Objective To investigate the causal relationship between circadian rhythm disruption and inflammatory bowel disease(IBD)and the mediating effect of gut microbiota based on Mendelian randomization(MR).Methods Summary statistics of Genome-wide Association Study(GWAS)for circadian rhythm disruption(n=205 527)and IBD(n=214 053)were obtained from IEU OpenGWAS database.Summary statistics of GWAS for the gut microbiota were obtained from the MiBioGen database(n=18 340).Two-sample MR analysis was used to estimate the genetic correlation and causality between circadian rhythm disruption and IBD,and the mediating effect of the gut microbiota was analyzed by two-step MR analysis.Results There was a suggestive causal relationship between circadian rhythm disruption and IBD(odds ratio=1.255,P＜0.05).Rikenellaceae id.967 played a mediating role in the causal chain between them(the mediating effect was-0.028 740).Sensitivity analysis confirmed that the results were not interfered by level pleiotropy and heterogeneity.Conclusion There is a genetic correlation between circadian rhythm disruption and IBD,and gut microbiota may play a mediating role between them.

Objective: To analyze the characteristics of first-visit cases of herpes zoster in Zhoushan City, Zhejiang Province from 2021 to 2023, so as to provide the reference for improving herpes zoster prevention and control measures. Methods: Data on the incidence and vaccination of first-visit herpes zoster cases at all levels of public medical institutions in Zhoushan City from 2021 to 2023 were collected through the Zhoushan Comprehensive Health Information Platform and Zhoushan Immunization Program Information Management System. The incidence and outpatient proportion were calculated. The population distribution, seasonal distribution, and clinical consultation status of first-visit herpes zoster cases were described. Results: From 2021 to 2023, a total of 15 156 first-visit herpes zoster cases were reported in Zhoushan City, with an average annual incidence of 5.36‰. The incidences for each year were 5.78‰, 5.29‰ and 5.02‰, respectively, and the outpatient proportions were 0.15%, 0.14% and 0.11%, respectively, showed decreasing trends (both P<0.05). The number of doses of recombinant herpes zoster vaccine or live attenuated herpes zoster vaccine administered were 130, 312, and 633, respectively. The main consultation department was dermatology, with 11 004 cases (72.60%). The primary clinical diagnosis was visceral herpes zoster, with 5 901 cases (38.94%). A total of 1 936 cases (12.77%) had at least one underlying medical condition. The mean age of onset was (56.08±16.23) years, and the incidence showed an upward trend with increasing age (P<0.05). There were 7 386 male cases and 7 770 female cases, with a male-to-female ratio of 0.95∶1. The incidence among males aged ≥50 years was lower than that among females (6.53‰ vs. 8.69‰, P<0.05). The onset of the disease exhibited a significant seasonal pattern, with a peak period from June 21st to August 21st, covering 75% of the cases Conclusions From 2021 to 2023, the incidence and outpatient proportion of herpes zoster in Zhoushan City decreased. Summer was the peak season for onset, and women and the elderly were the key populations. It is necessary to strengthen the collaborative diagnostic and treatment capabilities of key departments such as dermatology and enhance the enthusiasm for vaccination among key populations.

INTRODUCTION: Health literacy plays an essential role in one's ability to acquire and understand critical medical information in the coronavirus disease 2019 (COVID-19) infodemic and in other pandemics. We aimed to summarise the assessment, levels and determinants of pandemic-related health literacy and its associated clinical outcomes. METHODS: A systematic review was performed in Medline ® , Embase ® , PsycINFO ® , CINAHL ® and four major preprint servers. Observational and interventional studies that evaluated health literacy related to the novel COVID-19, severe acute respiratory syndrome (SARS) and Middle East respiratory syndrome (MERS) were included. Items used in health literacy instruments were grouped under the themes of knowledge, attitudes and practices. Determinants of health literacy were grouped into five domains: sociodemographic, medical, psychological/psychiatric, health systems-related and others. RESULTS: Of the 2,065 articles screened, 70 articles were included. Of these, 21, 17 and 32 studies evaluated health literacy related to COVID-19, SARS and MERS, respectively. The rates of low pandemic health literacy ranged from 4.3% to 57.9% among medical-related populations and from 4.0% to 82.5% among nonmedical populations. Knowledge about the symptoms and transmission of infection, worry about infection, and practices related to mask usage and hand hygiene were most frequently evaluated. Sociodemographic determinants of health literacy were most frequently studied, among which higher education level, older age and female gender were found to be associated with better health literacy. No studies evaluated the outcomes associated with health literacy. CONCLUSION The level of pandemic-related health literacy is suboptimal. Healthcare administrators need to be aware of health literacy determinants when formulating policies in pandemics.

