1.Summary of the best evidence for the prevention and management of chyle leak after thyroid cancer surgery

Objective:To retrieve and obtain relevant evidence at home and abroad on the prevention and management of chylous leakage after thyroid cancer surgery, and provide reference for clinical practice.Methods:Based on the 6S pyramid model system, evidence related to the prevention and management of chylous leakage after thyroid cancer surgery in the databases of UpToDate, BMJ best practice, Guidelines International Network (GIN), National Guideline Clearinghouse (NGC), PubMed, EMbase, Wanfang Database, China National Knowledge Infrastructure (CNKI) and other databases, including guidelines, expert consensus, systematic reviews, evidence summary, clinical decisions, and position statements. The retrieval time was from database establishment to June 1st 2024.Results:A total of 16 articles were included, including 5 guidelines, 9 expert consensus, and 2 systematic reviews. The 30 best pieces of evidence were summarized from 6 aspects: preoperative evaluation and preparation, intraoperative prevention, diagnosis, diet and nutrition management, monitoring and treatment, and health education.Conclusions:This study summarized relevant evidence and provides reference for medical staff to formulate intervention measures and management plans for chyle leakage after thyroid cancer surgery.
2.Efficacy of nasolacrimal duct stent placement combined with endonasal endoscopic dacryocystorhinostomy in the treatment of chronic dacryocystitis and recurrence rate analysis

Objective:To investigate the efficacy of nasolacrimal duct stent placement combined with endonasal endoscopic dacryocystorhinostomy (EES-DCR) in the treatment of chronic dacryocystitis and analyze the recurrence rate of this condition.Methods:A retrospective analysis was performed on 312 patients with chronic dacryocystitis admitted to Department of Otolaryngology Head and Neck Surgery, Xi'an People's Hospital (Xi'an Fourth Hospital) from April 2021 to April 2023. All patients were divided into the non-implantation group (EES-DCR only) and the implantation group (nasolacrimal duct stent placement combined with EES-DCR) based on the surgical procedure used. Baseline data were excluded using propensity score matching method, and 156 patients were included in each group. Clinical efficacy, incidence rates of complications, and epiphora status (epiphora degree, tear film breakup time, Schirmer test strip wetting length) before surgery and at 1 month after surgery, as well as recurrence rate within 1 year of treatment were compared between the two groups.Results:The total efficacy in the implantation group was significantly higher than that in the non-implantation group [98.08% (153/156) vs. 93.59% (146/156), χ2 = 3.93, P = 0.047). The overall incidence rate in the implantation group was significantly lower than that in the non-implantation group [5.13% (8/156) vs. 12.18% (19/156), χ2 = 4.90, P = 0.027). At 1 month after surgery, both the degree of epiphora and the wetting length of Schirmer test strip significantly decreased in both groups, while the tear film breakup time significantly increased (both P < 0.05) compared with before surgery. The degree of epiphora and the wetting length of Schirmer test strip in the implantation group were (1.25 ± 0.26) points and (12.37 ± 2.24) mm, respectively, which were significantly lower than those in the non-implantation group [(1.44 ± 0.29) points, (13.32 ± 2.60) mm, t = 4.81, P < 0.001; t = 3.45, P < 0.001]. The tear film breakup time in the implantation group was (12.43 ± 2.31) seconds, which was significantly longer than that in the non-implantation group [(11.38 ± 2.15) seconds, t = 4.15, P < 0.001]. The total recurrence rate in the implantation group was 5.13% (8/156), which was significantly lower than that in the non-implantation group [11.54% (18/156), χ2 = 4.19, P = 0.041]. Conclusions:Nasolacrimal duct stent placement combined with EES-DCR effectively treats chronic dacryocystitis, reduces the degree of epiphora, and lowers the recurrence rate of the disease. This combined therapy has few complications and is safe and effective.

