OBJECTIVE To compare the efficacy and safety of evolocumab and probucol in patients with ultra-high-risk atherosclerotic cardiovascular disease （ASCVD） following percutaneous coronary intervention （PCI）. METHODS A retrospective analysis was conducted on 156 ultra-high-risk ASCVD patients who underwent PCI in our institution between January 1， 2023 and December 31， 2024. According to the lipid-lowering regimen， the patients were categorized into evolocumab group （ n ＝86） and probucol group （ n ＝70）. Changes in lipid parameters [total cholesterol （TC）， low-density lipoprot ein cholesterol （LDL-C）， high-density lipoprotein cholesterol （HDL-C）， triglycerides， lipoprotein （a）， and lipid goal achievement rate ] ， inflammatory markers [interleukin-6 （IL-6） and C-reactive protein （CRP） ] ， and cardiac function indices （left ventricular ejection fraction， left ventricular end-systolic diameter， left ventricular end-diastolic diameter， and N-terminal pro-B-type natriuretic peptide） were compared between two groups at baseline and after 6 months of treatment. The incidence of adverse clinical events during treatment， including acute myocardial infarction， in-stent restenosis， acute heart failure， cerebral hemorrhage， and stroke， was also evaluated. RESULTS No statistically significant differences were observed between the two groups at baseline （ P ＞0.05）. After 6 months of treatment， both groups demonstrated significant improvements in lipid profiles （except HDL-C） and inflammatory markers compared to those at baseline （ P ＜0.05）. The evolocumab group exhibited greater reductions in TC， LDL-C， IL-6， and CRP， along with a higher lipid target achievement rate， compared with the probucol group （ P ＜0.05）. There were no statistically significant differences in the cardiac function-related indicators before and after treatment between the two groups， nor in the incidence of adverse events during the treatment （ P ＞0.05）. CONCLUSIONS For ultra-high-risk ASCVD patients after PCI， both of the above treatment options are associated with improvements in blood lipid and inflammatory response， with good safety during short-term follow-up. Evolocumab shows superior efficacy in TC， LDL-C and inflammatory markers reduction and lipid target achievement， compared to probucol.

ObjectiveTo observe the clinical effectiveness and safety of Qingfei Shenshi Decoction （清肺渗湿汤） combined with western medicine for patients with bronchial asthma of heat wheezing syndrome， and to explore its potential mechanism of action. MethodsEighty-six participants with bronchial asthma of heat wheezing syndrome were randomly divided into treatment group and control group， each group with 43 participants. The control group received conventional western medicine， and the treatment group was additionally administered Qingfei Shenshi Decoction orally on the basis of the control group， 1 dose per day. Both groups were treated for 14 days. The primary outcome measure was clinical effectiveness； secondary outcome measures included traditional Chinese medicine （TCM） syndrome score， asthma control test （ACT） score， pulmonary function indices such as forced expiratory volume in 1 second （FEV₁）， forced vital capacity （FVC）， peak expiratory flow （PEF）， serum inflammatory factor levels including interleukin-4 （IL-4）， tumour necrosis factor-α （TNF-α）， and high-sensitivity C-reactive protein （hs-CRP）， and immune function indices including CD3⁺， CD4⁺， CD8⁺， CD4⁺/CD8⁺. All outcome measures were evaluated before and after treatment. Vital signs were monitored， and electrocardiography， blood routine， urine routine， liver function， and renal function tests were performed before and after treatment. Adverse events and reactions during the study were recorded. ResultsA total of 80 patients completed the trial with 40 in each group. The total clinical effective rate of the treatment group was 97.5% （39/40）， which was significantly higher than that of the control group （85.0%， 34/40， P＜0.05）. After treatment， both groups showed decreased TCM syndrome scores， IL-4， TNF-α， hs-CRP， and CD8⁺ levels， as well as increased ACT scores， CD3⁺， CD4⁺， CD4⁺/CD8⁺， FEV₁， FVC， and PEF levels （P＜0.05 or P＜0.01）. Moreover， the improvements in these indices were more significant in the treatment group than in the control group （P＜0.05 or P＜0.01）. No significant abnormalities in safety indicators were observed in either group， and no adverse events or reactions occurred. ConclusionQingfei Shenshi Decoction combined with conventional western medicine for patients with bronchial asthma of heat wheezing syndrome can effectively improve the clinical symptoms， pulmonary function， and clinical effectiveness， with good safety. Its mechanism may be related to reducing inflammatory factor levels and regulating T lymphocyte subsets to improve immune function.

