1.Material Basis of Anti-Inflammatory Efficacy and Mechanism of Action of Bushen Tongdu Prescription Based on UPLC-LTQ-Orbitrap-MS and Network Pharmacology

ObjectiveThis paper aims to investigate the material basis of the anti-inflammatory efficacy and mechanism of action of Bushen Tongdu prescription (BSTDP). MethodsThe chemical components of BSTDP and its blood-absorbed components in vivo were systematically identified by using ultra-performance liquid chromatography-linear ion trap-electrostatic field orbitrap high-resolution mass spectrometry (UPLC-LIT-Orbitrap-MS). Network pharmacology was employed to screen blood-absorbed bioactive components and potential targets of this formula. A protein-protein interaction (PPI) network of core targets was constructed to conduct enrichment analysis. Molecular docking was further utilized to verify the binding affinity between key components and targets. The inflammatory model was established and verified in vivo by using a transgenic zebrafish Tg (mpx: GFP). At three days post-fertilization (3 dpf), larvae of zebrafish were randomly assigned to blank group, model group, positive drug dexamethasone acetate group (75 μmol·L-1), and BSTDP groups with low, medium, and high doses (500, 1 000, and 2 000 mg·L-1). The distribution and quantity of neutrophils in the yolk sac region were observed under a fluorescence microscope. The mRNA expression levels of key genes in the toll-like receptor 4 (TLR4)/myeloid differentiation factor 88 (MyD88)/nuclear factor kappa-B (NF-κB) signaling pathway and inflammatory factors including interleukin (IL)-1β, IL-6, and tumor necrosis factor-α (TNF-α) were detected by Real-time quantitative polymerase chain reaction (Real-time PCR). ResultsA total of 120 chemical components were identified in BSTDP, among which 26 original components were confirmed by using serum pharmacochemical methods. A total of 227 common targets linking rheumatoid arthritis (RA) and the blood-absorbed components were screened by network pharmacology. It is suggested that pseudobrucine, vomicine, sinapine, rehmannioside, cinnamyl alcohol glycoside, and methylephedrine exert anti-inflammatory effects by acting on core targets including protein kinase B1 (Akt1), signal transducer and activator of transcription 3 (STAT3), tumor necrosis factor (TNF), TLR4, mitogen-activated protein kinase 14 (MAPK14), and phosphatidylinositol-4,5-bisphosphate 3-kinase catalytic subunit α (PIK3CA), thereby modulating multiple signaling pathways such as TLR4 and NF-κB. In vivo verification in zebrafish demonstrates that the maximum tolerable concentration of Bushen Tongdu Formula is 2 000 mg·L-1. Compared to those in the blank group, zebrafish in the model group showed a significantly higher number of neutrophils in the yolk sac region (P<0.01) and rising mRNA levels of TLR4, MyD88, NF-κB, TNF-α, IL-6, and IL-1β (P<0.01). Compared to that in the model group, the number of neutrophils was significantly reduced in BSTDP groups with medium and high doses, as well as the dexamethasone acetate group (P<0.05, P<0.01). There was no statistically significant difference in the low dose group. The mRNA expression levels of TLR4, MyD88, NF-κB, TNF-α, IL-6, and IL-1β were significantly down-regulated (P<0.05, P<0.01). ConclusionThis paper identifies the material basis of the efficacy of BSTDP, demonstrating that the formula can exert an anti-inflammatory effect through the TLR4/MyD88/NF-κB signaling pathway. The results provide scientific experimental evidence for its further clinical application.

3.Rehmanniae Radix Preparata in Common Central Nervous System Diseases: A Review

Central nervous system diseases refer to complex disorders that affect the structure and function of the central nervous system (brain and spinal cord). Due to their complex pathogenesis and the lack of effective therapies, the diseases have become one of the leading causes of human death and have imposed a heavy economic burden on families and society. Rehmanniae Radix Preparata, a representative traditional Chinese medicine for "nourishing essence and enriching marrow", has multi-target and multi-pathway advantages in the treatment of central nervous system diseases. It contains various active components such as catapol, verbascoside, and echinacoside and has multiple pharmacological effects including anti-depression, neuroprotection, antioxidation, anti-inflammation, and immunomodulation. In Alzheimer's disease, Rehmanniae Radix Preparata can regulate the processing of beta-amyloid precursor protein to reduce its production, and its antioxidative and neuroprotective effects can alleviate related pathological damage. In Parkinson's disease, it can protect dopaminergic neurons by regulating intracellular signal transduction pathways. In ischemic stroke, its antioxidative, anti-inflammatory, and neuroprotective effects can alleviate pathological reactions. Multiple sclerosis is an autoimmune demyelinating disease, and its pathogenesis is related to the autoimmune response attacking the myelin sheath of the central nervous system. The anti-inflammatory effect of Rehmanniae Radix Preparata can help inhibit this autoimmune response, reduce the damage of inflammation to the myelin sheath, and protect damaged neurons while promoting myelin sheath repair. In depression, Rehmanniae Radix Preparata can improve neural plasticity, and its anti-inflammatory effect can regulate neuroendocrine function and influence the metabolism of neurotransmitters. This paper explored the action mechanism of Rehmanniae Radix Preparata in central nervous system diseases and summarized the relevant basic research and research progress on its active components and traditional Chinese medicine formulas for treating central nervous system diseases, providing a theoretical basis for the development of traditional Chinese medicine products for the prevention and treatment of central nervous system diseases with Radix Rehmanniae Preparata.