BACKGROUND:Although traditional therapies,including drugs and surgery,cannot repair the damaged myocardial tissue,the mortality rate of myocardial infarction remains high.Stem cells provide the possibility to solve this problem due to their self-renewal and multi-directional differentiation potential. OBJECTIVE:To analyze the research progress of stem cell therapy for myocardial infarction in recent ten years by bibliometric analysis. METHODS:The related articles on stem cells and myocardial infarction published in SCI-E and SSCI from January 1,2012 to December 1,2022 in the Web of Science database were searched.EXCEL,CiteSpace and VOSviewer software were used to make statistical and visualization analyses of the data such as the number of publications,authors,institutions,journals,countries and keywords. RESULTS AND CONCLUSION:A total of 3 210 core articles were published,and the total number increased year by year.hausenloy,derek j.is the author with the largest number of publications,China is the country with the largest number of publications,and the Fourth Military Medical University is the institution with the largest number of publications.The research hotspots in this field are changing from cell experiments and animal experiments to clinical trials.In the past ten years,research in this field has been highly popular and still has great development prospects.It is necessary to promote international and inter-agency exchange and learning,and further explore the role of stem cells in the treatment of myocardial infarction.
6.Mediating effect of chronic disease self-efficacy between fatigue and social support in breast cancer patients undergoing chemotherapy

Objective:To explore the mediating effect of chronic disease self-efficacy between fatigue and social support in breast cancer patients undergoing chemotherapy.Methods:This study was a cross-sectional study. From July 2020 to April 2021, convenience sampling was used to select 376 patients with breast cancer undergoing chemotherapy in the Shandong Provincial Hospital Affiliated to Shandong First Medical University as the research object. The General Information Questionnaire, Fatigue Scale-14, Chronic Disease Self-Efficacy Scale and Social Support Rating Scale were used to investigate the patients. Structural equation modeling was used to test the mediating effect among variables. A total of 376 questionnaires were distributed, and 372 valid questionnaires were recovered, with a valid recovery rate of 98.94%.Results:Among 372 breast cancer patients undergoing chemotherapy, the total scores of the Fatigue Scale-14, Chronic Disease Self-Efficacy Scale and Social Support Rating Scale were (10.61±1.81) , (5.00±2.53) and (43.42±12.59) , respectively. Chronic disease self-efficacy was positively correlated with social support in breast cancer patients undergoing chemotherapy ( r=0.493-0.648, P<0.01) , and social support was negatively correlated with fatigue ( r=-0.703--0.234, P<0.01) , and self-efficacy was negatively correlated with fatigue ( r=-0.729--0.220, P<0.01) . Chronic disease self-efficacy played a partial mediating role between fatigue and social support in breast cancer patients undergoing chemotherapy (β=-0.335, P<0.01) , and the mediating effect accounted for 47.59% of the total effect. Conclusions:Social support can indirectly affect fatigue through chronic disease self-efficacy in breast cancer patients undergoing chemotherapy. Nurses should focus on improving the chronic disease self-efficacy and social support of breast cancer patients undergoing chemotherapy.
7.Rapid Health Technology Assessment of Albumin-bound Paclitaxel in the Treatment of Breast Cancer

OBJECTIVE:To evaluate th e effectiveness ,safety and economy of albu min-bound paclitaxel (nab-PTX)in the treatment of breast cancer by using rapid health technology assessment (HTA),and to provide evidence-based reference for drug selection. METHODS :Retrieved from PubMed ,the Cochrane Library ,CNKI,Wangfang database and other databases ,systematic evaluation/Meta-analysis,HTA and pharmacoeconomic studies about nab-PTX in the treatment of breast cancer were included ;the conclusions were classified and analyzed by using descriptive analysis. RESULTS :A total of 5 systematic reviews/Meta-analysis , 8 pharmacoeconomic studies were included in this study. Compared with conventional taxanes ,nab-PTX increased pathological complete response (pCR)rate [OR =1.39,95%CI(1.16,1.67),P<0.001] and event-free survival (EFS)[HR=0.69,95%CI(0.57, 0.85),P<0.001] in neoadjuvant chemotherapy (NAC)-treated breast cancer patients. However ,there were no significant differences in overall survival (OS),progression-free survival (PFS),objective response rate (ORR)and disease control rate (DCR)in metastatic breast cancer (MBC)patients between 2 groups. In the terms of safety ,nab-PTX increased the incidence of grade 3-4 sensory neuropathy [OR =1.89,95%CI(1.36,2.61),P<0.001] in MBC patients ,and increased the incidence of neutropenia [OR = 1.52,95%CI(1.23,1.88,P<0.001],sensory neuropathy [OR = 2.17,95%CI(1.38,3.40),P<0.001],rash [OR =1.46,95%CI mei1213@163.com (1.18,1.80),P<0.001] and fatigue [OR =1.28,95%CI(1.04, 1.56), P=0.02] in NAC -treated breast cancer patients.Pharmacoeconomic studies showed that nab-PTX could improve the quality adjusted lif e years of MBC patients compared with traditional taxanes ,and it was a economical option. CONCLUSIONS:Nab-PTX enhances pCR in NAC-treated breast cancer patients ,but has no significant advantage in the effectiveness of MBC patients ,and increases the occurrence of ADR. Nab-PTX may have a cost-utility advantage over conventional taxanes for MBC.
8.Effects of different conversion time to open surgery on prognosis after laparoscopic radical resection for colorectal cancer

