1.Resting brain function study of executive function changes in patients with type 2 diabetes mellitus

Objective To explore the changes in neural activity in patients with type 2 diabetes mellitus(T2DM)and their corre-lation with executive function,and to analyze the neural mechanisms underlying the decline in executive function in T2DM patients.Methods Thirty-one T2DM patients(T2DM group)and thirty-two healthy controls(HC)(HC group)matched for body mass index(BMI)underwent resting-state functional magnetic resonance imaging(rs-fMRI)scans and N-back task tests were included.Differ-ences in the amplitude of low-frequency fluctuation(ALFF),regional homogeneity(ReHo),and seed-based functional connectivity(FC)between the two groups were compared,and partial correlation analyses were performed between the difference results and N-back task performance.Results The T2DM group showed prolonged reaction time(RT)in the 1-back and 2-back tasks.T2DM patients exhibited increased ALFF in the bilateral caudate nucleus,left medial superior frontal gyrus,and right postcentral gyrus,as well as elevated ReHo in the right putamen.FC analysis revealed significant alterations in FC between the caudate nucleus,putamen,and multiple brain regions in T2DM patients,with some of these FC changes significantly correlated with RT and accuracy(ACC)in the N-back task.Conclusion The decline in executive function in T2DM patients may be associated with abnormal neural activity in brain regions such as the striatum,salience network,and frontoparietal control network.FC further decreases under increased cognitive load.These findings provide evidence for the study of the neural mechanisms of executive function impairment in T2DM patients.
2.Prenatal ultrasonic findings of fetal malformation of cortical development

Objective To observe prenatal ultrasonic findings of fetal malformation of cortical development(MCD).Methods Totally 37 singleton fetuses with MCD diagnosed by fetal cranial MRI and/or genetic examination were retrospectively enrolled,and prenatal ultrasonic manifestations were observed.Results Among 37 fetuses,29(29/37,78.38%)were found with abnormal Sylvian fissure,26(26/37,70.27%)with widened/asymmetrical lateral ventricles,22(22/37,59.46%)with abnormal surface sulci,14(14/37,37.84%)with abnormal cavity of septum pellucidum(CSP)/corpus callosum(CC),11(11/37,29.73%)with midline deviation/curvature,11(11/37,29.73%)with widened subarachnoid space,10(10/37,27.03%)with head circumference<2 standard deviation below mean value,9(9/37,24.32%)with abnormalities in the cerebellar hemisphere/vermis,7(7/37,18.92%)with abnormal echoes in the brain parenchyma,7(7/37,18.92%)with midline cysts and 2(2/37,5.41%)with enlarged ganglion eminence.Conclusion Fetal MCD mainly presented as abnormal Sylvian fissure,widened/asymmetrical lateral ventricles and abnormal surface sulci,which might accompanied by abnormal CSP/CC,midline deviation/curvature,widened subarachnoid space and abnormal head circumference in prenatal ultrasound.

Cognitive frailty, a novel concept in geriatric medicine, has become a focal point of recent research. Dual-task training, which innovatively integrates physical activity with cognitive rehabilitation, has shown promise in simultaneously enhancing motor function and cognitive performance in older adults with cognitive frailty. This review summarizes the concept of dual-task training, its application modalities, and its effectiveness in this population. It also proposes strategies for further implementation, aiming to provide a reference for future research and practical applications of dual-task training in cognitively frail older adults.
4.Analysis of changes in peripheral blood NK cell activity,CD4+T cell subset counts and circulating tumor cell levels in patients with pulmonary infection after esophageal cancer surgery

