treatment of respiratory ADV infections.METHODS Respiratory samples from 15,315 hospitalized children in Hubei Mater-nal and Child Health Care Hospital from Jan.to Dec.2023 were collected.Pathogen-targeted high-throughput se-quencing technology(tNGS)/multiplex respiratory virus detection kits were used to detect respiratory pathogens in the samples.A retrospective analysis was conducted on the clinical and laboratory characteristics of the patients,as well as the subtypes and epidemiological features of adenovirus(ADV).RESULTS Of the 15,315 samples,436 were confirmed to be ADV-positive,with an overall detection rate of 2.85%.Summer was the peak season for ADV respiratory infections,with the highest detection rate of 1.46%(224/15,315).The detection rates in spring,autumn and winter were 0.43%,0.41%and 0.54%,respectively.The detection rate was highest among children aged 2 to<6 years(3.24%,247/7,621).The ADV detection rates differed significantly among different age groups(P<0.05).The 436 samples contained 4 species(B,C,D and E)and 7 serotypes(ADV-1~7),and the ADV-B3 and ADV-B7 subtypes accounted for the majority of genotypes,accounting for 73.62%of the total.The main clinical symptoms of ADV-positive children were cough,fever,tonsillar enlargement with purulent se-cretions and abdominal pain.Among the 436 ADV-positive hospitalized children,296(67.89%)had single ADV infection,while 140(32.11%)had co-infection with other respiratory pathogens.Children in the coinfection group had higher proportions of fever,high fever,fever duration,cough,shortness of breath,mild to severe pneumonia and longer hospital stays compared to the single infection group(P<0.05).CONCLUSIONS The detection rate of ADV is associated with age and season.Common clinical manifestations are cough,fever,tonsillar enlargement with purulent exudates and abdominal pain.ADV-3 and ADV-7 are the most commonly detected types in children diagnosed with respiratory infections.
