1.Summary of the best evidence for home nutrition management in patients after gastric cancer surgery

Objective To retrieve,evaluate and summarise the best evidence in home nutrition management for patients after gastric cancer surgery,so as to provide a reference for medical staff.Methods According to the"6S"mode of evidence-based resource retrieval,desktop work was conducted to search the databases of UpToDate,PubMed,Embase,Cochrane Library,Web of Science,China National Knowledge Infrastructure(CNKI),VIP database,Wanfang database,Scottish Intercollegiate Guidelines Network(SIGN),the website of National Institute for Health and Care Excellence(NICE),Guidelines International Network(GIN),National Guideline Clearinghouse(NGC),the website of Registered Nurses'Association of Ontario(RNAO),the guidelines website of Chinese Society of Clinical Oncology(CSCO),Physiotherapy Evidence Database(PEDro),National Comprehensive Cancer Network(NCCN),the website of China Medical Association(CMA),the website of American Society for Parenteral and Enteral Nutrition(ASPEN),the website of European Society for Clinical Nutrition and Metabolism(ESPEN),and Yimaitong for clinical guidelines,systematic evaluations,expert consensuses,clinical decisions,evidence summaries and randomised controlled trials(RCTs)in home nutrition management for the patients after gastric cancer surgery.The retrieved data was from 1st January,2014 to 30th June,2024.Two researchers who had systematic training in evidence-based nursing independently screened and evaluated the retrieved literature.Results A total of 13 documents were finally included,covering 5 clinical guidelines,5 expert consensuses,1 clinical decision and 2 RCT studies.Thirty-two pieces of the best evidence were summarised from seven aspects of nutritional follow-up,nutritional risk screening and assessment,health education of oral nutrition,health education of tube-fed enteral nutrition,health education of nutrient requirement,complication management and life management.Conclusion The evidence summarised in this study on home nutrition management for the patients after gastric cancer surgery can be used as a reference for medical staff.
2.Current status and visual analysis of the burn-related sepsis.

OBJECTIVE:
To explore the current status, evolution, hot topics, and future research trends in the field of burn-related sepsis research through a visual analysis of literature.
METHODS:
A bibliometric method was employed to retrieve articles related to burn-related sepsis from January 1, 1994, to May 16, 2024, in the China National Knowledge Infrastructure (CNKI) and the Web of Science database. The CiteSpace 6.3.R1 software was used to analyze the retrieved literature. The number of publications, authors, countries, and institutions in both Chinese and English literature was statistically analyzed. Co-occurrence analysis, clustering analysis, and co-citation analysis of keywords were performed.
RESULTS:
A total of 1 090 articles from the CNKI database and 1 143 articles from the Web of Science database were retrieved. Over the past 20 years, the volume of Chinese publications has remained stable, although there has been a slight decline in the past two years. In contrast, the number of English publications, after a period of growth, showed a sharp decline over the past three years. In Chinese literature, 1 457 authors published articles on burn-related sepsis as first authors, with 14 core authors publishing four or more articles. In English literature, 98 authors published articles on burn-related sepsis as first authors. Research on burn-related sepsis was conducted by 76 countries, with the United States having the most collaborations and publications. Globally, 1 349 institutions published articles on burn-related sepsis, with the top institutions being the First Affiliated Hospital of the PLA General Hospital (8 articles) for Chinese literature and the University of Texas Medical Branch (57 articles) for English literature. In the co-occurrence analysis, 208 Chinese keywords and 211 English keywords were included. Excluding keywords related to search terms, the top five most frequent keywords in Chinese literature were burn, sepsis, infection, severe burn, and procalcitonin; the top five most frequent keywords in English literature were sepsis, septic shock, mortality, injury, and burn injury. Chinese keyword analysis identified six clusters, with the largest being sepsis, followed by procalcitonin, infection, and severe burn. English keyword analysis identified seven clusters, with the largest being expression, followed by epidemiology, inhalation injury, and acute kidney injury. The persistent clusters in Chinese literature were procalcitonin, with recent emerging nodes being severe burn, inflammatory response, platelets, and predictive value. In English literature, the persistent clusters were inhalation injury and nitric oxide, with recent emerging nodes being continuous renal replacement therapy, hemorrhagic shock, and early enteral nutrition. The longest-lasting emergent keyword in Chinese literature was delayed resuscitation (2003-2010), with the highest emergent strength being severe burn. In English literature, the longest-lasting emergent keywords, each lasting five years, were nitric oxide (2007-2012), management (2019-2024), and impact (2019-2024), with the highest emergent strength being thermal injury.
CONCLUSIONS
Research on burn-related sepsis has shifted from focusing on early studies on pathogenesis and mortality to focus on prevention, treatment, and early diagnosis. Future research is expected to focus on early diagnosis and risk factors of burn-related sepsis.