INTRODUCTION: Achieving low-density lipoprotein cholesterol (LDL-C) levels is key to preventing atherosclerotic cardiovascular events. However, many high-risk cardiovascular patients still experience poor LDL-C goal attainment and receive suboptimal lipid-lowering therapy (LLT) prescriptions. Herein, we evaluated LLT prescription patterns, LDL-C goal attainment and cardiovascular mortality among this population group in Singapore. METHODS: This prospective observational cohort study included 555 patients with ischaemic heart disease (IHD) admitted to the hospital in 2020. The LLT prescriptions, corresponding LDL-C levels and cardiovascular outcomes were assessed over a 24-month period. RESULTS: Most participants were male (82.3%), with 48.5% identified as Chinese. High-intensity statin prescriptions increased from 45.4% at hospital admission to 87.1% at discharge and remained stable at approximately 80% at 6, 12, and 24 months post-discharge. Combination LLT prescriptions increased from 12.3% at discharge to 33.8% by 24 months. Ezetimibe was the most commonly prescribed second-line LLT (40.8%), followed by inclisiran (1.09%) and anti-proprotein convertase subtilisin/kexin type 9 monoclonal antibody therapies (0.87%). Over 24 months, LDL-C goal attainment rates were 22.1% for LDL-C < 1.4 mmol/L and 47.2% for LDL-C < 1.8 mmol/L. Multivariable Cox proportional hazards regression indicated that achieving LDL-C < 1.8 mmol/L goal was associated with a reduction in all-cause mortality at 24 months (hazard ratio 0.53, 95% confidence interval 0.30-0.94, P = 0.030). CONCLUSION Treatment gaps in lipid management persist in 80% of the study population, indicating that statin monotherapy alone is insufficient to achieve LDL-C goals. Greater efforts to improve LDL-C goal attainment rates in high-risk cardiovascular patients are imperative.

Objective To evaluate the prognostic value of serum tissue factor pathway inhibitor-1(TFPI-1)combined with nuclear factor-κB(NF-κB)in severe traumatic brain injury(STBI).Methods The medical re-cords of 127 patients with STBI admitted to the hospital from July 2022 to August 2024 were retrospectively ana-lyzed and followed up for 6 months.They were divided into poor prognosis group(n=53)and good prognosis group(n=74)according to the prognosis of STBI patients.The serum NF-κB level,clinical data and serum TFPI-1 level of the two groups were compared.The factors affecting the adverse prognosis of STBI patients was screened,and the value of serum NF-κB and serum TFPI-1 in predicting the adverse prognosis of STBI patients were ana-lyzed.Results The serum NF-κB level in the poor prognosis group was higher than good prognosis group,and the serum TFPI-1 level was lower than good prognosis group(P<0.05).The proportion of patients aged>60 years old was higher than good prognosis group(P<0.05).Serum TFPI-1 level(OR=0.328,95%CI:0.156～0.689)was a protective factor for poor prognosis in STBI patients(P<0.05),serum NF-κB level(OR=3.773,95%CI:1.797～7.924)and age>60 years(OR=3.543,95%CI:1.687～7.441)were independent risk factors for poor prognosis in STBI patients(P<0.05).The area under the curve(AUC)of serum TFPI-1 and NF-κB levels and their combined prediction of poor prognosis in STBI patients were 0.784,0.847 and 0.931,respectively(P<0.05),and the AUC value of the combined TFPI-1 and NF-κB levels was higher(P<0.05).Conclusion Serum NF-κB combined with serum TFPI-1 has higher prognostic value in STBI patients.