Objective To investigate the effects of early and delayed conversion to open surgery on the prognosis after laparoscopic radical resection for colorectal cancer.Methods The retrospective cohort study was conducted.The clinicopathological data of 119 patients who were converted to open surgery in laparoscopic radical resection for colorectal cancer in the ZiBo First People's Hospital from January 2008 to December 2014 were collected.There were 66 males and 53 females,aged from 20 to 84 years,with an average age of 55 years.Of the 119 patients,82 who were converted to open surgery within 60 minutes after the start of laparoscopic surgery and 37 who were converted to open surgery after 60 minutes since the start of laparoscopic surgery were allocated into early conversion group and delayed conversion group.Observation indicators:(1) surgical situations and postoperative recovery;(2) perioperative complications;(3) follow-up.Follow-up using telephone interview,mail and outpatient examination was performed to detect patients' survival once 3 months within 2 years and once a year after 2 years postoperatively up to September 2017.Measurement data with normal distribution were represented as Mean±SD,and comparison between groups was analyzed using the independent sample t test.Count data were represented as absolute number or percentage,and comparison between groups was analyzed using the chi-square test or Fisher exact propability.Comparison of ordinal data was analyzed using the rank sum test.The survival rate was calculated using the Kaplan-Meier method and Log-rank test was used for survival analysis.Results (1) Surgical situations and postoperative recovery:patients in the two group underwent laparoscopic radical resection of colorectal cancer successfully.The operation time,time to first defecation,duration of postoperative hospital stay were (202±44) minutes,(2.6 ± 1.1) days,(9 ± 5) days in the early conversion group and (230±45) minutes,(3.7±2.1) days,(12±6) days in the delayed conversion group,showing statistically significant differences between the two groups (t=-3.106,-3.450,-2.865,P<0.05).The time to first flatus was (1.8± 0.6) days and (2.0 ± 0.8) days in the early conversion group and delayed conversion group,respectively,with no statistically significant difference between the two groups (t =-1.245,P> 0.05).(2) Perioperative complications:8.5% (7/82) of patients in the early conversion group had perioperative complications,including 2 of early postoperative inflammatory bowel obstruction,2 of postoperative incisional infection,1 of chylous fistula,1 of urinary retention,3 of anastomotic fistula;the same patient can merge multiple complications.Patients with perioperative complications were cured after symptomatic and supportive treatment.Meanwhile,35.1% (13/37) of patients in the delayed conversion group had perioperative complications,including 8 of early postoperative inflammatory bowel obstruction,3 of postoperative incisional infection,6 of urinary retention,3 of anastomotic fistula;the same patient can merge multiple complications.Patients with perioperative complications were cured after symptomatic and supportive treatment.There was a statistically significant difference in the incidence of perioperative complications between the two groups (x2=12.902,P<0.05),a statistically significant difference in the early postoperative inflammatory bowel obstruction and urinary retention between the two groups (P<0.05),and no statistically significant difference in the postoperative incisional infection,chylous fistula,anastomotic fistula between the two groups (P>0.05).(3) Follow-up:112 out of 119 patients were followed up for 5.2-101.9 months,with a median time of 32.1 months.The 5-year survival rate was 70.5% and 63.6% in the 79 patients of early conversion group and 33 of delayed conversion group,showing no statistically significant difference between the two groups (x2 =0.038,P>0.05).Conclusions Delayed conversion after 60 minutes since the start of laparoscopic surgery will lead to the slower recovery of intestinal function and prolonged hospitalization time,and increase the rates of early postoperative inflammatory intestinal obstruction and postoperative urinary retention.Therefore,a comprehensive analysis of the feasibility of laparoscopic surgery in the initial exploration is recommended and a fast decision on early conversion is necessary.
9.Evaluation of the sensitivity and specificity of corneal visualization scheimpflug technology biomechanical index in the diagnosis of keratoconus

