1.Catheter-directed thrombolysis for acute entire lower extremity deep venous thrombosis: a comparative study of calf deep vein and contralateral femoral venous access

Objective:To compare the clinical efficacy of catheter-directed thrombolysis (CDT) via the contralateral femoral vein approach (CFVA-CDT) and the calf deep vein approach (CVA-CDT) in the treatment of acute mixed-type lower extremity deep vein thrombosis (DVT).Methods:Patients treated with CFVA-CDT and CVA-CDT for acute mixed-type DVT were retrospectively collected from January 2018 to December 2021, totaling 49 and 32 patients, respectively. The relevant technical indicators, thrombolysis rates in the iliac-femoral vein segment and femoral-popliteal vein segment, clinical efficacy, and the incidence of lower extremity deep vein patency, venous valve insufficiency, and post-thrombotic syndrome (PTS), as well as the severity of chronic venous disease in the affected limb (VCSS score) during a 2-year follow-up period were retrospectively compared between the two venous access CDT groups. The t-test was used for comparing quantitative data, while the chi-square test or Fisher′s exact test was used for categorical data.Results:During CFVA-CDT procedure, 6-8 F vascular sheaths were used, and balloon dilation of 2~6 mm was more frequently employed (65.31%, 32/49) to expand venous stenosis/occlusion segments before successful sheath placement compared to the CVA-CDT group (37.50%, 12/32), and the difference was statistically significant ( P=0.014). In the CVA-CDT group, 31.25% (10/32) of patients had a maximum sheath size of 6 F, while the remainder used 4 or 5 F sheaths. Among them, 34.38% (11/32) of patients required re-puncture of the popliteal or femoral vein for larger sheaths (≥8 F) for thrombus aspiration and subsequent endovascular treatment during or after thrombolysis. The effective thrombolysis rates (≥50%) in the iliac-femoral vein segment were not significantly different between the two groups ( P=0.778). The effective thrombolysis rate of the femoral-popliteal venous segment is related to the presence or absence of popliteal vein opacification on lower extremity venous antegrade venography. There was no significant difference between the groups when the popliteal vein was visualized ( P=1.000). While the popliteal vein was not visualized, the CVA-CDT group (75.0%, 15/20) was significantly better than the CFVA-CDT group (34.38%, 11/32), and the difference was statistically significant ( P=0.004). There was no significant difference in clinical efficacy between the two groups ( P=0.819). During follow-up, the femoral-popliteal vein patency rate in the CVA-CDT group (87.50%, 28/32) was significantly higher than in the CFVA-CDT group (44.90%, 22/49), the difference was statistically significant ( P<0.001). Conclusions:Successful CFVA-CDT requires the assistance of more ancillary devices, while the use of larger sheaths is more limited in CVA-CDT due to the smaller caliber of the calf deep veins. The presence or absence of popliteal vein opacification on lower extremity venous antegrade venography may influence the effective thrombolysis of the femoral-popliteal venous segment thrombus in patients with acute mixed deep vein thrombosis (DVT) treated with CFVA-CDT and CVA-CDT. Compared to CFVA-CDT, CVA-CDT can improve the patency rate of the femoral-popliteal venous segment.