3.Intervention programs for fertility concerns in breast cancer survivors of childbearing age:a scoping review

Objective:To analyze the relevant studies on fertility concerns intervention programs for breast cancer survivors of childbearing age, and identify the specific content, outcome indicators and intervention effects of intervention, in order to provide reference for clinical staff to implement relevant intervention.Methods:According to the research method of scope review, the studies on the fertility concerns with breast cancer patients of childbearing age in China National Knowledge Infrastructure, WanFang, VIP, China Biomedical Literature Database, CINAHL, PubMed, Cochrane Library, Web of Science etc. were searched. The search period was from database establishment to May 9, 2024. The included literature was classified and analyzed.Results:A total of 12 articles were included, 9 were randomized controlled trials, 2 were self-controlled before and after studies, and 1 was a prospective cohort study. Fertility concerns intervention programs included group fertility counseling centered on couples, fertility support based on a multidisciplinary team collaboration model, and fertility management based on "Internet + reproductive health". The duration of intervention was usually 4 or 24 weeks. The intervention had different effects on the level of fertility concerns, psychological state and marital status.Conclusions:At present, the intervention programs for fertility concerns of breast cancer survivors of childbearing age reflect the importance, professionalism, immediacy and interactivity, but China is in the initial stage, and there are still shortcomings in multidisciplinary teams, fertility referral system, and Internet platform construction. Future research should build a breast cancer fertility intervention program suitable for our country, and promote its application through the Internet.

6.Non-pharmaceutical intervention of Traditional Chinese Medicine in stable chronic obstructive pulmonary disease: a best evidence summary

Objective:To systematically review, assess, extract, and summarize the most effective evidence on non-pharmacological traditional Chinese medicine interventions for promoting pulmonary rehabilitation in stable patients with chronic obstructive pulmonary disease, aiming to provide a theoretical foundation for clinical nurses to implement traditional Chinese medicine nursing interventions.Methods:According to the "6S" evidence model, relevant evidence on non-pharmacological traditional Chinese medicine interventions for stable chronic obstructive pulmonary disease was retrieved from domestic and foreign databases, guideline websites and professional society websites, including clinical decisions, guidelines, evidence summaries, systematic reviews, expert consensus, etc. The retrieval time was from January 1, 2018 to August 10, 2023. Two researchers evaluated the quality of the included literature, and extracted data and summarized evidence.Results:A total of 23 articles were included, including 6 guideline, 1 evidence summary, 7 systematic reviews, 8 Meta analysis and 1 expert consensus. The 37 evidences were summarized from 11 aspects, including target population, intervention focus, moxibustion therapy, acupoint sticking therapy, transcutaneous electrical acupoint stimulation, acupuncture therapy, acupoint injection therapy, traditional Chinese exercises, diet therapy, follow-up procedures, safety considerations.Conclusions:This study summarizes the evidences of traditional Chinese medicine non-drug pulmonary rehabilitation in the stable stage of chronic obstructive pulmonary disease. When applying the evidence in clinical practice, the traditional Chinese medicine nursing ability of nurses and the actual situation of patients should be fully considered to formulate an individualized traditional Chinese medicine nursing plan.
7.Identification of a COL2A1 mutation in a Chinese family with Stickler syndrome type 1 via whole exome sequencing