Objective In order to understand the current resistance status of Aedes albopictus(Ae. albopictus) adult population to commonly used insecticides and the resistance dynamics development in Zhejiang Province and provide a theoretical basis for the scientific and rational use of hygienic insecticides. Methods Resistance determination was carried out using the diagnostic dose method(GB/T 26347-2010)recommended by WHO in the adult mosquito contact cylinder method to monitor the resistance of Ae. albopictus field populations in 11 cities and districts in Zhejiang Province in 2024. Results The monitoring result showed that adult Ae. albopictus in 11 areas were resistant or suspected resistant populations to both cis-cypermethrin and permethrin; suspected and resistant populations to the organophosphate insecticide malathion were as high as 81.82%; while suspected resistance to the carbamate insecticide residual carbofuran accounted for 45.45%, and the rest of the monitoring sites were sensitive to it. Conclusions The result suggest that the generalized resistance of Ae. albopictus populations to pyrethroids and organophosphorus insecticides in the field in Zhejiang Province needs to be highly emphasized. In future mosquito vector and mosquito-borne disease control, insecticides should be selected based on the resistance monitoring information, while following scientific guidelines for insecticide use.

Objective To investigate the types and incidence of bifid mandibular canal using cone beam computed tomography(CBCT)technology so as to enhance our understanding of these anatomical features and help reduce complications caused by trauma to the neurovascular bundle within them.Methods CBCT data of 803 patients from College of Stomatology,Xi'an Jiaotong University,were collected,and the types and incidence of bifid mandibular canal were statistically analyzed according to the Naitoh classification method.Results The incidence of bifid mandibular canal was 54.05％,with the occurrence rates for the retromolar canal and forward canal being 26.40％ and 32.75％,respectively.Conclusion To ensure the safety of treatments,it is recommended that oral clinical practitioners use CBCT to obtain three-dimensional images for precise assessment of bifid mandibular canal,thereby avoiding potential complications during the perioperative period.

