Objective To investigate CT manifestations of pulmonary graft-versus-host disease(GVHD)after allogeneic hematopoietic stem cell transplantation(allo-HSCT)in children.Methods A total of 311 children with pulmonary GVHD who underwent allo-HSCT were retrospectively analyzed.According to the onset time of pulmonary GVHD within 100 days after allo-HSCT or not,the patients were divided into acute group(n=82)and chronic group(n=229).Chest CT manifestations were observed and compared between groups,and children survival after treatments were recorded an d compared between groups.Results The main CT manifestations of pulmonary GVHD after allo-HSCT in children included ground glass opacity(GGO)(82/311,26.37％),air trapping(78/311,25.08％),thickening bronchial wall(32/311,10.29％),bronchiectasis(28/311,9.00％),pleural effusion(43/311,13.83％),thickening interlobular septum(43/311,13.83％),thickening pleura(41/311,13.18％)and thoracic air leak syndrome(41/311,13.18％).The proportion of localized GGO,diffuse GGO,pleural effusion and thickening interlobular septum in acute group were all higher,while of air trapping and bronchiectasis were both lower than those in chronic group(all P＜0.05).No significant difference of other menifeatations was found between groups(all P＞0.05).After treatments,85 cases died,and the mortality rate was 27.33％(85/311),which in acute group(30/82,36.59％)was higher than in chronic group(55/229,24.02％)(x2=4.802,P=0.031).Conclusion CT manifestations of pulmonary GVHD after allo-HSCT in children had certain characteristics.

Objective To explore the expression and clinical significance of microRNA (miR)-155 and miR -222 in plasma of patients with ventricular septal defect(VSD),and to analyze the possible mechanism.Methods A total of 20 children with VSD who received treatment at the Department of Cardiovascular Surgery from August 2012 to June 2013 were enrolled (the VSD group)and 15 patients with fracture (the control group).The plasma miR -155 and miR -222 expression levels were measured by real -time quantitative reverse transcription -polymerase chain reaction (RT -qPCR).The potential target genes of miR -155 and miR -222 were predicted by using 3 current-ly available prediction programs,including TargetScan,mirbase and Miranda,and the signaling pathway of miRNA was predicted by Pathway -express analysis.Results Compared with the control group,the expression levels of miR -155 (P =0.033)and miR -222(P <0.001)in the VSD group decreased significantly;miR -155 and miR -222 predic-ted target genes included 74 and 50,respectively.The Pathway -express analysis indicated that 7 signaling pathways played important roles in the occurrence of fetal VSD,including signaling pathways for heart development,such as:mito-gen -activated protein kinase(MAPK)signaling pathway.Conclusions The expression levels of plasma miR -155 and miR -222 in VSD were significantly decreased.The target genes were related to signaling pathways for heart deve-lopment (MAPK signaling pathway),which indicates that miR -155 and miR -222 may be involved in the pathological process of VSD,and may serve as an independent evaluation indicator for the diagnosis of VSD.

Objective To explore risk factors of different locational leukoaraiosis in elderly patients with cerebral infarction.Methods Patients were collected who aged 60 to 85 with cerebral infarctions came to the Fourth Affiliated Hospital of China Medical University and took cerebral magnetic resonance imaging (MRI) [fluid-attenuated inversion recovery (FLAIR), T1, and T2].The patients were divided into periventricular white matter changes (PVWMCs) and deep white matter changes (DWMCs), and scored by Fazekas scale, respectively.We registered past medical history, medical systems and neurological examination, blood lipids, regular blood test, and resting blood pressure measurements in the morning.Univariate analysis was used to choose risk factors which ~fected white matter changes.After that, rank multivariate logistic regression analysis was used to test risk factors of PVWMCs and DWMCs, respectively.Results There were significant age differences in both PVWMCs and DWMCs, the degree of risk was associated with age.Conclusions PVWMCs and DWMCs risk factors vary, and we should take control of risk factors actively for the elderly in order to reduce the harm arising from WMC.

Objective:To analyse the biological function of anti-IL-6Rβ(gp130) monoclonal antibody and its regulatory effect on IL-6 signaling.Methods:Biological characteristics of anti-IL-6Rβ(gp130) mAb were assessed by Western blot analysis, capture ELISA and peptide ELISA .The phosphorylation of STAT 3 was tested by Western blot analysis in IL-6-stimulated U266/RA-FLS/RA-PBMC with or without anti-IL-6Rβ(gp130) mAb treatment.Results:3 strains of mouse anti-human gp130 mAb were with high affini-ty and different binding epitopes , the kaff of 10A1 was 2.62E-10.In U266, RA-PBMC and RA-SFMC, IL-6 signaling highly activated STAT3 which could be inhibited by anti-gp130 mAb.Conclusion: Anti-IL-6Rβ( gp130 ) mAb might have different binding epitopes and could affect IL-6 stimulated phosphorylation of STAT3, which provides a preliminary experiment for analyse the correlation of IL-6 signaling and RA .