Objective:To identify the disease-causing mutation in a Chinese family with Stickler syndrome type 1.Methods:The pedigree investigation was conducted.A Chinese family with Stickler syndrome type 1 was enrolled in the Shantou International Eye Center in June 2012.Medical history collection and clinical examinations, such as vision, intraocular pressure, slit lamp microscopy and fundus, were carried out in all the included family members and the diagnosis was made by clinical experts.Total genomic DNAs were extracted from the peripheral blood samples (5 ml) obtained from 5 patients and 4 healthy members.The potential variant of the proband's father Ⅲ-5 were screened by whole exome sequencing (WES) and stepwise bioinformatic analysis.The segregation and mutation conformation of the variant was verified by Sanger sequencing.The pathogenicity of the variant was predicted by SIFT, Polyphen2, and MutationTaster.Conservation and three-dimensional structure of amino acid mutation were analyzed by multiple sequence alignment and UniProt.This study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Joint Shantou International Eye Center (No.EC20110310[2]-P02).Written informed consent was obtained from each subject or the guardian.Results:An autosomal dominant inherence in 39 members of 4 generations including 15 patients and 24 phenotypically normal members was found in the family.The proband (Ⅳ-4) showed high myopia, retinal detachment and strabismus in the right eye, and the left eye was blind.A patient (Ⅲ-5) showed high myopia and cataract in the right eye, atrophy in the left eye.A patient (Ⅳ-9) showed binocular high myopia.A heterozygous variation, c.1693C>T: p.Arg565Cys, within the exon 26 of COL2A1 gene was revealed in patient Ⅲ-5, which was only found in the patients and not in phenotypically normal members, indiacating co-separation in this family.The variant was predicted to be a severe damage by SIFT, Polyphen2 and MutationTaster.The amino acid mutation at position 565 was highly conservative among human, mouse, rat, bovine and Xenopus laevis, which caused the arginine to cysteine substitution at the X position in triple helix repeat region Gly-X-Y, affecting the function of fibrous protein and becoming pathogenic. Conclusions:Variant c.1693C>T: p.Arg565Cys in COL2A1 gene is disease-causing in this family and this is the first report about the variant in China.
8.Physical activity among senior primary school students on weekends in Beijing

Objective:
To analyze the duration and influencing factors of moderate and vigorous physical activity(MVPA) on weekends for primary school students in grades 4 to 6 in Beijing, and to provide a reference for formulating health education and promotion measures.
Methods:
Multi stage stratified cluster sampling method was used to randomly select 2 515 students from grades 4-6 in 14 primary schools in Beijing, and a self administered questionnaire was used to record MVPA on weekend, social demographic characteristics, other related health behaviors and knowledge. Multivariate Logistic regression analysis was used to explore the influencing factors of MVPA on weekends.
Results:
The prevalence of insufficient MVPA on weekends in Beijing was 63.54 %, and the prevalence was higher among girls (69.92%) than boys (57.81%) ( χ 2=39.65, P <0.01). Multiple Logistic regression analysis revealed that girls ( OR =1.74), living in rural areas ( OR =1.41), participants attending general schools ( OR = 1.34 ), from divorced family ( OR =1.46), and short sleep duration ( OR =1.50) were more likely to fail to meet the MVPA recommendations( P <0.05).
Conclusion
It is quite common that no sufficient weekend MVPA among senior primary school students, among them, the outer suburbs and schools with relatively weak teaching resources are the key places that need attention, and girls are the key groups that need attention.
9.Progress of congenital sodium diarrhea in children

Congenital sodium diarrhea(CSD)refers to a rare and intractable secretory diarrhea of intrauterine onset with high fecal sodium loss which is caused by abnormal intestinal sodium absorption.Congenital sodium diarrhea is a disease with clinical and genetical heterogeneity.It can be classified as non-syndromic congenital sodium diarrhea(non-sCSD)and syndromic congenital sodium diarrhea(sCSD)according to the clinical features.The diagnosis of CSD is made based on the manifestation and genetic analysis.The mutations of CSD include SLC9A3 gene、GUCY2C gene and SPINT2 gene.The main treatment of CSD is symptomatic treatment and some of these patients will develop inflammatory bowel disease(IBD). Lifelong clinical and biological follow-up of patients is necessary.
10.Prevalence and associated factors of electronic products usage among primary school students of grade 4-6 in Beijing

Objective:
To explore the prevalence and associated factors of non learning-based screen time of 4-6 grade school students in Beijing, and to provide a basic data for further research on hazard control measures such as myopia, overweight and obesity among children and adolescents in Beijing.
Methods:
Multistage stratified random cluster sampling method was adopted. A total of 2 515 primary school students were randomly selected in from schools in Beijing, self-developed questionnaire was used to investigate on the time for each electronic products usage for non-learning purpose, the total of electronic products usage every day and other information.
Results:
The rates of screen time >15 minutes each time and ≥1 h daily were 48.43%, 22.90%, respectively. Multiple Logistic regression analysis showed that ordinary school, 6 th grade, male, single-parent family and other types of family, and lack of moderate-to-vigorous physical activity at weekend were associated with the students’ non learning-based screen time (OR=1.66, 2.28, 1.27, 1.44, 1.87, 2.20, P<0.05).
Conclusion
The situation of excessive screen time of primary school students was improved in Beijing, but still be prevelent, male students of grade 4 should be given more attention, and more importance should be attached to the offect of family in children.