Objective To compare the technical indicators and clinical effect of catheter-directed thrombolysis(CDT)via two types of non-popliteal venous access in the treatment of acute mixed-type lower extremity deep vein thrombosis(DVT).Methods The clinical data of 119 patients with acute mixed-type lower extremity DVT,who were admitted to the Affiliated Jiangning Hospital of Nanjing Medical University and the Affiliated Nanjing Hospital of Nanjing Medical University of China to receive CDT treatment from January 2016 to June 2022,were retrospectively analyzed.Of the 119 patients,CDT via deep calf vein access was carries out in 45(calf vein group)and CDT via healthy-side femoral venous access was performed in 74(femoral vein group).The success rate of vascular puncture,success rate of catheterization technique,number of successful CDT venous puncturing,time spent for sheath placement,time spent for catheterization,thrombolysis time,used amount of thrombolytic agent and associated complications(including vein puncturing and anticoagulant thrombolysis-related complications),the thrombolytic effect of different anatomical segments,and the clinical efficacy during the follow-up period for at least 12 months were compared between the two groups.Results Successful catheterization via deep calf vein access and via healthy-side femoral vein access was obtained in 31 and 58 CDT patients respectively,with a technical success rate of 68.89%(31/45)and 78.38%(58/74)respectively,the difference between the two groups was not statistically significant(P=0.248).In 26 patients(67.74%)of the calf vein group,more than two times of puncturing were needed before the sheath placement could be successfully achieved.The time spent for sheath placement in the femoral vein group was(1.84±0.87)min,which was remarkably shorter than(10.52+6.13)min in the calf vein group(P<0.001),but the time spent for catheterization in the femoral vein group was(41.60±13.31)min,which was obviously longer than(20.06+4.46)min in the calf vein group(P<0.001).The thrombolysis time in the femoral vein group and the calf vein group was(5.34+1.43)days and(5.06±1.18)days respectively(P=0.354),and the used amount of thrombolytic agent in the femoral vein group was(352.16±71.98)×104 U,which was prominently larger than(284.68±77.64)× 104 U in the calf vein group(P<0.001).The last follow-up check showed that the patency rate of the popliteal vein in the calf vein group was significantly higher than that in the femoral vein group(P=0.037).No statistically significant differences in the incidence of post-thrombotic syndrome(PTS)and the mean VEINES-QOL/Sym scores existed between the two groups(all P>0.05).Conclusion Compared with CDT via healthy-side femoral vein access,CDT via deep calf vein access can better remove the thrombus in the popliteal vein and superficial femoral vein,and improve the femoropopliteal vein patency rate,although it has no obvious advantages in reducing the occurrence of PTS and in improving the VEINES-QOL/Sym score,moreover,the deep calf vein puncture and sheath placement require a high-level technique.

Objective To investigate the effectiveness of a higher-order thinking-oriented BOPPPS-based blended teach-ing model in otolaryngology-head and neck surgery education,focusing on its impact on the academic performance and teaching satisfaction of undergraduate clinical medicine students.Methods A total of 199 undergraduate clinical medicine students from Guangzhou Medical University were enrolled,divided into a control group(2021-2022 academic year,n=118)and an experi-mental group(2022-2023 academic year,n=81).The control group received conventional blended teaching via Chaoxing plat-form combined with case discussions,while the experimental group implemented the BOPPPS-integrated blended teaching model.Results Students in the experimental group achieved significantly higher average scores than the control group(Δ=11.71 points),with the excellent rate increasing from 0% to 11.1% and the failure rate decreasing to 1.2% .Additionally,the experi-mental group reported high satisfaction with the BOPPPS-integrated blended teaching model,with an overall satisfaction rate of 80.25%.Furthermore,54.32% of students expressed a preference for blended teaching approaches.Students widely acknowl-edged that this model facilitated flexible knowledge application.Conclusion The BOPPPS-integrated blended teaching model ef-fectively enhances the academic performance and teaching satisfaction of undergraduate clinical medicine students,providing a valuable reference for medical education reform oriented toward fostering higher-order thinking and clinical competency.
4.Impact and mechanism of resveratrol on myocardial energy metabolism in exercise-induced fatigue rats

