1.miR-27a-3p promotes the proliferation of human hypertrophic scar fibroblasts by regulating mitogen-activated protein kinase signaling pathway

BACKGROUND:Multiple studies have confirmed that mitogen-activated protein kinase(MAPK)signaling pathway is involved in cell proliferation,and microRNA(miR)is involved in the occurrence and development of hypertrophic scars.Therefore,the role of miR-27a-3p and MAPK signaling pathways in pathological scar formation has been further explored. OBJECTIVE:To explore the effect of miR-27a-3p on the proliferation of human hypertrophic scar fibroblasts through the MAPK signaling pathway. METHODS:The primary fibroblasts were isolated and collected from the skin samples.The primary fibroblasts were observed by inverted microscope and verified by immunofluorescence.The relative expression level of miR-27a-3p in tissues was detected by qRT-PCR.The target genes of hsa-miR-27a-3p were predicted using the database,and then the predicted target genes were enriched by gene ontology function analysis and biological pathway enrichment analysis of the Kyoto Encyclopedia of Genes and Genomes.There were seven groups:blank control,negative control,miR-27a-3p mimic,miR-27a-3p inhibitor,miR-27a-3p mimic+p38 MAPK inhibitor,miR-27a-3p mimic+extracellular regulated protein kinase inhibitor,miR-27a-3p mimic+c-Jun N-terminal kinase inhibitor.Western blot was used to detect the levels of extracellular regulated protein kinase,c-Jun N-terminal kinase inhibitor.and p38 kinase and their phosphorylation levels.Cell counting kit-8 and EdU were used to detect cell proliferation. RESULTS AND CONCLUSION:Compared with normal skin fibroblasts,hypertrophic scar fibroblasts had stronger proliferative activity(P<0.05)and faster proliferation level(P<0.001).Compared with normal skin,miR-27a-3p was highly expressed in hypertrophic scars(P<0.001).Compared with the negative control group,overexpression of miR-27a-3p could promote cell proliferation activity(P<0.001)and proliferation levels(P<0.001).Compared with the negative control group,knockdown of miR-27a-3p could inhibit the proliferation activity(P<0.05)and proliferation levels(P<0.001).Compared with the negative control group,overexpression of miR-27a-3p promoted the phosphorylated levels of extracellular regulated protein kinase,c-Jun N-terminal kinase,and p38 mitogen-activated protein kinase(P<0.05).Compared with the negative control group,knockdown of miR-27a-3p inhibited the phosphorylated levels of extracellular regulated protein kinase,c-Jun N-terminal kinase,and p38 MAPK(P<0.05).Compared with the miR-27a-3p mimic group,specific inhibitors of extracellular regulated protein kinase,c-Jun N-terminal kinase,and p38 MAPK reversed the effects of miR-27a-3p on the proliferative activity(P<0.01)and proliferation level(P<0.001)of fibroblasts.To conclude,these results suggest that miR-27a-3p promotes the proliferation of human hypertrophic scar fibroblasts by activating the MAPK signaling pathway.
2.Application of signature pedagogy in teaching "basic nursing" course

Objective:To explore the impact of "basic nursing" course with signature pedagogy on undergraduate nursing students' critical thinking, professional identity, and professionalism.Methods:A signature pedagogy method for the "basic nursing" course was established using narrative as a theoretical lesson and scenario-simulation as a laboratory lesson. Ninety sophomore nursing students in the 2020 undergraduate nursing program were selected as the participants of a controlled pre-post study. The students were evaluated using the Critical Thinking Inventory, the Professional Identity Questionnaire, and the Professionalism Inventory before and after the course. Semi-structured interviews were conducted at the end of the course to gain an in-depth understanding of their learning experience. Data analysis was conducted using SPSS 26.0 and Nvivo 11.0 Colaizzi's seven-step method was used for analysis of qualitative research.Results:A total of 87 nursing students participated in the course and 83 were included in the quantitative analysis. The total scores of critical thinking skills (286.83±17.44), professional identity (58.95±6.43), and professionalism (67.08±11.21) were significantly higher after the course than before the course ( P<0.001). Qualitative study identified the four themes of promoting effective learning, developing critical thinking and empathy, building professional attitudes, and expecting curriculum optimization. Conclusions:The "basic nursing" course based on signature pedagogy for undergraduate nursing students helps to enhance their critical thinking, foster a sense of professional identity, and develop professionalism.
3.A case of MRKH syndrome combined with adenomyosis without endometrium and literature review

Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is characterized by congenital vaginal hypoplasia and primordial uterus caused by Müllerian anomalies. Case reports concerning adenomyosis associated with MRKH syndrome are limited, and the formation and intervention of adenomyosis without endometrium are unclear. Here, we present the case of a patient with MRKH syndrome complicated with adenomyosis without endometrium, a hypoechoic solid mass next to the right ovary. According to the intraoperative observation and histopathological examination findings, the mass were diagnosed as adenomyosis of the primordial uterus. Moreover, our report provides insights about its mechanism of occurrence and interventions in MRKH syndrome combined with adenomyosis without endometrium.
4.Three-dimensional ultrasound combined with microvascular flow imaging for prenatal diagnosis of fetal intracranial anomalies