Objective:To compare the efficacy and safety of irradiation-incorporated and chemotherapy only-based myeloablative conditioning regimens in haploidentical hematopoietic stem cell transplantation (haplo-HSCT) for patients with high-risk myeloid malignancies.Methods:This study retrospectively collected clinical data from 63 high-risk acute myeloid leukemia/myelodysplastic syndrome (AML/MDS) patients who underwent haplo-HSCT at the Fifth Medical Center of the Chinese PLA General Hospital from January 2015 to December 2019. These patients were classified into the irradiation ( n = 17) and chemotherapy ( n = 46) groups based on different conditioning regimens. The differences between the two groups were compared in terms of hematopoietic reconstitution, cumulative incidence of acute/chronic graft-versus-host diseases (aGVHD and cGVHD), non-relapse mortality (NRM), relapse rate (RR), overall survival (OS), and disease-free survival (DFS), followed by the analysis of prognostic factors. Results:The median follow-up time for the irradiation and chemotherapy groups was 78.5 and 72.3 months, respectively. The median time for neutrophil engraftment was 14.0 days in the irradiation group and 14.5 d in the chemotherapy group, and for platelet engraftment was 15.0 and 13.0 d, respectively. As a result, the two groups showed no statistically significant differences in hematopoietic reconstitution ( P > 0.05). The cumulative incidence of aGVHD and cGVHD was higher in the irradiation group compared to the chemotherapy group, yet showing no statistically significant differences ( P > 0.05). Specifically, the cumulative incidence of grade Ⅱ-Ⅳ aGVHD within 100 d was 29.4% and 21.7% for the irradiation and chemotherapy groups, respectively. The cumulative incidence of grade Ⅲ-Ⅳ aGVHD was 23.5% and 13.0%, respectively. The cumulative incidence of severe cGVHD within five years was 11.8% in the irradiation group and 8.7% in the chemotherapy group. In terms of long-term survival, the cumulative 5\|year RR and NRM were 20.2% and 28.4% in the irradiation group, 5.9% and 23.9% in the chemotherapy group, respectively, showing no statistically significant differences ( P > 0.05). The 5-year DFS and OS rates were 73.9% and 47.7% in the irradiation group, and 81.1% and 54.4% in the chemotherapy group, respectively, without statistically significant differences ( P > 0.05). Notably, the irradiation group manifested more favorable DFS and OS survival curves compared to the chemotherapy group. The survival curves indicate that the irradiation-incorporated regimen exhibited better trends in OS, DFS, and cGVHD-free relapse-free survival (GRFS). However, multivariate analysis did not reveal that irradiation conditioning is an independent prognostic factor affecting survival [ HR = 0.532 (0.163-1.735), 0.370 (0.091-1.516), 0.683 (0.248-1.882), P > 0.05]. Conclusions:In haplo-HSCT for high-risk myeloid malignancies, the irradiation-incorporated conditioning regimen demonstrates lower RR and NRM, higher DFS and OS, and potentially superior survival outcomes compared to the chemotherapy only-based regimen. Therefore, the irradiation-incorporated conditioning regimen may be preferentially considered in haplo-HSCT.

Objective:To investigate the clinical effectiveness of vitamin D combined with conventional anti-infective therapy and formula feeding in the treatment of preterm infants with infectious diseases.Methods:A total of 150 premature infants with infectious diseases treated in Hebei Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from January to December 2022 were prospectively selected as the study objects, and divided into the control group and the study group according to the admission sequence number, with 75 cases in each group. The control group was treated with routine anti-infection regimen and formula feeding, and the study group was combined with early vitamin D supplementation on the basis of the control group. Both groups were treated continuously for 4 weeks. Serum prealbumin (pre-Alb), 25-hydroxyvitamin D[25-(OH)D], immune indexes and inflammatory response indexes were compared between the two groups before and after treatment, and neonatal complications and adverse reactions were compared between the two groups.Results:After treatment, the levels of serum 25-(OH)D and pre-Alb in the study group were higher than those in the control group: (37.58 ± 4.02) nmol/L vs. (27.20 ± 3.75) nmol/L, (141.27 ± 16.79) mg/L vs. (132.83 ± 16.36) mg/L, there were statistical differences ( P<0.05). After treatment, the levels of serum complement C3, C4, immunoglobulin (Ig) M and IgG in the study group were higher than those in the control group : (0.92 ± 0.15) g/L vs. (0.77 ± 0.19) g/L, (0.18 ± 0.05) g/L vs. (0.15 ± 0.04) g/L, (0.24 ± 0.04) g/L vs. (0.22 ± 0.05) g/L, (8.07 ± 1.05) g/L vs. (7.68 ± 1.13) g/L, there were statistical differences ( P<0.05). After treatment, the serum levels of interleukin-2 (IL) in the study group was higher than that in the control group, while the levels of IL-4, amyloid A (SAA), soluble human tumor necrosis factor receptor 1 (sTNFR-1) and Toll-like receptor 4 (TLR4) in the study group were lower than those in the control group: (4.61 ± 1.14) pg/L vs. (3.81 ± 0.91) pg/L, (20.53 ± 2.61) pg/L vs. (23.97 ± 3.12) pg/L, (5.56 ± 1.78) mg/L vs. (7.24 ± 1.93) mg/L, (4.16 ± 1.27) mg/L vs. (6.08 ± 1.54) mg/L, (75.21 ± 7.39) mg/L vs. (81.42 ± 8.17) mg/L, there were statistical differences ( P<0.05). The incidence rates of neonatal pneumonia, necrotizing enterocolitis (NEC) and metabolic bone disease in the study group were lower than those in the control group: 1.33%(1/75) vs. 10.67%(8/75), 1.33%(1/75) vs. 10.67%(8/75), 0 vs. 9.33%(7/75), there were statistical differences ( P<0.05). Conclusions:Vitamin D supplementation can effectively maintain the nutritional status of premature infants with infectious diseases and improve the immune system function, regulate inflammation and reduce complications.

