Objective To explore the current status of penile erection hardness and its influencing factors in patients with non-obstructive azoospermia. Methods From January to December 2024,450 patients with non-obstructive azoospermia were surveyed at the Reproductive Medicine Center of Hospital.A self-made general information questionnaire was used to collect their demographic data. The Erectile Hardness Scale(EHS)was employed to investigate the current status of their penile erection hardness, and a self-made questionnaire was utilized to explore the influencing factors. Results Among the 450 patients with non-obstructive azoospermia, during sexual intercourse,35.3%of the patients reported that their penile erection hardness could reach grade 4(normal state),54.5%reported that it only reached grade 3(sub-optimal state),9.3%reported that it only reached grade 2(slight penile erection),and 0.9%reported that it only reached grade 1(inability to achieve an erection). In the survey of satisfaction with sexual life quality, among the 450 patients, only 24.9%were very satisfied with their sexual life quality;57.3%were basically satisfied;9.6%considered it average;4.0%were dissatisfied;3.1%were very dissatisfied;and 1.1%had no sexual life.alcohol consumption(OR=2.393,95%CI:1.493~3.836),satisfaction with the quality of sexual life(OR=1.455,95%CI:1.118~1.894),educational attainment(OR=0.709,95%CI:0.549~0.917),and the sleep quality in the past month(OR=0.641,95%CI:0.452~0.907).Conclusions Clinical studies have shown that factors such as drinking habits, sexual life satisfaction, sleep quality, and educational attainment collectively influence the penile erection hardness in patients with non-obstructive azoospermia. Therefore, the medical team needs to customize personalized intervention plans and educational materials based on individual differences among patients.Through psychological counseling and lifestyle guidance, they can improve erectile function and the quality of sexual life, promote harmonious marital relationships, and enhance the overall life experience of the patients.
2.The microbe-immune-virus regulatory network in pediatric asthma:From mechanism to clinic
The Journal of Practical Medicine 2025;41(20):3297-3304
Pediatric asthma is a complex and heterogeneous disease, with susceptibility in early life co-shaped by the interplay of host genetics, environmental exposures, microbial colonization, and immune development.Early-life airway viral infections are a well-established risk factor, though their pathogenic effects are highly host-dependent.Emerging evidence indicates that the gut microbiome remotely regulates pulmonary immune homeostasis via metabolites such as butyrate(the gut-lung axis),playing a pivotal role in the disease trajectory.Multi-omics research has expanded the scope of this regulatory network, identifying cross-kingdom microbial members, including the gut virome(phages)and protozoa. These members influence asthma susceptibility by activating specific immune pathways, such as the Tfh13-IgE axis and innate immune memory.Crucially, many children exposed to risk factors remain healthy, highlighting the roles of individual resilience and protective factors.Elucidating this network is crucial for clarifying disease heterogeneity and provides a basis for developing novel biomarkers(e.g.,the oropharyngeal microbiome, serum Extracellular Vesicles[EVs])and constructing multi-dimensional risk prediction models integrating artificial intelligence. This research ultimately aims to advance personalized precision prevention and management for pediatric asthma.

Objective To verify the accuracy of the"manual tracing+summation method"for calculating the area of subcap aponeurotic hematoma using CT images in the practice of judicial expertise. Methods Subcap aponeurotic hematoma at different locations were simulated using"skull+colored clay plastic"model. The simulated hematoma areas were calculated via RadiAnt DICOM Viewer(64-bit)after CT scanning and compared with manual measurements.Results(1)For completely scaned simulated hematoma, the area of manual calculation is smaller than that measured by the expert assistance system for human injury identification, yet demonstrated relatively high accuracy;(2)Scanning planes parallel to the three scan baselines has the greatest impact on the imaging of top hematoma, often leading to incomplete scanning of both the top of the skull and hematoma. Conclusion When the scan of hematoma is complete, the subcap aponeurotic hematoma calculated by manual tracing is close to true values. However, incomplete scans may lead to disputable results and time-consuming and laborious manual calculations. In view of the current situation, pixel level semantic segmentation technology in the field of computer vision can be combined with the application of level set, gray reconstruction and other segmentation algorithms to obtain the three-dimensional outline of the hematoma area,so as to calculate the hematoma area and provide the most direct data analysis for judicial identification.

Objective:To identify the risk factors associated with conversion to cesarean delivery in parturients receiving epidural labor analgesia and to construct a predictive model. Methods:In this prospective cohort study, 718 parturients with a singleton fetus at 37-42 weeks of gestation age, aged 22-45 yr, of American Society of Anesthesiologists Physical Status classification Ⅱ, who received epidural labor analgesia at the Women′s Hospital affiliated to Zhejiang University School of Medicine between November 2022 and August 2023, were included and divided into 2 groups based on the final mode of delivery: cesarean delivery group (CD group) and vaginal delivery group (VD group). Data collected included maternal age, height, body mass index at labor onset, gestational age, pregnancy complications, mode of conception, parity, premature rupture of membranes, mode of labor onset and details of labor analgesia. Risk factors for conversion to cesarean delivery were analyzed using multivariate logistic regression analysis, and a prediction model was developed. The receiver operating characteristic curve was ploted, and the area under the receiver operating characteristic curve was calculated to evaluate the accuracy of the predictive model. Results:A total of 682 parturients were included in the final statistical analysis, with 103 in CD group and 579 in VD group. Multivariate logistic regression analysis showed that maternal age≥35 yr, height<160 cm, body mass index at labor onset ≥28 kg/m 2, nulliparous parturients, the use of Foley balloon catheter or dinoprostone suppositories for cervical ripening were risk factors for cesarean delivery, and cervical dilation during analgesia was a protective factor. The area under the receiver operating characteristic curve for the predictive model constructed based on the aforementioned influencing factors yielded predicting conversion to cesarean delivery due to failed vaginal trial of labor was 0.791 (95% confidence interval 0.744-0.837, P<0.001), with a sensitivity of 73.6% and a specificity of 70.9%. Conclusions:Nulliparous parturients, maternal age≥35 yr, height<160 cm, body mass index at labor onset≥28 kg/m 2, and the use of Foley balloon catheter or dinoprostone suppositories for cervical ripening are risk factors for cesarean delivery, and cervical dilation during analgesia is a protective factor. The predictive model established based on these influencing factors has high predictive performance.