Myocardial fibrosis is a serious cause of heart failure and even sudden cardiac death. However, the mechanisms underlying myocardial ischemia-induced cardiac fibrosis remain unclear. Here, we identified that the expression of sterile alpha and TIR motif containing 1 (SARM1), was increased significantly in the ischemic cardiomyopathy patients, dilated cardiomyopathy patients (GSE116250) and fibrotic heart tissues of mice. Additionally, inhibition or knockdown of SARM1 can improve myocardial fibrosis and cardiac function of myocardial infarction (MI) mice. Moreover, SARM1 fibroblasts-specific knock-in mice had increased deposition of extracellular matrix and impaired cardiac function. Mechanically, elevated expression of SARM1 promotes the deposition of extracellular matrix by directly modulating P4HA1. Notably, by using the Click-iT reaction, we identified that the increased expression of ZDHHC17 promotes the palmitoylation levels of SARM1, thereby accelerating the fibrosis process. Based on the fibrosis-promoting effect of SARM1, we screened several drugs with anti-myocardial fibrosis activity. In conclusion, we have unveiled that palmitoylated SARM1 targeting P4HA1 promotes collagen deposition and myocardial fibrosis. Inhibition of SARM1 is a potential strategy for the treatment of myocardial fibrosis. The sites where SARM1 interacts with P4HA1 and the palmitoylation modification sites of SARM1 may be the active targets for anti-fibrosis drugs.
5.The research progress of A-PRF and CGF in regeneration and repair of implant-related alveolar bone defects

The advanced platelet-rich fibrin(A-PRF)and concentrated growth factor(CGF)are platelet concentrates(PCs)obtained through the collection of venous blood from the patient and using different centrifugation procedures.Both are rich in growth factors,and the internal fibrin structure within them serves as a natural three-dimensional scaffold for the release of growth factors.They possess characteristics such as low cost,high safety and superior regenerative potential.Currently,they have gained wide-spread attention in the field of regeneration and repair of soft and hard tissues in oral implantation.This review provides the progress of A-PRF and CGF research,focusing on their preparation methods,application methods,potential regeneration mechanisms and a comparison of components and scaffold structures promoting alveolar bone defect repair between A-PRF and CGF.
6.Construction and effect of preventive maintenance pathway based on risk control model for medical equipment in operating room

Objective:To construct a preventive maintenance path based on risk control model for medical equipment in the operating room,and analyze its application effect in the management for medical equipment in the operating room.Methods:Based on 7 key factors included equipment characteristics,usage duration,startup frequency,operating efficiency,operator skills,maintenance frequency,and technical support in risk control model,we followed management mode of preventive maintenance,and constructed a preventive maintenance path from six dimensions which included management of equipment application,preventive maintenance and upkeep,predictive maintenance,fault repair,post upkeep,and quality monitoring.A total of 215 used medical equipment in operating room at Xiaolan People's Hospital of ZhongShan from January 2022 to December 2022 were selected,and the equipment during January and December 2022 received maintenance management by using conventional management method,and these during January and December 2023 received maintenance management by using preventive maintenance path method based on risk control model for medical equipment(model management method).The quality of management and operation for equipment between two kinds of management methods were compared.A self-designed questionnaire was adopted to investigate the operators'satisfaction for service quality of equipment in operating rooms.Results:The average standardization degree of using equipment,efficiency of maintenance,timely maintenance,and qualification rate of quality inspection of adopting model management method were respectively(94.43±4.26)%,(97.74±1.53)%,(86.78±6.72)%and(96.48±3.02)%,all of which were higher than those of adopting conventional method(t=22.583,34.738,14.820,18.577,P<0.05).The average rate of starting equipment and self-repair rate of the model management method were significantly higher than those of the conventional method,and the differences were statistically significant(t=10.355,7.624,12.811,P<0.05).The satisfaction scores of operators who used management for adopting model management method were higher than those for adopting conventional management method in applying and managing equipment,preventive upkeep,predictive maintenance,fault repair,post upkeep and quality monitoring,and the differences were statistically significant(t=18.653,22.942,18.752,23.673,40.055,37.120,P<0.05).Conclusion:The preventive maintenance path based on risk control model for medical equipment in operating room can improve the management quality for equipment in operating room,and enhance management effectiveness and operators'satisfaction.
7.Qualitative study on the psychological acceptance mechanism of appropriate Traditional Chinese Medicine techniques among patients with abnormal uterine bleeding