Autoimmune oophoritis is a rare cause of primary ovarian insufficiency(POI), and some patients may also have concurrent adrenal insufficiency. This condition significantly impairs reproductive function and is often difficult to distinguish from common causes of POI, leading to missed or incorrect diagnoses. This paper reports a case of autoimmune oophoritis treated in our department and provides an updated summary of recent advances in the understanding of this condition, with the aim of raising clinical awareness and improving diagnostic accuracy among healthcare professionals.

OBJECTIVE To analyze the risk factors for excessive microbial contamination level tested for repaired endoscopes before reuse and to formulate targeted management strategies,providing a reference for handling such events.METHODS A total of 54 repaired endoscopes reused in the Digestive Endoscopy Room of a hospital in Shanxi Province from Oct.2021 to Jun.2024,with a total of 105 repairs,were selected as the study subjects.Mi-crobial contamination levels were tested for all repaired endoscopes before reuse,and they were divided into a posi-tive group(colony count＞20 CFU/item)and a negative group(colony count ≤20 CFU/item)based on the test results.The risk factors for excessive microbial contamination levels of repaired endoscopes before reuse were summarized.The positive rates of microbial contamination level tests for repaired endoscopes before reuse were compared between the pre-intervention period(from Oct.2021 to Jun.2024)and the post-intervention period(from Jul.2024 to Dec.2024).RESULTS The results of microbial contamination level tests of endoscopes with 105 repairs before reuse showed a positive rate of 27.62％(29/105).Shortening duration between cleaning and disin-fection before and after repair(OR=0.285)was a protective factor,scratches/grooves/leak repairs in the endo-scope tubes(OR=3.211),improper cleaning and disinfection(OR=5.257)and the less number of enzymatic brushing washes(OR=2.438)were identified as risk factors for excessive microbial contamination levels of re-paired endoscopes before reuse(P＜0.05).Before the intervention,the positive rate of microbial contamination level tests for repaired endoscopes was higher than that for non-repaired endoscopes(27.62％vs.3.45％,P＜0.001).After the intervention,there was no statistically significant difference in the positive rate of microbial con-tamination level tests between repaired and non-repaired endoscopes(5.26％vs.4.17％,P=0.681),but the posi-tive rate for repaired endoscopes was lower than that before the intervention(5.26％vs.27.62％,P=0.004).CONCLUSIONS The occurrence of excessive microbial contamination levels of repaired endoscopes before reuse frequemly occur.Formulating and implementing targeted management strategies may guarantee the qualified rate of cleaning and disinfection of endoscopes before the reuse,and enhance the safety for the reuse of repaired endoscopes.

