1.Applied anatomy and clinical application of the perforator flap at the neck of radial first metacarpus

Objective:Through anatomical observations, anatomical data on the radial perforating vessels around the neck of first metacarpus were observed and classified, hence to provide an anatomical evidence for clinical application of the flap based on the perforating vessels.Methods:From July 2022 to June 2023, 15 fresh adult upper limb specimens were studied at the Department of Hand Surgery, Northwest University First Hospital. All perforators within the zone for flap design around the neck of radial first metacarpus were observed. The number, origin and outer calibre of the perforators were recorded. Perforators with an outer diameter greater than 0.13 mm were defined as dominant arteries. SPSS 20.0 statistical software was used to count the number and outer diameter of cutaneous branches originating from the radial artery of the first metacarpal bone, the radial palmar proper artery of thumb and other arteries. The results were expressed as mean ± standard deviation ( ± s), with P<0.05 indicating statistical significance. From July 2023 to May 2024, 14 patients (10 males, 4 females and aged 27-53 years) with digital soft tissue defects were treated. The defects were of 3 thumbs, 4 index fingers, 5 middle fingers, 1 ring finger and 1 little finger, and sized from 2.5 cm × 2.0 cm to 4.0 cm × 2.5 cm. The perforator flaps of the neck of radial first metacarpus (3.0 cm × 2.5 cm - 4.5 cm × 3.0 cm) were harvested and donor sites were closed directly in the emergency surgery. Follow-up through the visits of outpatient clinic and via telephone and WeChat interviews to assess the flap survival, digital function and aesthetics of the flap. Results:In the specimens, it was found that about 75% of dominant perforators were originated from the first metacarpal radial artery. There were statistical significance in the number and outer calibre of the perforators between the first metacarpal radial artery, radial proper palmar digital artery of thumb and other arteries ( P<0.05). Postoperative follow-up lasted for 5-12 (mean 10.1) months, all flaps survived after surgery. Sensation regained according to the British Medical Research Council (BMRC), and they were: S 2 in 6 flaps and S 3 in 8 flaps. According to Evaluation Trial Standards of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association, 11 digits were excellent and 3 in good. Scar evaluation scored 3-4 (mean 3.43) points according to the Vancouver Scar Scale (VSS). Conclusion:The perforator flap around the neck of first radial metacarpus is primarily supplied by the radial artery of the first metacarpus. This flap achieves satisfactory functional and aesthetic outcomes with good sensory recovery, therefore it is an ideal flap for reconstruction of digital soft tissue defect.

Plant-derived nanovesicles have gained attention given their similarity to mammalian exosomes and advantages such as low cost,sustainability,and tissue targeting.Thus,they hold promise for disease treatment and drug delivery.In this study,we proposed a time-efficient method,PEG 8000 combined with sucrose density gradient centrifugation to prepare ginger-derived nanovesicles(GDNVs).Subse-quently,curcumin(CUR)was loaded onto GDNV by ultrasonic incubation.The optimum conditions for ginger-derived nanovesicles loaded with curcumin(CG)were ultrasound time of 3 min,a carrier-to-drug ratio(GDNV:CUR)of 1∶1.The study achieved a high loading capacity(94.027%±0.094%)and encapsu-lation efficiency(89.300%±0.344%).Finally,the drugs'in vivo distribution and anti-colitis activity were investigated in mice.CG was primarily distributed in the colon after oral administration.Compared to CUR and GDNV,CG was superior in improving disease activity,colon length,liver and spleen coefficients,myeloperoxidase activity,and biochemical factor levels in ulcerative colitis(UC)mice.In addition,CG plays a protective role against UC by modulating serum metabolite levels and gut flora.In summary,our study demonstrated that GDNV can be used for CUR delivery with enhanced therapeutic potential.