Objective Investigating the effect and mechanism of resveratrol on cardiac function in exercise-induced fatigue rats by regulating mitochondrial energy metabolism.Methods Forty-eight healthy male Sprague Dawley rats(6~8 weeks old)were selected and divided randomly into a blank control(C)group,resveratrol administration(R)group,exercise-induced fatigue(E)group,and exercise-induced fatigue+resveratrol administration group(ER)(n=12 rats per group).Rats in the E and ER groups were subjected to weight-bearing 5%of their body weight for 60 minutes each day and exercise for 6 days per week for 6 weeks.Rats in the R and ER groups received resveratrol 50 mg/kg by gavage,1 hour after exercise.The biochemical kit was used to detect the indicators of motor fatigue,myocardial injury and mitochondrial energy metabolism enzymes in rat myocardium.The mRNA expression levels of myocardial energy metabolism related regulatory factors were detected by real-time fluorescent quantitative PCR(RT-qPCR).Results Plasma levels of blood urea nitrogen(BUN)and creatine kinase(CK)were significantly increased in rats in group E compared with group C(P<0.05,P<0.05),CK-MB and cTnI activity were also significantly increased(P<0.01,P<0.05),while activity levels of myocardial cytochrome C oxidase(COX)and succinate dehydrogenase(SDH)were significantly reduced(P<0.01,P<0.05).mRNA expression levels of the mitochondrial energy metabolism-related genes silent information regulator 1(SIRT1),peroxisome proliferator-activated receptor γ coactivator 1-α(PGC-1α),and estrogen receptor-related receptor α(ERRα)were also significantly reduced(P<0.01,P<0.01,P<0.01).Serum levels of BUN and CK were significantly decreased in the ER group compared with group E(P<0.05,P<0.05),while CK-MB and cTnI activities were significantly decreased(P<0.05,P<0.05),myocardial COX and SDH activities were significantly increased(P<0.01,P<0.05),and SIRT1,PGC-1α and ERRα mRNA levels were also significantly increased(P<0.05,P<0.01,P<0.01).Conclusions Resveratrol can effectively activate the sirtuin 1/peroxisome proliferator-activated receptor γ coactivator 1α/estrogen-related receptor α signaling pathway,improve myocardial energy metabolism in exercise-fatigued rats,and protect against myocardial injury.

8.Observation of analgesic efficacy of liposomal bupivacaine for local infiltration anesthesia in unicompartmental knee arthroplasty: a prospective randomized controlled study.

OBJECTIVE:
A prospective randomized controlled study was conducted to investigate the early postoperative analgesic effectiveness of using liposomal bupivacaine (LB) for local infiltration anesthesia (LIA) in unicompartmental knee arthroplasty (UKA).
METHODS:
Between January 2024 and July 2024, a total of 80 patients with knee osteoarthritis (KOA) who met the selection criteria were enrolled in the study. Patients were randomly assigned to either the LB group or the "cocktail" group in a 1∶1 ratio using a random number table, with 40 patients in each group. Baseline characteristics, including gender, age, body mass index, operated side, Kellgren-Lawrence grade, and preoperative American Society of Anesthesiologists (ASA) classification, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, and knee joint range of motion, showed no significant difference between the two groups ( P>0.05). Both groups received LIA and comprehensive pain management. The surgical duration, incision length, pain-related indicators [resting and activity visual analogue scale (VAS) scores, total dosage of oral morphine, WOMAC scores], knee joint range of motion, first ambulation time after operation, length of hospital stay, and postoperative adverse events.
RESULTS:
There was no significant difference between the two groups in surgical duration, incision length, first ambulation time after operation, length of hospital stay, total dosage of oral morphine, and pre-discharge satisfaction with surgery and WOMAC scores ( P>0.05). At 4, 12, and 24 hours after operation, the resting and activity VAS scores in the "cocktail" group were lower than those in the LB group; at 60 and 72 hours postoperatively, the resting VAS scores in the LB group were lower than those in the "cocktail" group, with the activity VAS scores also being lower at 60 hours; all showing significant differences ( P<0.05). There was no significant difference in the above indicators between the two groups at other time points ( P>0.05). On the second postoperative day, the sleep scores of the LB group were significantly higher than those of the "cocktail" group ( P<0.05), while there was no significant difference in sleep scores on the day of surgery and the first postoperative day ( P>0.05). Additionally, the incidence of complications showed no significant difference between the two groups ( P>0.05).
CONCLUSION
The use of LB for LIA in UKA can provide prolonged postoperative pain relief; however, it does not demonstrate a significant advantage over the "cocktail" method in terms of short-term analgesic effects or reducing opioid consumption and early functional recovery after UKA. Nevertheless, LB may help reduce postoperative sleep disturbances, making it a recommended option for UKA patients with cardiovascular diseases and insomnia or other mental health issues.