Objective To study the sensitivity and specificity of parameter Corvis biomechanical index (CBI)— a new biomechanical index of Corvis ST in the diagnosis of keratoconus and evaluate the role of CBI in the diagnosis of keratoconus and the change of biomechanic.Methods A diagnostic trial study was adopted,and 66 eyes from 49 keratoconic patients (keratoconus group) and 91 right eyes from 91 myopic patients (control group) from April in 2018 to August in 2018 in Henan Eye Hospital were enrolled.Pentacam and Corvis ST examinations were performed by the same operator after the basic eye examinations.Using the evaluation of diagnostic test,consistency test,and receiver operating characteristic (ROC) curve analysis,gain the outcome of the sensitivity,specificity,consistency,Youden index,and area under the ROC curve of the parameter CBI.This study protocol was approved by Ethic Committee of Henan Eye Hospital and followed the Declaration of Helsinki.Written informed consent was obtained from each patient prior to any medical examination.Results Sixty eyes were diagnosed as keratoconus by the parameter CBI of Corvis ST.Evaluation of diagnosis test:the sensitivity was 97.0%,and the specificity was 97.8%;consistency check:Kappa =0.948,P<0.05.ROC curve analysis:the sensitivity was 98.5%,the specificity was 96.8%,Youden index was 96.3%,P<0.000 1 and AUC was 0.996.Conclusions CBI can separate healthy eyes from keratoconic eyes with highly sensitivity and specificity,which was highly consistency with results of Rabinowitz keratoconus diagnostic criteria.CBI could be used as a new biomechanical indicator for the diagnosis of keratoconus.
10.Surgical treatment in heamophilic patients with musculoskeletal disorder

Objective To study the surgical treatment strategy for heamophilic arthropathy and musculoskeletal appara?tus. Methods A total of 120 heamophilic patients underwent 166 primary operations from January 1996 to June 2015 in Pe?king Union Medical College Hospital, with the average age of 29.7±12 years (from 6 to 61 years). Hemophilic type A accounted for 109 patients and hemophilic type B accounted for 11. Eighty?seven patients presented with bleeds within the joints, with 63 cases for knee involved, 29 cases for hip, 11 cases for ankle. Fifteen patients presented with intramuscular bleeds, 21 patients with heamophilic pseudotumors. Strategy of clotting factor replacement therapy was designed according to the different level of opera?tion procedure. The clinical manifestation, operative strategy, clinical outcome and complications were retrospectively recorded. Results 41 cases (34.2%) of patients underwent surgeries for more than one location. Totally, 166 procedures were performed for 120 patients. There were 103 procedures of joint arthroplasty (62.0%, 103/166), 21 procedures of pseudotumor resection (12.7%, 21/166), 15 procedures of tendon lengthening, 12 procedures of ankle arthrodesis, 13 procedures for knee flexion contracture. There were 30 cases of postoperative complications, with the rate of 18.1% (30/166). The coagulation related complication was 4.8%in this group (8/166). The surgical procedure related complication was 7.2%(12/166). All the preoperative symptoms were relieved during the follow?up. Conclusion Surgical treatment was effective for the heamophilic arthropathy and lesion of mus?culoskeletal apparatus, under the reasonable clotting factor replacement therapy. The incidence of perioperative complication in heamophilic patients is higher than that in non?hemophilic patients. Closely inspection was inevitable for perioperative treat?ment in hemophilic patients.