Objective To observe the value of three-dimensional ultrasound combined with microvascular flow imaging for prenatal diagnosis of fetal intracranial anomalies.Methods Totally 118 fetuses with intracranial anomalies diagnosed through cranial MRI/induced labor specimen dissection who underwent prenatal ultrasound examination were retrospectively enrolled.Two-dimensional,three-dimensional ultrasound and microvascular flow imaging manifestations of fetal intracranial anomalies were observed,and the accuracy of three-dimensional ultrasound combined with microvascular flow imaging for prenatal diagnosis of fetal intracranial anomalies was analyzed.Results The accuracy of three-dimensional ultrasound combined with microvascular flow imaging for prenatal diagnosis of fetal intracranial anomalies was 93.22%(110/118),of isolated and non-isolated intracranial anomalies was 97.47%(77/79)and 84.62%(33/39),respectively.Six fetuses were missed diagnosis of malformations of cortical development(1 fetus of tuberous sclerosis,4 of abnormal morphology of the sulci gyrus and 1 of schizencephaly)and 1 fetus of intracranial softening lesion,while posterior fossa arachnoid cyst in 1 fetus was misdiagnosed as mega cisterna magna.Conclusion The accuracy of three-dimensional ultrasound combined with microvascular flow imaging for prenatal diagnosis of fetal intracranial anomalies was relatively high.
5.Prenatal ultrasound for diagnosing fetal intracranial arachnoid cyst and predicting outcomes

Objective To observe the value of prenatal ultrasound for diagnosing fetal intracranial arachnoid cyst(IAC)and predicting outcomes.Methods A total of 240 fetuses with IAC confirmed by fetal brain MR examination or clinical follow-up were retrospectively enrolled,and the prenatal ultrasonic manifestations were observed.The outcomes of pregnancy were recorded,and the neurological development and treatment after birth were regularly followed up,then the fetuses were classified into good outcome or poor outcome groups.Univariate and binary logistic regression were used to analyze clinical data and prenatal ultrasonic findings,and the impact factors of outcomes of fetal IAC were screened.Results Among 240 fetuses,IAC located in the supratentorial area in 192(192/240,80.00%)and in the infratentorial area in 48(48/240,20.00%)fetuses.The size of IAC was 0.3-6.2 cm,with an average of(1.4±0.9)cm.Single lesion was found in 230 fetuses(230/240,95.83%),while multiple lesions were detected in 10 fetuses(10/240,4.17%).IAC presented as thin-walled cystic echoes with clear intracranial boundaries and good intracapsular sound transmission in all 240 fetuses,including 150(150/240,62.50%)with simple IAC and 90(90/240,37.50%)complicated with intracranial or other system malformations.Totally 189 fetuses were delivered successfully,while 51 underwent induction of labor.The newborns were followed up to 6-84 months.Among 240 fetuses with IAC,171(171/240,71.25%)had good outcome and 69(69/240,28.75%)had poor outcome.The size,number of lesion and whether combined with malformations or not on prenatal ultrasound and gender were all independent impact factors of outcomes of fetal IAC(OR=1.558,6.323,0.911,2.662,all P<0.05).Conclusion Prenatal ultrasound could accurately diagnose and predict the outcomes of fetal IAC.
6.Prenatal ultrasonic findings of fetal malformation of cortical development

Objective To observe prenatal ultrasonic findings of fetal malformation of cortical development(MCD).Methods Totally 37 singleton fetuses with MCD diagnosed by fetal cranial MRI and/or genetic examination were retrospectively enrolled,and prenatal ultrasonic manifestations were observed.Results Among 37 fetuses,29(29/37,78.38%)were found with abnormal Sylvian fissure,26(26/37,70.27%)with widened/asymmetrical lateral ventricles,22(22/37,59.46%)with abnormal surface sulci,14(14/37,37.84%)with abnormal cavity of septum pellucidum(CSP)/corpus callosum(CC),11(11/37,29.73%)with midline deviation/curvature,11(11/37,29.73%)with widened subarachnoid space,10(10/37,27.03%)with head circumference<2 standard deviation below mean value,9(9/37,24.32%)with abnormalities in the cerebellar hemisphere/vermis,7(7/37,18.92%)with abnormal echoes in the brain parenchyma,7(7/37,18.92%)with midline cysts and 2(2/37,5.41%)with enlarged ganglion eminence.Conclusion Fetal MCD mainly presented as abnormal Sylvian fissure,widened/asymmetrical lateral ventricles and abnormal surface sulci,which might accompanied by abnormal CSP/CC,midline deviation/curvature,widened subarachnoid space and abnormal head circumference in prenatal ultrasound.
7.New advances in fetal neurological ultrasound examination