5.Research progress on aluminum load in children and the mechanism of aluminum neurotoxicity

Abstract
Aluminum is a chronic and cumulative environmental neurotoxin. Human exposure to aluminum is inevitable and impossible to quantify. The level of aluminum load is on the rise, the neurobiological toxicity of which is a cause for great concern. Aluminum intake, absorption, and load levels in the body are often higher in children in stages of rapid growth and development than in ordinary adults. Long term low dose aluminum load levels may cause a variety of chronic subclinical damage and long term irreversible damage to the neurodevelopmental function of children. Therefore, this review primarily elucidates the status of aluminum load in children and its effect on neurodevelopmental functioning. Furthermore, we examine research progress on the related mechanism of impaired neurodevelopmental functioning so as to provide a theoretical basis for the treatment and prevention of health problems caused by aluminum load in children and outline proposals for healthcare in this area.
6.Value of strain means of quantitative parameters in diagnosis of liver fibrosis.

OBJECTIVETo explore the value of the strain means of the quantitative parameters of organization diffusion in determining the degree of liver fibrosis.
METHODSA total of 130 patients with chronic hepatitis B were examined for quantitative analysis of organization diffusion before liver biopsy to analyze the performance of the strain means in liver fibrosis of different degrees. All the cases were confirmed by pathology.
RESULTSThe strain means differed significantly between different stages of liver fibrosis. Using the best cut-off values of 116.2, 111.4, 101.6 and 97.5 for diagnosis of S≥1, S≥2, S≥3 and S=4, the diagnostic sensitivities were 91.2%, 90.8%, 95.8% and 89.7%, with specificities of 90.4%, 98.6%, 93.8% and 95.6% and accuracies of 90.6%, 94.7%, 95.5% and 91.8%, respectively.
CONCLUSIONThe strain mean can provide specific numerical indicators to determine the degree of liver fibrosis.

9.Clinical efficacy of Qingyi decoction combined with DAR for the treatment of acute pancreatitis

Objective To explore the clinical efficacy of Qingyi decoction (a preparation of Chinese herbal medicine) combined with DAR (combined use of dexamethasone,anisodamine and rhubarb) for the treatment of acute pancreatitis.Methods A total of 387 eligible patients met the criteria of acute pancreatitis were enrolled from January 2005 to April 2012 for prospective study.All patients,mild acute pancreatitis (MAP) or severe acute pancreatitis (SAP),were divided (random niumber) into four groups,namely conventional therapy (T),DAR therapy (DAR),Qingyi decoction therapy (Q) and Qingyi decoction combined with DAR therapy (Q + DAR).Outcome,fasting time,serum amylase,abdominal pain relief time,pancreatic or peri-pancreatic complications and average hospital-stay were analvzed with SPSS 13.0 statistic software.P < 0.05 was considered statistically significant.Results None of MAP patients died.Of SAP patients,there was no difference in mortality among different groups (P > 0.05).length of fasting time,tine elapsed for abdominal pain relief,time required for normalized serum amylase level and length of hospital stay in MAP patients were significantly shorter than those in SAP patients regardless of different therapies (P < 0.05).Either patients of SAP or MAP treated with Q + DAR or DAR suffered shorter length of time than those treated with T or Q in respect of fasting,abdominal paiu relief,serum amylase level normalization and hospital stay (P < 0.05).For SAP or MAP patients,there was no difference in abdominal pain relief time between receiving DAR and Q + DAR treatment (P > 0.05),but the fasting time in Q + DAR was shorter than that in DAR (P < 0.05).Patients with SAP were more likely to suffer pancreatic or per-pancreatic complications than those with MAP,but there was no difference for SAP or MAP with different treatments.Conclusions DAR or Q + DAR was an alternative to conventional treatment for MAP or SAP,and they were both superior to conventional treatment.And Q + DAR was more advantageous than DAR when fasting time,hospital-stay time and cost were considered.
10.Evaluation of quantitative analysis of organization diffusion application in diagnosing liver fibrosis

Objective To explore the best cutoff point of the quantitative analysis of organization diffusion for differentiating liver fibrosis and to assess its diagnostic value.Methods Sixty-five patients with chronic hepatitis B were examined with quantitative analysis of organization diffusion before liver biopsy,and made a ROC analysis of diagnostic test.All cases confirmed by pathology.Results 114.95 was the best cutoff point of the strain average value which was one parameter of the quantitative analysis of organization diffusion,the area under the curve of ROC was 0.950 (P=0.000),and the sensitivity,specificity,accuracy were 92.3%,87.2% and 89.2%,respectively.Conclusions The quantitative analysis of organization diffusion is helpful in diagnosing liver fibrosis.