Objective:To explore the psychological acceptance mechanism of appropriate Traditional Chinese Medicine (TCM) techniques among patients with abnormal uterine bleeding (AUB) .Methods:Using purposive sampling, AUB patients experiencing menstruation but not yet menopausal were recruited from Zhumadian City, Henan Province, and Jiaxing City, Zhejiang Province, between November 2022 and January 2023. Semi-structured interviews were conducted, and the collected data were analyzed using content analysis.Results:A total of four main themes were identified: individualized disease perception and treatment experience; awareness and treatment experience of appropriate TCM techniques; cultural identity and influence of traditional beliefs; and the need for science communication and safety regarding TCM techniques.Conclusions:While AUB patients show a generally high level of acceptance toward appropriate TCM techniques, their understanding of both AUB and the relevant TCM therapies remains limited. Multiple factors influence patients' choices, and some concerns and doubts still persist during the decision-making process.
8.Construction and implementation of a new model for prevention and treatment of hepatitis B in communities in Guangzhou city

Objective:A novel grass-root community screening and management model of hepatitis B was developed in order to improve the diagnosis and treatment rate of hepatitis B in Guangzhou city.Methods:A three-tier collaborative framework[tertiary hospitals-center for disease control and prevention(CDC)-primary care clinics]implemented dual-track screening(fixed-site+mobile units)using rapid hepatitis B surface antigen(HBsAg)testing and structured surveys. Digital closed-loop management integrated screening,referral,and follow-up. Data were analyzed via SPSS 26.0.Results:Among 30 012 community-dwelling adults screened(Male∶Female=1∶1.68),overall HBsAg positive rate was 5.21%(1 565/30 012),peaking in the 50-59-year cohort( χ2=271.80, P<0.001). Hepatitis B knowledge awareness was critically low(39.24%). Of 140 referred HBsAg-positive individuals,15 chronic carriers required no immediate antiviral therapy per guidelines. Treatment linkage surged from 32.8%(41/125)to 86.4%(108/125)post-intervention. aMAP hepatocellular carcinoma(HCC)risk stratification(n=82)revealed low(36.6%),intermediate(32.9%),and high-risk(30.5%)profiles. Conclusions:This coordinated,digitally-enhanced strategy significantly improved hepatitis B detection and treatment access. However,persistent knowledge gaps underscore the imperative for targeted community education and adherence support.
9.Evaluation of short-term outcomes of surgical intervention for severe pulmonary stenosis in infants and young children

Objective:To evaluate the short-term efficacy of surgical treatment for severe pulmonary stenosis(PS) in infants and young children, and to clarify the impact of different surgical timings on the efficacy of PS treatment.Methods:A retrospective analysis was conducted on 24 infants and young children who underwent surgical treatment for severe PS at the General Hospital of Northern Theater Command, PLA, from January 1, 2020, to October 1, 2024. Among them, 13 were males and 11 were females. The average weight of the patients was(9.2±3.6) kg, the average gestational age was(39.3±1.7) weeks, and the average age was(15.0±13.5) months. Preoperative clinical symptoms and signs(e.g., cyanosis, shortness of breath), transpulmonary valve pressure gradient(TPVPG), right ventricular systolic pressure(RVSP), and Em/Am were recorded. The patients were divided into two groups based on surgical timing: the infant group(under 1 year old, n=12) and the toddler group(1-3 years old, n=12).Results:There were no deaths among all patients. The postoperative ICU stay was significantly longer in the infant group compared to the toddler group( P<0.05). Compared to preoperative values, surgical treatment significantly improved TPVPG, reduced RVSP, and enhanced right ventricular diastolic function, which stabilized by 3 months postoperatively( P<0.05). Intergroup comparisons revealed that the infant group had significantly lower TPVPG at 6 months postoperatively compared to the toddler group. Additionally, right ventricular diastolic function improved significantly in the infant group postoperatively, while no significant improvement was observed in the toddler group. Younger age and reduced right ventricular diastolic function were identified as major risk factors for prolonged mechanical ventilation(>24 hours). Conclusion:Surgical treatment for PS during infancy and early childhood is safe and effective. Comprehensive preoperative evaluation is crucial, and early surgical intervention is recommended for patients with impaired right ventricular function to improve prognosis.