8.Evaluation for real-world application of domestic and imported medical devices in Liaoning province

Objective:To investigate the current status of real time application of domestic and imported medical device in the real-world in Liaoning province,and provide data support for the promotion and application of domestic medical device.Methods:Data of the usage of medical device were collected from 77 medical institutions at all levels in 9 urban-rural demonstration regions with representativeness in Liaoning Province from November 2017 to June 2020.Descriptive analysis was used to characterize the distribution of medical device,which data were collected in urban-rural medical institutions at all levels in Liaoning Province.The total amount of receiving test of medical device,and the tested amount of each device in each month in urban-rural medical institutions at all levels were calculated.The median value of monthly average number of persons who received test of various kind of domestic and imported medical device in each month in hospitals with third level and secondary level were also calculated.Results:The total number of device that were collected from 77 medical institutions in 9 demonstration regions in Liaoning province was 325,among of which there were 153 domestic device(accounting for 47.1%)and 172 imported devices(accounting for 52.9%).A total of 6,261,000 person-time valid records were collected from 77 medical institutions,which average test number was 5,562 person-time in each device and each month.The total number of person-times of domestic device in diagnosis and treatment was 1,380,000 person-times(accounting for 22.0%),and that of imported devices was 4,881,000 person-times(accounting for 78.0%).The average test number of each device of domestic device in each month was 2,334 person-times(accounting for 42.0%),and that of imported device in each month was 3,228 person-times(accounting for 58.0%).In urban regions,imported device were dominant,while domestic medical device were dominant in rural regions in Liaoning province.The overall usage frequency of imported medical device was higher in hospitals with third level or secondary level,while the usage frequency of domestic medical device was higher in grassroots medical institutions.Conclusion:There are differences in the real time usage between domestic and imported medical device among 77 medical institutions at all levels in 9 demonstration regions in Liaoning province.Urban regions mainly tend to use imported device,while rural regions more tent to use domestic device.
9.Development of the Discharge Readiness Scale for Patients with Chronic Skin Wounds and its reliability and validity testing

Objective:To develop the Discharge Readiness Scale for Patients with Chronic Skin Wounds and evaluate its reliability and validity.Methods:The Discharge Readiness Scale for Patients with Chronic Skin Wounds was developed through literature review, semi-structured interviews, Delphi expert consultation, and a pre-survey. Convenience sampling was employed to select 320 patients with chronic skin wounds hospitalized at Chongming Hospital, Shanghai University of Medicine and Health Sciences and Qingdao Municipal Hospital from June 2023 to December 2024 for formal investigation, conducting scale reliability and validity analysis. The critical ratio method and correlation coefficient method were used for project analysis. Content validity of the scale was assessed using item-level content validity index and average scale-level content validity index, and construct validity was evaluated through exploratory factor analysis and confirmatory factor analysis. Reliability analysis employed Cronbach's α coefficient.Results:A total of 320 questionnaires were distributed, and 304 valid questionnaires were collected, with a valid response rate of 95.0%. The average scale-level content validity index was 0.91, and the item-level content validity index ranged from 0.80 to 1.00. Exploratory factor analysis revealed that the cumulative variance contribution rate of the five common factors was 67.483%. The overall Cronbach's α coefficient for the scale was 0.831. The final constructed scale comprised five dimensions of trajectory stage cognition, wound self-care skills, symptom management, psychosocial adaptation, and health information acquisition and resource utilization, with a total of 26 items.Conclusions:The Discharge Readiness Scale for Patients with Chronic Skin Wounds demonstrates good reliability and validity, providing clinical healthcare professionals with a standardized assessment tool.
10.Thoracoabdominal Injuries of Six-Year-Old Child Occupants in Reclined Seating Postures Based on 50% MPDB Scenario

Objective To investigate the risk of thoracoabdominal injuries in six-year-old child occupants in a reclined seating posture during frontal collisions,and provide a reference for developing child restraint systems(CRS).Methods Three validated biomechanical models of six-year-old child occupants in different seating postures with detailed anatomical structures were used.The acceleration curve from a sport utility vehicle crash test was applied to analyze the effects of seating posture on thoracic motion trajectory,chest acceleration,thoracoabdominal compression,viscous criterion(VC)of the chest and abdomen,internal organ strain,and spinal stress.Results Thoracic motion trajectories varied in the Z-direction under three seating postures.As the upper torso angle increased,thoracoabdominal kinematic injury parameters showed an upward trend.The thoracic and abdominal VC under 120° and 135° posture increased by 67%and 113%,10.7%and 25%compared with that under 105° standard sitting posture.The risk of thoracic internal organ injury was inversely related to the seating angle,while the risk of abdominal internal organ injury was positively related to the seating angle.The primary spinal injury mechanism was compression-flexion.Conclusions CRS protection evaluation should comprehensively consider thoracoabdominal kinematic parameters,internal organ biomechanics,and spinal injury risk.These findings have important implications for CRS development in intelligent driving systems and occupant protection strategy formulation.
