1.Clinical Efficacy of Shenqi Yangxin Decoction in Treatment of Patients with Ischemic Cardiomyopathy and Its Effect on Serum H2S and Ca2+

ObjectiveTo evaluate the clinical efficacy of Shenqi Yangxin decoction in the treatment of ischemic cardiomyopathy (ICM) with Qi and Yin deficiency and blood stasis syndrome and its effect on serum hydrogen sulfide (H2S) and calcium ion (Ca2+). MethodsA total of 64 ICM patients with Qi and Yin deficiency and blood stasis syndrome who met the inclusion criteria were randomly divided into a control group (n=32) and a treatment group (n=32). All patients received conventional Western medicine treatment. The treatment group was additionally given Shenqi Yangxin decoction. The TCM syndrome score, Minnesota Living with Heart Failure Questionnaire (MLHFQ) score, left ventricular ejection fraction (LVEF), N-terminal pro-B-type natriuretic peptide (NT-proBNP), 6-minute walk test (6MWT), New York Heart Association (NYHA) cardiac function classification, and serum H2S and Ca2+ levels were compared between the two groups pre- and post-treatment. ResultsTwo cases dropped out from each group during the study. Finally, 30 patients in each group were included in the analysis. There were no significant differences in age, gender, course of coronary heart disease, underlying diseases, and laboratory tests between the two groups. Compared with baseline, the TCM syndrome score, MLHFQ score, and NT-proBNP in both treatment group and control group decreased significantly (P<0.01), LVEF, 6MWT, and H2S increased significantly (P<0.01), and serum Ca2+ increased (P<0.05). Compared with the control group after treatment, the MLHFQ score and NT-proBNP in the treatment group decreased (P<0.05), the TCM syndrome score decreased significantly (P<0.01), LVEF, 6MWT, and serum Ca2+ increased (P<0.05), and H2S increased significantly (P<0.01). The improvement degree of the NYHA cardiac function classification in the treatment group was higher than that in the control group, but there was no significant difference. ConclusionShenqi Yangxin decoction is effective in treating ICM patients with Qi and Yin deficiency and blood stasis, which could significantly improve cardiac function and quality of life, and its therapeutic effect may be related to the regulation of serum H2S and Ca2+ levels.
2.Observation on the therapeutic effect of lower extremity exoskeleton robot training on walking and balance function in patients with incomplete paraplegia

Objective To explore the therapeutic effect of lower extremity exoskeleton robot(LEER)training on walking and balance function in patients with incomplete paraplegia.Methods According to the treatment method,70 patients with incomplete paraplegia were randomly divided into LEER group and control group,35 cases in each group.Both groups received regular rehabilitation therapy and functional walking training,and the LEER group received LEER training.Barthel index(BI),Berg balance scale(BBS)and functional ambulation category(FAC)were used to evaluate the activities of daily living,balance function and walking ability of the two groups before and 12 weeks after treatment.Results Compared with those before treatment,the BI and BBS scores of the LEER group and the control group were significantly increased(all P<0.05).There was no significant difference in BI and BBS scores between the LEER group and the control group before treatment(all P>0.05).The BI and BBS scores of the LEER group were significantly higher than those of the control group after treatment(all P<0.05).There was no significant difference in FAC grade distribution between LEER group and control group before treatment(P>0.05),and there was significant difference in FAC grade distribution between the two groups after treatment(P<0.05).Conclusion LEER training can effectively improve the daily living ability,balance and walking function of patients with incomplete paraplegia,which is superior to the single intervention path of conventional rehabilitation training.

4.Protective effect of sanguinarine on intestinal epithelial barrier in septic rats through modulation of MLCK-MLC signaling pathway