Objective To explore the feasibility and curative effect of extracellular matrix (ECM) fibrinolytic therapy in order to cure radiation-induced brachial plexopathy (RIBP) and to discuss the principle, indication and precautions. Methods The treatment was taken on 16 cases that had a definite clinical diagnosis and we took a systematic examination to make sure that there was no recurrence or metastasis of the tumor.Brachial plexus and tender area block was taken once a week by injection of mixture of hyaluronidase, hexadecadrol, Vitamine B12 and lidocaine. The therapy was applied for 6 to 12 weeks. Results All the patients were followed up for a period of 0.5 to 10 years. Four cases had an evident alleviation of the symptoms and got a function resumption of hands; 5 cases had an alleviation of pain and improvement of sensation and the progress of illness were terminated, but motor function didn't have improvement; 7 cases had a continued aggravation of the symptoms. All the cases found improvement in electromyography. Concluslon The aim of fibrinolytic therapy is to dissolve the extra ECM such as hyaluronic acid around nerves in order to achieve a new homeostasis. It can relieve the conglutination and entrapment at nerve inside and outside, reduce the pressure of neuraxon and create an availing condition for the regenesis of nerve and the regain of function.

Objective To observe expression and significance of inflammatory factor during perioperative and follow-up periods after endovascular stents in patients with cerebral arterial stenosis.Methods 54 patients diagnosed as cerebral arterial stenosis by digital substraction angiography (DSA) were selected as the stent group and treated with endovascular stents; another 32 subjects had the same disease but not accepted stenting were considered as the control group. Interleukin-6 (IL-6) and high-sensitivity C-reactive protein (hs-CRP) contents were measured at different time points during perioperative and follow-up periods in the two groups. Stage A represented as day one before angiography or catheterization; stage B as 6 hours postoperatively (stent group) or 6 hours after diagnostic angiography (control group); stages C～I as 12, 24, 48, 72 hours, 1 and 6 months after stent insertion (stent group) or the same time points after angiography (control group).Results Contents of IL-6 and hs-CRP of the stent group were similar as the control group in the stage A ( P>0.05), but significantly higher than that of the control group during stages B～I ( P<0.01～0.05). Among 54 patients of the stent group, 21 cases had restenosis 6 months postoperatively (38.89%). Contents of IL-6 and hs-CRP of the patients were similar as those without restenosis in stages A～F postoperatively ( P>0.05), but significantly higher than that of the cases without restenosis in stages G and I postoperatively ( P<0.01～0.05).Conclusion Endovascular stents can increase the contents of IL-6 and hs-CRP of patients with cerebral arterial stenosis; in addition, high expression of them is the risk factor of post-stent restenosis during follow-up period.

OBJECTIVE To investigate the occurrence and the genotype character of AmpC ?-lactamases-producing Escherichia coli isolated from the Second Hospital affiliated to Harbin Medical University.METHODS K-B disk diffusion test and FOX(≤17mm) test were used as initial screen tests to detect the clinical isolates;AmpC ?-lactamases were confirmed by three-dimentional extract tests;multiple-PCR was used for detecting plasmid-mediated AmpC gene and their genotypes were determined by DNA sequencing;the regulator genes of high producing AmpC isolates of E.coli were cloned to pUCm vectors and sequenced,the difference among them was analyzed by blast method.RESULTS Among total 586 E.coli clinical isolates,10 isolates(1.70%) resisted to cefoxitin;three-dimensional extract tests were positive for all 10 isolates;4 isolates of E.coli were plasmid-mediated DHA-1 type AmpC ?-lactamases by using multiplex PCR and DNA sequencing;6 isolates of chromosomal high level AmpC ?-lactamases-producing E.coli were sequenced and contrasted to that of E.coli K12,showed that they were gene polymorphism.CONCLUSIONS E.coli clinical isolates producing high level AmpC ?-lactamases not only acquire plasmid-mediated DHA-1 AmpC ?-lactamases but also cause by chromosomal ampC promotor or attenuator gene mutations in our hostiptal.