Objective:To evaluate the application effect of non opioid anesthesia using ketamine in endoscopic submucosal dissection under general anesthesia of gastroscopy and laryngeal mask.Methods:Sixty patients who underwent elective endoscopic submucosal dissection under general anesthesia gastroscopy and laryngeal mask at Nanjing Medical University Affiliated Nanjing Hospital from January to December 2022 were randomly divided into a conventional anesthesia group and a non opioid anesthesia group using a random number table method, with 30 cases in each group. The conventional anesthesia group used opioid drugs, while the non opioid anesthesia group used ketamine instead of opioid drugs. We recorded the incidence of postoperative nausea and vomiting in two groups of patients; Recorded the mean arterial pressure (MAP) and heart rate (HR) of two groups of patients before anesthesia (T 0), during intubation (T 1), at the start of surgery (T 2), and after surgery (T 3); We also recorded the lowest pulse oxygen saturation (SpO 2) after extubation in two groups of patients; At the same time, the surgical time, propofol dosage, extubation time, monitoring time in the anesthesia and resuscitation unit (PACU), incidence of postoperative nausea and vomiting, patient satisfaction, and other adverse reactions were recorded. Results:The difference in surgical time, MAP and HR from T 0 to T 3, and the lowest SpO 2 after extubation between the conventional anesthesia group and the non opioid anesthesia group was not statistically significant (all P>0.05); Compared with the conventional anesthesia group, the non opioid anesthesia group had a lower incidence of postoperative nausea and vomiting, lower dosage of propofol, shorter extubation time and PACU monitoring time, and higher patient satisfaction (all P<0.05); Both groups of patients had no serious adverse reactions. Conclusions:The application of ketamine in endoscopic submucosal dissection during non opioid anesthesia of gastroscopy laryngeal mask has certain advantages over conventional use of opioid drugs, including fast postoperative recovery, low incidence of nausea and vomiting, and high patient satisfaction.

AIM To explore the effects and mechanism of Polygoni multiflori Radix Preparata(PMRP)ona rat model of post-stroke depression(PSD).METHODS The models of middle cerebral artery occlusion(MCAO)established by thread embolism method were then randomly divided into the model group,the positive drug(fluoxetine)group,and the low-dose and high-dose PMRP groups,with 10 rats in each group,in contrast to the 10 rats of the sham operation group.After 7 days of MCAO modeling,the rats underwent their 21-day PSD modeling except those of the sham operation group,during which the rats had their the neurological functions and behaviors assessed on the 1st,7th,14th and 21st day;and their cerebral infarction area and brain water content detected on the 21st day.HE staining,Nissl staining and immunohistochemical staining were used to observe the pathological morphology of cerebral ischemic penumbra.ELISA and Western blot were applied in the detections of the cerebral protein expressions of aquaporins(AQP3,AQP4,AQP5)and brain-derived neurotrophic factor(BDNF).RESULTS Compared with the model group,the high-dose PMRP group displayed improved neurological functions and behavioral scores(P＜0.05,P＜0.01),and reduced cerebral infarction area and brain water content(P＜0.05).Compared with the model group,each treatment group demonstrated clearer brain structure in ischemic penumbra,smaller intercellular space,increased neuron counts,decreased cerebral protein expressions of AQP3,AQP4 and AQP5(P＜0.05,P＜0.01),and increased BDNF protein expression(P＜0.05).CONCLUSION PMRP can reduce the depression of PSD rats by improving the microenvironment of neurons to promote their growth and survival,and eliminating the brain edema to enhance neurological functions via reduced protein expressions of AQP3,AQP4 and AQP5,and increased BDNF protein expression.

ObjectiveTo investigate whether there exists gender differences in mechanical pain hypersensitivity induced by the subcutaneous injection of macrophage colony-stimulating factor （M-CSF） in normal mice and to explore the preliminary mechanism. MethodsThirty 10-week-old C57BL/6J mice were randomly divided into three groups， (n = 10 mice/group, half male and half female). The albumin control group （BSA， 0.3 μg）， low dose M-CSF group （L M-CSF， 0.075 μg） and high dose M-CSF group （H M-CSF， 0.3 μg） received 50 μL BSA or M-CSF injected subcutaneously into the left medial thigh once daily for 3 consecutive days. Before and after drug administration， von-Frey mechanical sensitivity test was used to detect the mechanical paw withdrawal threshold （PWT） in each group. Immunofluorescence was performed to examine the expression changes of Ionized calcium-binding adaptor molecule 1 (Iba1) in skin， calcitonin gene-related peptide （CGRP） and phosphorylated ERK1/2 （p-ERK） in L5-L6 DRG and lumbar spinal dorsal horn. ResultsIn female mice， only high dose of M-CSF caused mechanical allodynia， whereas in male mice both doses produced marked allodynia. Mechanically， high-dose M-CSF induced massive aggregation of subcutaneous macrophages （marked by Iba1） in male and female mice， but more dramatic dependence in female mice. Similar gender differences were also found in the increase of p-ERK and CGRP expression in dorsal root ganglion （DRGs）. Notably， CGRP expression was especially elevated in the fibers of DRG in male mice. Correspondingly， the expressions of p-ERK and CGRP⁺ terminals in the superficial spinal dorsal horn of male mice were significantly higher than those of female mice after M-CSF treatment. ConclusionSubcutaneous injection of M-CSF triggers sexual dimorphism in mechanical pain hypersensitivity, which is related with differential changes in peripheral macrophage expansion and sensitization of the nociceptive pathway.