Objective:To investigate the effect of sanguinarine(SAG)on the intestinal epithelial barrier in septic rats and to explore the potential mechanism initially.Methods:Rats were divided into Sham group,CLP group,low-dose SAG group(L-SAG group),medium-dose SAG group(M-SAG group)and high-dose SAG group(H-SAG group)according to the random number table method,10 rats in each group.The rat model of sepsis was induced by cecum ligation puncture(CLP)method.HE staining was used to observe the pathological damage of intestinal epithelium.Serum DAO activity was detected by diamine oxidase(DAO)kit.Detec-tion of urinary lactose/mannitol(L/M)ratio by high performance liquid chromatography.Detection of serum TNF-α,IL-6 and IL-1β levels by ELISA.Protein expression levels in intestinal epithelial tissues were detected by Western blot.Results:The intestinal mucosa was intact and the intestinal villi were neatly arranged of rats in the Sham group;the intestinal mucosa of CLP group showed congestion and edema,inflammatory cell infiltration,irregular arrangement of intestinal villi,and enlarged villi gap;the pathological damage of intestinal mucosa in M-SAG group and H-SAG group were reduced to different degrees compared with the CLP group.Chiu's score,se-rum DAO activity and urinary L/M ratio were significantly higher in the CLP group compared with the Sham group;Chiu's score,se-rum DAO activity and urinary L/M ratio were significantly lower in the M-SAG and H-SAG groups compared with the CLP group(P<0.05).The serum TNF-α,IL-6 and IL-1β levels were significantly lower in the CLP group than in the Sham group,while the serum TNF-α,IL-6 and IL-1β levels were significantly higher in the M-SAG and H-SAG groups than in the CLP group(P<0.05).Claudin-3,ZO-1 and ocludin protein expressions were significantly lower in the intestinal epithelium of rats in the CLP group compared with the Sham group,while MLCK,MLC and p-MLC protein expressions were significantly increased.Compared with the CLP group,Claudin-3,ZO-1 and occludin protein expressions were significantly increased in the intestinal epithelium of rats in the M-SAG and H-SAG groups,while MLCK,MLC and p-MLC protein expressions were significantly decreased(P<0.05).Conclusion:SAG can reduce in-testinal barrier damage,inhibit intestinal barrier permeability,and protect intestinal barrier integrity in septic rats,which is at least partially achieved by inhibiting MLCK-MLC signaling pathway activation.

Objective:To revise and enlarge the specification of pharmaceutical excipient sucrose palmitate.Methods:Reference to JP2018,USP-NF 2023,EP1 1.0,Chinese Pharmacopoeia 2020 edition of the four general rules and the first supplement of sucrose stearate pharmacopoeia standards and other relevant requirements for standard research.Results:According to the quality of the product and the actual application in the formulation,the quality standards for the pharmaceutical excipient sucrose palmitate will be studied.At present,the quality standard of sucrose palmitate for pharmaceutical excipients has been disclosed.Conclusion:The established stand-ard will provide the quality guarantee for the application of sucrose palmitate in medicines.

Creutzfeldt-Jakob disease(CJD)is a rapidly progressive and fatal neurodegenerative disorder caused by prions,with certain infectivity and iatrogenic transmission risks.With the rapid progress and application of new dia-gnostic biomarkers and detection methods,as well as the construction and improvement of surveillance and reporting systems,the detection of CJD in patients domestically and internationally has shown an increasing trend year by year.Due to its long incubation period and heterogeneity of early symptoms,early identification and diagnosis of the disease is difficult,increasing the risk of transmission within medical institutions.Currently,there is a lack of con-sensus on the infection prevention and control of CJD.In order to timely identify and diagnose CJD as well as effec-tively block its transmission in medical institutions,this consensus summarizes 15 clinical concerns and formulates 24 specific recommendations based on the latest domestic and international research findings and clinical evidence,as well as combines with clinical practice,aiming to standardize healthcare-associated infection prevention and control measures for CJD and reduce its transmission risk in medical institutions.
8.Psychosocial adaptation and its influencing factors in young and middle-aged patients after colorectal cancer surgery

Objective:To investigate the current status of psychosocial adaptation in young and middle-aged patients after colorectal cancer surgery and to explore its influencing factors, providing a basis for the development of targeted interventions.Methods:A total of 200 postoperative young and middle-aged colorectal cancer patients undergoing follow-up at the outpatient department of Shanxi Cancer Hospital from June 2023 to March 2024 were selected by convenience sampling. Data were collected using a general demographic questionnaire, the Self-Report Psychosocial Adjustment to Illness Scale (PAIS-SR), the Family APGAR Index Questionnaire, and the Simplified Ways of Coping Questionnaire (SWCQ). Multiple linear regression analysis was performed to identify factors influencing psychosocial adaptation.Results:A total of 200 questionnaires were distributed, and 191 valid responses were collected, yielding a valid response rate of 95.5%. The total PAIS-SR score of participants was (57.91±23.45). Multiple linear regression analysis revealed that education level, self-perceived economic status, employment status, presence of a stoma, time since surgery, family care, and positive coping style were statistically significant influencing factors ( P<0.05), explaining 46.4% of the variance in psychosocial adaptation. Conclusions:Healthcare providers should pay particular attention to patients with lower educational levels, poor self-perceived economic conditions, unemployment, a stoma, and shorter postoperative duration. Enhancing family care—starting with patient caregivers—and encouraging the use of positive psychological interventions to foster active coping strategies may help improve psychosocial adaptation in young and middle-aged colorectal cancer patients.