Objective:To investigate the dynamic changes of serum long non-coding ribonucleic acid antisense non-coding RNA in the INK4 locus (LncRNA ANRIL), long non-coding ribonucleic acid maternally expressed gene 3 (LncRNA MEG3) and inflammatory factors [interferon-γ (IFN-γ), interleukin-6 (IL-6), interleukin-18 (IL-18) and interleukin-1β (IL-1β)] in patients with acute ischemic stroke and their correlation with neurological impairment.Methods:This prospective study included fifty-two patients with acute ischemic stroke admitted to the Central Hospital of Dalian University of Technology were included, and the neurological impairments of the patients were scored using the National Institutes of Health stroke scale (NIHSS) on the first day of the onset of the disease, and serum LncRNA ANRIL, LncRNA MEG3 and inflammatory factors (IFN-γ, IL-6, IL-18 and IL-1β) were detected using enzyme-linked immunosorbent assay (ELISA) at the first, second and seventh day of the onset of the disease. The correlation between serum LncRNA ANRIL, LncRNA MEG3 and inflammatory factor levels and neurological impairment at each time point was analyzed by Spearman correlation analysis.Results:Among 52 patients, mild group had 22 cases (NIHSS score <5 scores), and moderate-to-severe group had 30 cases (NIHSS score ≥5 scores). In acute ischemic stroke patients with moderate-to-severe group, the expression level of LncRNA MEG3 was higher than in the mild group, but the intergroup difference did not reach statistical significance ( P>0.05). LncRNA ANRIL, LncRNA MEG3, IFN-γ, IL-18 and IL-1β peaked at the same time point in acute ischemic stroke patients, all within second day of the onset of the disease. Spearman correlation analysis showed that the serum LncRNA MEG3 level on the first day of the onset of the disease was significantly positively correlated with NIHSS score ( P = 0.006), and serum LncRNA ANRIL level on seventh day was significantly positively correlated with NIHSS score ( P = 0.049). Conclusions:Serum LncRNA ANRIL, LncRNA MEG3 and inflammatory factors (IL-18, IL-1β and IFN-γ) are highly expressed in patients with acute ischemic stroke, peaking on the second day of onset. LncRNA MEG3 levels on the first day of onset and LncRNA ANRIL levels on the seventh day of onset are helpful in determining the degree of neurological impairment in patients with acute ischemic stroke.

Objective:To analyze the association of sedentary time and physical activity with the risk of developing chronic obstructive pulmonary disease (COPD) in non-smoking women aged 40 years and above in Songjiang District, Shanghai.Methods:Based on a natural population-based cohort in Songjiang, a total of 18 707 non-smoking women who were aged 40 years and above and without COPD at baseline survey were enrolled in the study. Cox proportional risk regression model was used to analyze the associations of the duration of sedentary behavior, physical activity with the risk for COPD at baseline survey, and the hazard ratio ( HR) of risk for COPD and its 95% CI were calculated. Stratified analyses were performed based on age, BMI, history of respiratory diseases and so on. Sensitivity analyses were conducted by excluding the cases diagnosed with COPD within one year after the baseline survey. Results:As of March 31, 2024, a total of 691 new COPD cases had been recorded after a median follow-up time of 6.96 years with an incidence density of 53.22 per 10 000 person-years. After adjusting for relevant confounders, in the tertile subgroups of sedentary time, the risk for COPD reduced by 17% in the short sedentary time group compared with the long sedentary time group ( HR=0.83,95% CI:0.70-0.99). Compared with the low physical activity level and long sedentary time group, the risk for COPD reduced by 24% in the high physical activity level and short sedentary time group ( HR=0.76, 95% CI: 0.61-0.95) and by 23% in the low physical activity level and short sedentary time group ( HR=0.77, 95% CI: 0.60-0.97). Compared with the non-physical exercise and long sedentary time group, the risk for COPD reduced by 28% in the non-physical exercise and short sedentary time group ( HR=0.72, 95% CI: 0.60-0.87). These associations remained when the cases diagnosed with COPD within one year of the baseline survey were excluded. Conclusions:Increasing physical activity and reducing sedentary time have beneficial effects to prevent COPD in non-smoking women, and reducing sedentary time alone may also reduce the risk for COPD if increasing physical exercise or other physical activity is not possible.