9.Analysis of serum humoral immunity related indicators in patients with occupational medicamentose-like dermatitis due to trichloroethylene

Objective To analyze the changes of serum humoral immunity related indicators, including immunoglobulins (Ig), complement and C-reactive protein (CRP) in patients with occupational medicamentose-like dermatitis due to trichloroethylene (OMDT) at different stages. Methods A total of 131 OMDT patients were selected as the study subjects using a retrospective analysis method, and liver function and humoral immunity related indicators were collected for analysis in the early stage and the recovery stage of the disease. Results The abnormality of liver function among study subjects was 89.3% (117/131) in the early stage of the disease, with mild, moderate, and severe liver damage accounting for 61.8%, 5.3%, and 22.1%, respectively. The levels of serum alanine aminotransferase, aspartate aminotransferase, γ-glutamyl transferase, total bilirubin, alkaline phosphatase, total bile acid, IgG and CRP in the early stage increased ( all P<0.01), while the level of serum cholinesterase and C3 decreased (both P<0.01) compared with the recovery stage. However, there was no significant difference in the level of serum albumin, globulin, IgA, IgM and C4 or albumin/globulin ratio between the early stage and the recovery stage (all P>0.05). The level of patients′ serum IgA in the group with severe liver function injury was lower than that in the group with mild to moderate liver function injury in the recovery stage (P<0.05). The level of serum C3 in the patients with severe liver function injury was lower than that in the group with mild to moderate liver function injury in the early stage (P<0.05). Conclusion Serum IgG and CRP levels increased and C3 level decreased in OMDT patients at the early stage of the disease, which was correlated with the degree of liver function injury. It is suggested that humoral immune response is involved in the autoimmune liver injury in OMDT patients.

Objective:To investigate the effect of short-chain fatty acid (SCFA) sodium butyrate (NaB) on the polarization of lipopolysaccharide(LPS) induced M1 type alveolar macrophages and the mechanism of action.Methods:Mouse alveolar macrophages (MH-S) were randomly(random number) divided into control group (Control group), sodium butyrate group (NaB group), LPS group, LPS+NaB group (LB group), and LPS+NaB+adenylate activated protein kinase (AMPK) inhibitor (Compound C) group (LC group).The mRNA expression levels of interleukin 6 (IL-6), interleukin 1β(IL-1β), tumor necrosis factor α(TNF-α), cluster of differentiation 86 (CD86), inducible nitric oxide synthase(iNOS) in MH-S cells, and zonula occludens 1 (ZO-1), tight junction protein 4(Claudin-4), and closed protein(Occludin) in mouse lung epithelial cells (MLE-12) were detected by qRT-PCR;Protein levels of IL-6, IL-1β, and TNF-α in the supernatant of MH-S cell medium were measured by ELISA;Western blot determed the protein expression of AMPK, P-AMPK, nuclear factor E2-related factor 2 (Nrf2), and heme oxygenase 1 (HO-1) in MH-S cells;Expression of M1 type macrophage-associated markers CD86 and iNOS were determined by flow cytometry.Results:(1) qRT-PCR and ELISA results were consistent, M1 type macrophage-associated proinflammatory cytokines IL-6, IL-1β and TNF-α significantly reduced in the LB group after NaB addition compared with the LPS ground (all P <0.05); (2)The results of qRT-PCR and flow cytometry were consistent,compared with the LPS group, the LB group showed a significant decrease in M1 type macrophage-related polarization indicators CD86 and iNOS after NaB addition(all P<0.05); (3) Western blot was used to detect the expression of the AMPK/Nrf2/HO-1 signaling pathway,compared with LPS,the addition of NaB in the LB group enhanced the expression of P-AMPK/AMPK, Nrf2 (nucleus), and HO-1 (all P<0.05); compared with the LB group, the LC group decreased the expression of P-AMPK/AMPK, Nrf2 (nucleus), and HO-1 (all P<0.05);the results of flow cytometry showed that compared with the LPS group, the addition of NaB significantly decreased the expression level of iNOS + in the LB group ( P<0.05); compared with the LB group, the addition of Compound C in the LC group reversed the inhibitory effect of NaB on iNOS + ( P <0.05);(4) The qRT-PCR results of MLE-12 cells showed that compared with the LPS group, the LB group showed a significant increase in Z0-1, Claudin-4, and Occludin after the addition of NaB(all P<0.05). Conclusions:SCFA inhibits LPS-induced polarization of M1-type alveolar macrophages and ameliorates the inflammatory response by activating the AMPK/Nrf2/HO-1 signaling pathway.