Objective:To analyze the etiological characteristics in patients pulmonary infection after esophageal cancer sur-gery and clinical significance of detecting peripheral blood activity of natural killer cell(NK),CD4+T cell subsets,and circulating tu-mor cell(CTC).Methods:A total of 108 patients with pulmonary infection undergoing radical resection of esophageal cancer in Chi-feng Songshan District Mongolian Medicine Hospital,from July 2020 to July 2022 were enrolled as infection group,while another 108 patients without pulmonary infection during the same period were enrolled as non-infection group.The levels of NK cells,CD4+T cell subsets and CTC in the peripheral blood of the two groups were detected 3 days after the operation.The disease severity was evaluated by clinical pulmonary infection score(CPIS).The correlation between CD4+T cell subsets,NK activity,CTC levels and CPIS were an-alyzed by Pearson and Spearman correlation analysis.The application value of each index for predicting pulmonary infection after esophageal cancer surgery was evaluated by receiver operating characteristic(ROC)curves.Multivariate Logistic regression analysis was used to analyze the risk factors of pulmonary infection after esophageal cancer surgery.Results:The levels of peripheral blood NK,CD4+T cells and CD4+T/CD8+T in infection group were significantly lower than those in non-infection group,while positive rate of serum CTC was significantly higher than that in non-infection group(P<0.05).The levels of peripheral blood NK,CD4+T cells and CD4+T/CD8+T in CTC positive group were significantly lower than those in CTC negative group(P<0.05).CPIS in patients with lung in-fection after esophageal cancer surgery was(4.18±1.29)points.Correlation analysis showed that levels of peripheral blood NK,CD4+T cells were significantly negatively correlated with CPIS,while CTC were significantly positively correlated with CPIS(P<0.05).AUC values of NK,CD4+T cell subsets and CTC for predicting lung infection were 0.694,0.757 and 0.574,respectively.The predictive value of NK and CD4+T cells subsets was better than that of CTC(P<0.05).Multivariate Logistic regression analysis showed that low levels of NK and CD4+T cell were independent risk factors of postoperative lung infection(P<0.05).Conclusion:Patients with pulmo-nary infection after esophageal cancer surgery have abnormal immune function,and low levels of NK cells and CD4+T cells are inde-pendent risk factors for postoperative pulmonary infection.Focusing on monitoring the levels of NK cells and CD4+T cells in the periph-eral blood of patients is helpful to grasp the development of the patient's condition and predict the occurrence of postoperative pulmo-nary infection.
5.Protective effects of Sophora subprostrate polysaccharide against oxidative dam-age in IPEC-J2 cells

The objective of this study was to evaluate the protective effect and possible related mechanisms of Sophora subprostrate polysaccharide(SSP)on intestinal epithelial cell injury in-duced by Tert-Butyl hydroperoxide(TBHP).The optimal dose of TBHP and the safe concentra-tion range of SSP were determined using the MTT method.In this study,IPEC-J2 cells were divid-ed into five groups:the control group,the model group,the SSPL group,the SSPM group and the SSPH group,and the cell morphology,cell survival rate and LDH release rate were observed and measured.The content of intracellular reactive ROS was observed and determined by DCFH-DA staining.The content of MDA in the supernatant and the antioxidant index of cells were determined by the reagent kit.Transcriptome technology was employed to analyze the potential mechanisms by which SSP mitigates oxidative damage in IPEC-J2 cells.The results showed that treatment with 625 μmol/L TBHP for 2 h significantly reduced the activity of IPEC-J2 cells,markedly increased LDH release(P<0.05),inhibited CAT superoxide SOD and glutathione GPX activities(P<0.05),and significantly elevated MDA and ROS levels(P<0.05).Compared to the model group,after SSP treatment,intracellular ROS levels were significantly reduced(P<0.05),while CAT,SOD,and GPX activities were significantly increased(P<0.05),and MDA content and LDH re-lease were significantly decreased(P<0.05)in a dose-dependent manner.Transcriptome analysis revealed that TBHP treatment significantly altered the transcriptional profiles of IPEC-J2 cells,while SSP treatment could restore the transcriptional profiles of the damaged cells to a certain ex-tent.Gene ontology(GO)and Kyoto encyclopedia of genes and genomes(KEGG)indicated that the differentially expressed genes between the CC and TBHP groups were significantly enriched in oxidative phosphorylation,ribosome,and other pathways.Meanwhile,the differentially expressed genes between the SSP and TBHP groups were mainly enriched in oxidative phosphorylation,ap-optosis,glyoxylate and dicarboxylate metabolism,and other pathways.These results suggest that TBHP may disrupt normal oxidative respiration in IPEC-J2 cells by affecting oxidative phospho-rylation and interfering with metabolism pathways involving glycine,serine,and threonine,leading to oxidative damage in intestinal epithelial cells.Conversely,SSP treatment may potentially restore oxidative phosphorylation processes,alleviate lysosomal damage,reduce cell apoptosis,and miti-gate oxidative damage in intestinal epithelial cells through modulation of oxidative phosphoryla-tion,apoptosis,and lysosomal pathways.This discovery provides a theoretical basis for the clinical application of SSP in alleviating oxidative damage in the porcine intestinal tract.
6.Correlation between the expression level of miRNA-4319 and miRNA-4262 in early gastric carcinoma tissue and recurrence after endoscopic submucosal dissection