To investigate the distribution and development of common allergens in children in Hangzhou area, and to provide an epidemiological basis for the prevention, diagnosis, and treatment of allergic diseases in this area. This study is a retrospective study, selecting 3 524 children who underwent allergen screening at DiAn Medical Laboratory Center (Hangzhou) Co., Ltd. from January 2022 to January 2024 as the research subjects, including 2 012 males and 1 512 females. Among them, there were 1 098 infants (0-1 year olds), 1 673 toddlers (1-3 year olds), and 753 children (3-14 year olds). Immunoblotting was used to detect 21 allergen-specific immunoglobulin E (sIgE) antibodies. The positive rates of various allergens were calculated according to gender, age, and other factors, and the epidemiological characteristics and trends were analyzed. The results showed that the overall positive rate of sIgE was 60.33% (2 126/3 524). The main inhaled allergens were Dermatophagoides pteronyssinus/Dermatophagoides farinae (992, 28.15%), dog dander (295, 8.37%) and cat dander (181, 5.13%). The main food allergens were milk (696, 19.75%), egg white (541, 15.36%) and shrimp (205, 5.83%). Boys had significantly higher sIgE positivity rates than girls for Dermatophagoides pteronyssinus/Dermatophagoides farinae(642, 31.90%, χ2=10.10, P=0.001), house dust(61, 3.02%, χ2=5.12, P=0.024), cat dander(124, 6.16%, χ2=4.06, P=0.044), mold combinations(103, 5.14%, χ2=7.51, P=0.006), and tree pollen combinations(42, 2.07%, χ2=5.44, P=0.020) (all P<0.05); With age, there was a significant increase in positivity rates for house dust mite/dust mite, house dust, mold combinations, milk, and beef (all P<0.05), and a significant decrease in positivity rates for cockroach consumption, egg whites, shrimp, crab, cod, lobster/scallop, and soybeans (all P<0.05), the positive rates of only ingestive allergens decreased significantly ( P<0.001), and the positive rates of only inhalant and mixed allergens increased significantly ( P<0.001). In conclusion,in the Hangzhou area, dust mites are the most common inhalant allergens among children with allergic diseases, while milk is the most common food allergen. Boys are more sensitive to inhalant allergens, and as children grow older, the positive rates for different allergens undergo significant changes. Dynamic monitoring of changes in specific IgE antibodies to various allergens can assist in the diagnosis, prevention, and treatment of allergic diseases.

Objective To systematically compare the efficacy of different virtual reality(VR)technologies on motor dysfunction in patients with Parkinson's disease(PD)through a network meta-analysis(NMA).Methods Following the PRISMA-NMA guidelines, randomized controlled trials(RCT)that compared the effect of at least two interventions on motor function in patients with PD were searched in PubMed,Embase,Cochrane Li-brary,Web of Science,CNKI,Wanfang data and VIP.The interventions included immersive virtual reality(IVR),non-immersive virtual reality(VRT),augmented reality(AR),treatment as usual(TAU)and active control(AC).The primary outcomes included Unified Parkinson's Disease Rating Scale Part Ⅲ(UPDRS-Ⅲ),Berg Balance Scale(BBS)and Timed Up and Go Test(TUGT).A frequentist-based random-effects model was used to conduct NMA,and the surface under the cumulative ranking curve(SUCRA)was used to rank the interventions. Results A total of 20 RCT involving 890 patients were included.IVR performed best in improving the score of UPDRS-Ⅲ(SUCRA=97.7%)and was significantly superior to TAU(SMD=-0.82,95%CI-1.28 to-0.37).IVR showed the highest probability of being the best option for the score of BBS(SUCRA=85.2%)and was signifi-cantly superior to TAU(SMD=3.94,95%CI 1.08 to 6.80).IVR performed best in the outcome of TUGT(SU-CRA=95.1%)and was significantly superior to VRT(SMD=1.06,95%CI 0.40 to 1.72),AR(SMD=-1.09,95%CI-1.98 to-0.19)and TAU(SMD=-1.38,95%CI-1.95 to-0.82).However,subgroup analysis revealed that the efficacy advantage of IVR was mainly confirmed in short-term(four to six weeks)interventions, while the evidence for long-term efficacy was currently very limited. Conclusion The efficacy of interventions on motor rehabilitation in patients with PD appears to be positively correlated with the technology's level of immersion. As the most immersive technology,IVR is the optimal choice for im-proving overall motor function, balance and mobility in patients with PD.VRT and AR serve as effective adju-vants and are superior to TAU.