Objective To investigate the correlation between the expression of microRNA-4319(miRNA-4319)and microRNA-4262(miRNA-4262)in early gastric carcinoma tissue and postoperative recurrence after endoscopic submucosal dissection(ESD).Methods From May 2018 to March 2020,396 patients with early gastric carcinoma who underwent ESD were regarded as the disease group,meantime,98 patients with normal gastric mucosa gastritis confirmed by pathological examination were as the control group.Quantitative reverse transcription polymerase chain reaction(qRT-PCR)method was applied to detect the relative expression levels of miRNA-4319 and miRNA-4262 in tissues;follow up on postoperative recurrence of early gastric carcinoma patients,and the expression levels and clinical pathological characteristics of miRNA-4319 and miRNA-4262 were compared between recurrent and non recurrent patients.Multivariate Logistic regression analysis was applied to analyze the influencing factors of recurrence in early gastric carcinoma patients after ESD;receiver operating characteristic curve(ROC curve)was applied to analyze the predictive value of miRNA-4319 and miRNA-4262 in early gastric carcinoma tissue for recurrence after ESD.Results The expression levels of miRNA-4319 and miRNA-4262 in the disease group were lower than those in the control group,the differences were statistically significant(P<0.05);The expression levels of miRNA-4319 and miRNA-4262 in the recurrence group were decreased,and the proportion of submucosal invasion was increased,the differences were statistically significant(P<0.05).The depth of invasion to the submucosal level(OR=3.158,95%CI:1.395~7.151)was a risk factor for recurrence after ESD in patients with early gastric carcinoma(P<0.05),and miRNA-4262 ≥ 0.76(OR=0.561,95%CI:0.370~0.852)and miRNA-4319 ≥ 0.69(OR=0.482,95%CI:0.255~0.911)were protective factors for recurrence after ESD in patients with early gastric carcinoma(P<0.05);The area under the curve(AUC)of miRNA-4319,miRNA-4262 and their combination for predicting recurrence after ESD in patients with early gastric carcinoma was 0.889(95%CI:0.854~0.918),0.914(95%CI:0.882~0.940)and 0.964(95%CI:0.940~0.980),respectively,which was superior to the individual predictions of miRNA-4319 and miRNA-4262 respectively(P<0.05).Conclusion MiRNA-4319 and miRNA-4262 are low expressed in tissues of early gastric carcinoma patients,they are closely related to postoperative recurrence after ESD.

9.Meta-synthesis of qualitative studies on the experience of body quality management in overweight or obese pregnant women during pregnancy

Objective:To systematically evaluate the experience of body quality management in overweight or obese pregnant women during pregnancy, so as to provide reference for developing targeted intervention plans.Methods:Searched China National Knowledge Infrastructure, Wanfang, VIP, Chinese Biomedical Database, PubMed, Web of Science, CINAHL, Embase, Medline databases to screen the qualitative studies on the experience of body quality management of overweight or obese pregnant women during pregnancy, and integrated them with a pooled integration method. The search deadline was from database creation to December 11, 2024.Results:A total of 11 literatures were included and 32 results were extracted, which were summarized into 9 categories and integrated into 3 results including complex emotional experience, face a variety of challenges and difficulties; strategies to improve body quality management ability.Conclusions:Medical staff should pay attention to the psychological experience of body quality management in overweight or obese pregnant women during pregnancy, actively mobilize their positive emotions, constantly strengthen their social support system, and break through the dilemma of body quality management.

Creutzfeldt-Jakob disease(CJD)is a rapidly progressive and fatal neurodegenerative disorder caused by prions,with certain infectivity and iatrogenic transmission risks.With the rapid progress and application of new dia-gnostic biomarkers and detection methods,as well as the construction and improvement of surveillance and reporting systems,the detection of CJD in patients domestically and internationally has shown an increasing trend year by year.Due to its long incubation period and heterogeneity of early symptoms,early identification and diagnosis of the disease is difficult,increasing the risk of transmission within medical institutions.Currently,there is a lack of con-sensus on the infection prevention and control of CJD.In order to timely identify and diagnose CJD as well as effec-tively block its transmission in medical institutions,this consensus summarizes 15 clinical concerns and formulates 24 specific recommendations based on the latest domestic and international research findings and clinical evidence,as well as combines with clinical practice,aiming to standardize healthcare-associated infection prevention and control measures for CJD and reduce its transmission risk in medical institutions.
